Malignant Peripheral Nerve Sheath Tumor of the Sciatic Nerve in a Patient with Neurofibromatosis - A Case Report - (신경섬유종증 환자의 좌골 신경에 발생한 악성 신경초종 - 증례 보고 -)

  • Malignant peripheral nerve sheath tumors(MPNSTs) are uncommon sarcomas that mostly arise in the soft tissue. They can develop from the pre-existing neurofibromas or schwannomas, or denovo from the peripheral nerves, or they can occur following the radiation therapy. We report a case of MPNST that developed in the sciatic nerve of the patient with neurofibromatosis type-1(NF-1). The patient was a 39-year-old man with the history of NF-1, who's main symptom was a rapidly enlarging painful mass in his posterior thigh. The well demarcated tumor, $6.5{\times}5{\times}4.5$cm in size, was composed of closely packed spindle cells. Since the patients with NF-1 have a high risk for developing a recurred MPNST, the importance of the clinical follow up is emphasized.

Effects of swimming on functional recovery and brain-derived neurotrophic factor (BDNF) mRNA expression after sciatic crushed nerve injury in rats

  • 말초신경은 외상이나 질병 등 여러 가지 원인으로 손상되기 쉬우며, 손상의 정도가 심하거나 치료가 지연되는 경우에는 심각한 기능 소실을 초래할 수 있다. 본 연구에서는 수영이 말초신경손상후 운동기능의 회복과 뇌유인성 신경영양인자 (brain-derived neurotrophic factor, BDNF) mRNA의 발현에 미치는 효과를 알아보기 위하여, 흰쥐 좌골신경에 압박 손상을 가하고 수영을 적용한 후 보행궤적분석 (walking track analysis)과 역전사연쇄반응 (reverse transcription-polymerase chain reaction, RT-PCR)을 실시하였다. 그 결과, 좌골신경 압박손상된 쥐는 특징적인 보행패턴을 나타내어 좌골신경기능지수 (sciatic function index, SFI)가 현저히 낮아졌으며, BDNF mRNA의 발현이 증가하였다. 좌골신경 압박 손상후 수영을 한 쥐에서는 SFI가 현저히 향상되었으며, BDNF mRNA의 발현은 억제되었다. 이러한 결과는 말초신경손상후 수영이 BDNF mRNA의 발현을 조절함으로써 기능 회복을 촉진시키는 효과적인 치료방법이 될 수 있음을 제안하고 있다.

  • Ganglion cysts that occurred within sheaths of peripheral nerves have been documented in literatures, but it is relatively rare condition. The peroneal nerve is the most common site of involvement. Other reported sites of involvement are the radial, ulnar, median, sciatic, tibial and posterior interosseous nerves. We report a case of the intraneual ganglion cyst within peroneal nerve and another case of the intraneural ganglion cyst within a medial plantar nerve that were successfully excised without neurologic complication.

Effect of Transplantation of Intravascular Cultured Neural Stem Cell upon Peripheral Nerve Regeneration (혈관내에 배양한 신경줄기세포의 이식이 말초신경 재생에 미치는 영향)

  • The ultrastructural change of sciatic nerve and immunohistochemical changes of NGF, PCNA were studied at the transplanted segment of intravascular cultured neural stem cell in the rat sciatic nerve by 5 months after the sciatic nerve transection. The transplanted intravascular neural stem cells were differentiated into Schwann reals at the 20th day and these cells began to regenerate by the proliferation and hypertrophy. There were many remyelinating Schwann cells in the transplanted nerve in term of stimulation. According to NGF finding, we suggest preexisting Schwann cells may induce the differentiation of neural stem cells into regenerating Schwann cells. Electron microscopic changes were the remyelinating appearance, the increase of intraaxonal microtubules and enlarged mitochondria and contacting tell processes each other.

The Effect of Acrylamide on the Ultrastructures of Nervous System of the Mouse (생쥐 신경계의 미세구조에 미치는 Acrylamide의 영향)

  • The effed of acrylamide on the nervous system has heen morphologically studied using light and electron microscopes. The light micrographs on central and pedpheral nervous tissues of mouse treated with acrylamide monomer showed total vacuolation of spinal cord, cell degradation containing neuron and neuroglia, and distal nerve fiber degeneration. The electron micrographs showed ultrastrudural changes. Abnormal mitochondria in neuron, splitting of myelin sheath in lumbar ventral root nerve, partial disintergration of myelin sheath and axoplasmic degeneration in sciatic nerve, and overafl polyneuropathies in nervous system were observed. These results suggest that acrylarnide intoxicated mouse shows distal behavioral neuropathy as an earlist clinical sign, but the initial effect of acrylamide on the nervous system seems to appear at nearly the same time in both central and peripheral nervous systems.

