3D-Conformal Radiotherapy for Head and Neck Cancers at Asan Medical Center (두경부종양에서 3차원 입체조형치료의 서울아산병원 경험)

  • 목적: 다양한 발생부위에서 발생한 두경부종양을 대상으로 3차원 입체조형치료를 시도하여 표적체적 내 선량균일성과 주변장기의 선량분포변하를 알아보고자 하였다. 대상 및 방법: 1995년 1월부터 1996년 12월까지 3차원 입체조형치료를 시행 받은 38명에 분석을 시행하였다. 3차원 입체조형 치료는 동일평면 또는 비동일평면상에서 4개에서 14개의 조사면 수로 시행되었다. 3차원 입체조형치료계획시 표적 체적에 $50{\sim}82Gy$의 선량을 처방하였고, 이하선 안구, 척수, 측두하악관절 등을 보호하고자 하였다. 3차원 입체조형치료 계획을 기존의 2차원 치료계획과 비교하기 위하여 표적체적과 주변정상자기의 선량체적히스토그램, 평균선량, 표적체적 내에서 처방선량의 $95{\sim}105%$의 선량이 분포하는 체적을 비교하였다 치료계획에서 실제 치료시까지 소용되는 비용효과를 비교하였다. 대상환자의 평균추적기간은 34개월이었다. 결 과: 3차원 입체조형시료는 2차원 치료에 비해서 표적체적내 평균선량이 평균 10% 증가하였고, 주변정장기에 조사되는 방사선량이 현저히 감소됨을 관찰할 수 있었고 표적체적에 대한 등선량 곡선 분포가 우수함을 관찰할 수 있었다. 결 론: 3차원 입체조형치료는 두경부종양에서 표적체적의 선량 균일성이 증가하였고, 주변장기의 보존이 가능할 것으로 생각되었다. 따라서 본 저자들은 3차원 입체조형치료가 두경부종양에서 국소제어율과 무질병생존율 향상에 기여할 것으로 생각하였다.

Development of the Reference Korean Female Voxel Phantom (한국인 기준여성 체적소형 모의체 개발)

  • The objective of this study is for development of the reference Korean female phantom, HDRK-Woman. The phantom was constructed by adjusting a Korean woman voxel phantom to the Reference Korean data. The Korean woman phantom had been developed based on the high-resolution color slice images obtained from an adult Korean female cadaver. There were a total of 39 organs including the 27 organs specified in ICRP 103 for effective dose calculation. The voxel resolution of the phantom was $1.976{\times}1.976{\times}2.0619\;mm^3$ and the voxel array size is $261{\times}109{\times}825$ in the x, y and z directions. Then, the voxel resolution was changed to $2.0351{\times}2.0351{\times}2.0747\;mm^3$ for adjustment of the height and total bone mass of the phantom to the Reference Korean data. Finally, the internal organs and tissue were adjusted using in-house software program developed for 3D volume adjustment of the organs and tissue. The effective dose values of HDRK phantoms were calculated for broad parallel photon beams using MCNPX Monte Carlo code and compared with those of ICRP phantoms.

Evaluation of Dose Variation according to Air Gap in Thermoplastic Immobilization Device in Carbon Ion (탄소입자 치료 시 열가소성 고정기구의 공기층에 따른 선량 변화 평가)

  • Purpose: The purpose of this study is to find out the dose variation according to thickness of the air gap between the patient's body surface and immobilization device in the treatment plan. Materials and Methods : Four conditions were created by adjusting the air gap thickness using 5 mm bolus, ranging from 0 mm to 3 mm bolus. Immobilization was placed on top in each case. And computed tomography was used to acquire images. The treatment plan that 430 cGy (Relative Biological Effectiveness,RBE) is irradiated 6 times and the dose of 2580 cGy (RBE) is delivered to 95% of Clinical Target Volume (CTV). The dose on CTV was evaluated by Full Width Half Maximum (FWHM) of the lateral dose profile and skin dose was evaluated by Dose Volume Histogram (DVH). Result: Results showed that the FWHM values of the lateral dose profile of CTV were 4.89, 4.86, 5.10, and 5.10 cm. The differences in average values at the on the four conditions were 3.25±1.7 cGy (RBE) among D95% and 1193.5±10.2 cGy (RBE) among D95% respectively. The average skin volume at 1% of the prescription dose was 83.22±4.8%, with no significant differences in both CTV and skin. Conclusion: When creating a solid-type immobilization device for carbon particle therapy, a slight air gap is recommended to ensure that it does not extend beyond the dose application range of the CTV.

