A Survey on Health Service Activities of Public Health Nurses in Pusan, Kyung Nam (부산.경남지역 보건소 간호사의 보건업무 수행활동에 관한 조사)

  • The purpose of this survey was to measure the health service activity of the Public health nurses and analyze the related factors influencing to their activities. The subjects of this study were 249 PHN in Health Centers, Pusan and Kyung Nam area and survey was conducted from 5th, January to 28th, February, 1987. The results of this study were as follow; 1. Total performance average of health service activities was 2.52. Among the 1 t health categories, health service planning (3.05) was the most and others health care (2.07) was the least active. 2. In performed degree of activities in department of Health Center. The hightest performed activities were T.B care in T.B clinic (3.79), family planning in family planning clinic (3.94), screening for disease in medical office (3.42), prenatal care in M.C.H. room (3.46) work planning in injection room (3.76), postpartum care in MCH center (3.63). 3. There were significient difference in age, education, religion, section of service, duty between the general characteristics and health service activities.(p<.05).

간호사의 영양교육에 대한 인식 및 영양지식

  • 영양사들이 영양서비스에 대한 중요성을 인지하고 있으나 과중한 급식업무에 많은 시간을 할애하고 있으며, 또한 영양사 인력부족으로 인하여 환자에 대한 영양상담이나 영양교육이 제대로 이루어지지 않은 병원도 있어 간호사들이 영양교육을 실시하는 병원도 있는 형편이다. 따라서 영양사의 원활한 업무수행 및 역할 확대를 위하여서는 이에 영향을 미칠 수 있는 의사, 간호사 등 타 의료진의 영양관리에 대한 인식파악이 필요하다고 생각되어 마산시와 창원시 및 경남 함안군에 위치한 2-3차 의료기관에 종사하는 보건의료인 중 간호사(197명)와 간호조무사(94명) 291명을 대상으로 영양관리 중 영양교육에 대한 인식 및 영양지식을 조사하였다. 환자에 대한 영양교육이 ‘매우 필요하다’에 74.2%, ‘약간 필요하다’에 32.8%를 보였으며, 간호사와 전문대 졸업이상에서 간호조무사와 고졸자에 비하여 영양교육의 필요성에 대한 인식이 높게 나타났다(P < 0.001). 영양관련 과목을 이수하지 않은 자는 47.9%, 이수한 자는 52.1%이였으며, 간호조무사(60.4%)가 간호사(42.1%)에 비하여 (P < 0.01), 그리고 근무경력 2년 이상(51.5-59.4%)에서 2년 미만(34.4%)에 비하여(p < 0.05) 영양관련 과목을 이수하지 않은 율이 높게 나타났다. 영양관련 과목 이수자 중 영양지식 습득정도는58.7%가 ‘부족’한 것으로, 40.6%가 ‘보통’인 것으로 답하여 대체적으로 부족하다고 인정하고 있었으며, 학력이 낮을수록 ‘부족’하다고 답하여 학력에 따른 유의적인 차이가 있었다(P <0.05). 영양교육 연수경험이 있는 대상자는 8.6%, 없는 자는 91.4%로 간호사에게 영양교육 연수가 거의 없는 것으로 나타났다. 영양교육 담당자의 적임자로는 영양사 69.3%, 간호사 21.3%로 답하였으며, 간호조무사는 영양교육 적임자로 영양사 82.7%, 간호사 5.8%로 답한 반면 간호사는 영양사 63.5%, 간호사 27.9%로 답하여 유의적인 차이가 있었다(p<0.001). 기회가 주어지면 영양교육에 참여하겠다는 대상자가 47.2%, 참여하지 않겠다는 자가 52.8%로 약1/2정도는 기회가 온다면 참여하겠다는 의지를 보였다. 영양교육에 참여하지 않겠다는 대상자의 43.4%가 ‘전문지식 부족’, 40.5%가 ‘전문가가 담당하는 것이 바람직하다’, 11.9%가 ‘업무량 과중’을 이유로 들었다. 환자치료 시에 영양에 대한 내용을 지도한 경험이 있는 대상자는 48.8%, 지도경험이 없는 자는 51.2%로 나타났으며, 간호사(60.9%)가 간호조무사(23.4%)에 비하여 지도경험율이 높고(p <0.001), 기혼자(57.4%)가 미혼자(44.2%)에 비하여 높으며(p < 0.05), 근무경력 2년 이상이 2년 미만에 비하여(P <0.05), 그리고 전문대 졸업 이상이 고졸에 비하여 유의적으로 높게 나타났다(p<0.001). 영양지식점수의 분포는 good group 55.3%, fair group 41.2%, Poor group이 3.4%이었으며, good group의 평균 점수는 16.1 $\pm$ 1.1점, fair group은 12.7 $\pm$ 1.4점, poor group은 6.6 $\pm$ 2.8점으로 세 군간에 유의적인 차이가 있었다(p < 0.001). 영양지식 점수의 전체평균은 20점 만점에 14.3 $\pm$ 2.5점이였다. 이상의 결과에서 환자의 영양교육 및 상담업무를 원활하게 수행하기 위하여 영양과 내에 급식관리와는 별도로 임상영양 및 영양교육을 담당하는 부서가 설치되어야 하며 영양사의 인력확보가 필요하다고 사료된다. 이상의 연구결과에서 간호사들은 영양교육이 필요하다고 인식하고 있었으나 간호사를 영양교육의 적임자로 보는 시각이 비교적 높았고 약 1/2정도는 영양교육에 참여하겠다는 의지를 가지고 있을 뿐만 아니라 실제로 영양지도를 한 경험이 있는 것으로 나타났다. 그러나 1/2 정도가 영양관련과목을 이수하지 않았으며, 91.4%가 영양교육 연수경험이 없는 것으로 나타났다.

