Comparison of Trend Tests for Genetic Association with Sibship Data (형제 자료에 근거한 유전연관성 추세 검정법의 비교)

  • Extensively used case-control designs in medical studies can also be powerful and efficient for family association studies as long as an analysis method is developed for the evaluation of association between candidate genes and disease. Traditional Cochran-Armitage trend test is devised for independent subjects data, and to apply this trend test to the biologically related siblings one has to take into account the covariance among related family members in order to maintain the correct type I error rate. We propose a more powerful trend test by introducing weights that reflect the number of affected siblings in families for the evaluation of the association of genetic markers related to the disease. An application of our method to a sample family data, in addition to a small-scale simulation, is presented to compare the weighted and unweighted trend tests.

Affecting factors of the Drinking Behavior of Liver Cirrhosis Patients The Aspects of Convergence of Drinking Behavior and Disease-related of factors (간경변증 환자 음주행위 관련변인 -음주행위와 질병관련 변인의 융복합 측면-)

  • This study was conducted to determine the affecting factors among patients with drinking behavior of liver cirrhosis. Data were collected by questionnaire from 157 patients who were diagnosed with liver cirrhosis at a tertiary hospital located in D-city. Measurements included patients' demographic characteristics, clinical characteristics, disease-related of symptom experience, emotional-factors of anxiety-depression and social-factor of social support. Data were analyzed using t-test, and logistic regression analyses. The incidence rate of drinking behavior was 31.8%. Multivariate analysis revealed that smoking(yes), men, symptom experience, social support and anxiety were more likely to report high level of drinking behavior. Future research should consider managing drinking behavior as an essential component of comprehensive care for patients with liver cirrhosis.

Computer-Aided Diagnosis for Pulmonary Tuberculosis using Texture Features Analysis in Digital Chest Radiography (질감분석을 이용한 폐결핵의 자동진단)

  • There is no exact standard of detecting pulmonary tuberculosis(TB) in digital image of simple chest radiography. In this study, I experimented on the principal components analysis(PCA) algorithm in the past and suggested six other parameters as identification of TB lesions. The purpose of this study was to develop and test computer aided diagnosis(detection) method for the detection and measurement of pulmonary abnormalities on digital chest radiography. It showed comparatively low recognition diagnosis rate using PCA method, however, six kinds of texture features parameters algorithm showed similar or higher diagnosis rates of pulmonary disease than that of the clinical radiologists. Proposed algorithms using computer-aided of texture analysis can distinguish between areas of abnormality in the chest digital images, differentiate lesions having pulmonary disease. The method could be useful tool for classifying and measuring chest lesions, it would play a major role in radiologist's diagnosis of disease so as to help in pre-reading diagnosis and prevention of pulmonary tuberculosis.

Histopathology of unidentified viral disease of cultured carp Cyprinus carpio (양식 잉어, Cyprinus carpio에서 발생하는 미동정 바이러스성 질병의 병리조직학적 특징)

  • Unidentified viral disease with high mortalities has been recorded consistently since 1998 in cultured carp Cyprinus carpio in Korea. In the present study, we investigated the histopathological characteristics of the diseased carp to evaluate the relatedness to koi herpesvirus disease (KHVD). Histopathogical examination revealed severe necrotic changes and vacuous cells in the kidney, spleen, liver, pancreas, heart and intestine, but eosinophilic intranuclear inclusion, a typical characteristic of KHV infection was not detected. These results suggest that KHVD may be not a cause for the high mortality occurring among cultured carp in Korea and possibly other unknown viral disease could are related.

Health Status and Self-management Barriers in People with Diabetes -A Comparison by Medicaid Beneficiary Status- (성인당뇨병환자의 건강수준 및 질병관리장애요인 -의료급여환자와 건강보험환자의 비교-)

  • Academic interest has been drastically increased for the health disparities due to socioeconomic factors. For those who have a chronic illness such as diabetes, various psychosocial barriers related to illness management might affect and aggravate this disparity. This study focused on illness management barriers experienced by people with diabetes, and examined the differences by their Medicaid beneficiary status. The between group differences in illness management barriers, family support and health status were examined as well as the association between illness management barriers and health status. The sample of this study consists of 144 community dwelling adults who have diabetes, recruited from an outpatient diabetes clinic and a community welfare center. Medicaid beneficiaries reported poorer health status, and experienced more illness management barriers compared to their counterparts. Statistically significant differences were observed in barriers due to mental health, financial status, and lack of support. The between group difference in health status remained significant after controlling for the effect of demographic characteristics and illness related factors. Social work practitioners working with this population should address these illness management barriers to reduce socioeconomic health disparity.

