고령화 사회가 되면서 치매발병률 또한 증가하고 있다. 2017년 탈시설화와 통합돌봄의 본격적 도입으로 지역사회 기반 치매환자 돌봄이 더욱 중요해졌다. 치매에 대한 관점도 기존의 의료 모델, 사회적 모델에서 인간 중심 모델로 변화하고 있다. 본 연구에서는 장기요양재가이용시설인 노인주간보호센터의 돌봄 종사자들의 치매지식을 분석하여 치매서비스 개선방안을 제시하였다. 2곳의 주간보호센터에서 근무연한 2년 이상의 돌봄 종사자들을 대상으로 포커스 그룹 인터뷰를 진행하여 7가지 영역에서 소주제를 도출하였다. 개선방안으로는 지역기반 서비스의 중요성과 규칙적 일상 실천, 치매와 함께 동행하기 위한 치매인식 개선, 개별성과 다양성을 기반으로 한 인간중심 서비스의 실천 등이 제시되었다.

본 연구는 주간보호센터의 중풍노인이 이야기치료 집단상담을 통해 치매노인과의 갈등을 해결하는 과정을 탐색하는 연구이다. 중풍노인에 대한 이야기치료 집단상담을 7회 동안 실시한 과정을 사례연구방법을 통해 분석하였다. 분석결과 첫째, 중풍 노인들이 집단상담을 통해 치매노인을 '문제'가 아니라 '치매로 고통받는 사람'으로 보고, 치매노인을 비난하는 대신 도와줄 방법을 적극적으로 찾게 되었으며, 둘째, 자신들을 '치매노인의 피해자'가 아니라 '유능한 사람 으로 규정하게 되었고, 셋째, 중풍노인들끼리 협력적이고 친밀한 관계가 되었으며, 넷째, 중풍노인들이 주간보호센터 운영에 적극적으로 참여하게 된 것으로 나타났다. 이러한 변화에 이야기치료의 문제와 사람을 분리시키기, 클라이언트를 자기 문제의 전문가로 존중하기, 독특한 결과 찾기 등이 도움이 된 것으로 분석되었다. 이러한 결과를 바탕으로 장기요양보호대상자에게 보호를 제공할 뿐만 아니라 참여 기회를 확대하고, 이야기치료를 노인에게 적용할 것을 제안하였다.

본 연구에서는 주간보호서비스 이용노인들을 대상으로 서비스품질을 조사하고, 서비스품질과 고객만족, 추천의향 및 재이용의사 간의 인과관계 및 고객만족의 매개효과를 분석하였다. 구체적으로, 대전과 충남북 군 단위 지역에 위치한 주간보호시설을 이용하는 노인 208명을 대상으로 조사를 실시하였으며, 서비스품질은 Parasuraman 등(1985)이 제시한 SERVQUAL 모형에 따라 다섯 가지 요인 즉 유형성, 신뢰성, 응답성, 확신성, 공감성으로 측정하였다. 주요 연구결과는 다음과 같다. 첫째, 서비스품질의 하위요인 중 유형성과 응답성이 고객만족에 직접적인 영향을 미치는 것으로 나타났다. 둘째, 서비스품질은 추천의향 및 재이용의사에 직접적인 영향을 미치지 않았으나 고객만족을 통하여 영향을 미치는 것으로 조사되었으며, 고객만족의 매개효과가 검증되었다. 셋째, 고객만족은 추천의향과 재이용의사에 직접적인 영향을 미치는 것으로 나타났다. 연구결과를 바탕으로, 주간보호시설의 전반적인 서비스품질 및 고객만족도 제고 방안과 시설운영의 안정성 확보 방안 등을 제시하였다.

Purpose: The purpose of this study was to describe the operation, staffing, and services provided at adult day care centers in Korea. Methods: The study was a cross-sectional descriptive survey. The subjects were 209 centers among 280 centers registered in Korea (response rate 74.6%). The data was collected from August to December in 2006. Results: 48.8% of centers were located in a city area. The centers were based on the Social Model 65.5% of the centers were open Monday to Friday, an average of $7{\sim}8$ hours per day Dementia, stroke or frail elderly could use the center, and 57.4% of centers were used for dementia and stroke elderly together. The enrollment of elderly was 13.5. The number of total staff was 8.27, the number of RN's and social workers was 0.67, and 2.54 respectively. The social services(Meal preparation 98.6%, Special event 98.1%, Transportation, 97.1%) were provided more than the health services(Physical therapy 98.1%, ADL training 95.2%, Counseling 84 7%, Vaccination 82.8%, Health monitoring 78.9%, Health education 78.5%, Bathing 66.1%, and Speech therapy 28.2%). Conclusion: These results suggest we have to develop a health-focused adult day health care model based on the needs of elderly and their families. Nurses will have an important role in adult day health care.

In Germany, community based daycare facilities has been supported by long term care policies since the 1970s. With the legislation of Care Insurance, those policy has been developed further. As the use of daycare facilities decreased and the financial burden for consumers increased, administrators of facilities has been seeking for innovative programs and management methods in order to improve the service. For the same reason, policy makers have been pursuing new regulations of architectural standards of facilities. By looking at legislations (i.e., Law of Care Insurance, Law of Heim, and DIN18025) that stipulate architectural standard of facilities in Germany, this study will identify the development process of architectural change of daycare facilities. In addition, the study aims to contribute to the discussion on the use of day care facilities in Korea in terms of the legislation of Care Insurance soon to be introduced.

The purpose of this study was to understand the adult day care center as a place for the elders and adults and to develop an initial understanding of the services and activity programs for adult day care for the cognitively-impaired in the U.S.A. The data were collected from 12 Adult Day Care Centers(ADC) from 2001 to 2002 by personal interview and the documents about the programs. ADC programs provided primarily social, recreational, and health activities and services in a group setting. Centers offered participants to socialize, enjoy peer support, and receive health and social services in a stimulating and supportive environment that promotes better physical and mental health. For this purpose those centers offered not only professional health care, occupational.speech.physical therapies but also socio-recreation and therapeutic-recreation services.
