The Identification and Analysis of Selected Health Behaviors as Reported by Korean Adolescents in Seoul (한국 청소년들의 건강위험행동 실태파악 및 분석)

  • 본 연구의 목적은 한국청소년들의 건강행동실태를 파악하기 위한 것이다. 본 연구의 의의는 미국에서 행해지고 있는 청소년의 건강유지 및 증진과 아울러 성인병이나 조기사망에 대한 역학적인 접근방식을 한국청소년을 대상으로 하여 응용하는 데 있다. 이 연구에 사용된 연구도구는 미국의 Centers for Disease Control에서 청소년의 건강행동을 역학적으로 연구하기 위해 개발한 Youth Risk Behavior Survey를 한국 사회환경의 요구도에 맞게 수정한 것이다. 본 연구는 한국인 청소년들의 건강행동을 조사하기 위한 타당성있는 연구도구를 개발하기 위해 4개의 단계로 구성되어 있다. 두번의 focus group discussion과 pilot test, 그리고 실제조사로 이루어져 있다. 서울지역의 중.고등학생을 대표할 표본추출은 cluster sampling을 사용하였다. Cluster는 본 연구에서 구분한 서울의 지리적인 구역들, 즉 북부, 남부, 그리고 산업지역을 포함했고, 남자학교, 여자학교, 남녀공학학교를 포함했다. 총38개 중.고교 학생 4,747명이 조사되었다. 약 52.4%의 응답자가 심각하게 자살을 생각해본 적이 있다고 답했다. 그리고 23.6%가 설문조사전 30일 중에 담배를 피운 적이 있다고 응답했으며 49.7%는 설문조사전 30일중에 술을 마신 적이 있다고 답했다. 약 29%가 설문조사전 30일 중에 싸움에 가담한 적이 있다고 응답했다. 본 연구에서 조사된 건강관련행동 (건강위험행동 포함)들은 성, 연령, 성적, 사회경제적 위치, 학교 종류, 그리고 서울의 지리적인 구역에 따라 다양하게 나타났다. 남학생이 여학생보다 더 간강위험행동을 나타내었다. 건강위험행동은 나이가 증가함에 따라 증가했다. 사회경제적 위치가 낮다고 응답한 학생들과 학급에서 성적이 하위라고 응답한 학생들의 경우 건강위험행동은 더 많이 나타났다.

The Effects of Parent-Adolescent Communication and Depression on Suicide Ideation (부모-자녀 의사소통과 우울이 청소년 자살생각에 미치는 영향)

  • Purpose: The purpose of this study was to identify the effect of parent-adolescent communication and depression on suicide ideation in the adolescent population. Method: The participants for this study were 391 students from two middle schools, one located in Seoul and one in Kyung-gi do Province. A self-report survey was used for data collection, which was carried out from February 8 to 14, 2006. The collected data were analyzed using SPSS 11.0 Program. Results: The average score for father-adolescent communication was 61.70, and for mother-adolescent communication, 64.04. The average score for depression was 16.01, and for suicide ideation, 8.69. There was a statistically significant positive correlation between suicide ideation and depression. There was a statistically significant reverse correlation between suicide ideation and mother-adolescent communication, and between suicide ideation and father-adolescent communication. Mother-adolescent communication and depression account for 46.4% of variance in suicide ideation of adolescents. Conclusion: According to the results, programs designed to improve parent-adolescent communication and decrease depression should be developed and put into practice to decrease the rate of suicide ideation in adolescents in Korea.

A Comparison of Stressors and Coping Behaviors of the High School Students who have Suicidal Ideation vs those who do not (자살을 생각한 고등학생들과 그렇지 않은 학생들의 스트레스원과 대응행위 비교)

  • This study was conducted to identify and compare the stressors and coping behaviors of high school students who experienced suicidal ideation vs those who do not. Method: The data were collected from 1,441 high school students living in the middle region of Korea. Researchers collected the data using Adolescent Stress Inventory and Adolescent Coping questionnare from February to May of 1999. Data were analyzed using X2, T-test. Result: A significant proportion (31.9%) of subjects had thought about suicide during the past 6 months, 297 girls (35.3%) and 163 boys (27.2%). Major stressors of subjects who experienced suicidal ideation were academic problems, conflicts between family members, and changes in sleeping or eating patterns. Conflicts with family members contributed to have suicidal ideation. There was no significant difference in helpful coping behaviors between the two groups. The group who experienced suicidal ideation had lower scores and fewer coping behaviors. Boys perceived more stressors and had fewer helpful coping behaviors than girls. Conclusion: It is necessary to develope school-based suicide prevention programs. Our findings suggest that the program should focus on problems related to studying and conflicts within the family.

