Gender differences in Factors Associated with the Severity of Depression in Middle-aged Adults: An Analysis of 2014 Korean National Health and Nutrition Examination Survey (중년 성인에서 우울 중증도 관련요인의 성별 차이 : 2014년 국민건강영양조사 자료 분석)

  • The purpose of this study is to examine the prevalence of depression in Korean middle-aged adults and to identify the gender difference in factors associated with the severity of depression. The data of 2,241 individuals who participated in the Sixth Korea National Health and Nutrition Examination Survey in 2014 were used. Depressive symptoms were assessed using the Patient Health Questionnaire-9 (PHQ-9), and multivariate ordinal logistic regression analysis was performed. The prevalence rate of moderate to severe depression ($PHQ-9{\geq}10$) was identified at 5.2% (men 3.3%, women 7.0%). The factors associated with the severity of depression in men were younger age, living in metropolitan, unemployed, lower body mass index, and heavily drinking. On the other hand, women were affected by having no spouse, lower income, limited physical activity, and sleeping shorter hours.

  • We reported here on 2 cases of aortic regurgitation (AR) that were due to avulsion of the aortic valve commissure. Aortic valvuloplasty was attempted in both cases. In the 1st case, valvuloplasty was performed with reattaching the commissure using the 5-0 polypropylene continuous suture technique. However, aortic regurgitation recurred and this lead to reoperation on the postoperative $14^{th}$ day. The intraoperative finding revealed a completely re-detached commissure that required mechanical valve replacement. In the second case, we attempted to reattach the commissure using pledgetted multiple transverse mattress sutures with 5-0 polypropylene. Because the leaflet coaptation was incomplete, the aortic valve was replaced with a tissue valve.

Effects of a Web-Based Korean Triage and Acuity Scale Learning Program on Triage Self-Efficacy and Triage Performance Ability for Nurses in Emergency Department (웹기반 한국형 중증도 분류 체계 학습프로그램이 응급실간호사의 중증도 분류에 대한 자기효능감 및 수행능력에 미치는 효과)

  • Purpose: The Korean Triage and Acuity Scale (KTAS) is a tool used to classify the severity and urgency of emergency department (ED) patients, focusing on their symptoms. In consideration of the importance of the KTAS, a web-based learning program has emerged as a new mode of education; it enables ED triage nurses to access it anytime and anywhere, and according to their own learning abilities. This study aimed to develop a web-based KTAS learning program and evaluate its effects on self-efficacy and triage performance ability in ED nurses. Methods: A quasi-experimental design with a non-equivalent control group pretest-posttest was used. The conceptual framework was Bandura's self-efficacy theory. There were 30 participants in the experimental group and 29 in the control group. The experimental group attended an orientation and 4 sessions of a web-based KTAS learning program. The learning program lasted 280 minutes over five weeks, consisting of 40 minutes of orientation and four 60-minute sessions. Results: The scores of self-efficacy, triage performance ability in KTAS level, and chief complaints significantly increased in the experimental group compared to the control group. In addition, the numbers of under-triage in KTAS significantly decreased in the experimental group in comparison to the control group. Conclusion: The results suggest that the learning program was effective in improving ED nurses' level of self-efficacy and triage performance ability (KTAS level and KTAS chief complaint). Accordingly, the web-based KTAS learning program can be applied as an education intervention to improve ED nurses' triage skill.

  • The incidence of degenerative valve disease also increasing with the increasing life expectancy of the elderly population. Rheumatic valve disease is decreasing gradually and the incidence of calcified degenerative aortic stenosis (AS) is growing. Echocardiography is a very important tool for evaluating the prognosis and treatment method as well as the time of operation and diagnosis of heart valve disease. When evaluating valvular heart disease, 2-dimensional echocardiography, which observes all heart valves in detail, should take precedence. Understanding the clinical findings of degenerative valve disease and performing precise echocardiography are extremely important. In addition, an assessment of the severity of aortic stenosis is necessary to determine the surgical indications. An assessment of the severity by echocardiography was explained with three cases of degenerative aortic stenosis. To perform echocardiography accurately, it is necessary to understand degenerative valve disease and its clinical findings accurately.

A Convergence Study about Factors on Disease related Knowledge in Patients with Congestive Heart Failure (울혈성 심부전 환자의 심부전 관련 지식 영향 요인에 관한 융복합적 연구)

  • Purpose: The purpose of this convergence study is to examine the factors influencing disease related knowledge in patients with congestive heart failure. Methods: A descriptive regression design was used and the participants were 100 patients with heart failure from one general hospital in 2015. Data analysis included .multiple regression. Results: The average scores were $9.59{\pm}1.77$ for disease related knowledge. The significant factors affecting disease related knowledge were severity(${\beta}=.32$, p<.001), duration of disease diagnosis(year) (${\beta}=.30$, p=.003). The regression model explained approximately 17.3 % of disease related knowledge. Conclusion: The findings recommend that nurses have to improve health related knowledge of patients with congestive heart failure by considering severity and duration of disease diagnosis.