Competencies of Nurses in a Maternal Early Childhood Sustained Home-Visiting Program (영유아 가정방문간호를 위한 간호사의 역량)

  • Purpose: Competency is a key foundation of the nursing education curriculum and a baseline element of nursing practice. The purpose of this study was to explore the competencies of community health nurses in a maternal early childhood sustained home-visiting program based on nurses' field experiences. Methods: The participants were 21 nurses who had more than 1 year of experience in this program. Reflective interviews were performed; lasting 1 hour per nurse, on August 18, 2016, and the collected data were analyzed using qualitative directed content analysis. Results: Twenty-four themes were extracted for 9 competencies. The major themes included implementing a long-term relationship-based approach, providing client-led service rather than expert-led service, helping mothers with psychosocial difficulties, and applying skills in a practical manner in the home environment. Conclusion: In order to develop a home-visiting program as part of maternal-early childhood nursing practice, nurses should develop competencies that help them make significant and positive interventions. Results indicate that the curriculum for community health nurses should be changed to improve competencies in building relationships with clients and to focus on the application of skills in specific cases and in clinical practice.

산업간호사의 근무조건 - 서울 경인지역을 중심으로 -

  • 본연구는 보건관리자로서의 산업간호사의 역할을 뒷받침 할 수 있는 근무조건을 파악하기 위한 기초조사연구이며 산업간호사의 근무조건 개선에 기여코저 한다. 연구방법은 1991년 3월부터 4월까지 서울, 인천, 경기 지역에 위치한 산업장의 건강관리실에 근무하는 산업간호사 131명을 대상으로 설문지를 이용하여 전화 또는 산업간호사회를 통해 자료를 수집하였다. 조사 결과는 다음과 같다. 1) 조사대상의 일반적 특성 서울, 경기, 인천지역에 상시근로자수가 300인 이상에 근무하는 산업간호사 배치율은 49.3%로 나타났다. 조사대상의 지역별 분포는 서울이 61개사업장이었고 인천 경기의 순이었다. 또한 산업간호사의 교육수준은 전문대졸이 84.0%이었고, 연령별 분포는 20대가 65.7%이었으며, 규모는 500~999사이가 42.7%였고, 노조가 있는 사업장은 73.3%인 것으로 나타났으며 산업간호사 1인이 근무하는 곳이 90.7%였고 보건관리자가 간호사뿐인 곳이 63.3%이며 1000명이상 근로자를 가진 산업체중 31.9%가 전임산업의나 산업위생사 없이 간호사 혼자서 근무하는 것으로 나타났다. 2) 근로조건 산업간호사는 주당근무시간이 44시간이하인 경우가 59.6%였고, 1부제 근무만하는 경우가 93.1%였으며, 월차휴가는 94.7%가 사용할 수 있었으며, 보수는 기본급이 25~40만원 사이가 69.5%였고 면허수당은 43.8%만 받는 것으로 나타났으며 상여금은 년 600%를 받는 산업장이 42%였고 총급여액이 35~55만원을 받는 경우는 전체의 67.3%를 차지하였다. 또한 연장근로인정여부는 일부만 인정하거나 인정하지 않는 경우가 61.8%였고, 과거경력 은 34.4%가 인정하지 않았고 승진은 66.4%가 불가능한 것으로 나타났다. 산업간호사가 별정직이나 촉탁으로 취업한 경우가 35.1%였고 일반 고졸이나 전문대졸 대우를 받고 있는 경우가 68.9%였다. 또한 전문단체활동 지원정도는 대한간호협회나 산업간호사회지원이 95 %를 넘었으며 산업간호사가 원하는 것 모두 지원하는 경우가 68.7%였으며 회비와 참석 모두를 지원하는 경우가 83.7%였다. 3) 모성보호 산업간호사중 81.7%가 결혼 후 근무가 가능했으며, 94.7%가 생리휴가를 사용할 수 있는 것으로 나타났다. 그리고 산전산후휴가는 76.3%가 사용할 수 있었다. 그러나 결혼후 근무나 산전산후휴가는 명문화되어 있었으나 실제로 이용한 경우는 없는 것으로 나타났다. 본 연구결과는 산업간호사의 근무시간, 전문단체활동지원은 비교적 좋은 조건이나 보수를 비롯 승진이나 대우, 모성보호에 대한 개선여지가 있음을 보여주었다. 따라서 결혼후 지속근무, 산전산후 휴가제 등이 보장되어야 전문직으로서의 산업간호사 인력확보, 활용이 용이할 것으로 기대된다. 더불어 전문단체의 조직적 활동과 연구, 행동적인 지원이 계속되어져야 할 것을 제언하는 바이다.

Comparison of Working Conditions among Non-regular Visiting Nurses in Public Health Centers based on Their Employment Types (전국 보건소 비정규직 방문간호사의 고용형태별 직무실태 비교)