Association Between Internet Use and Suicidal Ideation Among South Korean Adolescents (청소년기 인터넷 사용 수준과 자살생각과의 연관성)

  • Purpose: Internet is one of the essential tools in South Korean adolescent life. Recently, researchers suggest that Internet use is positively associated with teens' mental health problems such as depression and suicide. Thus, this study explored a relationship between Internet use and suicide ideation among South Korean adolescents. Methods: Secondary data, the three-wave Korean Youth Panel Survey, were analyzed. First, in terms of the levels of Internet use and suicidal ideation, t-test was used for examining a gender difference, and Analysis of Variance were used for examining differences across three points in time(waves 2 - 4). Second, logistic analysis was performed to examine the significant association between Internet use and suicide ideation after controlling for the other factors' effects. In this analysis, Fixed Effect Regression Method(FERM) was incorporated as well. FERM was useful in adjusting for the effects of time-invariant factors even without measuring them. As a result, it was possible to obtain more accurate findings. Results: Adolescent depression was an important factor influencing the association between Internet use and suicide ideation. After controlling for the effect of depression, the relationship between Internet use and suicide ideation was no longer statistically significant. Conclusion: As a proxy to identify teens who are obsessed with suicide ideation, levels of Internet use do not seem useful. Instead, it is necessary to examine teens' access to websites related to suicide.

The Relationship Between Trajectories of Child Maltreatment and Adolescent Suicidal Ideation (아동의 종단적 학대 경험과 청소년기 자살생각 간 관계)

  • Objective: The purpose of this study was to identify the longitudinal relationship between child maltreatment and adolescent suicidal ideation. Several studies suggest that child maltreatment is associated with adolescent suicidal ideation. However, these studies have not focused on the duration of child maltreatment. Methods: Participants were 420 adolescents (52.6% male; 47.4% female) involved in the Korea Welfare Panel Study. Data were collected via youth self-report in 2006, 2009, and 2012. A self-report assessment of children and youths' experiences of physical, emotional, or neglect had been surveyed longitudinally. Questionnaires on adolescent suicidal ideation were answered in 2012. A growth mixture model and multiple regression were conducted using Mplus 5.21. Results: The study results showed that 93.5% of the children had experienced no maltreatment; 6.5% had experienced increasing maltreatment longitudinally. The trajectory of increasing maltreatment was positively associated with adolescent suicidal ideation. Conclusion: These results show that children suffering from chronic maltreatment are subject to a high risk of suicide and therefore require continuous supervision and management.

A Meta-analysis of The Factors Related to Bullying Damage Experience of Adolescent (청소년의 집단따돌림 피해경험과 관련된 요인에 대한 메타분석)

  • This study conducts meta-analysis of the effect size of related factors using research papers published within the past 20 years in relation to adolescent bullying experience in Korea. The verification results are as follows. First, in inducing factors, the overall effect size, personal psychology factor, and school life factor group showed a medium effect size. In the causative factors, the aggression, anxiety, stress, depression, suicidal thoughts, and school stress of school psychological factors showed a moderate effect size. Second, in terms of deterrent, the overall effect size, family environment factor, personal psychology factor, and school life factor group showed a medium effect size. In the sub-factors of determinants, the parenting attitude, parental support, and self-esteem and self-resilience of the family environmental factor group also showed a medium effect size. In conclusion, this study is meaningful in that it tried to quantitatively integrate the previous studies to examine objective results. It is meaningful that it provided basic data in program development and policy making to prevent and reduce the bullying of youth.

Association between High-caffeine Energy Drink Intake and Suicidal Ideation in Korean Adolescents (우리나라 청소년의 고카페인 에너지 음료 섭취와 자살생각과의 관련성)

  • Purpose: The purpose of this study is to examine the association between consuming high-caffeine energy drinks and suicidal ideation in Korean adolescents. Methods: This study used data drawn from the 2015 Korea Youth Risk Behavior Web_Based Survey. The data of 66,068 students in total (33,374 middle school students and 32,694 high school students) were included in the analysis. The $x^2$ test, univariate and multivariate logistic regressions were conducted with the use of SAS 9.3. Results: Adjusted for the variables associated with suicidal ideation, multivariate logistic regression revealed that consuming highly caffeinated energy drinks had a significant effect on suicidal ideation (middle school students, 1~2 times per week: OR=1.24, 3~4 times per week: OR=1.88, 5~6 times per week: OR=2.20, everyday: OR=2.66, high school students, 1~2 times per week: OR=1.26, 3~4 times per week: OR=1.84, 5~6 times per week: OR=2.42, everyday: OR=3.89). Conclusion: This study strongly suggests adolescents be provided with health education and information about consuming an adequate amount of high-caffeine energy drink and the drink's side effects. In addition, regulations and polices on high-caffeine drink intake should be implemented.

