Ultrasound-Guided Local Steroid Injection Therapy for Acute Calcific Tendinitis of Shoulder (초음파를 이용한 급성 극상근 석회화 건염의 스테로이드 국소 주사 요법)

  • Purpose: To analyze the clinical outcome after ultrasound guided multiple dry needlings and local steroid injection for acute calcific tendinitis of shoulder. Materials and Methods: Twenty patients with acute episode of pain by calcific tendinitis of shoulder with average age 58.2 (50~70 years) and follow-up of 18 months in average (range, 12~24) were included in study. There were 18 patients with right and 2 with left sided involvement. All patients had calcific deposits in the supraspinatus tendon. All patients underwent standardized nonoperative treatment protocol, consisting of 5~12 MHz high resolution ultrasound guided multiple dry needlings with 18 guage needle, followed by 2% lidocaine 1cc and 40 mg/ml depomedrol 1cc injection at site of calcific tendinitis. The outcome was assessed by UCLA shoulder score, range of motion and VAS score. A statistical analysis with ANOVA and Tukey's post-hoc test with the significance level at 5% was performed using SAS 9.1 software (SAS Institute, Cary, NC). Results: All patients got continuous relief of pain right after the procedures until final follow-up. Before the procedures, the UCLA scores were fair in 15 patients and poor in 5. After the procedures, the UCLA scores were excellent in 16 patients and good in 4. All cases revealed no limitation of shoulder function. The average VAS score decreased from 8.9 before the procedures to 0.5 at final follow-up (p<0.0001). No complication was encountered. Conclusion: An ultrasound guided local steroid injections following multiple dry needlings would be one of the useful treatment modality for the acute calcific tendinitis of shoulder.

  • Study Design: Literature review. Objective: Ultrasound-guided injections are a common clinical treatment for lower lumbosacral pain that are usually performed before surgical treatment if conservative treatment fails. The aim of this article was to review ultrasound-guided injections in the lumbar and sacral spine. Summary of Literature Review: Ultrasound-guided injections, unlike conventional interventions using computed tomography or C-arm fluoroscopy, can be performed under simultaneous observation of muscles, ligaments, vessels, and nerves. Additionally, they have no radiation exposure and do not require a large space for the installation of equipment, so they are increasingly selected as an alternative method. Materials and Methods: We searched for and reviewed studies related to the use of ultrasound-guided injections in the lumbar and sacral spine. Results: In order to perform accurate ultrasound-guided injections, it is necessary to understand the patient's posture during the intervention, the relevant anatomy, and normal and abnormal ultrasonographic findings. Facet joint intra-articular injections, medial branch block, epidural block, selective nerve root block, and sacroiliac joint injections can be effectively performed under ultrasound guidance. Conclusions: Ultrasound-guided injections in the lumbar and sacral spine are an efficient method for treating lumbosacral pain.

  • Ultrasonic Machining process is an efficient and economical means of precision machining on glass and ceramic materials. However, the mechanics of the process with respect to crack initiation and propagation, and stress development in the ceramic workpiece subsurface are still not well understood. In this research, we investigate the basic mechanism of chemical assisted ultrasonic machining(CUSM) of glass through the experimental approach. For the purpose of this study, we designed and fabricated the desktop micro ultrasonic machine. The feed is controlled precisely by using the constant load control system. During the machining experiment, the effects of HF(hydrofluoric acid) characteristics and machining condition on the surface roughness and the material removal rate are measured and compared.

  • 본 논문은 시청자의 위치에 최적화된 입체음향을 제공하는 TV 개발에 관한 것으로 2 개의 TV 스피커만으로 5.1 채널 스피커가 주는 입체음향 효과를 제공해준다. 기존의 Speaker Virtualizer 기술은 시청자가 특정 위치(Sweet Spot)를 벗어나면 입체음향 성능이 현저히 저하된다. 반면, 본 논문에서 제안하는 Adaptive Virtualizer 기술은 초음파가 장착된 리모콘을 사용하여 시청자의 위치를 인식하고 인식된 시청자의 위치 정보를 활용하여 청취위치에 해당하는 HRTF로부터 설계된 Filter를 Update 하고 두 스피커의 출력레벨 및 시간지연 값을 보정함으로써 최적의 입체음향을 재현한다. 본 논문에서는 실시간 구현을 위해 Speaker Virtualizer의 계산량을 최소화하는 기술을 제안하고 다양한 청취 위치에 해당하는 Filter를 설계하고 설계된 Filter를 효율적으로 Update 하는 Adaptive Virtualizer 기술을 제안한다. 또한, 초음파를 이용한 시청자 위치 인식 기술 및 전체 시스템 통합 기술을 제시한다.

