Efforts to Improve Antibiotic Prescribing Trends for Acute Upper Respiratory Infections in a South Korean University Hospital (급성 상기도 감염에서 항생제 처방개선에 관한 연구)

  • 목적: 급성 상기도 감염에 사용되는 항생제 처방유형을 조사하여 항생제 사용을 개선하는데 있다. 방법: 2011년 1월부터 6월까지 경상대학교 병원을 포함한 10개 국립대학병원을 대상으로 급성 상기도 감염에 사용한 항생제 처방율을 조사하고, 경상대학교병원에서 급성 상기도 감염에 사용한 2011년 1월부터 6월까지의 외래환자를 대상으로 항생제 처방 내역을 전자의무기록을 통하여 후향적으로 분석한 후 처방의를 대상으로 항생제 사용 적정성 검토를 위한 그룹미팅 및 교육, 급성 상기도 감염에 항생제 처방 시 경고 안내문을 보여주는 등 중재활동 후 2011년 12월에 처방유형을 조사 하였다. 결과: 경상대학교병원에서 2011년 1월부터 6월까지 급성 상기도 감염에 항생제 사용은 1739명의 상기도 감염 외래환자 중에서 874명 (42.3%)으로 나타났다. 진료과별 급성 상기도 감염에 대한 항생제 처방은 소아과, 이비인후과, 내과, 응급의학과, 호흡기내과, 흉부외과 등에서 처방하였으며 소아과에서 1044명의 상기도 감염환자 중 556건(53.3%)로 가장 빈번하게 사용하였으며 처방율은 이비인후과에서 58.9% (225/382)로 가장 높았다. 사용한 항생제로는 amoxicillin-clavulanic acid가 371례 (36.3%)로 가장 빈번하게 처방된 약제이며, azithromycin이 85례 (9.7%) 처방되었다. 급성 상기도 감염 중 급성 편도염에 항생제 처방율이 가장 높았으며 (70.8%, 80/113), 급성 인두염에 가장 빈번하게 사용되었다 (61.1%, 319/522). 균동정을 위한 혈액배양 의뢰 건수는 1739 상기도 감염 환자 중 15명 (항생제 미사용 4명, 사용 11명)이 의뢰되었으며 모두 음성이었다. 중재활동 후 2011년 12월 상기도 감염에 항생제 처방건수는 소아과에서 1건, 이비인후과에서 2건으로 나타났다. 결론: 처방의를 대상으로 적절한 항생제를 사용을 권장하는 지속적인 교육 및 항생제 처방시 경고 안내문을 띄우는 등의 중재활동과 지속적인 모니터링 및 피드백은 급성 상기도 감염에 있어서 항생제 처방유형에 변화를 보였다.

Smart Exercise Prescription of Elderly Users using Visual Path Map (비쥬얼패스맵을 이용한 고령자 대상의 스마트 운동처방)

  • Exercise programs for elderly users are operated by each department and facility, but it is not enough to visualize exercise prescription and effect followed by elderly users physical conditions. The purpose of this study is to suggest exercise prescription for elderly users with a visual path map. A visual path map is to visually present types of users classified according to physical strength conditions, the process of exercise prescription, and effects of exercise. Exercise prescription is divided into four stages: analysis of physical conditions, exercise prescription by the visual path map, smart exercise prescription, and exercise for elderly users. The first stage, analysis of physical conditions is to classify physical conditions by each type by mechanically learning elderly users' physical test values. The second stage, exercise prescription by the visual path map, is to present exercise prescription suitable for elderly users' physical conditions. The third stage, smart exercise prescription, is to offer exercise prescription of the day when exercise is carried out using elderly users' smart phones in consideration of their situations. The fourth stage, exercise for elderly users, is to provide information by their smart phones when they exercise. In conclusion, this study will be able to induce elderly users to do continuous exercise by motivating them.