Effects of Smoking, Drinking and Drug use on the Adolescent's Suicidal Ideation by using the Data of the Korea Youth Risk Behavior Web-based Survey through from 2008 to 2014 (2008~2014년까지 청소년건강행태 온라인조사를 활용하여 흡연·음주·약물사용이 청소년 자살생각에 미치는 영향)

  • Purpose: This study aims to investigate the association between suicidal thoughts and the co-occurrence of alcohol, tobacco, and drug use among adolescents. Methods: This study used the data of the Korea Youth Risk Behavior Web-based Survey from 2008 through 2014. The subjects of this study were 517,866 students, in total (Male 266,500, Female 251,366). Frequency analysis was conducted on the data about the subjects' general characteristics and their experience of smoking, drinking, and drug use. The relationship between suicidal thoughts and the co-occurrence of alcohol, tobacco, and drug use was analyzed through ${\chi}^2-test$ and logistic regression. The analyses were done using SAS 9.4. Results: According to the study, adolescents who used alcohol, cigarettes, and drugs at the same time engaged in suicidal ideation 3.69 times more than their peers who didn't use such substances (95% CI: 3.40~4.00, p<.001). Also, adolescents who used only tobacco, only alcohol, and only drugs engaged in suicidal ideation 1.49 times (95% CI: 1.43~1.56, p<.001), 1.28 times (95% CI: 1.26~1.31, p<.001) and 2.34 times (95% CI: 2.04~2.69, p<.001) more than the students who didn't use any substances, respectively. Conclusion: Adolescents' suicidal ideation and the co-occurrence of alcohol, tobacco, and drug use had a statistically significant relationship.

Combined Influence of Smoking Frequency and Intensity on Suicidal Ideation and Attempts in Korean High School Students (고등학생 청소년의 자살생각과 시도에 대한 흡연빈도와 강도의 수준별 결합효과)

  • Purpose: The purpose of this study was to investigate the combined influence of the frequency and the intensity of smoking on suicidal ideation and attempts in Korean high school students. Methods: This cross sectional study used secondary data from the 2014 Adolescent Health Behavior Online Survey. A total of 35,094 high school students were included in the study. The study set up two categories for the frequency of smoking - intermittent and daily - and another two for the intensity of smoking - heavy and light. Complex samples logistic regression was used to analyze the combined influence of smoking frequency and intensity on suicidal ideation and attempts. Results: Regardless of frequency and intensity, smoking was more likely to increase suicidal ideation and attempts than non-smoking. Among smokers, intermittent heavy smokers (suicidal ideation: Adjusted Odds Ratio [AOR]: 1.406, 95% Confidence Interval [CI]: 1.015~2.161, p=.012; suicidal attempts: AOR: 2.977, 95% CI: 1.814~4.886, p<.001) or daily heavy smokers (suicidal ideation: AOR: 1.274, 95% CI: 1.017~1.596, p=.035; suicidal attempts: AOR: 1.717, 95% CI: 1.250~2.359, p=.001) had significantly higher odds of suicidal ideation and attempts than intermittent light smokers. Conclusion: Based on the results, smoking prevention and cessation are essential to preventing suicides in adolescents. Especially, smoking intervention programs are needed to reduce smoking intensity in smokers.

Effect of Sleep Duration on Suicidal Ideation in Korean Adolescents (우리나라 청소년의 수면시간이 자살생각에 미치는 영향)

  • Purpose: The purpose of this study is to examine the effect of sleep duration on suicidal ideation in Korean adolescents. Methods: This study used the data drawn from Korea Youth Risk Behavior Web-Based Survey in 2013. A total data of 63,301 adolescents were included in the analysis. Included variables were general characteristics, health status variables, hours of sleep, and suicidal ideation. For sample description weighted percentage and means were used. Rao-Scott ${\chi}^2$ test and simple and multiple logistic regression with dummy variables were conducted with use of SAS 9.2. Results: Among general characteristics and health status variables, gender, grade, school level, school achievement, family's economic status, living with parents, subjective health, happiness, stress, and feeling of despair were found to be significantly related to suicidal ideation. Rates of having suicidal ideation were 22.4% in the case of students with less than 5 hours of sleep and 12.9% in students with 8 or more hours of sleep. After controlling general characteristics and subjectively perceived variables, hours of sleep was still significantly related to suicidal ideation. That is, compared to adolescents with 8 hours or more of sleep, those with less than 5 hours of sleep showed significantly higher risk for having suicidal ideation (adjusted odds ratio: 1.20, p<.001). Conclusion: These findings suggest that short sleep has negative effects on psychological health in adolescents. Particularly, less than 5 hours of sleep has harmful effect on adolescents' increased risk for having suicidal ideation.
