A study on hospice care awareness and user satisfaction with intensive care unit facilities -Centering on capital district- (호스피스 케어인식도와 시설이용 만족도에 관한 연구 -수도권 중심으로-)

  • This study aims to investigate hospice care awareness, user satisfaction with intensive care unit facilities and the difference in hospice care awareness level. A survey of 160 people including the intensive care patients in capital district areas and their family members was conducted from March $10^{th}$ until $July31^{st}$ of 2013. As an analytical method, the SPSSWIN18.0 program was used and the sample size followed Cohen's sample extracting formula of G*Power 3.1, an analytical program for test power. The mean, standard deviation and regression analysis have been statistically carried out. The results indicate that the care awareness of hospices have meaningful relations with age, religion and marital status. As for the satisfaction level with hospices, the facilities in the countryside are preferred to those in the cities, and the hospice rooms with Korean floor heating system is preferred to rooms with bed. The implication of this study is expected to become a basic reference to the improvement of the hospice system, seen from the result of survey with highly favorable response for care units in hospices.

Anxiety, Depression and Quality of Life of Married Women with Urinary Incontinence Living in the Community (지역사회 거주 기혼 여성의 요실금 중증도에 따른 불안, 우울 및 삶의 질)

  • Purpose: The purpose of this study was to compare anxiety, depression and quality of life according to the severity of urinary incontinence in married women. Method: The participants were 168 married women aged 30-65 years who experienced urinary incontinence. The data were collected from May to July, 2006 using a structured questionnaire. Frequencies, percent, means and standard deviation, t-test, ANOVA, $X^2-test$ and Scheffe test with SPSS win 14.0 program were used to analyze the data. Results: The distribution of urinary incontinence severity was mild 87.5%, or moderate 12.5%. Significant differences in the severity of urinary incontinence were found for the general characteristics of age, education level, having a job and having a spouse and for the obstetric characteristics of type of delivery, and menopause. Differences in the score for anxiety(t=-2.41, p=0.001) and quality of life(t=5.50, p<0.001) according to the severity of urinary incontinence were statistically significant. Conclusion: Women with moderate to severe urinary incontinence should be screened for psychosocial factors. Severity of urinary incontinence in married women negatively affects their quality of life. Further research is needed to determine factors predicting the quality of life for incontinent women.

The Severity of the Pediatric Patients admitted at NICU using Therapeutic Intervention Scoring System (환자분류체계를 이용한 NICU 입원 환아의 중증도)

  • This study was attempted to help in explore new direction about classification of the severity of the pediatric patients admitted at NICU. Data were collected from 230 patients who admitted at Neonatal Intensive Care Unit of 3 University hospitals and 1 General hospital during 7 months period from september 1, 2000 to April 30, 2001. The results were as follows: 1. The degree of severity of the pediatric patients admitted at NICU shown ranged 1-102 and averaged 17.7. 2. With the respect to the severity of the pediatric patients admitted at NICU, there were statistically significant relation in passing day(s) to admission(r=-.153, p=.020), hospital day(s)(r-.501, p=.000), gestational age(r=-.354, p=.000), birth weight(r=-.280, p=.000), Apgar score at 1 min and at 5 min(4=-.340, p=.000; r=-.322, p=.000), present body weight(r=-.151, p=.023). 3. The severity of the pediatric patients according to general characteristics, there were significant difference in admitting day of the patients(t=2.339, p=.020), Apgar score at 1min and 5min(F=7.893, p=.000; t=3.568, p=.001).

  • The bronchial varices in mitral stenosis are uncommon and incidentally discovered during bronchoscopy. Although bronchial varices are primarily associated with bronchial or pulmonary disease, the bronchial vein can be dilated with increased pulmonary venous pressure secondary to mitral stenosis. The bronchial varices may present massive hemoptysis. The hemoptysis can be controlled by mitral commissurotomy or mitral valve replacement in case of mitral stenosis. We report a case of bronchial varies in a patient with severe mitral stenosis. The bronchial varices were found incidentally during bronchoscopy and they were nearly disappeared by mitral valve replacement.

