Validity and Reliability of the Korean Version of Advance Directives Attitude Scale for Nurses (한국어판 사전연명의료의향에 대한 간호사 태도 도구의 타당도와 신뢰도)

  • Purpose : This study aimed to verify the validity and reliability of the Korean version of the nurses' attitude scale toward advance directives of patients. Methods : We translated and back-translated the original tool consisting of 20 questions and tested and verified its content validity. Questions for which its content validity has been verified, a preliminary investigation was conducted among 20 nurses working in ICU, followed by the actual investigation. Data were analyzed using SPSS version 24.0 for Windows and Mac and AMOS version 24.0. To verify the validity, an item analysis was conducted for all 398 samples, and then an exploratory factor analysis for 200 samples that were randomly selected, followed by a confirmatory factor analysis for the remaining 198 samples. Results : Korean version of advance directives attitude scale (K-ADAS) consisting of 14 questions - 7 questions on 'patient rights,' 4 questions on 'role of a nurse,' and 3 questions on 'ethical judgment' - was verified its validity and reliability. Conclusion : In this study, the validity and reliability of the K-ADAS have been verified. We expect the verified tool to be useful in various fields that measuring the nurses' attitude toward advance directives of patients.

Burnout and compassion competency to explain job satisfaction among nurses working in intensive care units or emergency rooms (중환자실과 응급실 간호사의 소진과 공감역량이 직무만족도에 미치는 영향)

  • Purpose : This study aimed to examine the relationships of burnout and compassion competency with job satisfaction among nurses working in the intensive care units and emergency rooms. Methods : A correlational survey was conducted from August 1 to August 14, 2018 and involved in 102 nurses working at four university hospitals for more than 1 year. A structured questionnaire was used to measure burnout, compassion competency, and job satisfaction, and data were analyzed using hierarchical multiple regression and mediation effect analysis. Results : The mean scores of burnout, compassion competency, and job satisfaction were 2.58, 4.35, and 2.95, respectively. Burnout and compassion competency along with working night shifts and a wish to transfer explained 35.3% of variance in job satisfaction. Compassion competency showed a small but significant mediating effect in the relationship between burnout and job satisfaction (indirect effect -.0446, 95% CI -.1133, -.0075). Conclusion : Burnout and compassion competency were the significant influencing factors of job satisfaction among nurses working in intensive care units and emergency rooms. Since compassion competency showed a significant mediating effect, further studies should focus on intervention strategies for compassion competency to improve job satisfaction in this population.

Factors Influencing Radiation Protection Behavior of Nurses in Intensive Care Units (중환자실 간호사의 방사선 방어 행위 영향 요인)

  • Purpose : The purpose of this study was to identify the factors that influence the behavior of intensive care unit (ICU) nurses regarding radiation protection, and to provide basic data to improve radiation protection educational programs. Methods : This descriptive research study included 143 nurses working in ICUs. Data were collected through a self-reported structured questionnaire and analyzed using t-test, Pearson's correlation coefficients, and hierarchical multiple regression analysis. Results : Radiation protection environment (β=.35, p =.001), radiation protection self-efficacy (β=.25, p =.002), type of hospital (β=-.25, p =.002) and experience in ICU (β=-.18, p =.022) affected the radiation protection behavior of ICU nurses. The total explanatory power of model was 60.0% (R2=.64, Adj. R2=.60, F=17.71, p <.001). Conclusion : In order to enhance the standards of radiation protection behavior among intensive care unit nurses and improve radiation protection self-efficacy, educational and training programs should be developed. Additionally, administrative support should be considered to create environments with better radiation protection at the organizational level.

Validity and Reliability Tests of Neonatal Patient Classification System Based on Nursing Needs (간호요구 정도에 의한 신생아중환자 분류도구의 타당도 및 신뢰도 검증)

  • Purpose: This study was done to verify validity and reliability of a neonatal patient classification system (NeoPCS-1). Methods: An expert group of 8 nurse managers and 40 nurses from 8 Neonatal Intensive Care Units in Korea, verified content validity of the measurement using item level content validity index (I-CVI). The participants were nurses caring for 469 neonates. Data were collected from November 11 to December 14, 2011 and analyzed using descriptive statistics, ANOVA, intraclass correlation coefficient, and K-cluster analysis with PASW 18.0 program. Results: Nursing domains and activities included 8 items with 91 activities. I-CVI was above .80 in all areas. Interrater reliability was significant between two raters (r=.95, p<.001). Classification scores for participants according to patient types and nurses' intuition were significantly higher for the following patients; gestational age (${\leq}29$ weeks), body weight (<1,000 gm), and transfer from hospital. Six groups were classified using cluster analysis method based on nursing needs. Patient classification scores were significantly different for the groups. Conclusion: These results show adequate validity and reliability for the NeoPCS-1 based on nursing needs. Study is needed to refine the measurement and develop index scores to estimate number of nurses needed for adequate neonatal care.