Clinical Course according to Antipsychotics Prescription Pattern in Delirium (섬망 환자에서 항정신병약물 처방 유형에 따른 임상 경과의 차이)

  • Objectives : Although antipsychotics are commonly used to control symptoms of delirium, there is a lack of research on the prescription pattern and its clinical effects. The purpose of this study was to investigate the effect of antipsychotics prescription pattern on clinical course of delirious patients consulted to psychiatry. Methods : During the period from July 2016 to February 2017, 212 patients who were referred for delirium were reviewed for their medical records. The duration of delirium was monitored using CAM-ICU, and duration of admission, mortality, and delirium at discharge were reviewed. Clinical course was compared among three groups according to the antipsychotic drug administration pattern: Continuous use group, optimal use group and PRN use group. Results : The pattern of taking antipsychotic medication longer than duration of delirium did not associated with better clinical course compared with the pattern of adapting to the period of delirium and rather increased the risk of taking antipsychotic medication at discharge. When used for a shorter period than the delirium period, it was associated with poor clinical course. Conclusions : The results of this study suggest that a strategy to administer antipsychotics for a minimum period, according to periods of delirium, is appropriate. Also, efforts are needed to minimize the use of antipsychotic drugs after recovery from delirium.

  • This study focuses on the necessity of developing the material for teachers who are involved in diagnosing and prescribing students' mathematical errors. And it also intends to stimulate the related research of this area. For this, it tries to suggest the fundamental components-(1)types and frequencies of errors, (2) diagnostic test kit, (3)causes of errors, (4)ideas for prevention, (5)ideas for correction, (6)practice for settlement, and (7) performance test kit and frame of the teaching guide book for the teachers according to the general procedure of diagnosis and prescription. Finally it provides the concrete research areas for the future study.

An Analysis of Nursing Research on Child Rearing in Korea (자녀양육에 대한 국내 간호학분야 연구논문 분석)

  • Purpose: The purpose of this study was to analyze papers on child rearing in Korea and to suggest future research and nursing practice directions for child rearing. Methods: An on-line search via 5 web sites yielded 201 research papers. The analysis of key concepts was based on the attributes of parenting and type of knowledge held by the parents. Results: The research papers found in the search were 27.3% experimental and 72.7% non-experimental. Among the participants, 77.0% of parents raising the child were mothers and the children were mostly young children under the age of one year. Types of knowledge related to child rearing varied widely, but explanatory knowledge (55.2%) was the most common, followed by prescriptive knowledge (30.0%) and descriptive knowledge (14.8%). For attributes of parenting, reaction to parenting (49.9%) followed by parenting behavior/practice (43.1%) was most common. Conclusion: Findings of this study indicate that further research is necessary regarding fathers and surrogate caregivers and on prescriptive knowledge for health promotion of children at different developmental stages. Also the results suggest a need for research toward developing child care policies.

  • The purpose of this study is to design and implement the web-based counselling system in aspects of learning type and method by PBL in Constructivism. Learning problem is classified into learning difficulty, learning disability, emotional disability, memory/cognition, ADHD(attention deficit hyperactivity disorder), classwork, and school circumstance. Counselling for individual, offline visit, parents, electronic mail, and group with professionals who were involved in the design and implementation of learning counselling were used to supplement the clinic process for learning. The result of this study aims to administer the distance counselling system for learning without prominent computer knowledge. Each learner can solve diverse private learning problems which can be widely applied for situated learning. Also these online strategies by PBL indicate that providing learning types and methods are critical for overall understanding in online counselling system for learning.

  • For using the knowledge of experts diversely and efficiently, it is very important that the system could represent the complex knowledge in well structured manner and also adapt well to the frequent schema changes. In this paper, we propose an efficient method for knowledge base management by using XML based templates. In our proposed method, to solve the inefficiency of management of many XML documents, we represent the complex knowledge on XML-based templates, and manage the templates structurally by using a meta-template that defines relationship among the templates. In order to show the effectiveness of our method, we developed a knowledge base to formulate an exercise prescription for a subject and an application program to classify the subject's fitness. The experimental study shows that XML-based templates provide a flexible, extensible and structured way of representing expert knowledge. The inference using XML-based templates can be controlled systematically and efficiently by using meta-template.