A Study on the Relationship between the Breast Self-Examination and the severity of breast cancer (유방암 자가검진 유무와 질병의 중증도와의 관계연구)

  • The purpose of this study is to determine the need of education about BSE for the medical members, to survey about the performance and the knowledge of BSE in patients who operated on subtotal and total mastectomy and to compare the regularity of BSE with the severity of breast cancer. The subjects were 163 patients with operated on mastectomy who participated were interviewed in the S university Hospital. The collected data were analyzed by descriptive statistics and t-test, One-way ANOVA and Pearson Correlation with SPSSWIN program. Results were obtained as follows: 1. There was no statistical difference between in the regular group of the mammography and the regular BSE group(F=0.558, P=0.458). 2. There was statistical difference between the method of detection and the severity of the breast cancer(F=3.359, P=0.011). 3. There was statistical difference between regularity of the BSE and the severity of breast cancer(F=3.301, P=0.019). 4. There was showed higher severity of the breast cancer in the elderly patients(r=0.172, P=0.019). 5. There was showed higher severity of the breast cancer in the lower educational level(r=-0.170, P=0.033). According to this study need to the development of the educational program about the BSE and the future research about the regular BSE of the high risk group in the breast cancer.

Association between Quality of Life and Symptom Severity in Obsessive-Compulsive Disorder Patients Using EQ-5D (EQ-5D를 이용한 강박장애 환자의 삶의 질과 증상 중증도의 연관성 연구)

  • Objectives Obsessive-compulsive disorder (OCD) is a disabling psychiatric disorder, and more attention is recently paid on the quality of life (QoL) in OCD patients. The Euro-QoL-5D (EQ-5D) is a widely used self-report to calculate a single score which represents 'health utilities'. The aim of this study was to assess the health-related QoL for patients with OCD using the EQ-5D and to examine the relationship between health-related QoL and symptom severity. Methods Seventy-three patients with a Diagnostic and Statistical Manual of Mental Disorders (DSM-5) diagnosis of OCD were recruited from the outpatient clinic in Seoul National University Hospital. Symptom severity was assessed using the Yale-Brown Obsessive-Compulsive Scale (YBOCS), and QoL was assessed with the EQ-5D-5L questionnaire. Using Korean valuation study, a single score of QoL was calculated. Results Most of the OCD patients were relatively young (< 45 years) with the mean YBOCS total score of 19.33. The mean EQ-5D score was 0.71 and significantly correlated with symptom severity (r = -0.483, p < 0.001). 25% of the EQ-5D score was predicted by the YBOCS total score (b = -0.011, p < 0.001) by regression analysis. Conclusions OCD patients suffer from lower health-related QoL and QoL significantly decreased as symptom severity increased. The results of the EQ-5D would enable further studies on QoL comparison across medical disease and mental disorders.

Subjective oral dryness and stimulated salivary flow rate in medicated patients in chronic severe psychiatric patients (일부 만성 중증 정신질환자의 약물 복용에 따른 구강건조증상과 자극성 타액분비율)

  • Objectives : The purpose of the study is to investigate the xerostomia in the chronic severe psychiatric patients in Korea because there were few reports on xerostomia in the psychiatric patients. Methods : The subjects were 61 psychiatric patients in the mental hospital by convenience cluster sampling. A self-reported symptom questionnaire was filled out by the three researchers on the basis of medical records by the informed consent. The stimulated salivary flow rate of the patients was measured by saliva sampling. Results : The subjects consisted of 45.9% of male and 54.1% of female. High school graduation accounted for 40.0% and 20.0% did not attend the school. The majority of the patients were medicaid recipients. Schizophrenia accounted for 86.9% and most patients were long term care recipients. A total of 68.9% of the patients suffered from salivary dysfunction. The medication in schizophrenia seemed to decrease the stimulated salivary flow rate and made the patients difficult in chewing and swallowing due to xerostomia and low saliva secretion(p<0.05). Conclusions : Medication in schizophrenic patients caused the salivary dysfunction. So the collaboration between the psychiatry doctors and dental hygienists is very important to improve the salivary secretion in the schizophrenic patients. The continuous and long term care of the xerostomia will help the patients maintain the good oral hygiene.