Adaptation of Enteral Tube Feeding (ETF) Nursing Practice Guideline (경장영양요법 간호실무지침 수용개작)

  • Purpose: This study was conducted to develop a Korean version of evidence-based enteral tube feeding (ETF) guidelines through adaptation of existing ETF guidelines. Methods: The guideline adaptation process was conducted into 24 steps according to a manual for guideline adaptation version 2.0 developed by NECA. Results: The adapted ETF nursing practice guideline was consisted of 9 domains and 20 recommendations, including confirmation of tube placement, risk of aspiration, assessment gastric residual volume, body positioning, treating feeding tube occlusion, administration rate, medication, tube flushes, and interruption of feeding. The results of the grading of recommendations assessment by expert penal showed that 8 recommendations in Grade A, 4 in grade B, and 8 in Grade C were emerged from the process. The range of content validity index scores by expert penal was 0.8-1.0. Conclusion: It is expected that the adapted ETF nursing practice guideline could be helpful for nurses to practice evidence-based ETF for their patients.

The Relationship between the Work Environment and Person-centered Critical Care Nursing for Intensive Care Nurses (중환자실 간호사의 근무환경과 인간중심 간호의 관계)

  • Purpose : The purpose of this study was to investigate the effect of the perceived nursing work environment (NWE) on person-centered critical care nursing (PCCN) practices among nurses in intensive care units (ICUs). Methods : A cross-sectional research design was adopted for this study; participants were 126 ICU nurses working in five hospitals in B city. From December 2018 to February 2019, a survey questionnaire on NWE and PCCN were conducted. The collected data were analyzed with a multiple hierarchical regression analysis. Results : The mean score for NWE was $2.98{\pm}0.39$. Among the factors, basic work system was the highest ($3.39{\pm}0.41$), followed by leadership of the head nurse ($3.31{\pm}0.55$), interpersonal relationships ($2.77{\pm}0.62$), and institutional support ($2.58{\pm}0.52$). The mean score for PCCN was $3.52{\pm}0.46$. Among the factors, comfort was the highest ($3.77{\pm}0.62$), followed by respect ($3.55{\pm}0.56$), compassion ($3.43{\pm}0.60$), and individuality ($3.38{\pm}0.56$). The interpersonal relationship (${\beta}=.31$, p=.001) and the number of patients per shift (${\beta}=-.23$, p=.005) had a significant effect on PCCN, accounting for 25.9% of the total variance. Conclusion : These results suggest that it is necessary to enhance the relationship-oriented culture and reduce the workload of nurses in order to improve the quality of nursing care in ICUs.

The Relationship between Person-Centered Nursing and Family Satisfaction in ICUs (중환자실에서의 인간중심 간호와 가족 만족도 관계)

  • Purpose : This study aims to identify the relationship between person-centered critical care nursing (PCCN) and family satisfaction for intensive care unit (ICU) nurses. Methods : This research used a cross-sectional survey. The participants were 142 nurses and 121 family members from 8 ICUs in 4 hospitals in B city. The questionnaire survey was conducted between December 2018 and March 2019. The eight ICUs were divided into those that scored high and those that scored low for person-centered care relative to the average score for PCCN, and the difference in family satisfaction between the ICUs with high and low scores was compared using t-test. Results : The mean score of PCCN was $3.68{\pm}0.40$ out of 5. In the subcategories of PCCN, "comfort"was the highest at $3.95{\pm}0.49$, followed by "respect" at $3.73{\pm}0.57$, "compassion" at $3.59{\pm}0.57$, and "individuality" at $3.47{\pm}0.54$. The average score of family satisfaction with the ICUs was $3.45{\pm}0.67$ out of 5. In its subcategories, "emotion" was the highest at $3.69{\pm}0.72$, followed by "information" at $3.61{\pm}0.75$, "participation" at $3.30{\pm}0.79$, and "resources" at $3.20{\pm}0.80$. The family satisfaction ($3.75{\pm}0.54$) with the ICU of the highest-scoring PCCN was significantly higher than the family satisfaction ($3.25{\pm}0.53$) with the ICU with the lowest-scoring PCCN (t=4.97, p<.001). Conclusion: The results suggest that the PCCN scores of ICU nurses should be promoted to improve family satisfaction. It is necessary to pursue a variety of ways to secure the provision of person-centered nursing practice in ICUs.

