Temporomandibular Disorder Caused by Periapical Abscess of Third Molar (제 3 대구치의 치근단 농양으로 인한 측두하악장애)

  • Mouth opening limitation is generally caused by masticatory muscle or temporomandibular joint pain, disc dislocation without reduction, adhesion or ankylosis of the temporomandibular joint, and muscle contracture. But otorhinolaryngologic disease, neurologic and vascular disease, tumor, inflammation and infection may cause pain and mouth opening limitation which mimics temporomandibular disorders. Re-evaluation for possibilities of inflammation, infection and tumor should be in cases that do not show symptom improvement or appear with continuous aggravation despite of proper treatment. In this case, we report of medial pterygoid muscle pain and mouth opening limitation caused by periapical abscess of third molar spread to the pterygomandibular space.

Work related diseases in Korean fire fighters (소방공무원의 직무관련질환과 건강실태 조사)

  • The purpose of this study is to investigate work related diseases in Korean firefighters and to prepare the preventive measure for health management for the firefighters. Structured self-reported questionnaire was distributed to the firefighters. The result revealed that 37.7% of fire fighters had chronic diseases and 12.3% had chronic low back pains. The affected parts included lumbar region(41.5%), shoulder(33.0%), and neck(23.6%). The affected parts caused physical pain and work discomfort for more than a month. A total of 54.1% firefighters visited to the hospital within a year. This results suggest that it is necessary to develop the health promotion program for the prevention of musculoskeletal disease and regular health checkup for the acute disorders caused by workload.

  • The purpose of this study is to identify the oral health behaviors of the Sanitation Workers and to lay the basis for the policy direction for promoting industrial oral health. A total of 79 self - filling questionnaires were analyzed for Sanitation Workers in Gwangju Metropolitan City. As a result, oral health education experience was about 25% compared to 80% of scaling experience. Oral hygiene is related to academic background, type of employment, and number of years of work. Periodontal status was higher in 'high sensitivity' and 'gum edema' than high school education or higher. The higher the income and education, the more perceived periodontal status was 'normal' or 'worse'. Therefore, it is meaningful that we analyzed sanitation worker and it is necessary to take various efforts and countermeasures to improve the oral health of the industry by expanding the opportunities for oral health education by taking into consideration the Sanitation Workers and developing active and complex programs.

Factors Associated with Relapse to Smoking Behavior Using Health Belief Model (건강믿음모형을 이용한 금연성공자의 재흡연에 영향을 미치는 요인 분석: 금연클리닉 등록자를 중심으로)

  • Objectives: This study aimed to identify factors associated with smoking relapse. Methods: The study sample was recruited among subjects who were enrolled in the smoking cessation clinic of a public health center and had succeeded in quitting smoking for at least six months. A total of 159 male subjects were followed via mail survey one year later. The independent variables in the analyses were socio-demographic characteristics, smoking history and behavior, receipt of smoking cessation aids, health behaviors and components of the health belief model (HBM). The dependent variable was smoking relapse assessed one year after quitting. Ordered logit regressions were used to identify factors associated with smoking relapse. Results: The relapse rate of the ex-smokers in our sample was 25.8%, and the occasional smoking rate was 17.0%. Univariate analyses revealed that only factors related to the HBM, such as perceived susceptibility to diseases (p<0.01), perceived severity of diseases (p<0.01), perceived health benefits of not smoking (p<0.01), perceived barriers to quitting smoking due to increasing stress and difficulty in social life (p<0.01), and self-efficacy (p<0.01) were associated with the likelihood of relapse for ex-smokers. Ordered logit analyses yielded two significant factors affecting the likelihood of relapse, the perceived barriers to quitting smoking and self-efficacy. Conclusions: Our results indicate that higher levels of barriers to quitting smoking and lower levels of self-efficacy were significantly related to risk of smoking relapse. These findings may be useful for identifying those at highest risk for relapse and choosing the optimal strategies for prevention of relapse for ex-smokers.

