Analysis of the Emergency Medical Service Area Using GIS: the Case of Seoul (GIS를 이용한 응급의료 진료권 분석: 서울시를 사례로 하여)

  • The purpose of this study is to analyze the service area of emergency medical centers using GIS-based spatial analysis methods in Seoul, focusing on the behaviour of patients on the choosing of emergency centers. For this analysis, six emergency centers were selected to collect data for the information on the addresses of patients from September to November hi 2003. Analysis on the service area, which was carried out by measuring the distribution of patients in terms of distance from emergency medical centers, clearly reveals that the majority of patients was located within or adjacent districts at the emergency medical center. However, the size of the primary service area f3r six emergency medical centers was much different, implying that the decision to visit specific emergency medical center by patients was closely related to the size, perception, and preference of the emergency medical center. Based on the results of the spatial characteristics of emergency medical service area, this research tries to construct the surface map of the emergency medical service level supplied by 32 regional emergency medical centers located in Seoul. Considering the levels of infrastructure for emergency medical centers, the coverage for the degree of supply of emergency medical service by each emergency medical center was constructed in terms of a distance decaying in the distribution of patients from emergency medical center imposing different weights on distance bands. Spatial overlay utilizing map algebra function was performed in order to calculate total supply level of emergency service. The results clearly show that spatial inequality exists in the supply levels of the emergency medical service among local areas of Seoul.

  • 정보기술의 발달과 인터넷의 확산 등의 사회적인 변화에 따라 고객을 효과적으로 만족시킬 수 있도록 기업은 고객과의 장기적인 관계를 구축하는 고객관계관리(CRM: Customer Relationship Management)을 사용하고 있다. 최근에는 블로그나 SNS등에 기업이 상품이나 서비스를 팔고자 하는 소비자들이 가득 모여 있기 때문에 실시간으로 소비자의 니즈를 파악할 수 있는 방법으로 트위터, 블로그, 카페 등 SNS 상의 빅 데이터를 분석하는 시스템을 이용한다. 본 논문에서는 고객의 보다 효율적인 피드백 수집분석을 위해 기존의 기업/기관에서 운영 및 관리하는 내부 CRM 데이터와 SNS 상의 외부 데이터를 연동하여 분석할 수 있는 이기종 데이터의 통합 분석엔진 시스템을 제안한다. 이를 의료서비스에 적용하여 내부 데이터인 매출, 방문자 수, 진료과 정보, 환자 정보, 고객 불만 유형 등을 분석하고 소셜데이터를 통해 해당 의료기관에 대한 소비자 경험 (진료, 시설 등) 정보를 수집한다.

  • Introduction : 최근 의료기관 내 진료비 정산으로 인한 전산화사업이 적극 추진되면서 각 의료기관은 생성된 자료의 목적과는 무관하게 대량의 자료를 지니고 있으면서도 병원경영과 관련하게 자료를 활용하고 있지 못하다.Objectives : 병원 입원환자의 데이터를 분석하여 입원환자의 이탈요인을 파악하여 이탈을 방지하고 지속적인 의료서비스를 제공할 수 있도록 하고자 한다. Method : 본 연구는 일개병원을 퇴원환자를 대상으로 2006년 1월 1일~2007년 12월 31일 동안의 퇴원환자 데이터를 기반으로 연구되었다. 퇴원환자 38,359명의 병원데이터 베이스를 기반으로 동일 연구기간 외래 누적데이터 375,659건의 외래환자 데이터와 비교하여 퇴원환자 중 재입원 예약 및 외래진료 예약 부도여부를 비교하는 후향적 연구를 수행하였다. 2008년 1월 1일 기준 재입원 예약 및 외래진료 예약부도 발생여부를 기준으로 계속진료환자 그릅과 이탈환자그룹을 생성하였다. 중복을 제거한 총 대상은 3,503명이였다. Discussion :본 연구에서 얻어진 결과를 기초로 퇴원환자의 이탈률이 높은 환자그룹을 집중 관리하여 이탈을 방지하고 관심을 갖고 향후 잠재적 이탈 고객을 다시 유입할 수 있는 많은 방안들이 마련되어야 할 것이다.

Analysis of Selection Criteria of Consumers for Dental Clinic (의료소비자의 특성별 치과의료기관 선택기준에 관한 분석)

  • The subjects of this study were the patients of H Dental Hospital in Busan, who visited for the purpose of treatment in patients over 18 years of age and 151 patients, analyzed the data of the results of the selection criteria of the dental health care consumer. The gender of the subjects by cross-analysis, importance order of selection criteria for dental hospital were cleanness and hygiene(78.1%), equipment and facilities(60.9%), career and word-of-mouth(57.6%), staff kindness(57.6%), ease of use(32.5%) and scale of dental hospital(22.5%), it was appeared similar in comparison of cross-analysis and average-analysis. Major consideration of the study subjects to select criteria for dental hospital was pain control(31.8%), infection control(30.5%), cost(29.8%), time(7.9%) respectively. Gender as a primary consideration to the part of a man in pain control(39.2%), women cost(39.1%) was most common (p<0.001). In dental health care consumer to select criteria, clean hygiene and infection control at the heavily appeared to affect the direction of change in the consciousness of consumers seeking and it is necessary to improve hospital services for the dental care of their qualitative changes.

