Factors Affecting the Satisfaction Level of Orthodontic Patients with Dental Service (교정환자의 치과의료서비스 만족도에 영향을 미치는 요인)

  • The purpose of this study was to examine the satisfaction level of orthodontic patients with medical service in a bid to provide information on the diverse needs of orthodontic patients and the improvement of the competitiveness of dental institutions. The subjects in this study were 226 orthodontic patients who visited Y and B dentist's offices respectively located in Busan and Daegu. A survey was conducted from December 3 to 20, 2008, and SPSSWIN 14.0 program was utilized to analyze the collected data. The findings of the study were as follows: 1. As to the reason why they chose the dentist's offices, the greatest group made that choice through the recommendation of people around them(50.7%). Approximately half them(47.8%) tended to talk about grievances when they had any, and the greatest group(58.3%) expected to have a regular set of teeth when they received orthodontic treatment (58.3%). And the largest group(80.9%) expected that treatment to improve their oral health a lot. The greatest group(70.6%) wanted to receive whitening treatment after completing orthodontic treatment. 2. As to factors significantly affecting their satisfaction level with treatment service, satisfaction level with the employees(dental hygienists) had the most significant impact, followed by satisfaction with amenities, medical fee, dentists and friendliness. 3. Their satisfaction level with medical fee exerted the most influence on intention of Recommendation offices they visited to others, followed by satisfaction with dentists and employees(dental hygienists).

Effects of National Dental Screening on Dental Caries Experience, Treatment, and Cost in Children (국가 구강검진이 어린이의 치아우식 경험, 치료 및 비용에 미치는 영향)

  • This study aimed to evaluate the effects of national dental screening for children in Korea in the aspects of prevention of dental caries, the number of treatments related to dental caries, and cost of dental care. The study used the national cohort data on children health screening provided by National Health Insurance Service, and analyzed 32,937 1st dental screening, 22,608 2nd dental screening, 13,708 3rd dental screening, and 241,043 cases from the dental care of 27,944 children born in year 2008 and 2009. The decayed-filled teeth index and decayed-filled person rate, calculated from the results of the 2nd and 3rd dental screening, decreased as the number of preceding dental screening increased. Glass ionomer and amalgam restoration, pulp treatment of primary teeth and extraction of primary canine and molar were shown to decrease as the number of examined dental screening increased, and the total cost of dental care covered by national health insurance also decreased as the number of dental screening increased. In conclusion, national dental screening for children is an efficient and economical oral health care method that prevents dental caries and lowers the possibility of dental caries related treatment, thereby reducing cost of dental care.

  • During the main ruling in 2018, it is difficult to find a new judiciary, which is understood to be due to a certain degree of jurisprudence established and focusing mainly on contentious disputes within the framework of damages. The cases in which the court's judgment is reversed helped to understand the reason and the judiciary, and it was confirmed that the dispute in the medical lawsuit became more and more intense. Decisions on responsibility restrictions and medical records were also noticeable, with a significant increase in the number of verdicts relating to the doubt about medical records. This is considered to be part of the increasing number of cases in which the parties raise questions about medical records, and several cases were categorized and introduced at this opportunity. We also introduce the case of forced discharge of long-term hospitalized patients and medical fee bill, because it was judicial interest after the Supreme Court ruling that the cost of treatment for the after-effects of medical malpractice can not be claimed to the patient.

Status of Infectious Disease Inpatients at Long-Term Care Hospitals in Korea (국내 요양병원의 감염병 입원환자 실태 분석)

  • This study investigated the status of infectious disease inpatients at long-term care hospitals in Korea. A descriptive study was conducted on patients with 14 infectious diseases at 798 long-term care hospitals during 2016-2017. The number of infected patients, total admission days, and total medical expenses were higher in 2017 than in 2016. The most common infectious diseases were enterocolitis due to Clostridium difficile, influenza, and scabies. The number of hospitals with patients who had enterocolitis due to C. difficile and resistance to carbapenem was higher in 2017 than in 2016. Hospitals with 150-299 beds had higher numbers of infectious disease patients than those with under 150 or over 300 beds. Therefore, intensive efforts are needed to control the most common diseases at long-term care hospitals, such as enterocolitis due to C. difficile, influenza, and scabies. It is recommended to apply relevant guidelines related to infection control management as well as implement educational programs. It will also be necessary to develop applicable infection monitoring standards and support the facilities and health workforce of long-term care hospitals under 300 beds through an effective infection surveillance system.