Comparison of Therapeutics on Chloramphenicol Injection-induced Sciatic Nerve Injury in Dogs (개에서 Chloramphenicol주사에 의한 좌골신켱손상시의 치료효과의 비교)

  • Chloramphenicol주사에 의한 좌골신경손상을 유발한 후 Prednisolone을 이용한 약물처치군, 전침군 및 전침과 약물을 병용한 군에서 각각 회복속도를 비교하였다. 정상보행 횟수, pinching을 통한 감각신경의 통감반응,그리고 병리조직소견 등을 죠사한 결과를 요약 하면 다음과 같다. Enrofloxacin, cefazolin, cephalexin, penicillin 등의 좌골신경주위 주사게 서는 신경마비가 관찰되지 않았으나 chloramphenicol을 주사할 경우에는 좌골신경손상으로 인한 편측성 후지마비가 유발되어 knuckling 반응을 보였다. Chloramphenicol을 투여한후 좌골신경손상에 대한 각 처치에서 정상보행 횟수는 8-12주까지는 침술처치군이 약간 중가하 였으나, 처리군간 유의성은 관찰되지 않았다. 실험기간동안 chloramphenicol 에 의한 신경손 상에 대한 각 처치군별 병리조직 소견은 호전양상이 관찰되지 아니하였다.

Recording and Analysis of Peripheral Nerve Activity Using Multi-Electrode Array (다채널 신경전극 어레이를 이용한 말초 신경신호의 측정 및 분석)

  • Reliable recording and analysis of peripheral nerve activity is important to recognize the user's intention for controlling a neuro-prosthetic hand. In this paper, we present a peripheral nerve recording system that consisted of an intrafascicular multi-electrode array, an electrode insertion device, and a multi-channel neural amplifier. The 16 channel multi-electrode array was stably implanted into the sciatic nerve of the rat under anesthesia using the electrode insertion device. During passive movements and mechanical stimuli, muscle and cutaneous afferent signals were recorded with the multi-channel neural amplifier. Furthermore, we propose a spike sorting method to isolate individual neuronal unit. The muscle proprioceptive units were classified as muscle spindle afferents or Golgi tendon organ afferents, and the skin exteroceptive units were categorized as slow adapting afferents or fast adapting afferents. Experimental results showed that the proposed method could be applicable to record and analyze peripheral nerve activity in neuro-prosthetic systems.

Role of MR Neurography for Evaluation of the Lumbosacral Plexus: A Scoping Review (요천추 신경총에 대한 자기공명신경조영술의 역할: 주제 범위 문헌고찰)

  • Purpose MR neurography (MRN) is an imaging technique optimized to visualize the peripheral nerves. This review aimed to discover an optimized protocol for MRN of the lumbosacral plexus (LSP) and identify evidence for the clinical benefit of lumbosacral plexopathies. Materials and Methods We performed a systematic search of the two medical databases until September 2021. 'Magnetic resonance imaging', 'lumbosacral plexus', 'neurologic disease', or equivalent terms were used to search the literature. We extracted information on indications, MRN protocols for LSP, and clinical efficacy from 55 studies among those searched. Results MRN of the LSP is useful for displaying the distribution of peripheral nerve disease, guiding perineural injections, and assessing extraspinal causes of sciatica. Three-dimensional short-tau inversion recovery turbo spin-echo combined with vascular suppression is the mainstay of MRN. Conclusion Future work on the MRN of LSP should be directed to technical maturation and clinical validation of efficacy.

Morphological changes of Schwann cells as neurotoxic responses (신경독성에 의한 Schwann 세포의 형태적 변화)

  • The early change observed in lead-induced neurophathy in the rat was Schwann cell swelling. In order to quantify this cell swelling, Schwann cell thickness and major diameter of the nucleus were measured using tranverse section with associated myelinated fiber of sciatic nerves. Group I rats were intoxicated with 0.5% lead acetate in the drinking water for 30 days; group II animals were treated as in group I and then restored to normal laboratory conditions for 30 days; and group III were controls. The results showed that the cell sizes were significantly greater in intoxicated animals, compared with control, and the cell sizes of group II did not differ significantly from control rats.