Mediating Effects of Diet Quality between Meal Frequency and Cardiometabolic Risk among Korean Adults: Data from the 7th Korea National Health and Nutrition Examination Survey (KNHNES) (한국 성인의 식사 빈도에 따른 심혈관대사질환 위험도와 식사 질의 매개효과 : 국민건강영양조사 제7기 자료)

  • Purpose : The 2020 Dietary Guidelines Advisory Committee specifically noted that meal frequency is associated with risks for cardiovascular disease, type 2 diabetes, and all-cause mortality, although the current evidence on meal frequency is conflicting. As meal frequency itself is affected by various factors, the aim of the study was not only to examine its relationships with cardiometabolic risk but also to identify the mediating effects of dietary quality. Methods : This study used a descriptive correlational design. In all 8,141 healthy adults participated in the study. Measurements included meal frequency, cardiometabolic risk, and diet quality. Data were analyzed using descriptive statistics, Pearson's correlation coefficient analysis, and process macro bootstrapping model 4. Results : The meal frequency was 3.52±0.61 times per day, the risk of cardiovascular metabolic diseases was 0.01±0.61 points, and the diet quality was 62.08±13.87 points. In mediation analysis, the effect of meal frequency on cardiometabolic risk score was completely mediated by diet quality. Conclusion : Improved diet quality in healthy adults should be considered when designing meal frequency interventions aimed at reducing their cardiometabolic risk, as the effect of meal frequency support on cardiometabolic risk was found to be mediated by diet quality.

Awareness of good death, perception of life-sustaining treatment decision, and changes in nursing activities after decision to discontinue life-sustaining treatment among nurses in intensive care units at tertiary general hospitals (상급종합병원 중환자실 간호사의 좋은 죽음인식, 연명의료결정 인식 및 연명의료중단 결정 후 간호활동의 중요도 변화)

  • Purpose : This study aims to explore nursing activities after the decision to discontinue life-sustaining treatment, awareness of a good death, and perception of life-sustaining treatment decisions among nurses in intensive care units (ICUs) at tertiary general hospitals. Methods : Participants were 173 nurses working in two tertiary general hospitals. The data were collected using structured questionnaires and analyzed using an independent t-test, paired t-test, one-way ANOVA, Scheffé's test, and Pearson's correlation coefficient. Results : Participants were 173 nurses working in two tertiary hospitals. The nursing activity increase was the greatest in the spiritual domain, and the physical domain was where the activities decreased the most. There were significant associations between Awareness of good death (Clinical) and Perception of life-sustaining treatment decision(r=.26, p <.001), Awareness of good death (Closure) and Perception of life-sustaining treatment decision(r=.36, p <.001), and Awareness of good death (Personal control) and Perception of life-sustaining treatment decision(r=.49, p <.001). Conclusion : Based on the results, systematic education programs and job training are required to improve the awareness regarding good death and perception of life-sustaining treatment decision for nurses in ICUs where discontinuing life-sustaining treatment decisions are made.

Reflections on the Prospects of Korean Advanced Practice Nurses : Based on Flexner's Professional Characteristics (한국 전문간호사의 전망에 관한 고찰: Flexner의 전문직 특성을 기반으로)

  • Purpose : This study explores the professional status of Advanced Practice Nurses (APNs) in Korea, who, despite being legally certified, face instability in their professional standing, including their scope of practice and compensation. Method : The study uses Flexner's professional characteristics as a framework to analyze and project the future trajectory of Korean APNs. Results : First, to ensure social accountability, professional bodies need to establish uniform nursing policies related to job roles, and healthcare institutions must adhere to these policies. Second, nursing leaders should spearhead the creation of nursing knowledge essential for the profession's advancement, aiming to establish it as the foundation for nursing practice through a consensus process within the nursing community. Third, the curriculum for APNs should enhance the quantitative and qualitative aspects of practice in response to societal needs. Fourth, professional bodies should formulate consistent nursing policies based on a thorough analysis of the healthcare environment and legal considerations, and guide their implementation in clinical practice through a consensus process within the nursing community. Lastly, guidelines should be established for professional standards suitable for the Korean context. Conclusion : Based on this review, it is recommended that all APNs adhere to the professional standards set by their respective organizations, actively participate in personal quality improvement initiatives, and fulfill their duties and roles as members of these professional bodies. Furthermore, these organizations should devise practical strategies to solidify the APN system and should spearhead a systematic consensus process that garners the agreement of all members within the nursing community.
