  • Dizziness is one of the common symptoms in many patients. The disorders of the labyrinth, vestibular nerve, vestibular neclei, or their central connections are responsible for practically all vertigo. Most disorders of the central connections are the vascular diseases and tumors. This study is based on the clinical consideration of one patient who sufferd from the cerebellar infarction and the upper respiratory infection. The patient, 61 - year - old man was diagnosed as the cerebellar infarction has been troubled with dizziness, a headache, a sore throat, a cough etc. The symptoms are classed as the Sanchopungyeul.(上焦風熱) Pungdam(風痰), and we prescribed Qingyanligetang(淸咽利膈湯) for him and his symptoms took a tum for the better.

Central Nervous System Complications of Coronary Artery Bypass Grafting - Comparison Between Off-Pump CABG and Conventional CABG (관상동맥 우회술 후의 중추신경계 합병증 - 심폐바이패스를 사용하지 않은 관상동맥 우회술과 기존의 관상동맥 우회술의 비교)

  • Background: Central nervous system complication after coronary artery bypass grafting(CABG) is one of the major prognostic determinants and the use of the cardiopulmonary bypass(CPB) may increase the incidence of this devastating complication. In this study, the outcomes after off-pump CABG were studied and compared with those following the conventional CABG using CPB. Material and Method: Among the consecutive isolated CABG's performed in SNUH during Feb. 1995 and Jun. 1999, 338 coronary artery bypass grafting were divided into two groups. 223 patients underwent CABG using the CPB(Group I), and 115 patients underwent CABG without CPB(OPCAB)(Group II). All patients enrolled in this study received extensive preoperative examinations including thorough neurologic examination before and after surgery, transcranial doppler study, carotid duplex ultrasonography, and magnetic resonance angiography if necessary. Central nervous system(CNS) complications were defined as stroke, seizure, metabolic or hypoxic encephalopathy and transient delirium after surgery. Result: There were 61 cases(27.3%) who developed postoperative CNS complication in Group I, whereas 8 cases(7.0%) of CNS complications developed postoperatively in group II(p<0.05). Statistically significant predictors of postoperative CNS complications in group I were age and the use of cardiac assist devices perioperatively. Conclusion: This study suggested that omitting the use of CPB in CABG resulted in significant decrease of the postoperative CNS complications. OPCAB should be more widely applied especially to the elderly who have preexisting cerebrovascular disease.

  • 폐흡충 감염자에서 나타나는 중추신경계의 병변을 모두 폐흡충이 중추신경계를 침범하여 발생한 것이라고는 할 수 없다. 그러므로 이 경우 원인진단을 위해서는 뇌척수액에 나타나는 폐흡충 특이항체의 측정이 필요하다고 생각된다. 우리는 확인된 뇌폐흡충중 2례와 척수 스파르가눔증 1례, 뇌병변이 없는 폐흡충증 환자 1례와, 기타 중추신경계 질환 환자 10례에서 얻은 뇌척수액을 희석하지 않고 면역효소진단법으로 특이항체를 측정하였다. 그 결과 흡광도 0.25를 양성 기준으로 하면 뇌 폐흡퉁증을 진단할 수 있다고 생각하게 되었다. 폐흡충중 환자 2례의 흉막삼출액에서 특이 IgG 항체가는 혈청에서의 측정치와 다르지 않아, 흉막삼출액도 폐흡충증의 진단에 이용할 수 있다고 생각되었다.

Comparison of Clinical Characteristics and Polysomnographic Findings between REM Sleep Behavior Disorder with and without Associated Central Nervous System Disorders (중추신경계질환 동반 여부에 따른 렘수면 행동장애의 임상 특성과 수면다원기록소견 소견 비교)

  • Objectives: REM sleep behavior disorder (RBD), characterized by excessive motor activity during REM sleep, is associated with loss of muscle atonia. In recent years, it has been reported that RBD has high co-morbidity with CNS disorders (especially, Parkinson's disease, dementia, multiple system atrophy, etc.). We aimed to assess differences in clinical and polysomnographic findings among RBD patients, depending on the presence or absence of central nervous system (CNS) disorders. Methods: The medical records and polysomnographic data of 81 patients who had been diagnosed as having RBD were reviewed. The patients were classified into two groups: associated RBD (aRBD, i.e., with a clinical history and/or brain MRI evidence of CNS disorder) and idiopathic RBD (iRBD, i.e., without a clinical history and/or brain MRI evidence of CNS disorder) groups. Twenty-one patients (25.9%) belonged to the aRBD group and 60 patients (74.1%) belonged to the iRBD group. The clinical characteristics and polysomnographic findings of the two groups were compared. Results: Periodic limb movement disorder (PLMD), i.e., PLMI (periodic limb movement index)>5, was observed more frequently in the aRBD group than in the iRBD group (p<0.001, Fisher's exact test). Also, obstructive sleep apnea syndrome (OSAS), i.e., RDI (respiratory disturbance index)>5, was found more frequently in the aRBD group (p=0.0042, Fisher's exact test). The percentages for slow wave sleep and sleep efficiency were significantly lower in the aRBD group than in the iRBD group. Conclusion: We found that 1 out of 4 RBD patients had associated CNS disorders, warranting more careful neurological evaluation and follow-up in this category of RBD. In this category of RBD patients, we also found more frequent PLMD and OSAS. These patients were also found to have lower slow wave sleep and sleep efficiency. In summary, RBD patients with associated CNS disorders suffer from more disturbed sleep than those without them.