  • This paper proposes an ultrasonic sensor based intelligent automatic door system which improves the performance of conventional door systems by adding more intelligent functions such that it offers more convenience to passersby and reduces power loss. The conventional automatic door systems employed passive and active infrared sensors for detecting objects and human bodies. But, they have problems such as power loss in door closing, not sensing fast approaching objects, and safety. The proposed automatic door system with ultrasonic sensors prevents unnecessary door closings to save the power and senses fast approaching objects to open the door at proper time, and improves safety. Thus, the proposed system improves the performance of the conventional systems in terms of operation, economy, and safety.

  • Ultrasonographic diagnosis and treatment in Orthopaedic fields had been widely used. Sonographic diagnosis and treatment of foot and ankle is convenient because of anatomical characteristics. The knowledge of the anatomy and biomechanics in foot and ankle area can help to diagnose and treat the disease around foot and ankle. 28 bone and many tendons, ligaments and muscles are consist of ankle and foot joint and the coordinative relation among these structures can allow the dual function, weight bearing and locomotion of ankle and foot during gait cycle. Foot and ankle have small room for many structures, so systemically physical examination is essential for diagnosis. Accurate understanding of foot and ankle anatomy and biomechanics could be helpful to using ultrasonograph.

  • De Quervain's disease is a stenosing tenosynovitis that affects the synovial sheath of the tendons of the abductor pollicis longus and extensor pollicis brevis; it can be considered a work-related disease, since it frequently is associated with highly repetitive movements in jobs using the hands. The usual sign and symptom of de Quervain's diseases are positive Finkelstein's sign and pain and tenderness around the radial styloid process. The majority of de Quervain's disease has a good results to conservative treatment, but surgery can be performed in case of recurrence or pain last more than 6 months without improvement in spite of conservative treatment. We diagnosed a patient with atypical de Quervain's disease by ultrasonography, treated by local steroid injection and pain was relieved. We report a case of atypical de Quervain's disease evaluated by ultrasonography with brief of literatures.

Efficacy of Ultrasonogram for the Diagnosis of Biceps Tendon Pathology (상완 이두건의 병소에 대한 초음파 검사의 유용성)

  • Purpose: We wanted to assess the accuracy of ultrasound for detecting abnormality of the long head of the biceps tendon in patients with rotator cuff disorders. Materials and Methods: Between January 2006 and March 2007, we reviewed the arthroscopic findings of biceps tendons in 67 patients with rotator cuff disorder and who underwent ultrasonography pre-operatively. The patients' average age was 58 years and there were 41 males and 26 females. The statuses of the biceps tendons were described as 'normal', 'dislocation', 'subluxation', 'partial or complete tears', and 'tendinopathy'. We investigated the correspondence between the arthroscopic and ultrasonographic findings. Results: On ultrasonography, the biceps tendons were 'normal' in 37 patients and 'abnormal' in 30 patients. On arthroscopy, 5 biceps tendons turned out to have partial tears, which were 'normal' on ultrasonography. On the other hand, 8 biceps tendons were 'normal' which were 'abnormal' on ultrasonography. Three dislocations and 4 complete tears of biceps tendons were identified on arthroscopy, and all of them were detected by ultrasonography. Of the 20 cases of 'tendinopathy' seen on ultrasonography, 15 patients had partial tears and the remaining 5 patients were normal on arthroscopy. Conclusion: Ultrasonography is accurate for detecting some biceps pathologies, such as dislocation or complete tear, but it is not accurate for the detection of partial tear.

  • In this study, piezoceramics/polymer composites with 3-3 connectivity were made by BURPS(Burnout Plastic Sphere) technique with PZT ceramics and plastic sphere. And the properties dependent on the Dextrin wt.% were investigated. The density of porous piezoceramic and piezoceramic/polymer composites were decreased almost linearly with increasing the Dextrin wt.%.

  • In this study, 3-D underwater object recognition using ultrasonic sensor fabricated with porous piezoelectric ceramics and SCL(Simple Competitive Learning) neural networks are presented. The recognition rates for the training data and the testing dara were 96 and 93%, respectively.