Measurement of Ambulatory Patients' Satisfaction and Its Influencing Factors in a Tertiary Hospital (병원 외래 방문 환자의 만족도 평가 및 관련 요인에 대한 연구 - 한 3차 병원 방문 환자를 중심으로 -)

  • Patients' evaluation of hospital care is one of the most important aspects of quality assessment. Survey allows patients to judge sujectively the events that occur during their hospital visit if performed properly. This study describes the result of a research effort to develop outpatient questionnaire that has sufficient validity and reliability to be used to measure patients' perception of satisfaction in Korea and to investigate influencing factors on patients' satisfaction. Self-administered questionnaire was developed for outpatient and the survey was conducted covering 827 outpatients in a tertiary hospital. It was confirmed by factor analysis that patients evaluate several components of ambulatory care distinctly ; hospital environment, administration and ancillary services, and medical care. We found strong evidence of construct validity and internal consistency for the above three dimensions of hospital process. On the contrary, reliability of overall outcome measures was low. It suggests that three items concerning overall outcome measures have some different meanings in patients' perception. Using logistic regression analysis it was found that previous health status, cost evaluation, and improvement in health status have significant influences on the level of patients' overall satisfaction and that patient's sex, experience of previous visit, expectation for improvement, cost evaluation, and improvement in health status are strongly related with intention to recommend hospital. In spite of some limitations the results of this study can be used helpfully as baseline informations for developing self-administered questionnaire and for exploring the influencing factors on patients' satisfaction. Further comprehensive research efforts should be made on the measurement of ambulatory patients' satisfaction and its related factors in current Korean situation.

Medical care utilization status and quality of life in diabetes mellitus patients (당뇨병환자의 의료이용 현황과 삶의 질)

  • This study aims to identify utilization status of medical care and factors to affect quality of life in diabetes mellitus patients. The research selected 6,146 adults aged over 50, who appeared with EQ-5D, from the Korean health panel. We analyzed states of medical care utilization using descriptive statistics. Multiple regression analysis was used to examine the main factors associated with quality of life in diabetes patients. The result shows that quality of life(EQ-5D) in diabetes is significantly associated with gender, age, insurance type, education level, household income, diabetes ambulatory medical cost and the number of ambulatory medical utilization for other diseases. Females, higher age, lower levels of education, medicaid, the lower household income, the higher diabetes ambulatory medical cost, and the higher the number of ambulatory medical utilization for other diseases were the significant factors of lower quality of life. Therefore, it is necessary to develop effective social programs and individualized approach to improve the quality of life in diabetes patients. In the future, these findings can be used as important data for health care policy and assessment.

Triage Accuracy of Pediatric Patients using the Korean Triage and Acuity Scale in Emergency Departments (한국형응급환자분류도구를 적용한 응급실에서 소아 환자의 중증도 분류 정확성)

  • This retrospective study investigates the accuracy of triage procedures for pediatric patients in emergency departments (EDs) using the Korean Triage and Acuity Scale (KTAS). The study includes 250 randomly selected initial nursing records and clinical outcomes of pediatric patients who visited one regional ED or a local ED from October 2016 to September 2017. The collected data were analyzed by a qualified expert to determine the true triage score. The accuracy of triage was defined as the agreement between the triage score of the emergency nurses (ENs) and the true triage score as determined by the expert. Based on expert comments, the cause of the triage error was analyzed and the KTAS score was compared with the discharge, length of stay (LOS), and medical cost. The results showed that the degree of agreement in the triage score between the experts and the ENs was excellent (weighted kappa=0.77). Among the causes of triage discordance, the most frequent was the incorrect application of vital signs to the KTAS algorithm criteria (n=13). Patients with high severity KTAS levels 1 and 2 were discharged less often (${\chi}=43.25$, p<0.001). There were significant differences in the length of stay (F=12.39, p<0.001) and cost (F=11.78, p<0.001) between KTAS scores when adjusting for age. The results of this study indicate that KTAS is highly accurate in EDs. Hence, the newly developed triage tool is becoming well established in Korea.