  • SMRs (standardized mortality rates) for major diseases, accidents, cancer are considered in small areas of administrative units such as Eup/Myeon/Dong from years 2005 to 2008. Due to small sample issue in small areas, the precision of directly estimated crude SMR for each area can be low. In this study, we consider the HGLM (hierarchical generalized linear model) with MRF (Markov random field) to account for the spatial correlations among the small areas. The effects of covariates for cause of mortality by Dongs in Seoul and disease maps based on the estimated SMR are presented. The results suggest how we analyze and interpret the difference in mortalities by small areas such as Dongs by revealing the spatial patterns.

Forensic analysis of toxic substances in fatalities with suspected companion animal cruelty (반려동물 학대 의심 폐사축에 대한 중독물질검사 연구)

  • The increasing prevalence of toxic substance-exposure in pets in South Korea endangers the health and safety of numerous companion animals, and has become a cause for concern. Notably, the annual incidence of forensic analysis in pets has increased by more than 150% in South Korea, mainly in populous regions such as Seoul, Incheon, and Gyeonggi. In response to this growing issue, veterinary forensic examinations were conducted on 549 dogs and cats from 2019 to 2022. This study revealed the presence of various toxic substances, including pesticides, insecticides, and drugs such as analgesics, anesthetics, antidepressants, and muscle relaxants, in pets. Among the 38 different toxins identified in pets, coumatetralyl, methomyl, terbufos, and buprofezin were the most frequently detected. In this study, toxic substances for pets were identified based on the "toxic agent list for humans," developed by the National Forensic Services, because no list of toxic agents for animals currently exists and data regarding potentially toxic substances for dogs and cats is limited. This is one of the limitations of this study, and necessitates the establishment of a toxic agent list for animals. Continued monitoring and research is also recommended to reveal the incidence, causes, and solutions of toxicity in animals.

Factors Affecting Hospitalized Children's Falls - Using Data in the National Hospital Discharge In-depth Injury Survey (입원 아동의 낙상영향요인 -퇴원손상심층조사 자료를 이용하여-)

  • This study investigated the characteristics and factors affecting inpatient infants, children, and adolescents who experienced falling, using NHDIS data from 2008 through 2017, The study analyzed data of 116 patients who were under 18 and who experienced injuries (KSCD, S00-S99) by falling (KSCD, W00-W19). Frequency analysis, cross-tabulations, and multiple regression analysis were conducted, using SPSS 23. There were more boys than girls, and most of the falls occurred at the ages of over one to under six years old. Most of the children had respiratory diseases, and most had open wounds or bruises due to falling. Also, most of the falls were related to the bed. In the factor analysis, age (β=.318), the main diagnosis (β=.231), and injury (β=.169) except gender affected falling. This suggests that it is necessary to conduct fall prevention education for children, considering the developmental stage characteristics and age group. It is necessary to screen the risk group such as children with a disease with relatively less restriction of activities or with a hyperactive disorder, and to develop a related manual. Hopefully, the results will be used as the basic data for fall prevention education and creating a fall prevention manual according to the characteristics of children's developmental stage for patients who need hospitalization, their caregivers, and the relevant medical team.

Factors Influencing Treatment Result in Inpatients with Tuberculosis (결핵입원환자의 치료결과에 영향을 미치는 요인)

  • The purpose of this study is to identify factors influencing treatment result in patients with Tuberculosis by patient characteristic, admission and disease characteristic, and hospital characteristic from 2006 to 2012. Survey data was using Korean national hospital discharge in-depth survey data produced by KCDC(Korea Center for Disease Control and Prevention). Study subjects were 8,305 inpatients with TB(A15.0~A19.9) and analyzed frequency, chi-square test, and logistic regression by using SPSS 20(Statistical Package for the Science). The results of this study show that influencing factors of treatment result were ages (20-39, 40-64, and over 65 years), type of insurance(medical aid), disease code (A16, A17, A18, A19), LOS (31-90, and 91-180 days), beds of hospital (300-499, 500-999, over 1,000 beds) and hospital district (non-metropolitan). These findings implied that it is necessary to support successful prevention and management for high risk TB groups and to build middle and long-term policies as well as short -term policy.
