Development and application of non-invasive drug delivery systems utilizing pulse power, and its application to mouse models (펄스파워를 적용한 비침습 약물 전달기 개발 및 마우스 모델로의 적용)

  • Some drugs can offer far better medical effectiveness as it is injected through the intradermal layer of the skin, known as a needle-free injection. However, conventional needle-free devices might deliver a relatively large amount of drug in a just single spot of skin, splitting open the tissue layer structure, which might cause bruising and bleeding. By injecting the small volume with a fast repetition rate in a large surface area of skin, the patient may get much fewer injuries and pain. To achieve that specification, the driving force must be instantaneous and short-pulsed. Such a form of an injection device has been developed but the efficacy of those devices has been rarely examined. Therefore, this study developed the laser-induced microjet device that ejects microjet whose speed is ~310 m/s, during the 400~800 ㎲ of pulse time. The device can eject ~1 µL of the drug at the rate at which each shot repeated 10 shots per second. Using this specification, we evaluated the efficacy of drug injection onto mouse models. After injecting the insulin solution into the mouse model, the blood insulin level is detected, resulting in 20 % of blood insulin level with the ordinary needle syringe injection method.

Economic evaluation of manual therapy for musculoskeletal diseases: a systematic review and narrative synthesis of evidence (근골격계 질환의 수기요법에 관한 경제성 평가: 체계적 문헌 고찰 및 근거 합성 연구)

  • 연구 목적 : 본 연구는 근골격계 질환에 대한 수기치료가 갖는 경제적 효과를 체계적으로 평가해 보기 위해 시행되었다. 연구 방법 : 2017년 2월 2일까지 국내외 관련 문헌을 체계적으로 검색 하였으며, 연관된 체계적 고찰 논문의 참고문헌을 조사하였다. 두 명의 연구자가 독립적으로 문헌을 선택하고, 비뚤림 위험 평가 및 경제성 평가 질 평가, 자료 추출을 시행하였다. 연구 결과 : 총 3,327개의 논문을 검토하여 최종적으로 18개의 무작위 대조 연구가 포함되었다. 경제성 평가는 수기요법과 다른 치료방법간의 유효성 비교를 통해 시행되었다. 요부 통증, 견관절 통증 및 외측 상과염 치료에 있어 침치료, 견인치료, 주사치료, 일반의 치료, 척추 안정화 기법 및 통증관리 치료보다 비용 효과적인 것으로 나타났다. 또한 또한, 수기치료는 경항통, 흉통, 고관절염 혹은 슬관절염, 경추신경병증 및 수부손상 치료에 있어 일반적 치료, 물리치료, 자가 치료 프로그램, 견인 치료에 비하여 효과적인 치료인 것으로 확인되었다. 결론 : 18개의 논문 중 10개의 논문에서 근골격계 질환 치료에 있어 수기치료가 경제적으로 효과가 있는 것으로 나타났다. 이러한 결과는 다른 치료방법에 비해 수기요법이 근골격계 질환에 있어서 경제적 가치가 있음을 시사하는 내용이다. 하지만, 근골격계 질환 수기치료의 경제성 평가에 대한 선행 연구는 전반적으로 부족한 실정으로, 보다 정확한 결과를 제시하고 정책결정을 위한 효과적인 제안을 위해서는 체계적인 추가 연구가 필요하다 사료되는 바이다.

