The purpose of this study was to classify risk groups according to Caries Risk Assessment (CRA) and to investigate the effect of caries prevention program after 1 year of caries prevention intervention program in 6-year-old infants with high caries risk. The subjects were selected based on responses to CRA questionnaires. At the first visit, oral examination, Cariview, bacteria and saliva flow test were performed. The caries risk group was classified accordingly. The subjects were given fluoride application and oral health education every four months and evaluated the same as the first visit after 1 year. As a result of classifying the risk level according to CRA, more than 80% of the subjects were in the high or extreme high risk. The dft index was increased in all risk groups after the intervention. There was a significant difference between the before and after intervention (p<0.05). The Cariview score showed a slight decrease after the intervention in the moderate and high risk groups. As a result of the evaluation of bacteria test, Streptococcus mutans were decreased to ${\geq}10^5CFU/ml$ saliva after intervention in all groups. Lactobacilli were decreased after intervention in high risk and extreme high risk groups. As a result of saliva flow, there was significant difference between caries risk groups before and after intervention (p<0.05). In conclusion, regular caries management has been shown to influence caries risk factors in high-caries risk children. Also, it is necessary to find out periodical dental risk management system which is suitable for domestic situation through the related studies.
This study was designed to investigate the using status and awareness of pit and fissure sealant, and how it affects on secondary caries when performed on above of initial caries. After classifying 446 occlusal surface into four groups, performed pit and fissure sealant, induced artificial caries, used DIAGNOdent (Kavo) to measure degree of secondary caries. Also, distributed a questionnaire on dental clinic in metropolitan area to find out using status and awareness of pit and fissure sealant, the following results were obtained. 1) On inspection and percussion, Group 4 corresponding to the enamel caries showed the highest secondary caries after sealant and was statistically significant difference in the order of initial group, stained group, sound group (p<0.05). 2) Inspection showed the highest percentage on tooth fissure caries diagnostic methods before sealant. 3) 56.6% didn't know about DIAGNOdent, 91.6% didn't have it. 4) In clinically, the most cause of secondary caries after sealant was a broken sealant, making caries on the downward. Based on the results of above study, degree of caries under sealant could affect on secondary caries, needs publicity about the use and necessity of objective fluorescence device.
The purpose of this study was to evaluate the efficacy of the newly-developed $Qraypen^{TM}$ (All In One Bio, Korea) system for the diagnosis of early proximal caries by comparing it with the conventional methods of visual inspection and periapical radiography. This study was carried out from July 2015 to April 2016 targeting 32 children aged 7~12 years who visited Y-Dental Clinic for school oral health examinations. Two investigators selected and examined a total of 153 primary molars that had not undergone restorative treatment. Comparisons were carried out between visual inspections, readings of posterior periapical radiography images, and readings of $Qraypen^{TM}$ images. This study revealed that the percentage of interproximal surfaces of primary molar teeth without caries incidence was 83.7% using $Qraypen^{TM}$ imaging and 84.9% using visual inspection and periapical radiography. The differences between the two methods were not statistically significant. Thus, $Qraypen^{TM}$ is expected to be a useful and convenient auxiliary diagnostic device that can facilitate the detection of hidden proximal caries in primary molars.

There is no adverse opinion on the anticariogenic effect of fluoride, so glass ionomer restoration which release the fluoride is recommended fer child patient. To study the anticariogenic effect of initial carious lesion of fluoride released from adjacent glass ionomer restoration, the in situ model was constructed. A microhardness test, polarized scope investigation and electron probe microanalysis was done for analyzing the distribution of fluoride which was precipitated from glass ionomer restoration to the enamel caries lesion. Fuji IX, the conventional glass ionomer, was used for experimental group and Z-100, composite resin that fluoride was not contained, was used for control group. On the microhardness test, the remineralization was accelerated by fluoride. And on the polarized investigation, the size of caries lesion was reduced in the oral cavity and that phenomenon was accelerated by fluoride, too. Electron probe microanalysis shows that the remineralization was accelerated by fluoride and the fluoride concentration on subsurface area was increased. It maybe that the subsurface area was critical to anticariogenic effect. In summary of these result, initial caries lesion can be remineralized in the oral cavity and that phenomenon can be accelerated by fluoride. The subsurface area of caries lesion was a major part of defense to cariogenic invasion and to conserve the subsurface area, the surface of lesion body have to conserved.

The purpose of this study is to analyze the cariogenicity of vitamin supplements for children by the Caries Potentiality Index (CPI), pH drop capacity, proliferation rate of Streptococcus mutans. Four vitamin supplements were selected - Noma (NM), Cenovis Kids (CK), Animal Parade (AP), and Character Vitamin (CV). CPI value decreased in the order of AP, CV, CK, and NM. Initial values of all experimental groups showed acidity below pH 7.0. Analysis of the colony forming units of Streptococcus mutans showed that NM and CV resulted a higher proliferation rate (p < 0.05) than CK and AP (p < 0.05). Bacterial activity of the control group was lower than other groups when observed with a confocal laser scanning microscope. Considering the bacterial activity and acidity of vitamin supplements, it is necessary to pay close attention when children taken the vitamin supplements for their oral health.

