Evaluation of Setup Uncertainty on the CTV Dose and Setup Margin Using Monte Carlo Simulation (몬테칼로 전산모사를 이용한 셋업오차가 임상표적체적에 전달되는 선량과 셋업마진에 대하여 미치는 영향 평가)

  • The effect of setup uncertainties on CTV dose and the correlation between setup uncertainties and setup margin were evaluated by Monte Carlo based numerical simulation. Patient specific information of IMRT treatment plan for rectal cancer designed on the VARIAN Eclipse planning system was utilized for the Monte Carlo simulation program including the planned dose distribution and tumor volume information of a rectal cancer patient. The simulation program was developed for the purpose of the study on Linux environment using open source packages, GNU C++ and ROOT data analysis framework. All misalignments of patient setup were assumed to follow the central limit theorem. Thus systematic and random errors were generated according to the gaussian statistics with a given standard deviation as simulation input parameter. After the setup error simulations, the change of dose in CTV volume was analyzed with the simulation result. In order to verify the conventional margin recipe, the correlation between setup error and setup margin was compared with the margin formula developed on three dimensional conformal radiation therapy. The simulation was performed total 2,000 times for each simulation input of systematic and random errors independently. The size of standard deviation for generating patient setup errors was changed from 1 mm to 10 mm with 1 mm step. In case for the systematic error the minimum dose on CTV $D_{min}^{stat{\cdot}}$ was decreased from 100.4 to 72.50% and the mean dose $\bar{D}_{syst{\cdot}}$ was decreased from 100.45% to 97.88%. However the standard deviation of dose distribution in CTV volume was increased from 0.02% to 3.33%. The effect of random error gave the same result of a reduction of mean and minimum dose to CTV volume. It was found that the minimum dose on CTV volume $D_{min}^{rand{\cdot}}$ was reduced from 100.45% to 94.80% and the mean dose to CTV $\bar{D}_{rand{\cdot}}$ was decreased from 100.46% to 97.87%. Like systematic error, the standard deviation of CTV dose ${\Delta}D_{rand}$ was increased from 0.01% to 0.63%. After calculating a size of margin for each systematic and random error the "population ratio" was introduced and applied to verify margin recipe. It was found that the conventional margin formula satisfy margin object on IMRT treatment for rectal cancer. It is considered that the developed Monte-carlo based simulation program might be useful to study for patient setup error and dose coverage in CTV volume due to variations of margin size and setup error.

  • 서로 섞이지 않는 두 비압축성 유체의 유동을 해석하기 위하여 VOF 방법에 기초한 수치 기법을 개발하였다. 유체간의 계면형상의 거동은 유동장내의 유체의 점유체적비의 변화에 의해 묘사되는데 이를 지배하는 이동방정식을 풀기 위한 새로운 대류항 계산법을 고안하였다. 대류항은 유체계면의 방향에 따라 풍상법과 역풍상법의 적절한 조합을 취하여 계산하는데 여기에 대각방향의 상류효과를 포함시켜 시간에 대한 2차 정확도를 갖도록 하였다. 또한 이 방법을 유량보정수송(FCT)법과 결합시켜 해의 단조성을 보장하였다. 몇 가지 단순 문제에 대한 시험 결과 이 기법이 수치오차에 의한 계면형상의 변형과 파손을 감소시킴을 확인하였다.

A comparison of bladder volume by sonogram and CBCT for Pelvic region cancer (골반부 암 치료 시 초음파검사를 통한 방광체적과 CBCT영상 방광체적의 비교)

  • Purpose: During the pelvic radiation therapy, it is performed with controlling bladder volume because the range of irradiation is changed depending on the bladder volume. Therefore in this study, we evaluate the accuracy of BVI by tracing the change of bladder volume using ultrasonic pulse diagnosis equipment(BVI 6100, Medical supply Co. LTD) Material and Methods: From January 2017 to September 2018, 19 patients who received pelvic radiation therapy at Pusan National University Hospital were included. To treat the patient, we compared that the bladder volume obtained from the BVI and the bladder volume obtained from the CBCT image then we evaluated for significance. Results: There was a significant correlation of r=0.773, BVI and CBCT in the whole volume section. However, based on the bladder volume in the RTP Image the p value was shown to be 0 at 125~175cc and lower than 0.05 at 175~275cc, And more than 0.05 at 275~375cc. Conclusions: In this study, the patient whose bladder volume is above than 275cc, there is a significance of bladder volume between BVI and CBCT image. However, we could make a decision to be undermined the reliability of BVI measurement in the case of the patient with emptied his urine. Therefore, it is possible to acquire a significant value for 175~275cc patients to use the correction value of BVI and the appropriate tolerance of volume.