  • Purpose: This study is to investigate working conditions including job stress among visiting nurses in public health centers in Korea. Methods: An social network based mobile survey was conducted in May 2017 (N=936, response rate: 47.0%). Results: The visiting nurses in this study had their average total career as a nurse is 13.7 years. The 68.3% of them were employed in an indefinite term, 17.0% were hired in a fixed term, and 11.0% came from outsourcing. They responded as high job-stress level including inadequate compensation (71.22/100) and job demands (71.91/100). They experienced down-talk (63.4%), swearwords (32.9%), being made a dirty face (39.9%), sexual jokes (30.8%), or being likened or evaluated with their appearance sexually (14.3%). Among the causes of job related conflicts and discrimination, deprived salary level was the most frequent reason (83.4%). The conflicts and discrimination were incurred by government officers (52.4%). There were no significant differences in overall job stress, emotional labor, organizational commitment, violence, and discrimination experience based on their employment types. Conclusion: The differences in working conditions among the non-regular nurses were trivial, and their overall working conditions were poor. It is necessary to improve non-regular nurses' working conditions in order to make up the limitations of the Korean healthcare system which is centered on hospitals.

How to Improve the Emergency Medical Service System: Levels of Knowledge and Techniques in Prehospital Care for 119 Relief Squad Members and Emergency Room Nurses (병원 전 단계 응급의료 서비스 개선방안에 관한 연구 : 119 구급대원과 응급실 간호사의 응급처치 지식 및 기술 숙련도 비교)

  • During the past decade or so in Korea, 119 relief squad members were supposed to respord first urgent or emergency cases. The primary purpose of this study was to assess what levels of knowledge and techniques in prehospital care the 119 relief squad members showed. Data regarding the knowledge and technique levels were collected from both the 119 relief squad members (n=63) and the emergency room nurses(n=46). The Results indicated that the 119 relief squad when compared to emergency nurses, showed higher scores for knowledge and techniques in some areas of prehospital care but not in other areas, However, no differences in knowledge and technique were found when the ANOVAs were calculated with two covariates : duration of their career in emergency medical services and frequency of their exposure to lectures on emergency medical technology. In addition, many respondents in the 119 relief squad group rated themselves poorly in knowledge and techniques of prehospital care, The findings imply that qualitatively better curricula should be given to the 119 relief squad members before they are allowed to play an important role in the emergency medical service system. These findings are also discussed in the context of improving the emergency medical service system.