Conservative and Early Arthroscopic Treatment of Calcific Tendinitis (석회화 건염의 보존적 치료와 조기 관절경적 치료 결과)

  • Purpose: We evaluated the level of pain and clinical presentation of calcific tendinitis after treatment with conservative and early arthroscopic operation. Materials and Methods: We reviewed 30 patients with calcific tendinitis with the minimum of 6 months of follow up period, treated from February 2002 to May 2008. We implemented Constant-Murley score to evaluate the pain of calcification and improvement of range of motion at the 2nd week, 12th week and 24th week with the patients who had treatment with steroid injection in 15 cases and operation in the other 15 cases. Results: Evaluating the level of pain using Constant-Murley score, we found that conservative treatment group had the scale of 3.2 before treatment and 13.6, 14.5 and 14.7 at 2nd, 12th, and 24th week, respectively since started treatment. The operation group, on the other hand, had the scale of 3.3 preoperatively and 10.2, 13.0, and 14.3 at 2nd, 12th and 24th week postoperatively. The range of motion in the conservative treatment group showed 14.3 on average before the treatment and 21.7, 31.3 and 35.7 at 2nd, 12th and 24th week after treatment. The operation group had 14.4 on average preoperatively, and 33.1, 35.8 and 36.4 at 2nd, 12th and 24th week postoperatively. The operation group had statistically significant improvement compared to the conservative group at the 2nd and 24th week, but showed no difference between the two groups at the final follow up. Conclusion: Early arthroscopic treatment of calcific tendinitis seems to be an excellent option that can increase the level of satisfaction of the patients and an efficient way of putting the patients back to their normal life promptly.

Ultrasound-Guided Local Steroid Injection Therapy for Acute Calcific Tendinitis of Shoulder (초음파를 이용한 급성 극상근 석회화 건염의 스테로이드 국소 주사 요법)

  • Purpose: To analyze the clinical outcome after ultrasound guided multiple dry needlings and local steroid injection for acute calcific tendinitis of shoulder. Materials and Methods: Twenty patients with acute episode of pain by calcific tendinitis of shoulder with average age 58.2 (50~70 years) and follow-up of 18 months in average (range, 12~24) were included in study. There were 18 patients with right and 2 with left sided involvement. All patients had calcific deposits in the supraspinatus tendon. All patients underwent standardized nonoperative treatment protocol, consisting of 5~12 MHz high resolution ultrasound guided multiple dry needlings with 18 guage needle, followed by 2% lidocaine 1cc and 40 mg/ml depomedrol 1cc injection at site of calcific tendinitis. The outcome was assessed by UCLA shoulder score, range of motion and VAS score. A statistical analysis with ANOVA and Tukey's post-hoc test with the significance level at 5% was performed using SAS 9.1 software (SAS Institute, Cary, NC). Results: All patients got continuous relief of pain right after the procedures until final follow-up. Before the procedures, the UCLA scores were fair in 15 patients and poor in 5. After the procedures, the UCLA scores were excellent in 16 patients and good in 4. All cases revealed no limitation of shoulder function. The average VAS score decreased from 8.9 before the procedures to 0.5 at final follow-up (p<0.0001). No complication was encountered. Conclusion: An ultrasound guided local steroid injections following multiple dry needlings would be one of the useful treatment modality for the acute calcific tendinitis of shoulder.

  • 타석에 관한 연구는 타석증을 보이는 환자에 대한 임상적 특징, 진단 및 치료에서부터 타석의 성분 및 구조 등에 이르기까지 다양한 범위에 걸쳐 이루어지고 있다. 타석의 미세구조에 관한 연구는 타석의 미세구조가 다양한 형태인 것으로 보고되고 있으며, 특히 최근 타석증의 치료에 새롭게 소개되고 있는 체외충격파쇄석술은 타석의 구조에 따라 그 효과가 영향을 받을 수 있을 것으로 사료된다. 이에 저자는 인간 타석의 미세구조에 관한 기본 자료가 필요할 것으로 사료되어 한국인 중년 여성으로부터 적출된 악하선 타석을 광학현미경 및 주사전자현미경을 이용하여 미세구조적 특징을 관찰한 결과, 다음과 같은 결론을 얻었다. 1. 타석은 중심부의 핵, 핵 주변의 층상구조 및 외피막으로 이루어져 있었다. 2. 핵은 비정형의 중심과 상대적으로 균질의 외곽부위로 구성되어 있었다. 3. 핵 주변은 대부분 동심원적인 층상구조를 보였지만 일부분에서는 균질의 구조를 보였다. 4. 타석 단면의 전체직경과 중심부 핵의 직경은 각각 $3,500{\mu}m$$1,500{\mu}m$였고, 층상구조를 이루는 각 층의 두께는 위치에 따라 약 $10{\sim}40{\mu}m$ 이내였다.

