Purpose: The purpose of this study was to investigate the emotional response of family members of physically restrained patients in the intensive care units (ICUs). Methods: The study subjects were 200 family members of ICU patients who had been on physical restraints in two university hospitals. Data were collected using the "Instrument of family's emotional response toward physically restrained patients". Results: The mean score of familial emotional response was 2.69 out of a possible 5. The subcategory of acceptance was the highest with 3.56 points followed by depression (3.02), helplessness (2.94), anxiety (2.87), shock (2.74), avoidance (2.64), and grudge (2.08). Multiple stepwise regression analysis indicated that the age of family members, side effects of restraints, and information provision were the variables influencing on negative emotional response of family. Conclusion: Family members showed slightly negative emotional response toward the physical restraints. This finding could be influenced by their limited knowledge of the need for the restraints. Educational programs or fact sheets to be given to family members may be helpful.
Purpose: The purpose of the study was to provide deep understanding of the reported experiences of families with their loved one in the intensive care unit (ICU), focusing on interactions with healthcare providers. Methods: The data were collected by individual interviews of eleven participants. The transcribed data were analyzed using qualitative content analysis to identify major themes and sub-themes that represented the experiences of families. Results: Five themes and 13 sub-themes emerged. "Captive of patients' delayed death: Fear and anxiety" describes psychological problems arising when the family member became critically ill enough to warrant being admitted to the ICU. "Families as the weak: Suppression and resistance" describes interpersonal difficulties arising due to lack of information and trust with healthcare providers. "Deprivation of authority and duty as families: Helplessness" illustrate situational barriers in attempting to protect and support family member. "Re-establishment of trust relationship with healthcare providers: Gratitude and appreciation" describes how they satisfied with themselves by regaining trust relationship. Lastly, "Acceptance of reality through direct care participation: Relief and peace" illustrates peace of mind by gaining sense of reality through active direct care participation. Conclusion: This study demonstrates the positive and negative experiences of families with ICU patients. The results will be useful in developing family-centered nursing interventions.
Purpose: The purpose of this study was to identify the effects of family visits upon the stress response of patients and their families, Methods: This study was the interrupted time series design, The subjects consisted of 197 patients and 197 family members in the cardiac intensive care unit of S Hospital in Bucheon. Physiological stress responses such as blood pressure, heart rates, respiration rates, and oxygen saturation were measured using HP monitors. VAS was used to measure the emotional stress. Collected data was analyzed using repeated measure ANOVA, t-test by SPSS 17.0 statistical program. Results: The family visits did not change patients' blood pressure, pulse rate, respiration rate and oxygen saturation, However the anxiety level of patients and their family members were decreased significantly during family visits. Furthermore, 30-minute family visit reduced more effectively patient's anxiety than 15-minute family visit. Conclusion: Family visits need to be used as a means of nursing intervention to ease the emotional stress of patients and their families. In addition, increasing of visiting time should be considered.
Purpose: The purpose of this study was to understand the meanings and nature of ICU admission experienced among patients with critical illness. The present study adopted a hermeneutic phenomenological method which was developed by van Manen. Methods: The participants for this study were 6 men and 3 women, who were over the age of 20 with ICU admission period more than 3 days. Data were collected by using in-depth interviews and observations from March, 2007 to September, 2007. The contents of the interviews were tape-recorded with the consent of the subject. Results: The essential themes that fit into the context of the 4 existential grounds of body, time, space and other people were as follows: a body that cannot react the way it wants, a wave of fear and insecurity everywhere, a struggle to survive, coming out from death's door, loss of time path, a long and continued waiting until escaping, more of machinery room than a patient's room, existence of life and death, an abyss of suffering seen thru another patient, taken care of by a doctor, trust and distrust, family, the ultimate safe zone. Conclusion: Critically ill patients in ICU experienced feelings of discomfort, unsafety, and insecurity. The result of this study can give nurses some insight into these experiences and help promote empathetic care.
Purpose: The purpose of this study was to identify the risk factors for a nosocomial urinary tract infection in intensive care units with a foley catheterization which showed a positive urine culture. Method: Three-hundred eighty-seven patients were included in the study. A retrospective review of the electrical medical record system's databases and medical record sheets in hospitalized patients from January 2003 to December 2003 was used. The collected data was analyzed by descriptive statistics, t-test, chi-square test and logistic regression analysis. Result: The frequency of the participants' nosocomial urinary tract infection was 72.9%. Significant risk factors for a nosocomial urinary tract infection were 'age', 'place of catheter insertion', 'frequency of catheter change', and 'duration of catheterization'. These variables explained 18.4% of variance in the experience of nosocomial urinary tract infection in intensive care units with foley catheterization. Conclusion: Medical personnel can decrease the incidence of a nosocomial urinary tract infection by recognizing and paying attention to the duration of catheterization, frequency of catheter change, and place of catheter insertion. As a result, specific and scrupulous strategies should be developed to reflect these factors for decreasing nosocomial urinary tract infections.