A Study on the Healthcare Utilization Pattern of Foreign Nationals with National Health Insurance in South Korea (외국인 건강보험가입자의 의료서비스 이용 현황 분석)

  • The purpose of this study is to analyze the status of medical service use of foreigners living in Korea by their nationality and types of national health insurance. As of 2018, 1,058,886 people were extracted from the qualification DB, excluding people whose insurance premium is missing. The data analysis showed that nearly 78% of foreigners with national health insurance used medical services and the countries with the largest number of users were China, Vietnam, and the United States. The total cost of treatment per capita was highest in the United States regardless of hospitalization and outpatient. The number of medical treatments per person, and the medical expenses of outpatients & inpatients services were highest among the regionally-insured and the length of stay per person was highest among the workers' dependents. Lastly, it was found that Chinese and regionally-insured received much more benefits than other groups compared to the premium they pay. After July 2019, foreign nationals residing in Korea for six months or longer are obligated to enroll in the national health insurance program. Since the latest data was in 2018, the result did not properly reflect the current situation, but it is meaningful that it made basic data for future comparative analysis.

The past, present and future of silkworm as a natural health food (천연 건강식품인 누에의 과거, 현재 그리고 미래)

  • Humans have been breeding the mulberry silkworm for the long period of time to obtain silk fabric and nutrient-rich pupae. Currently, silkworm larvae, pupae, and silk-Fibroin hydrolysates are registered as food raw materials, while silkworm feces and Bombyx batryticatus are registered as Korean traditional medicines. Among sericulture products, individually recognized health functional food ingredients include silk-protein acid-hydrolysates for immunity enhancement, Fibroin-hydrolysates for memory improvement, and freeze-dried 5th instar and 3rd-day-silkworm powder for lowering-blood sugar. Recently, HongJam produced by steaming and freeze-drying mature silkworms were reported to have various health-promoting effects such as preventing the onset of Alzheimer's disease and Parkinson's disease, enhancing gastro-intestinal functions, improving skin-whitening and hair growth, and extending healthspan. By consuming silkworm products with various health-promoting effects, it is possible to increase the healthspan of human beings, thereby reducing personal and national medical expenses, resulting in increasing the individual's happiness.

The Impacts of Organizational Commitment on Medical Service Fee Reduction Rate by Lifestyle with Environmental Factors as Medium (환경요인을 매개로 라이프 스타일에 따른 조직몰입이 진료비 삭감률에 미치는 영향)

  • This study identified the impacts of organizational commitment on medical service reduction rates by lifestyles of medical institution workers with a medium of environmental factors in a medical institution, and it conducted a survey targeting 300 medical institution workers in Jeolla-do, South Korea. The results are as follows. First, there was a significant difference in an organizational commitment by sociodemographic characteristics based on age, academic career, gender, marital status, occupational description, monthly earnings, the years of employment, and the type of hospital. Second, there was a significant difference in environmental factors by sociodemographic characteristics based on marital status, the years of employment, and the type of hospital. Third, there was a significant difference in the inpatient reduction rate based on the occupational description, years of employment, and the type of hospital. In the outpatient reduction rate, a significant difference was shown based on age, marital status, occupational description, the years of employment, and the type of hospital. Fourth, when analyzing the relationship between organizational commitment, environmental factors, and the reduction rate, there was a significant positive correlation between inpatient reduction rate and outpatient reduction rate. Fifth, when analyzing the impacts on the inpatient reduction rate by deploying organizational commitment and environmental factors at the same time, it was shown that the environmental factors mediated partially in organizational commitment and inpatient reduction rate. In the case of outpatient reduction rate, it was shown that the environmental factors mediated completely in organizational commitment and outpatient reduction rate. The present study is aimed to contribute to providing the baseline data for an efficient operational plan and effective workforce management of medical institutions.
