Center of Pressure and Ground Reaction Force Analysis of Task-oriented Sit-to-stand in Stroke Patients (뇌졸중 환자의 과제지향적 일어서기 시 신체압력중심과 지면반발력 특성 )

  • PURPOSE: This study examined the center of pressure (COP) and ground reaction force (GRF) characteristics during each task-oriented sit-to-stand in stroke patients. METHODS: Twenty stroke subjects were included in this study. The task consisted of sit-to-stand (SS), sit-to-stand for reaching (SR), and sit-to-stand for walking (SW). The response time, COP, and GRF were measured during each task. The COP and GRF data were obtained using a two-force plate. The force plates were placed on a chair (below the buttock) and floor (below the feet). RESULTS: Significant differences were observed between SS (1.48 ± .48 s) and SR (2.09 ± 0.82 s) and between SS and SW (2.27 ± .72 s) in the preparatory phase time during each sit-to-stand exercise (p = .002) and showed significant differences between SS (13.90 ± 6.44 cm) and SW (34.62 ± 39.38 cm) and between SR (16.14 ± 8.04 cm) and SW in the mediolateral COP range during each sit-to-stand exercise (p = .013). CONCLUSION: These findings suggest that more complex task-oriented sit-to-stand exercise requires a high-level motor programming process than a simple sit-to-stand task. Therefore, a variety of tasks-oriented sit-to-stand exercises will be useful training to achieve better ADL ability for stroke patients.

Comparison of Lower Extremity Electromyography and Ground Reaction Force during Gait Termination according to the Performance of the Stop Signal Task (정지신호과제의 수행에 따른 보행정지 시 다리 근전도 및 지면반발력 비교)

  • Purpose: The purpose of this study was to investigate the association between cognitive and motor inhibition by comparing muscle activity and ground reaction force during unplanned gait termination according to reaction time measured through the stop-signal task. Methods: Sixteen young adults performed a stop-signal task and an unplanned gait termination separately. The subjects were divided into fast and slow groups based on their stop-signal reaction time (SSRT), as measured by the stop-signal task. Electromyography (EMG) and ground reaction force (GRF) were compared between the groups during unplanned gait termination. The data for gait termination were divided into three phases (Phase 1 to 3). The Mann-Whitney U test was used to compare spatiotemporal gait parameters and EMG and GRF data between groups. Results: The slow group had significantly higher activity of the tibialis anterior in Phase 2 and Phase 3 than the fast group (p <0.05). In Phase 1, the fast group had significantly shorter time to peak amplitude (TPA) of the soleus than the slow group (p <0.05). In Phase 2, the TPA of the tibialis anterior was significantly lower in the fast group than the slow group (p <0.05). In Phase 3, there was no significant difference in the GRF between the two groups (p >0.05). There were no significant difference between the two groups in the spatiotemporal gait parameters (p >0.05). Conclusion: Compared to the slow group, the fast group with cognitive inhibition suppressed muscle activity for unplanned gait termination. The association between SSRT and unplanned gait termination shows that a participant's ability to suppress an incipient finger response is relevant to their ability to construct a corrective gait pattern in a choice-demanding environment.

Evaluation of the Impact Behavior of Inline Disk Wheel Made of Carbon Fiber Reinforced Composites (탄소섬유 강화 복합재로 구성된 인라인 디스크 휠의 충격거동 평가)

  • In this paper, The concept of a wheel with carbon fiber composite is to replace the conventional material used for a wheel hub, such as plastic, with a disk-type hub made of carbon fabric and epoxy resin. The impact load from the ground under real conditions was considered; a low-velocity impact test was conducted to evaluate the impact performance of the carbon wheel and compare it with that of a conventional plastic wheel. This study applied a 70 J impact load as a test condition. The impact energy was controlled in the test by adjustment of height and weight of impactor. The use of a carbon disk wheel hub was confirmed to reduce weight and generate an excellent repulsive force at low energy under conditions similar to real driving conditions. The results showed that the maximum load increased proportionally depending on the impact load, but the growth of the maximum load was reduced at a 20 J impact load and tended to decrease at a 45 J impact load. The carbon wheel showed excellent properties ; the level of rebounding was 35.3% and 19.1% of the total impact energy at impact loads of 5 J and 10 J, respectively. On the other hand, the carbon disk wheel rebounded less than 5% of the total energy due to crack generation of the thin carbon hub for impact loads of more than 20 J.

Locomotion Control of Biped Robots with Serially-Linked Parallel Legs (이중 병렬형 다리 구조를 가진 2족보행로봇의 보행제어)

  • In this paper, we propose a new parallel mechanism for the legs of biped robots and the control of the robot's locomotion. A leg consists of two 3-DOF parallel platforms linked serially: one is an orientation platform for a thigh and the other is the 3-DOF asymmetric parallel platform for the shank. The desired locomotion trajectory is generated on the basis of the Gravity-Compensated Inverted Pendulum Mode (GCIPM) in the sagittal direction and the Linear Inverted Pendulum Mode (LIPM) in the lateral direction, respectively. In order to simulate the ground reaction force, a 6-DOF elastic pad model is used underneath each of the soles. The performance and effectiveness of the proposed parallel mechanism and locomotion control are shown by the results of computer simulations of a 12-DOF parallel biped robot using $SimMechanics^{(R)}$.