  • 방사선치료의 발전으로 3차원치료보다 진보된 호흡연동방사선치료가 시행되어지고 있다. 호흡의 규칙성과 환자의 위치 재현성이 중요한 치료적응 인자이며, 호흡연동 방사선치료의 효율을 높일 수 있는 지표이다. 국가암통계상 고령의 암환자가 증가하며, 수술, 화학요법을 병행하는 암 치료법이 널리 이용이 되고 있다. 고식적인 치료를 요하는 고령의 복부 암 환자분들에 호흡연동 방사선치료법을 사용하는데 에는 호흡의 불규칙성과 체위의 재현성의 문제점으로 인한 치료 효율의 저하를 가져온다. 본 연구에서는 호흡에 의한 종양 움직임이 있는 방사선 치료에서 내부표적체적 기반의 치료계획과 호흡연동 기법을 적용한 치료계획과의 선량비교 분석하였다. 2가지 치료법 모두 정상조직 보호선량에 부합한 것으로 나타났으며 치료체적은 처방선량의 95%이상 포함된 선량분포로 적합하였다. ITV 설정을 통한 3D Plan은 고식적 치료을 목적으로 하는 고령의 환자, 체위 및 호흡의 불안정성 환자에게 처방선량의 95% 이상의 4D Plan의 치료법 보다 짧은 시간에 치료함으로써 치료효율을 높일 수 있을 거라 사료된다. 다만 정상조직보호선량(NTCP)에 부합하는지에 대한 평가가 전제되어야 한다.

Radiation Treatment Planning Evaluation by Internal Target Volume Settings (내부표적체적 설정을 통한 방사선치료계획 평가)

  • The study was conducted targeting 25 patients who underwent the respiratory gated radiation therapy in the abdominal region at Radiation Oncology of a University Hospital from December 2013 to June 2014 and types of cancer included liver(64%), CBD(8%), gastric(8%), GB(8%), pancreas(8%), SMA(4%). The means of ITV and PTV volume are 471.44 cm3 and 425.48 cm3, showing an increase in volume. Normal tissue volume was also found to have increased due to the increase of the section selected from PTV section to ITV section. Right kidney showed a significant increase in differences between increase in normal tissue volume, increase in target volume and increase in therapy irradiation area and difference between the means of dose applied to normal tissue. There was no significant difference in the mean dose applied to normal tissue according to the respiratory average. Both kidneys showed a significant difference in the difference between mean doses of target moving and normal tissue. In this study, both therapy methods through PTV section and ITV section volume setting were appropriate for protection doses of normal tissue and distributed over 95% of the prescribed dose and therefore, it is considered to be okay to be optionally used depending on the patient's therapeutic purpose. But in order to minimize the unexpected side effect, the plan of PTV section and ITV section should be established and used by evaluating normal tissue protection dose.

Evaluation of the reduced scalp dose at Volumetric Modulated Arc Therapy(VMAT) (전뇌조사의 체적변조회전치료 시 두피선량 감소에 관한 평가)

  • The use of WBRT(whole brain radiation therapy) has increased due to the increase in the incidence of metastatic brain tumors. The development of radiation therapy techniques is expected to improve the quality of life. The VMAT (Volumetric Modulated Arc Therapy) is an excellent treatment technique that can distinguish the dose in each volume. Therefore, this study compared conventional WBRT and VMAT for hair loss according to the scalp dose using a head phantom. The CI (Conformity Index), HI (Homogeneity Index) and QOC (Quality of Coverage) were measured brain tissue. A 20 percent and 50 percent dose was measured at the scalp, eyeball, lens, and c-spine. Conventional WBRT is excellent at 10 percent of brain tissue. VMAT is far superior at 1000 percent at the other organs. VMAT at the prescribed dose can be used as radiation therapy of metastatic brain tumors with less hair loss.

Study on Tumor Control Probability and Normal Tissue Complication Probability in 3D Conformal Radiotherapy (방사선 입체조형치료에 대한 종양치유확율과 정상조직손상확율에 관한 연구)

  • A most appropriate model of 3-D conformal radiotherapy has been induced by clinical evaluation and animal study, and therapeutic gains were evaluated by numerical equation of tumor control probability(TCP) and normal tissue complication probability (NTCP). The radiation dose to the tumor and the adjacent normal organs was accurately evaluated and compared using the dose volume histogram(DVH). The TCP and NTCP was derived from the distribution of given dosage and irradiated volume, and these numbers were used as the biological index for the assessment of the treatment effects. Ten patients with liver disease have been evaluated and 3 dogs were sacrificed for this study. Based on the 3-D images of the tumor and adjacent organs, the optimum radiation dose and the projection direction which could maximize the radiation effect while minimizing the effects to the adjacent organs could be decided. 3). The most effective collimation for the normal adjacent organs was made through the beams eye view with the use of multileaf collimator. When the dose was increased from 50Gy to 70Gy, the TCP for the conventional 2-port radiation and the 5-port multidimensional therapy was 0.982 and 0.995 respectively, while the NTCP was 0.725 and 0.142 respectively, suggesting that the 3-D conformal radiotherapy might be the appropriate therapy to apply sufficient radiation dose to the tumor while minimizing the damages to the normal areas of the liver. Positive correlation was observed between the NTCP and the actual complication of the normal liver in the animal study. The present study suggest that the use of 3-D conformal radiotherapy and the application of the mathematical models of TCP and NTCP may provide the improvements in the treatment of hepatoma with enhanced results.