Bladder volume variations of cervical cancer patient in radiation therapy using ultrasonography (초음파검사를 이용한 자궁경부암 환자의 방사선치료 시 방광 체적 변화)

  • Purpose : The bladder volume change was measured using ultrasonography for helping decrease the side effects and other organ variations in the location of radiation therapy for cervical cancer patients. An experiment was performed targeting patients who were treated with radiation therapy at PNUH within the period from September to December 2015. Materials and Methods : To maintain the bladder volume, each patient was instructed to drink 500 cc water before and after CT simulation, 60 minutes before the dry run. Also, the bladder volume was measured in each patient CT scan, and a 3D conformal therapy plan was designed. The bladder volumes measured before and after the CT simulation, dry run, and radiation treatment planning were compared and analyzed. Results : The average volume and average error of the bladder that were obtained from the measurement based on the CT scan images had the lowest standard deviation in the CT simulation. This means that the values that were obtained before and after the CT simulation were statistically relevant and correlative. Moreover, the bladder volume measured via ultrasonography was larger size, the average volume in the CT scan. But the values that were obtained Dry run and after the CT simulation were not statistically relevant. Conclusion : Drinking a certain amount of water helps a patient maintain his/her bladder volume for a dry run. Even then, it is difficult to maintain the bladder volume for the dry run. Also, whether or not the patients followed the directions for the dry run correctly is important.

Uncertainty Analysis of the Calculated Radioactivity in Liquid Effluent Released as Batch Mode from a Nuclear Power Plant (발전용원자로에서 뱃치방식으로 배출되는 액체상 방사성물질의 방사능 평가결과에 대한 불확도 해석)

  • A series of factors such as sampling, pretreatment measurement, volume estimation which induces uncertainty of the calculated radioactivity in liquid effluent released from a nuclear power plant were analyzed. It is innately impossible to estimate exact error of the calculated radioactivity, since most of the input parameters are determined by a single measurement and true value of the released radioactivity cannot be known. In this paper, a systematic model to calculate uncertainty of the released liquid radioactivity was developed based upon the guidance report published by the ISO in 1993, and the model was applied to a set of hypothetical batch release conditions. As a result, the Priority of each input parameter was turned out to be (1) wastewater volume, (2) sample volume, and (3) measured radioactivity of the sample. In addition, probability distribution of the released radioactivity was simulated by Monte Carlo method combining the probability distribution of each input parameter It was shown that the radioactivity released to the environment, which has been reported as a single value, has a certain form of probability distribution.