Review on the Japanese Super-Core Port Policy - An Assessment and New Politic Demands - (일본 슈퍼중추항만정책의 성과와 한계에 관한 고찰)

  • Japanese government has instituted and carried out Super-core Ports Policy to improve their operating efficiencies of international container ports from 2004. Despite of diverse institutional and administrative supports within this policy, this paper has assessed that they couldn't accomplished goals at the end of 2009 when the policy ended. In short, Japanese Super-core ports have explicitly lost the competitiveness for functioning as a hub-port in the East Asia region. We could find some defaults in the policy i.e. some limits of the private terminal operating units, inefficiencies in teaming between ports within Super-core port, lacking with system for collecting cargoes. In 2010, Japan is going to reinforce International Strategic Container Port Policy following Super-core Ports Policy. With this policy, they ought to prepare for the last leap on the basis of selection and concentration rules in international port. This new policy is particularly focused on recapturing their T/S cargo via Busan port. Regarding these changes, Busan port needs to prepare counter measurements for preserving Japanese T/S cargo firmly.

Case of Treatment Using Adaptive Servo-Ventilation in a Patient with Central Sleep Apnea after a Lateral Medullary Infarction (외측 연수 경색에 의해 발생한 중추성 수면 무호흡 환자의 자동-적응형 양압기를 사용하여 치료한 경험)

  • Central sleep apnea (CSA) is characterized by respiratory failure of at least 10 seconds without any effort of the chest and abdomen in the absence of upper airway resistance during sleep. In this case, the patient experiences respiratory failure that does not meet the CSA diagnostic criteria and CSA symptoms. Magnetic resonance imaging diffusion-weighted imaging (MRI DWI) scans revealed a lateral medullary infarction. Continuous positive airway pressure (CPAP) was applied as a primary treatment for CSA and respiratory failure. During the titration of CPAP, the apnea-hypopnea index (AHI) and arousal index (AI) were worse than the results before its use (AHI: 42.5/hr→82.8/hr, AI: 21.7/hr→40.8h). As a result, adaptive servo-ventilation (ASV) was chosen as the secondary treatment. Compared to the night-polysomnography results before the ASV treatment, the AHI improved (42.5/hr→8.6/hr). Therefore, ASV is a potential treatment for CSA and respiratory failure in these patients.

Necrotizing Primary Angiitis of the Central Nervous System Mimicking Brain Abscess: A Case Report and Literature Review (뇌농양을 모방한 괴사성 원발성 중추신경계 혈관염: 증례 보고와 문헌 고찰)

  • Primary angiitis of the central nervous system (PACNS) is a rare vasculitis in the central nervous system. Herein, we report a case of diagnosis and treatment of necrotic pattern PACNS, which was difficult to differentiate from a brain abscess. A 19-year-old male presented with blurred vision and a headache. Brain MRI revealed irregular rim-enhancing necrotic masses with central diffusion-high signal intensity in the corpus callosum and peripheral diffusion-high signal intensity in the left parietotemporal periventricular area. Susceptibility-weighted imaging revealed multiple punctate hemorrhages in the lesions. The patient was diagnosed with unusual abscess or tumefactive PACNS. Therefore, we initially treated the patient with antibiotics to rule out brain abscess. However, the brain lesions did not improve on follow-up MRI after the antibiotic treatment. Surgical biopsy was performed, and the histopathological diagnosis was PACNS with a necrotic pattern. The necrotic lesions became smaller on follow-up MRI after high-dose corticosteroid treatment.

흰쥐에서 출생 후 납중독에 의한 중추신경계 독성의 선택성 연구ㅤ

  • 출생직후부터 새끼쥐에 유발시킨 납중독이 중추의 특정 신경계에 미치는 신경독성의 선택성 여부를 알아보고자 하였다. 특정 신경계의 한 예로 모노아민성 신경계를 선택하여 납중독의 지표로 모노아민성 신경계의 효소인 MAO (mono-amine oxidase)의활성을 측정하였으며, 비특정 조직의 지표로 Na+K+-ATPase의 활성을 측정하였다. Wistar계의 흰쥐에서 태어난 새끼들에게 출생직후부터 전실험기간을 통해 0.05% 혹은 0.2% 초산납 (PbAc2)용액을 식수로 공급하여 납중독을 유발시켰다. 생후 2, 4, 6 및 8주된 새끼쥐의 MAO 및 Na+K+-ATPase활성을 대뇌, 간뇌, 중뇌, 뇌교-연수 및 다섯부위에서 각기 측정하였다.