This study was performed to evaluate the restoration combined with resin infiltration (RI) of early cavitated smooth surface caries lesion in terms of microleakage. Flowable resin and resin-modified glass ionomer cement (RMGIC) were compared. Sound 20 extracted 3rd molars were divided into 2 groups randomly. Artificial decalcified lesion was induced. Cavities were prepared on the mesial and distal surfaces, and randomly set as experimental and control group. RI was applied to the experimental group before cavity restoration. The control group was restored without RI. In group I and II, flowable resin and RMGIC was used for restoration respectively. After thermocycling and silver nitrate immersion, microleakage was assessed by μ-CT. Depth of microleakage was lower in experimental group than control group only in group II (p = 0.05). Microleakage depth was lower in group II than group I in both experimental and control groups (p = 0.05). RI pretreatment before restoration of early cavitated caries lesions might reduce the microleakage and help long-term maintenance of restoration. In this study, RMGIC was less polymerization shrinkage. Restoration with RMGIC after RI pretreatment reduced the microleakage of the restoration compared to the flowable resin.

This study investigated the effects of silver diamine fluoride (SDF) and potassium iodide (KI) treatments on the acid resistance of dentin exposed to secondary caries. Sixteen bovine dentin specimens with artificially induced caries were assigned to the following four groups: untreated negative control, untreated positive control, SDF-treated (SDF), and SDF and KI-treated (SDFKI). Multispecies cariogenic biofilms containing Streptococcus mutans, Lactobacillus casei, and Candida albicans were cultured on the specimens for 28 days, except for the negative control group. Specimens from the negative control group were stored in phosphate-buffered saline for that period. After a cariogenic biofilm challenge, the degree of demineralization was evaluated using micro-computed tomography (micro-CT). As a result of data analysis using micro-CT, the demineralization depths of the negative control, positive control, SDF, and SDFKI groups were 149.0 ± 7 ㎛, 392.0 ± 11 ㎛, 206.0 ± 20 ㎛, and 230.0 ± 31 ㎛, respectively. The degree of demineralization was significantly reduced in the SDF and SDFKI groups compared with that in the untreated positive control group. There were no significant differences between the SDF and SDFKI groups. This study confirmed that SDF and SDFKI treatments increase the acid resistance of dentin to secondary caries. KI did not significantly affect the caries-arresting effect of the SDF.

In this study, we observed the penetration of the resin infiltrant into natural initial caries by SEM and CLSM, evaluated the pH and viscosity of resin infiltrant and several adhesives(All-bond 2, Scotchbond Multi-purpose, Clearfil SE bond), and measured the surface roughness and the microhardness on permanent teeth before and after applied resin infiltrant into initial enamel caries. The viscosity(Pa.s) was observed for resin infiltrant($0.173{\pm}0.012$), All-bond 2($0.754{\pm}0.030$), Scotchbond Multi-purpose($1.46{\pm}0.137$), and Clearfil SE bond($1.28{\pm}0.125$). The pH was measured for resin infiltrant (4.7), All-bond 2(7.5), Scotchbond Multi-purpose(8.2), and Clearfil SE bond(4.67). An application procedure of resin infiltrant increased the surface roughness and the microhardness insignificantly(p>0.05), when compared to non-applied specimens.

The purpose of this study was to find out the effect of dentifrice containing Horseradish (Armoracia rustica) root extracts on improvement of oral hygiene. The clinical evaluation was done participated by 80 adults volunteers who provided a informed consent for their participation. The participants were divided into two groups ; control group (using toothpaste without Horseradish extracts) and experimental group (using toothpaste containing Horseradish extracts). The checklist of the experiment includes the plaque index, gingival index and anti-caries activity. The anti-caries activity was measured by cultivating Streptococcus mutans for 48 hours then estimating the number of increased bacteria using Dentocult SM. The double-blind method was used in this study. The results of this study can be summarized as follows; 1. The plaque formation decreased 28% in control group, 58% in experimental group using dentifrice with Horseradish extracts compared to baseline data. 2. For gingival index, control group and experimental group showed 26% and 40% decrease compared to baseline data, respectively. 3. The score of caries activity test decreased by 4% for control group and 69% for experimental group compared to baseline data. According to the results, a dentifrice containing Horseradish extracts can improve oral hygiene.
Antimicrobial activity of extract from some traditional oriental medicinal plants were evaluated for their antimicrobial activity against six oral pathogens, Streptococcus salivarius, Streptococcus oralis, Streptococcus mutans, Streptococcus sanguinis, Lactobacillus acidophilus and Lactobacillus casei, which are associated with caries disease. The antimicrobial activity was examined by determining the inhibition zone using the disc diffusion assay. In antibacterial activity test, extracts of Scutellaria baicalensis, Chrysanthemum indicum, Kochia scoparia, Hydnocarpus anthelmintica and Caesalpinia sappan showed inhibitory effects (40 mg/ml) against tested caries bacteria. Especially, the C. sappan extract showed the strongest activity on S. oralis (40 mm), L. casei (35 mm) and S. mutans (28 mm). Thus, this result suggests that C. sappan may be applicable to preventing dental caries.