A Study on Prospective Plan Comparison using DVH-index in Tomotherapy Planning (토모 테라피 치료 시 선량 체적 히스토그램 표지자를 이용한 치료계획 비교에 관한 연구)

  • Purpose: We proposed the method using dose-volume Histogram index to compare prospective plan trials in tomotherapy planning optimization. Materials and Methods: For 3 patients in cranial region, thorax and abdominal region, we acquired computed tomography images with PQ 5000 in each case. Then we delineated target structure and normal organ contour with pinnacle Ver 7.6c, after transferred each data to tomotherapy planning system (hi-art system Ver 2.0), we optimized 3 plan trials in each case that used differ from beam width, pitch, importance. We analyzed 3 plan trials in each region with isodose distribution, dose-volume histogram and dose statistics. Also we verified 3 plan trials with specialized DVH-indexes that is dose homogeneity index in target organ, conformity index around target structure and dose gradient index in non-target structures. Results: We compared with the similarity of results that the one is decide the best plan trial using isodose distribution, dose volume histogram and dose statistics, and the another is using DVH-indexes. They all decided the same plan trial to better result in each case. Conclusion: In some of case, it was appeared a little difference of results that used to DVH-index for comparison of plan trial in tomotherapy by special goal in it. But because DVH-index represented both dose distribution in target structure and high dose risk about normal tissue, it will be reasonable method for comparison of many plan trials before the tomotherapy treatments.

Decrease of Irradiated Volume using Rotational Treatment by Avoidance Sector in Radiation Therapy for Esophageal Cancer (식도암의 방사선치료에서 부분 각도에 의한 회전 치료를 이용한 조사체적의 감소)

  • In this study, plans to apply 3D conformal radiotherapy, intensity modulated radiotherapy, and volumetric intensity modulated arc radiotherapy to esophageal cancer radiotherapy were compared. In particular, arc therapy was applied to reduce irradiated volume and spread of low-dose during intensity modulated radiation therapy and volumetric intensity modulated arc radiotherapy by limiting part of irradiated angle, in order to compare target doses and dose for surrounding normal tissues of the two methods and those of 3D conformal radiotherapy. No significant difference in target dose was found among the three methods. The 5 Gy volume(V5) of the lung showed 56.53% of conformal radiotherapy, 52.03% of intensity modulated radiotherapy, and 47.84% of volumetric modulated arc therapy(CRT-IMRT p=0.035, CRT-VMAT p<0.001, IMRT-VMAT p<0.001). The 10 Gy volume(V10) showed a significant difference in conformal radiotherapy 35.12%, intensity modulated radiotherapy 34.04%, and volumetric modulated arc radiotherapy 33.28%, showing significant difference in intensity modulated radiotherapy(p=0.018), volumetric modulated arc therapy(p=0.035), no significant difference in dose was found at 20 Gy volume. The mean dose and 20 Gy volume of the heart were not significantly different according to the treatment plan, but the 30 and 40 Gy volumes were 37.16% and 22.46% in the volumetric modulated arc radiotherapy, showing significant differences(p=0.028) in comparison with conformal radiotherapy. It is believed that, by limiting part of the irradiated angle during intensity modulated radiotherapy and volumetric intensity modulated arc radiotherapy, the irradiated volume and, thereby, the 5-10 Gy area and toxicity of the lung can be reduced while maintaining dose distribution of the target dose.

Evaluation of DQA for Tomotherapy using 3D Volumetric Phantom (3차원 체적팬텀을 이용한 토모치료의 Delivery Quality Assurance 평가)

  • The study investigates the necessity of 3 dimensional dose distribution evaluation instead of point dose and 2 dimensional dose distribution evaluation. Treatment plans were generated on the RANDO phantom to measure the precise dose distribution of the treatment site 0.5, 1, 1.5, 2, 2.5, 3 cm with the prescribed dose; 1,200 cGy, 5 fractions. Gamma analysis (3%/3 mm, 2%/2 mm) of dose distribution was evaluated with gafchromic EBT2 film and ArcCHECK phantom. The average error of absolute dose was measured at $0.76{\pm}0.59%$ and $1.37{\pm}0.76%$ in cheese phantom and ArcCHECK phantom respectively. The average passing ratio for 3%/3 mm were $97.72{\pm}0.02%$ and $99.26{\pm}0.01%$ in gafchromic EBT2 film and ArcCHECK phantom respectively. The average passing ratio for 2%/2 mm were $94.21{\pm}0.02%$ and $93.02{\pm}0.01%$ in gafchromic EBT2 film and ArcCHECK phantom respectively. There was a more accurate dose distribution of 3D volume phantom than cheese phantom in patients DQA using tomotherapy. Therefor it should be evaluated simultaneously 3 dimensional dose evaluation on target and peripheral area in rotational radiotherapy such as tomotherapy.
