대학병원 중환자실 간호사의 전문직 삶의 질 영향요인 (Factors Influencing Professional Quality of Life in Intensive Care Unit Nurses of University Hospitals)

  • Purpose : This study aimed to identify the factors influencing the professional quality of life of intensive care unit (ICU) nurses working in university hospitals. Methods : A survey was conducted on 171 ICU nurses in university hospitals in B City, South Korea. This study used the Professional Quality of Life instrument, which consists of three subscales, namely, compassion satisfaction, burnout, and secondary traumatic stress. Data were analyzed using stepwise multiple regression analysis. Results : Compassion satisfaction was influenced by resilience, ICU job satisfaction, and innovation-oriented culture, and these variables explained 37.1% of the variance in compassion satisfaction. Burnout was influenced by resilience, a hierarchy-oriented culture, and ICU job satisfaction, and these variables explained 42.9% of the variance in burnout. Secondary traumatic stress was influenced by a task-oriented culture and resilience, and these variables explained 12.5% of the variance in secondary traumatic stress. Conclusion : These findings suggest the importance of improving resilience and job satisfaction to enhance the professional quality of life in ICU nurses. Moreover, creating an innovation-oriented culture rather than a hierarchical and task-oriented culture can effectively improve the professional quality of life of ICU nurses.

중환자실 간호사의 신체보호대 간호수행의 영향요인 (Factors Influencing Nursing Practice for Physical Restraints among Nurses in the Intensive Care Unit)

  • Purpose : This study aimed to identify the factors influencing physical restraint-related practice among nurses working in the intensive care unit (ICU). Methods : The participants consisted of 169 ICU nurses in three general hospitals in B and U cities. Data were collected from December 2021 to January 2022 through a structured self-administered questionnaire. Demographic characteristics, physical restraint-related practice, Person-centered Critical Care Nursing (PCCN), work environment, and attitudes toward physical restraint use were measured. For data analyses, hierarchical multiple regressions were conducted using SPSS/WIN 25.0. Results : Clinical careers in the ICU, better work environments, higher PCCN levels, and more positive attitudes toward physical restraint use were associated with a better practice of physical restraints, which together explained 35.5% of the total variance of the outcome. Conclusion : Our findings suggest that to promote a safe physical restraint-related practice among ICU nurses, it is important to improve the nursing environment, prepare guidelines for applying PCCN, and provide education for endorsing positive attitudes toward the use of physical restraints.

중환자실과 응급실 간호사의 소진과 공감역량이 직무만족도에 미치는 영향 (Burnout and compassion competency to explain job satisfaction among nurses working in intensive care units or emergency rooms)

  • Purpose : This study aimed to examine the relationships of burnout and compassion competency with job satisfaction among nurses working in the intensive care units and emergency rooms. Methods : A correlational survey was conducted from August 1 to August 14, 2018 and involved in 102 nurses working at four university hospitals for more than 1 year. A structured questionnaire was used to measure burnout, compassion competency, and job satisfaction, and data were analyzed using hierarchical multiple regression and mediation effect analysis. Results : The mean scores of burnout, compassion competency, and job satisfaction were 2.58, 4.35, and 2.95, respectively. Burnout and compassion competency along with working night shifts and a wish to transfer explained 35.3% of variance in job satisfaction. Compassion competency showed a small but significant mediating effect in the relationship between burnout and job satisfaction (indirect effect -.0446, 95% CI -.1133, -.0075). Conclusion : Burnout and compassion competency were the significant influencing factors of job satisfaction among nurses working in intensive care units and emergency rooms. Since compassion competency showed a significant mediating effect, further studies should focus on intervention strategies for compassion competency to improve job satisfaction in this population.

  • Purpose : This study aims to identify job stress, emotional intelligence, and exhaustion of the nurses in comprehensive nursing service units and then verify the moderating and mediating effects of emotional intelligence on the relationship between job stress and exhaustion. Methods : Participants are 118 nurses working in two general hospitals. The collected data are analyzed using the SPSS WIN 27.0 and AMOS 18.0 programs by assessing frequency and percentage, mean and standard deviation, independent t-test and one-way ANOVA, Scheffé test, Pearson correlation coefficient, and hierarchical multiple regression analysis and bootstrapping. Results : Exhaustion has a statistically significant positive correlation with job stress (r=.40, p <.001) and a statistically significant negative correlation with emotional intelligence (r=-.26, p =.004). A partial mediating effect of emotional intelligence is found between job stress and exhaustion, however, there is no moderating effect. Conclusion : It is expected that to reduce the exhaustion of nurses in comprehensive nursing service units, it is necessary to assess job stress and emotional intelligence, and strengthen emotional intelligence along with job stress intervention.