Gait Initiation in a Patient With Spastic Hemiplegia Cerebral Palsy With and Without a Dynamic Ankle Foot Orthosis: A Pilot Study (강직성 편측 뇌성마비 환자의 보행 기시에 미치는 동적 단하지 보조기의 효과: 사전연구)

  • 본 연구는 강직성 편측 뇌성마비 환자의 보행 기시(gait initiation)의 특성을 파악하고, 동적 단하지 보조기(dynamic ankle foot orthosis : DAFO)가 이런 환자들의 보행 기시에 미치는 영향을 알아보기 위하여 실시하였다. 연구대상자는 19세 선천성 강직성 우측 뇌성마비를 가진 여자 대학생이었다. 연구대상자는 4개의 다른 보행 기시 조건에서 각 조건당 7회의 보행 기시를 하였는데, 각각의 보행 기시 조건은: 1) 보조기 없이 좌측 하지로 보행 기시; 2) 우측 하지 보조기 착용 상태에서 좌측하지로 보행 기시; 3) 보조기 없이 우측 하지로 보행 기시; 4) 우측 하지 보조기 착용 상태에서 우측 하지로 보행 기시였다. 두개의 힘 판(force plate) 위에서 본인이 선택한 가장 편안한 방법과 속도로 2 m를 걷도록 하였다. 연구대상자가 보행 기시를 하는 동안 양쪽 가자미근과 앞정강근에 부착된 4개의 표면 전극으로부터 근 전도 신호와 보행 기시를 하는 연구 대상의 양 발 아래 놓인 두개의 힘 판의 자료를 분석하여 체중(body weight : BW)으로 정량화 하였다. 실험결과는 각 조건마다 7회 실시된 측정값들의 평균을 이용하였다. 본 사전연구의 결과는 다음과 같다. 1. 보행 기시 전 양다리로 선 상태에서 환측보다 건측으로 많은 체중지지를 하였으나(환측 42.57%BW, 건측 58.03%BW) 보조기의 착용이 환측으로의 체중지지를 향상시켰다(환측 44.46%BW, 건측 55.54%BW). 2. 보행 기시 중 가자미근과 앞정강근의 동시수축(coactivation)이 계속되었는데 이러한 양상은 보조기의 착용 유무에 따라 크게 달라지지 않았다. 3. 힘 판에서 들어온 지면 반발력(ground reaction force)의 수직 분력(vertical element, Fz)과 전후 분력(anterior-posterior element, Fx)을 분석한 결과, 건측 다리로 보행 기시를 할 때 보조기 착용은 가속을 촉진시켜 역학적으로 유리하였다. 이상의 연구 결과는 보조기의 착용이 건측 다리로 보행 기시를 할 때 미치는 신경학적, 역학적 장점들에 대한 구체적 연구의 필요성 및 근거를 제시하고, 환자들의 보행훈련 접근에 보다 효율적인 보행방법을 소개하는데 이용될 수 있다.

The Relationship between Grip Strength and Ground Reaction Force by Change of Position when Lifting Tasks (들기 작업할 때 자세의 변화에 따른 악력과 지면 반발력의 상관관계)

  • The purpose of this study, during the lifting task was researching the difference and a relationship between the ground reaction force and the grip strength by change of position. After grip strength has measured in symmetry position and asymmetry position at 45cm and 75cm of height of hand, ground reaction force was measured by same attitude lifting wooden box. We analyzed the difference of grip strength and ground reaction force in each position change. The results of grip strength, the grip strength of both hand were significant difference that in study subject symmetry and asymmetry position (p<0.01). The results of symmetry lifting task, the study subjects was significant difference of the ground reaction force difference by height (p<0.05). Asymmetry lifting task was significant difference of ground reaction force difference by direction of rotation was changed (p<0.01). The result of it will rotate with non-dominant hand side of lifting tasks from height 75cm where it easily maintains a balance possibility and decreasing the load of the hand. Therefore, from the workshop in the work people, it will be between the height 75cm and non-dominant hand side of trunk rotatory direction in the lifting tasks. Future study is necessary researched about the change of grip strength when the height of the hand is higher, and the difference of the ground reaction force when the change of weight.