Purpose: The purpose of this study was to identify vancomycin-resistant enterococcus (VRE) colonization rate in patients admitted to the intensive care unit (ICU), associated risk factors and clinical outcomes for VRE colonization. Methods: Of the 7,703 patients admitted to the ICUs between January, 2008 and December, 2010, medical records of 554 VRE colonized and 503 uncolonized patients were reviewed retrospectively. To analyzed the impact of colonization on patients' clinical outcomes, 199 VRE colonized patients were matched with 199 uncolonized patients using a propensity score matching method. Results: During the study period, 567 (7.2%) of the 7,703 patients were colonized with VRE. Multivariate analysis identified the following independent risk factors for VRE colonization: use of antibiotics (odds ratio [OR]=3.33), having bedsores (OR=2.92), having invasive devices (OR=2.29), methicillin-resistant Staphylococcus aureus co-colonization (OR=1.84), and previous hospitalization (OR=1.74). VRE colonized patients were more likely to have infectious diseases than uncolonized patients. VRE colonization was associated with prolonged hospitalization and higher mortality. Conclusion: Strict infection control program including preemptive isolation for high-risk group may be helpful. Further research needs to be done to investigate the effects of active surveillance program on the incidence of colonization or infection with VRE in the ICU.
Objectives: The purpose of this study was to investigate the oral health and hygiene in the neurosurgical patients in intensive care unit(ICU). Methods: The subjects were 92 neurosurgical patients in intensive care unit(ICU) from March, 2011 to December, 2012. The oral examination consisted of number of residual teeth, DMFT index, clinical attachment loss, gingival index, plague index, and Candida species colony of tongue and saliva. Plaque was inoculated from tongue and saliva and incubated in 36.5C incubator for 48 hours using $Dentocult^{(R)}$ CA(Orion Diagnostica, Espoo, Finland). Glasgow coma scale(GCS) was measured to evaluate the consciousness of the patients on the basis of medical record. Results: Oral health was poor in clinical attachment loss and gingival index. Oral hygiene in neurosurgical patients in ICU was very poor due to high plaque index and Candida colonization of tongue and saliva. Plague index was closely related to Candida colonization of tongue and saliva(p<0.05). Conclusions: Oral health and hygiene of patients in neurosurgical ICU were very poor. More careful oral hygiene care is very important and necessary to enhance the oral health improvement of the neurosurgical patients in ICU.

Purpose: The purpose of this study was to examine the effects of prior information about ICU environment on the anxiety and environmental stress of cardiac surgery ICU patients. Methods: A non-equivalent control group non-synchronized quasi-experimental research design was used. Participants were 60 (control 30, experimental 30) patients who had been admitted to ICU. Prior information about the ICU environment was provided to the experimental group. The anxiety level of subjects was measured by State-Trait Anxiety Inventory (STAI) and the stress level of subjects was measured by the Intensive Care Unit Environmental Stressor Scale (ICUESS). Data were analyzed using a Chi-square test or a Fisher's exact test, independent samples t-test, and paired samples t-test. Results: There was no difference in Anxiety (t=-0.58, p=.563), but there was a significant difference in environmental stress (t=10.46, p<.001). Conclusion: Providing prior information would be an effective nursing intervention to reduce environmental stress.
This study, aimed to categorize pattern of impatient's families in the ICU and to provide useful data for development of family-supportive program was conducted from June, 12 to Aug., 28, using Q-method. After reviewing the literature the researcher proceeded to interview 4 impatient's families in the ICU, developed the statement from consisting of 38 items, and then 30 impatient's families in the ICU were sampled. The collected data were coded after having been scaled from 1 to 9 points, and analyzed using the PC QUANL program. Three types were identified; The first type focused on self-regret about the impatient, the second type of families had some concerns for their health, and the third type was thinking of preparing for their future-lives, such as insurance and savings. It was common to feel powerlessness, regret, compassion, grief and anxiety in all types. They wanted to be given some information, the prognosis of patient and the way to overcome. The study will offer better understanding for family-centered approach and be useful for the system of orientation about impatients and their families.
Purpose: This study was conducted to analyze and clarify the meaning of the concept for relocation stress -focusing on patients transferred from an intensive care unit to a general ward. Methods: This study used Walker and Avant's process of concept analysis. Results: Relocation stress can be defined by these attributes as follows: 1) involuntary decision about relocation, 2) moving from a familiar and safe environment to an unfamiliar one, 3) broken relationship of safety and familiarity, 4) physiological and psychosocial change after relocation. The antecedents of relocation stress consisted of these facts: 1) preparation degrees of transfer from the intensive care unit to a general ward, 2) pertinence of the information related to the transfer process, 3) change of major caregivers, 4) change in numbers of monitoring devices, 5) change in the level of self-care. There are consequences occurring as a result of relocation stress: 1) decrease in patients' quality of life, 2) decrease in coping capacity, 3) loss of control. Conclusion: Relocation stress is a core concept in intensive nursing care. Using this concept will contribute to continuity of intensive nursing care.