A Descriptive Study of Oral Health Knowledge & Behaviors in Middle School Students (일부지역 중학생의 구강건강 지식 및 행동에 관한 조사연구)

  • Objectives: This study was designed to understand the oral health knowledge & conduct of middle-school students, search for the learning objective and the educational method in line with the subjects and of utilizing as the basic data for an effective oral health-care program. Methods: The samples to achieve the purpose of this research are composed of 139 students in middle-school, OO county. Chungcheongbuk-do, the number of male students 64, and female students 75. Data were statistically analyzed by frequency analysis, $x^2$-test or Fisher's exact test by using SPSS WIN Ver. 12.0. Results: Among items on oral-health knowledge in middle-school students. the awareness ratio on a cause and preventive method for oral disease was surveyed to be lower than the awareness ratio on symptoms of oral disease. As a result of examining by comparing knowledge and behavior on the time of tooth brush. both awareness and behavior were the level of 50% or less than it. In particular, 46.2% perceived after lunch. but practice just accounted for 33.0%. The frequency of tooth brush a day was the largest in a case(47.5%) of doing twice a day. However. there was also the response (5.8%) with saying of brushing once or not brushing even once. Thus, the practice of tooth brush was surveyed to be very low even if being a minority of students. The frequency of taking a light meal was 68.8% in less than twice a day. However, even students of taking more than five times were surveyed to be 9.8%. Out of the whole-body health in over 50%-59.9%. the oral health was surveyed to be perceived to be very important. Compared to the awareness level on importance of a tooth, the ratio of visiting a dentistry was analyzed to be very low. Conclusions: The study results suggest that the school oral-health project was examined to have the necessity of being expanded and carried out even in middle-and-high schools, by which the specific oral-health promotion program including oral-health education in this period is developed.

Doctor's Perception and Referral Barriers toward Palliative Care for Advanced Cancer Patients (말기암환자의 완화의료에 대한 의사들의 인식과 완화의료 의뢰 시 장애요인)

  • Purpose: This study was conducted to identify the perception regarding palliative care among Korean doctors and referral barriers toward palliative care for terminal cancer patients. Methods: Between May and June 2010, 477 specialists mainly caring cancer patients using a web-based, self-administered questionnaire. Results: A total of 128 doctors (26.8%) responded. All respondents (100%) deemed palliative care a necessary service for terminal cancer patients. More than 80% of the respondents agreed to each of the following statements: all cancer centers should provide palliative care service (80.5%); all terminal cancer patients should receive concurrent palliative care along with anti-cancer therapies (89.1%) and caring for terminal cancer patients requires interdisciplinary approach (96.9). While more than 58% of the respondents were satisfied with their performance of physical and psychological symptoms management and emotional support provided by patient's family members, 64% of the responded answered that their general management of the end-of-life care was less than satisfactory. Doctors without prior experience in referring their patients to palliative care specialists accounted for 26.6% of the respondents. The most common barrier to hospice referral, cited by 47.7% of the respondents, was "refusal of patient or family member", followed by "lack of available palliative care resources" (46.1%). Conclusion: Although most doctors do recognize the importance of palliative care for advanced cancer patients, comprehensive and sufficient palliative medicine, including interdisciplinary cooperation and end-of-life care, has not been put into practice. Thus, more active palliative consultation or referral is needed for effective care of terminal cancer patients.

The Needs for Discharge Education and Educational Performance of Nurses Perceived by Premature Infants Mothers In the ICT Medical Service (ICT 의료시설 기반에서 미숙아 어머니의 퇴원 교육 요구도와 간호사의 교육수행 정도)

  • The current research focused on the needs for discharge education and the educational performance of nurses perceived by mothers of premature infants. The subjects for the current research were 54 mothers of premature infants hospitalized in the neonatal intensive care unit at K general hospital in G metropolitan city. The data was collected between June 9, 2014 and September 30, 2014 through self-recording surveys. The needs for discharge education were $4.21{\pm}0.60$ and the perceived educational performance of nurses was $3.95{\pm}0.73$. There was a significant difference between the needs for discharge education and the perceived educational performance in abnormal symptom monitoring and management ($0.55{\pm}0.97$, p=0.001), excrement management ($0.45{\pm}1.11$, p=0.004) and growth development ($0.41{\pm}1.08$, p=0.007). The needs for discharge education was significantly different according to delivery type (vaginal delivery: $4.41{\pm}0.47$, caesarean section: $4.03{\pm}0.47$, p=0.040) and birth order (first: $4.37{\pm}0.53$, second: $4.25{\pm}0.51$, over third: $3.75{\pm}0.72$, p=0.031). Perceived educational performance of nurses was significantly different according to baby sitter (yes: $4.15{\pm}0.66$, no: $3.48{\pm}0.67$, p=0.002). ]