The Comparison of Symmetry of Vertical Ground Reaction Force on Pattern of Sit to Stand in the Chronic Stroke Patients (만성뇌졸중 환자의 일어서기 자세에 따른 수직지면반발력의 대칭성의 비교)

  • The purpose of this study is to evaluate the influence of posture elements on symmetrical weight bearing during STS (sit-to-stand) in patients with chronic stroke. The subjects were patients diagnosed with stroke: a total of 24 patients (16 males and 8 females) participated in this study. All the participants performed STS tasks(3 foot postures and 2 arm postures). Two force plates (AMTI) were used to measure the peak vertical ground reaction force(Peak Fz) and the symmetrical ratio to peak vertical ground reaction force. The data were analyzed using independent t-test and 2-way repeated analysis of variance. The results of this study were as follows: 1) The peak Fz placed more weight on the paretic leg during STS and 2) The symmetrical ratio to the peak Fz showed a significant difference according to the foot and arm posture (p<.05), and had the highest AYM_GA ($0.87{\pm}0.12$). These results indicate that arm and leg postures during STS in patients with chronic stroke had the highest AYM_GA. We believe that the outcome of this study will be a reference for the prognosis of STS in patients with stroke.

The Effects of Trunk Movement and Ground Reaction Force during Sit to Stand Using Visual Feedback (시각 되먹임을 이용한 앉은 자세에서 일어서기 시 몸통의 동작과 지면 반발력에 미치는 영향)

  • Purpose : This study was conducted to investigate the changes in trunk movement and ground reaction during sit to stand motion using visual feedback. Methods : Fifteen adults (average age: 23.53±1.77 years) participated in this study. An infrared reflective marker was attached to the body each participant for motion analysis, and the participants performed sit to stand motion while wearing a hat attached with a laser pointer, which provided visual feedback. First, the sit to stand action was repeated thrice without obtaining any visual feedback, followed by a three minute break. Next, the laser pointers attached to hats were irradiated on a whiteboard, located at a distance of 5 m in front of the chairs, on which the participants sat; a baseline was set, and the participants performed stand up movements three times under this condition. A visual feedback was provided to the participants to prevent the laser pointers from crossing the set baseline. During each stand-up movement, the position of the reflective marker attached to the subject's body was recorded in real time using an infrared camera for motion analysis. The trunk movement and ground reaction force were extracted through recorded data and analyzed according to the presence or absence of visual feedback. Results : The results indicated that in the presence of a visual feedback during the sit-to-stand movements, the range of motion of the trunk and hip joints decreased, whereas that of the knee and ankle joints increased in the sagittal plane. The rotation angle of the trunk in the horizontal plane decreased. The left and right movement speed of the center of pressure increased, the pressing force decreased, and the forward and backward movement speed of the trunk decreased. Conclusion : The results suggest that the efficiency and stability of the stand up movement of a body increase when a visual feedback is provided.

Method of Walking Surface Identification Technique for Automatic Change of Walking Mode of Intelligent Bionic Leg (지능형 의족의 보행모드 자동변경을 위한 보행노면 판별 기법)

  • In this paper, we propose a gait pattern recognition method for intelligent prosthesis that enables walking in various environments of femoral amputees. The proposed gait mode changing method is a single sensor based algorithm which can discriminate gait surface and gait phase using only strain gauges sensor, and it is designed to simplify the algorithm based on multiple sensors of existing intelligent prosthesis and to reduce cost of prosthesis system. For the recognition algorithm, we analyzed characteristics of the ground reaction force generated during gait of normal person and defined gait step segmentation and gait detection condition, A gait analyzer was constructed for the gait experiment in the environment similar to the femoral amputee. The validity of the paper was verified through the defined detection conditions and fabricated instruments. The accuracy of the algorithm based on the single sensor was 95%. Based on the proposed single sensor-based algorithm, it is considered that the intelligent prosthesis system can be made inexpensive, and the user can directly grasp the state of the walking surface and shift the walking mode. It is confirmed that it is possible to change the automatic walking mode to switch the walking mode that is suitable for the walking mode.

Foot/Ankle Roll-Over Characteristics According to Different Heel Heights of Shoe during Walking (보행 시 신발 굴 높이어 따른 발/발목 복합체의 Roll-Over특성)

  • This study investigated the effects on foot and ankle roll-over characteristics according to different heel heights during walking. Fifteen female volunteers who have neither musculoskeletal nor foot problems were participated in gait analyses, wearing four different pairs of shoes in different heel heights. To obtain roll-over shape of foot/ankle complex, we used trajectories of knee and ankle joints as well as the renter of pressure between initial contact and opposite initial contact. Results revealed that the entire roll-over shape moved distally as the heel height increased but roll-over characteristics showed uniformly with different heel heights. In addition, we found that nondisablea persons automatically adapted to their foot/ankle complex to varying heel heights within 6cm, by moving the origin of roll-over shape distally to maintain roll-over characteristics. However, since the balance of the gait only by the ankle joint could not be achieved beyond the heel height of 6cm, compensations at the knee and the hip joints occurred simultaneously. Roll-over characteristics in human walking would provide simpler and wider understanding of human walking, and furthermore could be applied to the wide understanding of prosthetics and orthotics of the lower extremity as well as orthopaedic shoes.