  • 우리나라 홍수 발생은 강수량이 집중되는 여름철에 집중되어 있어 홍수피해 방지에 주의가 필요하다. 보와 제방 등 이러한 홍수 피해를 대비하기 위한 구조물에 설계를 위해서는 하천의 유량 조사가 필수적으로 요구된다. 하지만 홍수기 직접적인 유량 조사는 안전상의 이유로 거의 이루어지지지 않고 있으며, 수위를 측정하여 수위-유량 관계를 만들어 유량을 측정하고 있다. 그러나 중소 규모의 하천의 경우 하도 경사가 급해 사류가 발생하거나 하도 단면이 급변하는 경우가 있어 수위-유량 관계를 그대로 적용하기 어려운 문제가 있다. 따라서 이러한 문제를 해결하기 위해 하천에 진입하지 않고 유속을 측정 할 수 있는 영상유속계와 같이 흐름 영상을 사용하여 유속을 측정하는 방법들이 개발되었다. 영상유속계의 측정 방법중 Spatio Temporal Image(시공간 영상)을 사용하는 방법은 일정시간의 시간평균 유속을 산정할 수 있고 한 장의 시공간 영상을 분석하기 때문에 유속 산정에 걸리는 시간이 작은 장점이 있지만 영상 내 흐름 방향을 정확히 설정하지 못하면 오차가 생길 수 있는 문제가 있어 주 흐름 방향을 정확히 탐색할 필요가 있다. 따라서 본 연구에서는 영상에서 시공간 체적을 만들어 주흐름 방향을 찾아내는 기법들의 장단점을 비교하고 안동 하천실험센터의 실규모 하천수로에 적용하여 결과를 비교하고 적용성을 평가하였다. 이를 위해 하천수로에 추적자를 살포하여 영상으로 녹화하였으며 녹화된 영상을 자기상관법과 시간적분법을 적용하였으며, 이를 통해 주 흐름 방향을 판별하였다. 또한 두 방법을 통해 결정된 주흐름 방향을 적용하여 시공간 영상을 제작하고 이를 이용하여 유속을 산정하여 비교하였으며, 주흐름 방향을 산정하는데 생기는 오차가 유속 계산에 얼마만큼의 영향을 끼치는지 분석하였다. 이러한 실험을 통해 하천에서 시공간 영상을 활용한 표면영상 유속계측 방법을 활용하는데 있어 도움이 될 것으로 기대된다.

Liver Splitting Using 2 Points for Liver Graft Volumetry (간 이식편의 체적 예측을 위한 2점 이용 간 분리)

  • This paper proposed a method to separate a liver into left and right liver lobes for simple and exact volumetry of the river graft at abdominal MDCT(Multi-Detector Computed Tomography) image before the living donor liver transplantation. A medical team can evaluate an accurate river graft with minimized interaction between the team and a system using this algorithm for ensuring donor's and recipient's safe. On the image of segmented liver, 2 points(PMHV: a point in Middle Hepatic Vein and PPV: a point at the beginning of right branch of Portal Vein) are selected to separate a liver into left and right liver lobes. Middle hepatic vein is automatically segmented using PMHV, and the cutting line is decided on the basis of segmented Middle Hepatic Vein. A liver is separated on connecting the cutting line and PPV. The volume and ratio of the river graft are estimated. The volume estimated using 2 points are compared with a manual volume that diagnostic radiologist processed and estimated and the weight measured during surgery to support proof of exact volume. The mean ${\pm}$ standard deviation of the differences between the actual weights and the estimated volumes was $162.38cm^3{\pm}124.39$ in the case of manual segmentation and $107.69cm^3{\pm}97.24$ in the case of 2 points method. The correlation coefficient between the actual weight and the manually estimated volume is 0.79, and the correlation coefficient between the actual weight and the volume estimated using 2 points is 0.87. After selection the 2 points, the time involved in separation a liver into left and right river lobe and volumetry of them is measured for confirmation that the algorithm can be used on real time during surgery. The mean ${\pm}$ standard deviation of the process time is $57.28sec{\pm}32.81$ per 1 data set ($149.17pages{\pm}55.92$).

  • Machine tool errors have to be characterized and predicted to improve machine tool accuracy. A real-time error compensation system has been developed based on volumetric error synthesis model which is composed of machine tool errors. This paper deals with new algorithm about verification of machine tool errors. This new algorithm uses a simplified volumetric error synthesis model. This simplified model is constructed with only main components among the error components of the machines. This main error components are analyzed by three-dimensional helical ball bar test. By substituting result of helical ball bar test fer simplified model, we could find that obtained error components are closed to real error components.

Real time compensation for quasistatic errors of a horizantal machining center (수평 머시닝 센터의 준 정적 오차의 실시간 보정)

  • A real time error compensation system was developed to improve the quasistatic volumetric accuracy of a machining center by using sensing, metrology, modeling, and computer control techniques. Including thermal errors, 32 error components are formulated in the time-space domain. Fifteen thermal sensors are used to characterize the temperature field of the machine. A compensation controller based on the IBM/PC has been linked with a CNC controller to compensate for machine errors in real time. The maximum linear displacement error in 4 body diagonals were reduced from 140 ${\mu}m$ to 34.5${\mu}m$ with this compensation system, and the spindle thermal drift in space was reduced from 147.3 ${\mu}m$ to 16.8 ${\mu}m$.