  • The purpose of this study is to identify job satisfaction and relative factors of job satisfaction in visiting nurses in public health centers. Data were collected by a self-administered questionnaire from May 1 to June 25, 1997. The subjects were 384 visiting nurses of public health centers in Korea. The results of this study were as follows; 1. Job satisfaction of Visiting Nurses was measured by a 5 point rating scale, the whole mean score was 3.08. The level of job prestige was highest among the six components of job satisfaction. The mean score of job perception was 4.05. 2. The factor affecting job satisfaction were job status, educational level and the number of visits home a month. 3. Factors affecting job perception were whether or not they had other certificates, whether they were educated about visiting nursing, and the status of these nurses. 4. Job perception and Job satisfaction had a significant correlation.

Customized Home Visiting Nurses' Experiences in Telephone Visiting (맞춤형방문간호사의 전화 방문 경험)

  • Purpose: The purpose of this study was to explore and understand customized home visiting nurses' experiences in telephone visiting. Methods: Qualitative data were collected from several discussions of 4 focus groups consisting of 20 customized home visiting nurses. All interviews were recorded and transcribed according to thematic content analysis processes. Results: The five main themes on customized home visiting nurses' experiences of telephone visiting were 'greeting', 'counseling', 'using strategies', 'maintaining attitude for enhancing relations' and 'enduring difficulties'. The category of 'counseling' was composed of confirming, educating, supporting, adjusting, and indicating. And, the category of 'using strategies' included complimenting, scolding, persuading, compensating, and ignoring. Conclusion: Developing a standardized manual on telephone visiting is needed to guide an effective counseling. In order for a customized home visiting nurse to be a good telephone counselor, training of motivational interviewing is recommended.

The Process of Nurses Experience in Public Health Program Performance (간호사의 보건사업수행 경험과정)

  • Purpose: This study was to describe and develop public health nursing. Methods: Data were collected from 19 nurses sampled from 5 public health centers. The grounded theory of Strauss and Corbin (1990) was applied. Results: As a result, 48 concepts, 23 subcategories, and 11 categories were deduced from open coding. In axial coding, casual conditions 'facing difficulties of practical administration' and 'attaching importance to health support for residents' and context condition 'assessing of residents' service needs' impacted phenomenon 'searching for efficient performing methods.' Intervening conditions 'forming of support system' and 'working with positive,' and action-interaction conditions 'improving of resident's health and medical accessibility' and 'striving for self-development' lead to consequences 'carried out promptly for field-based problem solving,' 'mastered of successful program planning methods' and 'solving the issues with existing style.' The periods of process were divided into 4 stages, confirming program-contents, probing program-method, developing program-strategies, and applying program-competencies. The core category, ‘strengthening of practical-planning work competencies' incorporated the relationship between and among all categories and explained the process. Conclusion: This study described public health nurses' performance in Korea. These findings have important implications for the practice and must be considered to develop competencies for planning and practice of public health.

Presenteeism and Absenteeism According to Health Problems on Nurses (간호사의 건강문제에 따른 프리젠티즘과 결근율의 관계)

  • Purpose: The purpose of this study was to examine the presenteeism and absenteeism in relation to the health problems of nurses. Method: Informed consent was provided before study. Data was collected from January 7 to March 6, 2008 by Stanford Presenteeism Scale-13. Data was collected from 3 hospitals in Seoul Gyeonggi area. The response rate of the questionnaires was 96.2%. Incomplete data was analyzed using SPSS program. Results: Shoulder, back and neck pain were the highest reported health problems and primary health Problem of nurses. Impaired presenteeism was ranked in this order: depression or anxiety, cold, and uterus or ovary disorder. Absenteeism was ranked in this order: uterus or ovary disorder, cold, and dermatological problem. Impaired presenteeism was significantly increasing according to the number of health problems. But Absenteeism was not. Presenteeism was significantly different by age, educational level, religion, whether or not the worker lived with their family, marital status, whether or not the worker has children, time spent in the workforce, experience of turnover, shift work, night work, and level of satisfaction with salary. But absenteeism was not significantly different given these criteria. Conclusions: Health problems of nurses can negatively effect the quality of nursing service and productivity at hospital. Therefore the hospital administrator should control the presenteeism and absenteeism through the management of health problems of nurses.