The Factors Influencing Preparedness on Disaster Nursing among Nursing Students (간호대학생의 재난간호 준비도 영향 요인)

  • The purpose of this study was to identify the factors influencing the disaster nursing preparedness of nursing students. A descriptive survey study was carried out from June 12 to October 16, 2017, the subjects were junior and senior grade nursing students. Data were analyzed by t-test, ANOVA, Pearson's correlation coefficients and multiple regression analysis using the SPSS program. The influencing factors on the disaster nursing preparedness were lower confidence of disaster nursing (β =-.21, p<.001) and disaster nursing knowledge (β=.15, p=002). 10.2% of the variance in disaster nursing preparedness was explained by these two factors on multiple regression analysis. In order to improve the preparedness of nursing students for disaster nursing, nursing students' confidence in disaster nursing should be improved, and a systematic and practical disaster nursing curriculum should be developed.

  • This study was attempted to analyze the status of emergency room use of severe trauma patients using the health insurance system and to understand their characteristics. This study used data from the 'Community-based Severe Trauma Surveillance' investigated from January 1, 2018 to December 31, 2019. As a result, there were differences in the degree of disability after injury and whether treatment(surgery, trauma embolism, transfusion) was performed according to the type of medical insurance (p< .001), it was found that there was a statistically significant difference between the degree of disability before and after damage depending on the type of medical insurance (p< .001). Reviews of the health insurance system located for the well-being of the people should be continued from various angles, and specific improvement plans should be proposed.

Current Strategy in Endovascular Management for Below-the-Knee Arterial Lesions (무릎 밑 동맥의 혈관 내 치료의 최신 지견)

  • The below-the-knee arterial tree is the thinnest of all the leg vessels and is an important path for blood flow to the foot. Hence, lesions including stenosis, especially obstruction, may lead to critical limb ischemia which represents the most severe clinical manifestation of peripheral arterial disease. It is characterized by the presence of ischemic rest pain, ischemic lesions, or gangrene attributable to the objectively proven arterial occlusive disease. Typically, the atherosclerotic disease process involving the below-the-knee arterial tree is diffuse in the majority of patients. The cornerstone of therapy is vascular reconstruction and limb salvage. Revascularization should be attempted whenever technically possible, without delay, in patients presenting critical limb ischemia and when the clinical status is not hopelessly non-ambulatory. Therefore, endovascular treatment can become the gold standard for the full range of patients including below-the-knee, limiting the clinical role of the classically trained surgeons.

Distribution of Disease Severity and Group A Rotavirus Genotypes (VP7 & VP4) in Children with Acute Rotavirus Gastroenteritis (급성 로타바이러스 장염 소아에서 질환의 중증도와 로타바이러스 VP7 & VP4 유전형의 분포에 대한 연구)

  • Purpose: We aimed to study the distribution of rotavirus genotypes (VP7 and VP4) and disease severity of rotavirus gastroenteritis prevalent in our community. Methods: Stool samples were collected from 156 children who were hospitalized with rotavirus gastroenteritis from December 2007 to June 2008. The disease severity of all patients was scored using the Vesikari scale. After extraction of ds-RNA of the rotavirus, cDNA synthesis using reverse transcription and polymerase chain reaction (RT-PCR) and multiplex PCR was performed. Following this, the final identification of genotypes was performed. Results: Of the 156 samples, VP7(G) and VP4(P) genotypes were identified in 147 (94.2%) and 140 (89.7%) samples, respectively. G1 (116 of 147 samples; 78.9%) and P[8] (137 of 140 samples; 97.9%) were the most prevalent, respectively. Of the 138 samples identified of combination types of VP7 and VP4, G1P[8] (111 samples; 80.4%) was the most prevalent. Other combination types varied with very low distribution rates. 9.4% of genotypes were not included in the new vaccines. The disease severity score was $11.8{\pm}3.3$ ($mean{\pm}2SD$). The distribution of disease severity was mild or moderate in 37.8% and severe in 62.2% of patients. Conclusion: The most prevalent genotype combination of rotavirus was G1P[8] and genotypes not included in the vaccines represented 9.4% in our community. Disease severity distribution of hospitalized children with rotavirus gastroenteritis was higher in the severe than in the mild and moderate categories.

Increased Incidence of Endometrial Polyps in Women with Endometriosis; the Association with Severity (자궁내막증 여성에서 증가된 자궁내막용종의 빈도; 질환의 중증도와의 관련성)

  • Objective: The aim of this study was to evaluate the correlation between severity of endometriosis and the incidence of endometrial polyp. Methods: The study population consisted of six hundred thirty-one women who had undergone laparoscopic operation due to infertility, severe dysmenorrhea or ovarian tumors. We divided two groups: 434 women with endometriosis (study group) and 197 women without the disease (control group). The presence of endometriosis was documented by diagnostic or therapeutic laparoscopic operation and the disease severity was scored according to revised The American Fertility Society classification. We confirmed the endometrial polyps by pathologic examination after hysteroscopic polypectomy, and compared endometrial polyp incidence according to severity of endometriosis. Results: There was no significant difference between groups with regard to age, mean duration of infertility. Endometrial polyps were found in 274 women (63.0%) with endometriosis and in 58 controls (29.8%, p=0.0000). The incidence of endometrial polyps differed significantly according to stage of endometriosis. The incidence of endometrial polyps were 77/142 (54.2%), 58/90 (64.4%), 73/108 (67.6%, p<0.05), 66/94 (70.2%, p<0.05) in endometriosis stage I, II, III, and IV. There was a linear correlation between stage of endometriosis and endometrial polyps incidence (p=0.008). Conclusion: Endometriosis is accompanied by endometrial polyps. This results showed positive correlation between severity of the endometriosis and incidence of endometrial polyps. It is the possible mechanism for low pregnancy rate in the severe endometriosis.
