A gamma nail has been used to treat intertrochanteric fractures since 1988. Although such cephalomedullary nails have mechanical advantages over extramedullary fixation devices, such as sliding hip screw, their beneficial effects on treating the Arbeitsgemeinschaft für Osteosynthesefragen/Orthopaedic Trauma Association (AO/OTA) 31-A1 and 31-A2 fractures are still controversial. During their 30-year history, many problems have been overcome, and new types of cephalomedullary nails have been introduced in clinical practice. New cephalomedullary nail systems facilitate nailing procedures and enhance the purchase capability of the femoral head by a lag screw. On the other hand, the failure rate still depends on the hands of the orthopedic surgeons. This review article focused on the basic principle of medial support and restoration of a medial buttress during the treatment of trochanteric fractures using a cephalomedullary nail.
The treatment of patients with severe periodontitis should be proceeded step-bystep through an accurate diagnosis of each patients' individual tooth and with a strategic treatment plan. Implant-supported fixed prosthetic restoration has the advantage of high patient satisfaction and stable vertical dimension compared to the removable partial denture. However, multiple teeth defect areas lacking hard tissue may be disadvantageous in aesthetic failure and longer treatment time. In addition, it takes a certain period of time to manufacture and install a conventional fixed prosthesis, and during this process, the provisional prosthesis must satisfy the mechanical, biological, and aesthetic requirements of teeth. The purpose of this article is to describe the fabrication of implant-supported fixed prosthesis through a step-by-step approach in a partially edentulous patient.
For fully edentulous patients, implant-supported overdenture can be considered to enhance chewing efficiency and denture stability. Implant planning software can be used to fabricate a surgical guide for a more precise consideration of anatomic factors and prediction of the shape of definitive prosthesis. Though there are many possible attachments for implant overdenture, milled bar can be useful due to its splinting effect of implants and rigid support of overdenture. This report presents a case of implant-supported milled bar overdenture after guided implant surgery performed with two surgical guide that was fabricated before and after bone reduction.
Objective: The aim of this study was to evaluate the biomechanical aspects of peri-implant bone upon root contact of orthodontic microimplant. Methods: Axisymmetric finite element modeling scheme was used to analyze the compressive strength of the orthodontic microimplant (Absoanchor SH1312-7, Dentos Inc., Daegu, Korea) placed into inter-radicular bone covered by 1 mm thick cortical bone, with its apical tip contacting adjacent root surface. A stepwise analysis technique was adopted to simulate the response of peri-implant bone. Areas of the bone that were subject to higher stresses than the maximum compressive strength (in case of cancellous bone) or threshold stress of 54.8MPa, which was assumed to impair the physiological remodeling of cortical bone, were removed from the FE mesh in a stepwise manner. For comparison, a control model was analyzed which simulated normal orthodontic force of 5 N at the head of the microimplant. Results: Stresses in cancellous bone were high enough to cause mechanical failure across its entire thickness. Stresses in cortical bone were more likely to cause resorptive bone remodeling than mechanical failure. The overloaded zone, initially located at the lower part of cortical plate, proliferated upward in a positive feedback mode, unaffected by stress redistribution, until the whole thickness was engaged. Conclusions: Stresses induced around a microimplant by root contact may lead to a irreversible loss of microimplant stability.

TMJ Osteoarthritis is a degenerative disease caused by overload on joint tissue, and often accompany with local tenderness on the joint, crepitus by mandibular movement, restriction of mandibular movement, and anterior openbite. In general, panoramic radiography, TMJ panoramic radiography, and transcranial radiography are conducted to diagnose osteoarthritis after clinical examination, however, these radiographic evaluations are limited in detecting minute bony changes of early pathologic lesion. The aim of this pilot study was to evaluate the limitation and usefulness of several TMJ imaging techniques, so we compare the findings from clinical examination, plain film radiographs, tomograph, and bone scan from 81 patients with temporomandibular joint disorder. The proportion of patients showing same findings in plain film radiographs, TMJ tomograph, and bone scan was high, however, it is desirable that clinician should conduct phased imaging examinations according to the clinical findings due to the possibilities of false negative findings in diagnosis of osteoarthritis.

