Effects of Chiropractic Using Sacro Occipital Technique on Pain and Physical Function in Patients with Low Back Pain (천골후두골 테크닉을 이용한 카이로프랙틱이 만성 요통환자의 통증과 신체기능에 미치는 영향)

  • The aim of this study was to identify the effects of chiropractic treatment using Sacro Occipital Technique (SOT) on low back pain (LBP) and physical functions in patients with chronic LBP. Forty-five women with chronic LBP were randomly assigned to the Diversified Technique (DT), SOT or DT+SOT group and received chiropractic treatment two times a week for 8 weeks. LBP was measured using Visual Analogue Scale (VAS), Oswestry Disability Index (ODI), and Roland-Morris Disability Questionnaire (RMDQ) at baseline and after 4 and 8 weeks of treatment. Physical functions were evaluated using lumbar strength, flexibility, and body symmetries. After 4 and 8 weeks, VAS, ODI, and RMDQ were significantly decreased in all 3 groups (all p<.001). Lumbar strength, flexibility, and shoulder and pelvic balances were significantly improved in all 3 groups (all p<.05). In conclusion, our results suggest that SOT and DT both may be effective for reducing LBP and improving physical functions.

  • This is the following report of the previous summary report 'Dental occlusion and relationship to TMD and systemic symptoms(I).' Among 5 representative theories about dental occlusion, stomatognathic system, and related systemic symptoms, this article will introduce two of them. Which are (1) Craniosacral mechanism and (2)Osteopathy. Brief history, definition, basic concepts, and the limitation of each theory are reviewed.

  • 본 연구는 40~60세의 중년여성을 대상으로 뉴로피드백 실험군 17명, 두개천골요법 실험군 17명, 혼합요법 실험군 19명, 대조군에 18명 등 총 71명을, 2007년 10월 1일부터 12월 14일까지 11주 동안 뉴로피드백(이하 NF), 두개천골요법(이하 CST) 및 이 두 요법의 혼합요법(이하 MT)을 적용하여 중년기 여성의 피로와 스트레스 감소 및 뇌기능 향상과 MT군의 시너지효과를 검증하고자 시도한 비동등성 대조군 전후 설계인 유사 실험 연구이다. 그 결과 1)NF, CST 및 MT는 중년여성의 피로와 스트레스를 감소시키고 두뇌 활성화를 통한 뇌기능을 향상시키는데 효과가 있었다. 2)전두엽의 활성화는 NF는 좌뇌를 CST는 우뇌를 활성화시켰다. 3)MT군은 두 군에 비하여 약간 더 효과가 있었다. 4)차시별 효과는 실험군 모두에서 10차시에서 유의한 증가를 보였다.

Dual Plate Fixation for Displaced Transverse Fracture of the Lower Sacrum (전위된 하부 천골 횡골절에 대한 이중 금속판을 이용한 내고정술)

  • Isolated fractures of lower sacrum are commonly treated conservatively in most cases because of low energy damage and stable fracture. However, surgical treatment is required in displaced unstable fracture. But the surgical technique is not established and even case reports are not common. We reported a case of displaced transverse fracture of the lower sacrum that was treated with an open reduction and dual plate fixation.

Analysis for Usefulness of Arterial Embolization on Sacral and Pelvic Giant Cell Tumors (천골 및 골반골에 발생한 거대세포종에 대한 동맥 색전술 치료의 효용성 분석)

  • Purpose: The purpose of this study is to determine the usefulness of arterial embolization on sacral and pelvic giant cell tumor (GCT). Materials and Methods: We retrospectively reviewed the medical records of 9 patients who had undergone serial arterial embolization between December 1996 and May 2008. We analyzed the clinical outcomes and therapeutic responsiveness of arterial embolization on sacral and pelvic GCT. Results: Six of 9 cases showed progression of disease (PD) status, even if 5 cases showed PD status despite of additional treatments including surgery and radiation, implying that serial arterial embolization on sacral and pelvic GCT is not effective. Three of 9 cases showed stable disease (SD) or continuous disease free (CDF) status and we analyzed associated factors with these good responses for embolization by ${\chi}^2$ test. The number of feeding vessels under six (p=0.048) and the number of collateral arterial supply under three (p=0.048) in the first angiogram showed significant relationships with good response for embolization, while remaining tumor staining by contrast after the first embolization and repeated embolization times were not significant. Conclusion: Although serial arterial embolization is not an effective modality on sacral and pelvic giant cell tumors, it may be a pilot modality under narrow indication of tumors with poor vascularity at first angiogram.

Treatment Outcomes of Sacral Giant Cell Tumor (천골 거대 세포종의 치료 결과)

  • Purpose: We analyzed the treatment outcomes of patients with sacral giant cell tumor. Materials and Methods: We retrospectively reviewed 7 patients with giant cell tumor of the sacrum who were treated at out institution between 1990 and 2012. Results: There were 2 men and 5 women with mean age of 23.6 years. The average follow up was 52.3 months (range, 15-73 months). Six patients received surgical treatment. Intralesional curettage was performed for the 5 patients and marginal resection for another one patient. The remaining one patient was received radiation only. The patients who received radiation therapy and marginal excision had no residual or recurrent tumors. Of 5 patients with intra-lesional excision, one patient needs one more operation; two patients need two more operation for local control of the giant cell tumor. The remaining two patients failed to gain local control in spite of additional treatments. Conclusion: For the treatment of sacral giant cell tumor, intralesional resection can be one of the treatments option with minimal neurologic injury. Furthermore, radiation therapy can be recommended when complete excision or curettage is impractical.

  • 욕창의 발생부위에 따른 수술방법 중 천골부 욕창의 치료에는 천골절골이 필요한 깊은 경우에는 대둔근피판술이, 갚지는 앓으나 광범위한 욕창일 경우에는 하둔대퇴피판술이 가장 성적이 좋았다. 대전자부 욕창의 치료에는 대퇴근막장근근피판술이 가장 성적이 좋았으며, 슬관절부 욕창의 치료에는 비복근판술 후 피부이식방법이 좋은 결과를 초래하였으며, 종골부 창상치료에는 발꿈치의 외후면 재건시는 lateral calcaneal flap이 좋으며, 종골절골이 필요치 않은 heel pad 재건시는 족저부 근막피판술이, 종골절골이 필요한 heel pad 재건에는 족지단골근근판술이 가장 좋은 성적을 가져 왔기에 문헌고찰과 함께 보고하는 바이다.

  • Abdominal pregnancy is a very rare form of ectopic pregnancy following IVF-ET procedure. Because the symptoms of abdominal pregnancy is not typical, it is difficult to diagnose early and the mortality rate is higher than other ectopic pregnancies. Liver, spleen, omentum and diaphragm are main implantation sites of abdominal pregnancy. We reported an unusual first case of an etopic pregnancy on uterosacral ligament following in vitro fertilization and embryo transfer with brief review of literature.