Evolution of Healthcare Service Disparities: A Case Study of Primary Care Services in Korea, 1995-2021 (보건의료 서비스의 공간적 불균등 분포 변이에 대한 연구: 1995년부터 2021년까지 초기진료기관을 대상으로)

  • While South Korea's universal healthcare system has garnered attention in public health, the issue of inequality in healthcare service provision among different age groups has incessantly become a significant concern. The focus of this concern is primarily on essential healthcare services, encompassing fundamental aspects of healthcare such as internal medicine, family medicine, and pediatric and adolescent care. This inequality is not limited to differences among age groups (both junior and senior demographics) but also extends to potential disparities in healthcare services based on geographic location, particularly in urban and rural contexts. This paper aims to investigate disparities in primary healthcare service resources in South Korea's evolving economic landscape between 1995 and 2021. We utilize a set of inequality indices with a spatial perspective through geographic cluster analysis. The findings reveal that concerns about inequality have been amplified during various economic events, including the IMF crisis in 1999, the global financial crisis in 2008, and the COVID-19 pandemic in 2020. These years are identified as significant phases that have contributed to manifesting spatial disparities in primary healthcare provisions, with a particular emphasis on the senior-aged population rather than junior or all population groups. Our findings underscore the pressing need to address the unequal distribution of essential healthcare resources as part of preparedness for potential economic impacts, requiring a comprehensive consideration of the interconnected nature of demographic and spatial dimensions in healthcare services.

A Comparative Study on the Satisfaction of Korean and Canadian Primary Care Based on the Survey of Overseas Korean in Canada (캐나다 재외국민 설문조사에 기초한 한국과 캐나다 1차 의료기관 만족도 비교 연구)

  • This study compares and analyzes satisfaction with Korean and Canadian primary medical care based on a survey of Koreans living in Canada. Based on data collected from Koreans who live in Canada, a paired sample t-test, analysis of variance (ANOVA), and GLM were used. The main conclusions of this study are as follows. First, overall satisfaction with Korean primary care was high. Second, the results of the analysis were stable, regardless of gender, age, education, and residential area. Third, overall satisfaction with Korean medical care was higher than for medical care in Canada. Fourth, satisfaction was high in terms of treatment skill and technology, and promptness in appointments, but was low in terms of medical expense. Fifth, there was a high level of confidence in physician skills and practices, but there was no significant difference in terms of sufficient counseling and explanations. Finally, the survey found that the tendency to increase medical treatment volume in order to increase a physician's own income was large. As a result of detailed analysis, it was concluded that Korea's primary care is more competitive than in Canada, but the primary medical doctors' patient interview and explanation obligations, and incentives to control income and medical expenses, need to be improved.

Analysis of the Continuity of Outpatient among Adult Patients with hypertension and its Influential Factors in Korea (우리나라 성인 고혈압환자의 외래진료 지속성과 이에 영향을 미치는 요인 분석)

  • The administration data of the national health insurance and health insurance bills were utilized in this study. The data of 485,953 patients who were at the age of 30 and up and used the out-patient departments of every medical institution located in some regions involving two southern and northern provinces once or more during a 184-day period from July to December, 2008. As a result of analyzing their Continuity of Ambulatory Care and factors affecting it, the following findings were given: The continuity of ambulatory care among the adult patients with hypertension in our country turned out to be on a high level(MMCI $0.96{\pm}0.13$, MFPC $0.96{\pm}0.12$). Given examining the outpatient medical-cure continuity level according to index, the averagely medical-cure continuity level was calculated to be high level with MMCI, $0.96{\pm}0.13$, and MFPC $0.96{\pm}0.12$. Thus, the tendency of visiting only one medical provider was high. The findings of the study illustrated that the average continuity of ambulatory care among the adult patients with hypertension in our country was on a high level, and it seemed that special care should be provided to patients with a low-level continuity of ambulatory care, such as women and elderly people aged 64 and over. The findings of the study are expected to serve as one of the barometers for the health care of patients with hypertension and for the performance of national hypertension management plans.

Differences of Cancer Patient's Health Care Utilizations between Medical Aid Program and National Health Insurance in the Elderly (노인 암환자의 건강보험과 의료급여 이용차이 분석)

  • This study to analyze differences of cancer patient's health utilizations in medical aid program and national health insurance by analysing health insurance claims data, and identify effects of health care systems. The majors results of the research were as follows. First, cancer patients in medical aid program more used total medical expenditures than in national health insurance mostly by many outpatient visits and long term hospitalization. Second, results of multiple regression, cancer patients in medical aid program more used total expenditures and inpatient expenditures. But, outpatient expenditures weren't different, cancer patients in medical aid program more visited medical institutions and hospitalized long term periods than in national health insurance. Therefore, it is too early to conclude that moral hazard is in health utilizations of medical aid program, because cancer patients in medical aid program many use in benefits for many nonbenefit burdens.

  • In this paper proposes the latest network-assisted online telemedicine service to coincide with the point being discussed for health care providers to match patients, patients with personalized medical service support system. In order to design the system, to understand the requirements of the patient personalized medical support service system, the data were normalized and were designed architecture client server structure. Further, in order to implement the system that was designed to define the structure of server and client, ontology repository, we implement the system. In this paper, as a result of the test by creating a scenario and prerequisites for testing patient personalized medical service support system that is design and implementation, selecting a patient's condition, department of symptoms by the selected but it was confirmed that the inference is, inference medical institutions that fits department inferred one following upon the items medical patient has the required.
