Development and Implementation of a Critical Pathway for Prevention of Adverse Reactions to Contrast Media for Computed Tomography (CT 조영제의 부작용 예방을 위한 표준진료지침서의 개발과 적용)

  • The purpose of this study is to develop a critical pathway (CP) for the prevention of adverse reactions to contrast media for computed tomography. The CP was developed and implemented by a multidisciplinary group in Seoul National University Hospital. The CP was applied to CT patients. Patients who underwent CT scanning were included in the CP group from March in 2004. The satisfaction of the patients with CP was compared with non-CP groups. We also investigated the degree of satisfaction among the radiological technologists and nurses. The degree of patient satisfaction with the care process increased patient information(24%), prevention of adverse reactions to contrast media(19%), pre-cognitive effect of adverse reactions to contrast media(39%) and information degree of adverse reactions to contrast media(19%). This CP program can be used as one of the patient care tools for reducing the adverse reactions to contrast media and increasing the efficiency of care process in CT examination settings.

  • 최근 의료 서비스의 질적 향상을 위해 지식 기반의 의사결정지원 시스템 (Decision Support System)의 도입이 지속적으로 이루어지고 있으며, 이의 대표적 예로 임상실행지침(CPG : Clinical Practice Guideline) 중심의 진료 시스템이 있다. 임상실행지침은 환자가 병원에서 거치는 프로세스를 표현한 것으로, 질환에 대한 환자의 표준화된 진료 프로세스 지식이다. 본 연구에서는 임상실행지침, 의료 지식, 환자의 실시간 데이터를 연결시켜 환자가 병원에서 받아야할 진료 과정을 동적으로 생성하는 의사결정지원 시스템을 제시한다. 본 시스템은 임상실행지침과 의료지식을 바탕으로 추상화된 진료 프로세스 템플릿을 생성하고, 이 템플릿의 인스턴스에 해당하는 환자의 실시간 데이터를 반영하여 이후의 진료 프로세스를 동적으로 생성한다.

Improvement of customer satisfaction with "Explanation Nurse Unit" (설명간호사실 운영 활성화를 통한 외래 환자 만족도 증진)

  • 문제: 본원의 외래 상황은 진료의가 짧은 시간 내에 많은 환자를 진료해야 하고, 진료 환자 모두에게 충분히 만족하도록 설명해주는 것은 매우 어려운 실정이었다. 2007년의 의료기관 본 평가에 앞서 병원 자체 평가 결과 외래환자의 설명 만족도는 57점으로 저조하였다. 목적: "이해하기 쉽게 설명하는 서울대학교 병원" 이라는 슬로건 하에 설명간호사 제도를 운영하기 시작하여, 설명간호사실 운영 활성화를 통한 외래 환자 만족도를 향상시키고자 한다. 의료기관: 서울시 종로구에 소재한 대학병원 질 향상 활동: 설명간호사실 방문율 증가 및 one-stop service care을 실현할 수 있는 방안을 모색하였고, 검사 시행 이유, 귀가 후 주의사항 검사 결과, 검사일정 외래 진료 안내 브로셔 및 표준화된 교육 자료를 개발하였다. 또한 설명간호사의 역할 및 업무의 확립을 위한 업무기술서를 개발 및 설명간호사실 환경을 개선하였다. 개선효과: 설명간호사실의 방문율이 내과는 2배, 신경과는 3배 정도 증가하였다. 표준화된 교육 자료 개발로 설명 요구도를 충족시켰고, '환자의 알 권리'와 '설명을 들을 권리'를 보장하였다. 업무기술서 개발로 설명간호사의 업무 및 역할을 확립하였으며 설명간호사실의 환경 개선으로 개인 신상 비밀을 보호받을 권리를 보장하였다. 외래 설명만족도가 2007년 57점에서 2008년 74점으로 향상되었다. 설명간호사실 자체 설문조사에서도 97%가 만족한다고 답하였고, 100% 모두 재방문 의사를 보였다. one-stop service care를 제공함으로써 외래가 복잡하다는 부정적인 이미지를 긍정적인 이미지로 변화시켰다.

The Effects of Medical Students' Self-Directed Learning Ability, Self-regulated Learning, and Communication Ability on Self-Efficacy in Performing Medical Treatment (의과대학생의 자기주도학습능력, 자기조절학습, 의사소통능력이 진료수행 자기효능감에 미치는 영향)

  • This study was a descriptive research study targeting medical students to determine the impact of self-directed learning ability, self-regulated learning, and communication ability on self-efficacy in performing medical treatment. This study randomly selected medical students from Region J, located in Province G, as the approximate population, and a total of 125 copies were finally analyzed. Descriptive statistics were analyzed using t-test, ANOVA, correlation, and multiple regression analysis using IBM SPSS/25. Self-efficacy in performing medical treatment was related to self-directed learning ability (r=.61, p<.001), self-regulated learning (r=.50, p<.001), and communication ability (r=.33, p<.001). There was a positive correlation with all of them. As a result of analyzing the variables that affect the subject's self-efficacy in performing medical treatment using hierarchical multiple regression, self-directed learning ability was found to be the factor that best predicts self-efficacy in performing medical treatment, followed by self-regulated learning and communication ability. The total explanatory power was 46.6%. Acquiring specialized knowledge and becoming a doctor after graduation through clinical practice and acquiring the basic clinical practice skills necessary to successfully perform one's duties are important tasks that medical students must accomplish. Therefore, in order to improve medical students' self-efficacy in performing medical treatment, the importance of improving health care, major satisfaction, and life satisfaction must be recognized and managed. In addition, efforts to develop programs and improve systematic systems that can improve self-directed learning, self-regulated learning, and communication skills should also be supported.

Predictors of Community Health Practitioners' Practice Regarding Urinary Incontinence (보건진료원들의 요실금 관련 간호행위에 영향을 미치는 요인)

  • Objectives: The purposes of this study were to investigate CHPs' knowledge, attitude, and practice regarding UI, and to identify predictors of their UI related practice. Methods: A descriptive-correlational study using self-administered questionnaire was conducted. A total of 330 members of the local Korean Association of CHPs were included in this study. A mailed survey was conducted to collect data. Findings: Of CHPs surveyed, 170(51.5%) returned completed questionnaire. Educational preparation of CHPs regarding UI was limited. The mean correct score of knowledge scale was 68.96%. Overall, CHPs exhibited positive attitudes toward UI. CHPs in this study were not actively participate UI related practice. Contrary to expectation, knowledge was not significantly related to CHPs' practice regarding UI. In the final analysis, CHPs' practice regarding UI was predicted by attitude toward the care of UI clients and educational needs for UI and these two variables explained 9% of variance of UI related practice. Conclusion: The results of this study suggest the importance of attitude and educational needs in shaping UI related practice among CHPs. To facilitate UI related practice among CHPs, efforts for attitudinal change related to UI among CHPs are needed. Moreover, continuing educational program for UI management should be developed, applied and evaluated.