중환자실 간호사의 임상 적응 경험 (Experience of Clinical Adaptation among Nurses in Intensive Care Unit)

  • Purpose : This study aimed to explore and describe intensive care unit (ICU) nurses' experience of clinical adaptation. Methods : The participants were 14 ICU nurses with more than two years of working experience in the ICU. Data were collected through in-depth individual interviews conducted between July and October 2021. Theoretical sampling was used to the point of theoretical saturation. Data were analyzed using the Strauss and Corbin method. Results : A total of 79 concepts, 37 subcategories, and 16 categories were identified through open coding. Axial coding based on the paradigm model revealed that the central phenomenon was "The harsh adversity faced in the nursing field where life and death are determined" and the core category was "Enduring the adversity of caring for critically ill patients and achieving self-realization." ICU nurses' clinical adaptation process was explained in five phases: "confrontation period," "turbulent period," "seeking period," "struggling period," and "stabilized period." The five phases that affect interventional conditions were "Support from reliable people," "Recognition of administrative and financial support." Conclusion : This study provided novel insights for a comprehensive understanding of ICU nurses' clinical adaptation processes. Furthermore, the findings are expected to be used as basic data to develop multifaceted strategies to help ICU nurses' adaptation to critical care.

중환자실 신규 간호사의 의사소통 상황 관련 교육 요구도 조사 (A Survey on Situation-related Communication Educational Needs for Novice Intensive Care Unit Nurses)

  • Purpose : This study sought to investigate novice nurses' communication education needs in the intensive care unit (ICU) using Importance-Performance Analysis (IPA) and Borich's need assessment model. This study identified communication challenges in clinical settings to develop a simulation program that enhances communication competencies based on educational requirements. Methods : A descriptive research design and a self-report questionnaire were used. The latter was developed and administered to 121 novice nurses with less than one year of experience in the ICU at various university hospitals in Korea. Data were collected via the online open chatroom from June 24th to July 28th, 2023. The communication education needs were identified using descriptive statistics, t-tests, IPA, and Borich's needs assessment model. Text analysis was used to categorize the participants' communication experience. Results : The results revealed that "communication with physicians," "communication with patients," and "communication with nurse on another shift" domains contained the most substantial educational needs for novice nurses working in the intensive care units. Conclusion : The results provide fundamental data for developing and enhancing customized communication education programs for novice ICU nurses. This valuable information could help ICU nurses and educators improve new nurses' communication skills, which would ultimately contribute to the advancement of nursing education and clinical practice.

중환자실 간호사의 임상추론역량, 긍정심리자본, 간호근무환경이 간호업무성과에 미치는 영향 (Factors Influencing Nursing Performance of Intensive Care Unit Nurses: Role of Clinical Reasoning Competence, Positive Psychological Capital, and Nursing Work Environment)

  • Purpose : This study aimed to investigate the impact of clinical reasoning competence, positive psychological capital, and nursing work environment on nursing performance of intensive care unit (ICU) nurses in tertiary general hospitals. Methods : Data were collected from 169 ICU nurses in tertiary general hospitals in Busan, South Korea, from July 27 to August 15, 2023, using a self-report questionnaire. After excluding 12 questionnaires with insufficient responses, 157 responses were analyzed. Descriptive statistics, independent t-test, one-way ANOVA, Pearson's correlation coefficient, and stepwise multiple regression analysis were employed. Results : Factors influencing nursing performance included clinical reasoning competence (β=.51, p <.001), hope as a subfactor of positive psychological capital (β=.31, p <.001), and the nursing foundations for quality of care in the nursing work environment (β=.21, p <.001). The explanatory power of the regression model was 62% (F=87.41, p <.001). Conclusion : The study underscores the importance of improving clinical reasoning competence among ICU nurses to enhance nursing performance and foster a hopeful attitude through reflective practices and personal growth. Organizational interventions, such as educational and psychological support programs, along with creating a supportive nursing environment, are crucial for improving nursing performance among intensive care unit nurses.

간호수가 누락률 감소활동 (Activity to reduce the record missing for nursing care charge)