기계학습을 활용한 중소기업 맞춤형 지식서비스 진단 및 처방 기술 연구

  • 많은 국가에서 중소기업의 기술혁신을 지원하기 위해서 다양한 직 간접적 지원 프로그램을 기획하고 운영한다. 이 연구는 R&D 투자 효율성을 제고하기 위한 간접 지원의 대표적인 사례 중에서 지식서비스, 특히 그중에서도 R&D 기획, 기술가치평가, 정보 지원 사업에 주목했다. 이 연구에서는 지식서비스의 일환으로 공공과 민간에서 다양하게 제공되고 있는 3가지 지원 사업을 대상으로 R&D 중소기업 중에서 수요 집단의 특징을 프로파일링 했다. 이런 프로파일링을 통해 기업의 해당 지식서비스 지원 적합성을 진단할 수 있는 계량적인 방법을 제안하고 나아가 이를 통한 처방 방법을 논의한다. 구체적인 연구 방법으로는 데이터 마이닝을 통한 분석 방법으로 수요 집단을 프로파일링 했으며, 전통적인 통계 방법인 판별분석 방법을 활용해서 구체적인 지식서비스 적합성 진단 모형을 제시했다. 본 연구에서는 특정 지식서비스 수요에 대한 중소기업의 특징을 프로파일링하기 위해서 설문정보를 활용했으며, 모형의 적정성을 검증하기 위해서 새로운 설문을 활용하는 등 총 6,600 기업에 대한 대규모 설문을 활용했다는데도 차별적 의의가 있다. 본 연구의 결과는 대규모 데이터를 바탕으로 한 중소기업에 대한 지식서비스 또는 R&D 진단과 처방 서비스를 제시해 중소기업과 정부 R&D 투자 효율성을 개선시키는데 기여할 것으로 기대된다.

  • 농촌진흥청에서 제공하고 있는 수치토양도는 수문 수자원 분야에서 SCS-CN법을 이용한 유효우량 및 유출곡선 산정에 가장 많이 활용되고 있다. 토양조사 국책사업 결과 전산화된 토양도 및 토양검정 데이터베이스에 기초한 토양정보 웹 시스템은 전국의 토양 전자지도와 토양 통계 자료를 주제별로 검색하거나 필지별 토양분석 성적에 따른 토양관리처방서를 조회하는데 사용되는 농업분야 이외의 사용자 그룹을 위해 필요한 정보를 제공하도록 활용 및 유통 요구도가 높아지고 있다. 수치토양도가 수문학적 토양유형 정보를 포함하도록 제공하는 것이 먼저 필요할 것으로 생각되고 다음으로는 수문 수자원 분야 활용 측면에서 수치토양도가 제공하는 속성, 축척, 제공형태, 좌표체계, 서비스 방식 등에 대하여 활용정책을 마련하여 이에 따라 자료가 유통될 수 있도록 하여야 할 것이다. 앞으로 활용이 가능하게 될 수치세부정밀토양도와 토양유형을 이용하여 충북 괴산군 소수면의 소유역에 대해 SCS 삼각법에 따른 단위도 작성, 유효우량 산출 및 유출곡선을 작성한 결과 농업과학기술원의 정 등(2006)이 분류한 토양유형을 이용한 결과 정 등(1995)에 따른 토양유형을 이용한 결과에 비해 CN값과 유효우량이 더 높게 나타났고 삼각단위도로부터 유도한 정점의 유출량과 시간별 유량 관측값에 더 가까운 것으로 나타났다.

  • The purpose of this study is to examine the effect of providing books to elementary school students that match their personality type on their reading attitude. For this purpose, this study investigated among the 226 elementary school students that are enrolled in fourth, fifth, and sixth grade classes at the elementary schools located in Gyeonggido, South Korea. The personality type of the students was determined by the Enneagram test. In this study, students were given with books that match their personality type and the effect of the provision was investigated. Main results of this study can be summarized as follows. First, the fact that books that match their own personality type made the children curious about the books, evoking their interest in reading. Second, the children exchanged their books with those who have similar personality type after they finish reading. As a result, the children could be exposed to more diverse books. Third, the students were curious about what kind of books were given to those with different personality type. The students responded that they want to read all of the different types of books too.