Bone loss may occur depending on the loading conditions. careful treatment planning and prosthetic procedures are very important factors for the proper distribution of stress. Evaluate the stress distributions according to numbers and location of implants in three-unit fixed partial dentures. A mandible missing the right second premolar, first molar and second molar was modeled. Using the CT data. we modeled a mandible with a width of 15mm, a height of 20mm and a length of 30mm, 2mm-thickness cortical bone covering cancellous bone mallow. An internal type implant and A solid type abutment was used. A model with 3 implants placed in a straight line, offset 1.5 mm buccally, offset 1.5 mm lingually and another model with 3 implants offset in the opposite way were prepared. And models with 2 implants were both end support models, a mesial cantilever model and a distal cantilever model. Three types of loading was applied; a case where 155 N was applied solely on the second premolar, a case where 206 N was applied solely on the second molar and a case where 155 N was applied on the first premolar and 206 N was applied on the first and second molar. For all the cases, inclined loads of 30 degrees were applied on the buccal cusps and vertical loads were applied on the central fossas of the teeth. Finite element analysis was carried out for each case to find out the stress distribution on bones and implants. This study has shown that prostheses with more implants caused lower stress on bones and implants, no matter what kind of load was applied. Furthermore, it was found out that inclined loads applied on implants had worse effects than vertical loads. Therefore, it is believed that these results should be considered when placing implants in the future.
Crossed occlusion can be treated either by overdenture and telescopic denture or by placing an implant at the edentulous area to reestablish the support on the occlusion. If alveolar bony support is sufficient and an the environment where an implant is inserted is favorable to restoring the masticatory and aesthetic function of a patient, the implant-supported fixed prosthesis can provide more definitive occlusal support and more aid for other oral functions. In this case report, a patient with a severe residual alveolar bone resorption following the extraction of teeth and who had a crossed occlusion was treated with sinus bone graft and alveolar bone augmentation in order to place the implants at prosthetically position. The definitive restoration was made to reflect the patient's occlusal and aesthetic function using the CAD/CAM double scanning method. Finally, the treatment had the masticatory and aesthetic function adequately restored, which is reported here.
Surgical microscrews were introduced and used as one method to provide absolute anchorage. Some clinicians implanted microscrews or miniscrews into the basal bone below the roots of the teeth to evade damage to the roots. Because the implanted microscrews were positioned too low the applied force was insufficient to retract the anterior teeth or protract the posterior teeth, and the use of microscrews or miniscrews seemed limited in applying vertical force. However Park implanted microscrews(micro-implants (1.2mm in diameter)) into the alveolar bone between the roots of the posterior teeth to change the direction of the applied force toward increasing horizontal component of the force. Moreover, these microscrew implants were positioned in the alveolar bone between the roots without causing discernable damage to the roots. This study was performed to provide guidelines and anatomic data to assist in the determination of the safe location for micro-implants. By measuring the CT images from 21 patients, anatomical data were obtained which were then used as a guide to determine the location for the implantation of micro-implants. The thickness of the cortical bones at the alveaolar bone region increased from the anterior to the posterior teeth area. The mandibular posterior teeth area showed thicker cortical bone. A greater distance was observed in distance between the second premolar root and first molar root in the upper arch, between the first molar root and the second molar root in the lower arch. The alveolar bone of the posterior teeth area is considered the best site for the implantation of micro-implants.

Longitudinal serial cephalometric films in the every second year from fifty girls were used for the analysis of growth pattern of maxillary alveolar bone (MAE). Six parameters regarding MAB height were measured and were compared with stature and chronological age. Descriptive analysis. correlation coefficient test and linear regression analysis were done for the statistical analyses. 1. The height of MAE increased until 14 years although growth velocity was gradually decreased. which was different with stature growth pattern. 2. The correlation coefficients between chronological age and MAB height were high and had no statistical differences with those of stature and MAB height. In addition. the correlation coefficients between chronological age groups within each parameter of MAB were very high. 3. The growth amount of MAB could be predicted using linear regression analysis on the basis of chronological age data. The growth pattern between MAE and statue was quite different because of the growth pattern difference between skeletal and neuronal system. Therefore. growth pattern of Korean MAB must be considered in dental treatment of youth. Also. close relationship between chronological age and MAB height would be useful when clinicians want to predict growth amount of MAE for the dental implant treatment.
This experiment was conducted to investigate of soil hardening degree before sowing, furrow depth at sowing and flooding time after sowing at furrowing in flooded rice paddy field that many people have an interest in as direct sowing method most recently. As hardening period was increased, the percentage of seedling stand and seed floating at flooding were increased slightly, while buried depth of stem at maximum tillering stage and cone penetration depth were decreased, respectively. Therefore, optimum degree of soil hardening was about 3 days after draining, at this time, cone penetration degree was about 6~7cm. According to furrow depth was more and more deep, buried depth of stem was increased gradually, but percentage of seedling stand was decreased considerbly. Also, root distribution ratio on surpace horizon and lodging degree were increased gradually according to furrow depth become more and more shallow. As flooding time after sowing was late, percentage of seedling stand and panicle number per $m^2$ were decreased slightly. These results apparently indicated that sowing after 3 days hardening when cone penetration degree was 6~7cm, furrow depth 3~4cm degree and flooding time just after sowing the best method to good establish of seedling stand.