Fibromyalgia from the Psychiatric Perspective (정신과적 관점에서의 섬유근통)

  • Fibromyalgia is a disorder characterized by the core symptom of chronic widespread pain, along with fatigue, sleep disturbances, mood changes, and cognitive difficulties. The etiology of fibromyalgia involves a combination of biological factors, such as genetic vulnerability, alterations in pain processing and stress response system ; psychological factors, such as anxiety, depression, anger, and perceived stress ; environmental factors, such as infections, febrile diseases, and trauma. Central sensitization, which is amplified in the process of sensory stimulation, has been emphasized as a key etiological factor, as supported by enhanced wind-up, delayed aftersensation, decreased nociceptive flexion reflex threshold and functional imaging studies. Several guidelines recommend that a multimodal approach be used to treat fibromyalgia, including both pharmacological and non-pharmacological treatments, tailored to each individual, and that clinicians should provide an intellectual framework through sufficient education and emphasis on the importance of self-management. The prevalence of mood disorders, anxiety disorders, and other psychiatric problems is 7-9 times higher in patients with fibromyalgia than in the general population ; moreover, the association between fibromyalgia and certain psychopathologies or sleep problems has also been suggested. Since psychiatric problems, with shared vulnerabilities and risk factors, interact with fibromyalgia bidirectionally and also affect the disease course, an integrated management approach is needed to determine the risk of comorbidities.

The Influence of Urinary Incontinence and Depression in Elderly on the Quality of the Life (노인의 요실금과 우울증이 삶의 질에 미치는 영향)

  • Objectives : Little is known about the influence of urinary incontinence and depression on individual's QOL(Quality of life). We aimed to clarify how the interaction between urinary incontinence and depression influences one's QOL. Methods : A total of 1262 patients were enrolled in this study from April, 2011 to July, 2011. We estimated the severity of depressive symptoms and QOL, using SGDS-K, EQ-5D. We also investigated the morbidity of urinary incontinence for each patient in person or by questionnaire. Comparisons of QOL between groups with or without depression, with or without urinary incontinence were established using t-test, ANOVA and Scheffe's post hoc analysis. The interaction between urinary incontinence and depression was analyzed by each domain of QOL, using multiple regression analysis. Results : Patients with depression and urinary incontinence showed significantly higher EQ-5D scores on every domain of QOL than other patients, which means significantly lower QOL. Patients with depression, no urinary incontinence reported lower QOL, especially in the domain of 'usual activity', 'anxiety' and 'visual analogue scale(VAS)', whereas those with urinary incontinence, no depression showed lower QOL in 'motility', 'usual activities' and 'pain' domain. Statistically significant interaction effects of two diseases were observed in the domain of 'VAS', 'self care' and 'anxiety'. Conclusions : Comorbidity of urinary incontinence and depression showed significantly lower QOL of patients, compared with urinary incontinence or depression respectively, which implies additive interaction effects of the two diseases. Optimal diagnosis and treatment of depression should be emphasized for patients with urinary incontinence.

A Study on Health Information and Medical Consulting via Internet Focusing on the Age Group of 20s (인터넷을 활용한 건강정보 및 의료상담에 관한 연구 (20대를 중심으로))

  • High Internet usage and the public's keen interest on health have influenced the health care system, and a potential value of the online health information and medical consultation market is immense. This study reveals results from data collected from Seoul residents in the age group of 20s in 2011. Out of 499 respondents, 75.2% answered that they used online health information; however, only 7.2% answered that they have used online medical consultation services. Findings on the purposes of using online medical consultation included treatments of symptoms(33.6%) and self-disciplines of one's health(19.5%). Mostly used Websites for health information search included search engines and blogs, but respondents preferred to use government sites and hospital sites in the future. When choosing a medical consultation, respondents preferred a certain website for different reasons including creditability of the consultant(23.7%), creditability of the organization(23.7%), rapid responses(21.2%), and more. Overall, although health information search via web is being highly utilized among people in their 20s, utilization of online medical consulting is not. Thus, promotion efforts to increase awareness and utilization of online medical consulting based on the site selection criteria, type of personal information disclosure, and other preferences are essential. Also, creating websites meeting these criteria is important.