  • Stellate ganglion block(SGB) is a widely used sympathetic block to diagnose or treat various painful conditions. We experienced a rare case who exhihited a contralateral Horner's syndrome following SGB. A 64-year-old female patient suffering from postherpetic neuralgia on mandibular branch of trigeminal nerve visited our pain clinic. She complained of severe burning and shooting pain on right side lower lip, ear and temporal area. We modified her previous medications and performed repeated right SGB daily, in combination with mandibular or mental and auriculotemporal nerve blocks twice a week. Her symptoms were progressively improved. A contralateral Horner's syndrome occured after the thirteenth SGB, which was performed under several attempts in the same manner and the same physician. She had no evidence of subarachnoid or brachial plexus blocks. She did not need any special treatment and returned home 2 hours later. Subsquent blocks were followed on ipsilateral Horner's syndromes.

  • A laser based needle-free liquid drug injection device has been developed. A laser beam is focused inside the liquid contained in the rubber chamber of micro scale. The focused laser beam causes explosive bubble growth, and the sudden volume increase in a sealed chamber drives a microjet of liquid drug through the micronozzle. The exit diameter of a nozzle is 125 ${\mu}m$ and the injected microjet reaches an average velocity of 264 m/s. This device adds the time-varying feature of microjet to the current state of liquid injection for drug delivery.

  • Hollow형 미세바늘 어레이는 주사기와 패치의 장점을 결합하여 여러 종류의 약물을 통증 없이, 전달할 수 있게 한다. 본 논문에서는 건식 식각 방법과 습식 식각 방법을 이용하여 hollow형 실리콘 미세바늘 어레이를 제작하는 제작 공정과 그 결과를 제시하였다. 미세바늘 어레이의 형태는 실리콘 웨이퍼의 앞면에서 세 번의 식각 공정을 이용해 제작되었는데, 첫 번째 건식 식각 공정으로 피부에의 침투를 원활히 하기 위해 바늘 끝을 형성하고, 두 번째 건식 식각 공정으로 바늘의 길이를 조절하며, 마지막 HNA solution을 이용한 습식 식각 공정으로 바늘을 더 가늘게 만들면서 끝을 더 날카롭게 식각한다. 바늘을 통해 약물전달이 가능하도록 웨이퍼의 뒷면으로부터 건식 식각 공정을 이용해 약물 주입통로를 형성하였다. 제작된 Hollow형 실리콘 미세바늘 어레이는 $170\;{\mu}m$의 너비와 $230\;{\mu}m$의 길이, 직경 $40\;{\mu}m$의 약물 주입통로를 가지고 있으며, $1\;cm^2$의 시편 위에 $1000\;{\mu}m$의 피치로 $9{\times}9$ 개의 바늘을 형성하였다.

  • Piriformis muscle syndrome is a condition that causes direct muscle pain around piriformis muscle or sciatica from irritated sciatic nerve and the diagnosis remains debatable. The main treatment is symptomatic relief from conservative therapy such as medication and piriformis stretching exercise, and various therapeutic injections including local anesthetic, corticosteroid, botulinum toxin can be considered for diagnostic and therapeutic purposes. In this case, a 54-year-old male who had sciatica and gait disturbance showed piriformis muscle hypertrophy in the pelvis MRI. From imaging studies, electrodiagnostic study and physical examination, he was diagnosed with piriformis muscle syndrome. He underwent trigger point injection and botulinum toxin injection into the piriformis muscle, and pain and gait disturbance significantly improved. This case reports a case of piriformis muscle syndrome with clinical symptom of gait disturbance, which was improved by botulinum toxin injection.
