Evaluation and Design of Ultrasonic Vibrator for Dental Surgery (치과용 골 수술기의 초음파 진동자 설계 및 평가)

  • A dental ultrasonic surgical instrument, commercially known as a scaler, is a high-value-added advanced technology that is used for tartar removal, implant operations, and gum and jaw bone surgery. In this study, the piezoelectric phenomenon for making linear motion associated with input electrical signals was studied, and the behavior of the ultrasonic vibrator was investigated by using the commercially available finite element program ANSYS(R) for the purpose of designing dental surgery tools. Modal analysis was carried out, and the optimal frequency range was calculated from the analyzed results. The ultrasonic vibrator was then redesigned based on the calculated optimal frequency range. The performance of the system was tested, and consequently, the proposed methodology was proven useful in vibrator design.

  • In the dentistry field, an ultrasonic surgery unit is widely used in bone cutting and scaling to reduce operation time and minimize hemorrhage. The purpose of this study was to evaluate bone cutting and the effect of a specimen's temperature on the two-type ultrasonic surgery unit using a handpiece moving system(HMS). A HMS, which can cut the bone, was developed to perform the experimental procedure with precision of motion control. Bone specimens were prepared from a combination of epoxy-hardener and cortical bone of bovine leg. Through the bone-cutting experiment, the cutting depth was evaluated by not only scanning electron microscopy, but also Vernier calipers. Also, the temperature distribution was measured by a thermo-graphic camera. This study may be applied methodically in various experimental evaluations on a performance test by a HMS.

A Study on Temperature Changes during Bone Scaling and Cutting of Dental Ultrasonic Scaling/Surgery System (치과용 초음파 스케일러/수술기 통합 시스템의 스케일링 및 절삭 시 온도 변화에 관한 연구)

  • While dental clinics still use the ultrasonic scaling/surgery tool for teeth scaling and cleaning the tool's use is expanding steadily to include treatment of damaged teeth and bone tissue. In this study, a handpiece moving system (HMS) was developed to evaluate bone scaling and cutting in the field of dentistry. The HMS, through a scaling test of bone using a scaler tip, it was able to identify surface damage. Additionally, a thermos-graphic camera was used to observe the temperature distribution that occurred during the bone scaling and cutting process. Consequently, we found that increasing the working load increased the amount of surface damage. Changes in temperature distribution occurred slowly and were maintained within safety bounds for 10 minutes. Going forward, we will compare the HMS performance on scaling and cutting with other devices.

  • Recently, research on bone fracture and osteoplasty using ultrasonic bone surgery unit has been ongoing in the fields of dentistry, plastic surgery, and otorhinolaryngology. However, detailed data evaluation with ultrasonic bone surgery unit has not been conducted to date. In this study, we developed handpiece moving system (HMS) for cutting performance evaluation. In the experimental setup of HMS, a handpiece was immobilized, and bone samples from cortical bone of bovine leg were prepared. Also, the experimental process was described in detail, and a basic experiment was carried out to evaluate the cutting performance. Future study is required on all experimental process conditions by HMS.

  • The use of ultrasonic surgery units and scalers are steadily increasing in the field of dentistry. Such units and scalers should be evaluated before they are commercialized because the mechanical performance and stability of equipment is crucial for patients. Hence, we created a hand-piece moving system that can test bone cutting and teeth scaling. The purpose of this study was to evaluate the teeth scaling performance of ultrasonic scaler unit. Additionally, we measured the temperature distribution and noise during the test. through an experimental test, we found that a high output of an Ultrasonic NX device can cause serious damage to the teeth surface, and it was not within range in heat generation distribution and noise test.

  • Developmental dysplasia of the hip broadly includes inadequate development of the hip joint involving the acetabulum or proximal femur, or both. Although ultrasonographic studies in neonates have greatly lowered the frequency of neglected or operatively treated cases, its sensitivity is less than desired. Hip dysplasia without subluxation is commonly diagnosed incidentally and strongly related to degenerative arthritis in females after the 4th decade. Hip dysplasia with subluxation shows symptoms through various periods, depending on its severity, especially for women with onset during pregnancy. A complete physical examination and early treatment for neonates are extremely important for obtaining satisfactory outcomes. To avoid underdiagnosis and to serve appropriate treatment on time, the authors recommend examining any suspicious hips in infants under two years of age. The study will discuss the diagnosis and primary treatment of developmental dysplasia of the hip.