  • 문제: 오늘날 간호사는 과거와 달리 현대적 보건의료 요구를 해결할 수 있는 체계적인 이론과 기술을 습득하도록 훈련된 고급 전문 인력이고, 간호행위를 생산하기 위해서는 서비스의 생산원가 뿐만 아니라 전문 인력으로서의 훈련 및 유지비용 등도 투입되어야만 가능하게 함으로서 2008년 7월부터 시행한 중환자실 간호등급에 따른 간호수가 책정으로 인해 중환자실 내에서 이루어지고 있는 많은 간호행위 중 수가를 받을 수 있는 부분이 생기게 되었고 간호행위의 중요성과 더불어 간호를 시행 후 받을 수 있는 수가에 대한 간호사들의 입력에 대한 의무도 늘어나게 되었다. 이에 빈번히 누락되고 있는 간호수가 관리를 통한 경제적 손실을 방지해야 할 필요성을 느껴 개선활동을 하고자 한다. 목적: 2008년 7월부터 시행한 중환자실 간호등급에 따른 간호수가 책정으로 인해 중환자실내에서 이루어지고 있는 간호 행위의 중요성을 인식하고 구체적인 문제 분석과 간호 현장에서의 개선안을 도출함으로써 경제적 손실을 방지할 수 있는 간호수가 누락률을 감소시키고자 함이다. 의료기관: 대구파티마병원 내과 중환자실. 질 향상 활동: 업무개선의 방법으로 환자 개인별 간호수가 입력 누락 방지 체크 리스트 사용 및 처방 전달 시스템의 간호수가 재입력 화면을 이용하여 입력의 용이성 도모하였고 입력된 간호수가를 처방 전달 시스템 간호수가 조회 프로그램을 이용해 익일 누락여부를 매일 모니터링 하였다. 교육 및 홍보활동으로는 간호수가 입력누락 방지를 위한 간호수가 입력 지침을 제작하여 전체간호사는 년 2회, 신규간호사는 개별 교육을 실시하였으며, 월별 간호수가 누락통계를 실명 공고 하여 간호수가 다 빈도 누락 간호사는 추가 개별교육을 실시하였다. 개선효과: 간호수가 입력 프로세스 개선을 통해 간호수가 평균 누락률이 개선 전 6.5%, 개선 후 1.2%로 5.3%의 누락 감소율을 보였으며 역치 5%를 달성하였고 간호수가 누락금액은 개선 전 2,992,752원, 개선 후 590,787원으로 2,401,965원의 누락 효과 비용을 구할 수 있었다. 본 QA활동으로 중환자실 간호수가 전산입력에 대한 체계적인 교육부재와 신규간호사들의 잦은 전산누락이 있어왔으나 표준화 된 체크리스트 지침과 입력확인 작업으로 전산입력 누락률이 감소하는 효과를 가져왔다. 추후 심평원 청구 작업을 하는 부서와 연계되어 실제 청구 누락률에 대한 비교와 간호사 근무연수와 간호수가 입력 누락률에 대한 상관관계 조사를 제언 해 본다.

소아중환자실 부모의 돌봄참여에 대한 간호사의 인식: 내용 분석 연구 (Nurses' Perceptions toward Parent Participation in Pediatric Intensive Care Unit: A Content Analysis)

  • 본 연구의 목적은 부모의 돌봄참여에 대한 소아중환자실 간호사의 인식을 파악하는 것이다. 연구대상은 서울에 위치한 상급종합병원 2곳의 소아중환자실에 근무하고 있는 간호사 5명이며, 일대일 심층 면담을 수행하였다. 자료수집 기간은 2016년 1월부터 2월까지였고 전통적 내용분석을 이용해 자료를 분석하였다. 연구 결과 소아중환자실 부모의 돌봄참여에 대한 간호사의 인식은 5개 범주와 43개 하위범주가 도출되었으며, 5개 범주는 필요성, 의미, 효과, 장애 요인과 촉진방안으로 나타났다. 간호사는 소아중환자실에서 집중 간호를 필요로 하는 자녀의 중증 상태로 인해 부모가 정서적 부담을 경험하며, 면회가 제한된 병원 정책이 이러한 부담을 악화시킨다는 점에서 소아중환자실에서 부모 돌봄참여의 필요성과 그 효과를 인식하고 있었다. 주요한 장애 요인은 부모의 돌봄참여에 대한 간호사의 지식과 역량의 부족, 과중한 업무 부담, 소아중환자실과 병원 내 정책이나 지침의 부족으로 나타났다. 촉진방안은 간호사 대상 부모 돌봄참여의 의미와 실천방안에 대한 교육 제공, 간호사뿐 아니라 부모를 포함하여 인식개선 활동, 전담 전문가를 지원하는 방안으로 나타났다. 본 연구의 결과를 기반으로 향후 소아중환자실에서 부모의 돌봄참여를 통합적으로 도입하기 위한 간호 중재의 개발과 효과 검증을 위한 후속 연구가 필요할 것이다.

의료인을 위한 구강점막욕창 시각적 감별도구의 신뢰도 (Reliability of the Visual Discrimination Scale on Oral Mucosa Pressure Ulcer for Healthcare Providers)

  • 본 연구는 구강점막욕창의 시각적 감별도구에 대한 신뢰성을 평가하기 위한 연구이다. 연구의 절차는 개발단계와 평가단계로, 개발단계에서는 자료수집, 자문단의 타당도와 신뢰도 평가, 최종도구개발이 수행되었으며, 평가단계에서 대상자들에게 9장의 사진도구를 평가하게 하였다. 세 군에서 측정자 간 신뢰도를 구한 결과, 상처 전문간호사군에서는 Krippendorff's alpha 가 .75이었고, 중환자 전문간호사군에서는 .70이었다. 치과의사군에서의 Cohen's weighted kappa값은 .78 로 나타났다. 측정자 내 신뢰도를 살펴본 결과, 중환자실 일반간호사군들의 intra-rater reliability는 .73으로 나타났다. 신뢰도가 입증된 9장의 사진은 실제 중환자실과 같은 구강점막욕창 위험을 가진 환자를 돌보는 임상환경에서 교육용으로 활용될 수 있을 것으로 보인다.