Personality Assessment Inventory Profiles of Patients with Mild and Severe Traumatic Brain Injury (경증 및 중증 외상성 뇌손상 환자의 성격평가 질문지 프로파일)

  • Objectives:This study was designed to investigate the characteristics of personality changes and emotional distress using the Personality Assessment Inventory (PAI) in patients with traumatic brain injury(TBI), divided into mild (MTBI) and severe (STBI) groups according to the severity of injury. Methods:The subjects were consisted of 25 patients with MTBI, 25 patients with STBI, and 25 normal controls. They were interviewed with the PAI. The data were analyzed by ${\chi}^2$ test, analysis of variance and Tukey test. Results:The results were the followings. First, Negative Impression in validity scales was elevated above cutoff point(T score 70) in both MTBI and STBI groups. Second, the clinical scales of which scores elevated above the cutoff point were Somatic Complaints, Anxiety, Anxiety-Related Disorders, Depression, and Schizophrenia in the MTBI, and Somatic Complaints and Depression in the STBI. Third, the clinical subscales above the cutoff point were Conversion, Somatization, Health Concerns, Affective Anxiety, Physiological Anxiety, Traumatic Stress, Cognitive Depression, Affective Depression, Physiological Depression, Thought Disorder, and Affective Instability in the MTBI, and Health Concerns, Cognitive Depression, Affective Depression, and Physiological Depression in the STBI. Fourth, Suicide Ideation in treatment scales was the only scale above the cutoff point in the MTBI and the others of the treatment and interpersonal scales in the MTBI and all of these scales of the STBI were not elevated above the cutoff point. Fifth, the scales of which scores showed significant difference between the MTBI and the STBI were Somatic Complaints, Anxiety, Depression, and Suicide Ideation, the subscales were Conversion, Somatization, Health Concerns, Affective Anxiety, Physiological Anxiety, Physiological Depression, and Psychotic Experiences. Conclusion:These results suggest that the patients with MTBI had more somatic and anxiety symptoms, depressed mood, and suicidal ideation than the patients with STBI. These characteristics are generally consistent with clinical observation and findings from previous studies of the patients with TBI, and the PAI seems to be a beneficial adjunctive assessment tool for the evaluation of patients with traumatic brain injury.

Two Cases of Severe Eczema Herpeticum after Herbal Medication in Children with Atopic Dermatitis (아토피 피부염 환아에서 한방 치료 후 발생한 중증 포진상 습진(Eczema Herpeticum) 2례)

  • It has been known that the eczema herpeticum is the one of most severe skin manifestation of external herpetic infection. It is developed by skin infection of Herpes simplex Virus(HSV) that superimposed on children with atopic dermatitis, and this could be fatal. The secondary bacterial infection happens frequently in eczema herpeticum. Staphylococcus aureus is the most frequently isolated aerobic strain. The important point of treatment of eczema herpeticum is the immediate start of systemic antiviral treatment and use of antibiotics to decrease the secondary infection. We report two cases of severe eczema herpeticum after herb medical treatment in atopic dermatitis. They have been improved after systemic acyclovir and antibiotic therapy with burn dressing.

A Case of Therapy of Aerosolized Ribavirin in a Leukemia Infant with RSV Infection (항암치료 중 RSV에 감염된 백혈병 영아에서의 리바비린 흡입치료 1례)

  • Respiratory syncytial virus (RSV) is the major cause of lower respiratory tract infection in infants. Life-threatening RSV infection is often reported in young children and immunocompromised hosts. Since there is no report on ribavirin therapy for RSV pneumonia in pediatric cancer patients in Korea, we report one case of RSV pneumonia that developed in an infant with acute lymphoblastic leukemia (ALL). Despite administration of oral ribavirin and intravenous immunoglobulin, the patient's respiratory distress worsened and admission to an intensive care unit was necessary. Chest x-ray showed multifocal consolidation, pneumothorax, and pneumomediastinum. Treatment with aerosolized ribavirin led to significant clinical improvement. The role of aerosolized ribavirin is still controversial, but it might have a therapeutic potential for severe RSV pneumonia in children with leukemia.
