임상에서 활동 중인 종양전문간호사의 직무분석-DACUM 기법 이용- (Job Analysis of Korean Oncology Advanced Practice Nurses in Clinical Workplace -Using the DACUM Method-)

  • Purpose: The purposes of this study were to identify job definition, duties and tasks of Korean oncology Advanced Practice Nurses (APNs) and assess frequency, importance, and difficulty for each of duty and task using the DACUM method. Methods: The DACUM committee was organized with nine oncology APNs. The committee documented the job definition, duties and tasks based on their daily job descriptions. Validity of the elicited duties and tasks was tested by a study team and 6 oncology APNs. Data were collected using a questionnaire survey from 53 oncology APNs. Results: The job description of oncology APNs was identified 12 duties and 66 tasks. Among the twelve duties, 'education for patients/families' was the most frequently performed and was the most important duty. 'Case management' was the most difficult duty. Among 66 tasks, 'providing education for patients/families' was most frequently performed. 'Providing education for patients/families' and 'assessing patient condition comprehensively' were the most important tasks. 'Implementation research' as the principal investigator was perceived as the most difficult task. Conclusion: In this study, Korean oncology APNs' job performances were identified and analyzed. It is recommended that job description for oncology APNs needs to be standardized and reshaped in advanced way in future.

종양전문간호사가 제공한 간호서비스의 구조.과정적 측면의 분석 (Analysis of Nursing Services of Oncology Advanced Practice Nurses from the View Point of Structure and Process)

  • Purpose: The purpose of this study was to investigate nursing services of Oncology Advanced Practice Nurses(OAPNs) from the view point of structure and process. Methods: Roles and practices of OAPNs for cancer patients on chemotherapy were investigated by semi-structured interview and survey. Subjects were 14 OAPNs in 3 hospitals. Results: OAPNs had high level of education and certification than registered nurses. Most subjects of OAPNs were patients on chemotherapy and OAPNs used most of their time in education and expert nursing practice. 57.1% of OAPNs used delegated order. They were satisfied with their job in general, but satisfaction in communication with nurse manager or resident doctor was low. Conclusion: These results will be the concrete explanation for the nursing services of OAPNs. On the basis of theses results, more study about effects of OAPNs will be needed.

종양전문간호사의 성과지표 사용현황 및 성과평가에 대한 인식도 (Oncology Advanced Nurses' Use of Outcome Indicators and Perception of Outcome Evaluation)

  • Purpose: The purpose of this study was to explore the use of outcome indicators and perception of outcome evaluation of oncology advanced practice nurses (APNs). Method: A total of 111 oncology APNs from hospitals which have over 400 beds were surveyed. The participants' use of outcome indicators and perception of outcome evaluation were collected using a questionnaire including 84 outcome indicators for APNs developed by Sung et al. and 13 items on APNs' contribution to the hospitals developed by Kleinpell (2005). Data were analyzed using descriptive statistics, t-test, and $x^2$ test. Result: Twelve items out of 84 outcome indicators were used over 50% of the time and 57 items could be used in the future by the participants. Seven of the 10 top-ranking outcome indicators in use were education related and were also expected to be used frequently in the future. The score for participants' perception of outcome evaluation was average-high, 3.82 out of 5. Conclusions: The results of the study show that the main outcome indicator for oncology APNs is education related, and thus the use of education related outcome indicators is recommended to make oncology APNs' activities be more visible.

방사선 치료의 국내 현황과 미래 (Present Status and Future Aspects of Radiation Oncology in Korea)

  • 목 적: 최근 국내 방사선 치료의 발전 경향, 시설 장비와 인적 현황 등 인프라와 특징을 조사하고, 국내 방사선종양학분야의 발전 방향을 모색하려 한다. 대상 및 방법: 2006년도 방사선종양학과 전공의 지도 감독에 관한 실태 조사를 위하여 2006년 7월, 전문의, 전공의, 의학 물리사, 간호사, 방사선사 등 인력 및 치료 장비 인프라 조사와 함께 향후 확장 계획에 관한 조사를 실시하였다. 국내 61개 기관의 치료 시설 등급 결정은 IAEA 조사연구에서 사용한 기준에 따라서 분류하였다. 결 과: 2006년 7월 현재 한국에 설치된 방사선종양학과는 61개 병원이며, 132명의 전문의와 50명의 전공의를 포함하여 선량계산사 및 의학물리사 64명, 방사선사 369명 간호사 130명 등 745명의 인력이 2004년도 기준으로 28,789명 신환의 방사선치료 업무를 담당하고 있다. 방사선치료기는 메가볼티지 치료기가 104대 설치되어 있고 선형가속기가 96대, 코발트 원격치료기 2대, 토모치료기 3대, 사이버나이프가 2대, 한 대의 양성자 치료기가 설치되어 있다. 강내치료 장치는 41개 병원에 설치되어 있으며 이 중 35개 병원에서 고선량률 강내치료기, 6개 병원에서 저선량률 강내치료 장치가 설치 운영되고 있다. 국내 61개 병원의 IAEA기준에 따른 분류에서는 level 0이 2곳, level 1이 15곳, level 2가 19곳이고, level 3 즉 강도조절 방사선치료, 정위적 방사선치료, 수술 중 방사선치료가 가능한 시설이 25곳으로 매우 높은 질의 치료를 시행함을 보여준다. 결 론: 최근 치료시설과 방사선치료 관련 인적 인프라는 현저한 증가 추세에 있다. 즉, 선형가속기의 증가와 함께 코발트 치료기는 현격한 감소를 보이며 토모치료기, 사이버나이프 등 첨단 치료 장치가 증가하는 경향이다. 한국의 메가볼티지 치료기의 숫자는 인구 백만 명당 2.1대로서 향후 방사선치료의 필요성과 적응증이 계속 증가되는 추세에 있어서 치료기의 증가가 필요한 실정이다.

일개 종합병원 종양간호사의 전문직 삶의 질 (Oncology Nurses' Professional Quality of Life in a Tertiary Hospital)

  • Purpose: The purpose of this study was to investigate oncology nurses' professional quality of life and its correlations with job satisfaction and job stress in a tertiary hospital in Seoul, South Korea. Methods: A cross-sectional survey design was utilized to investigate 210 oncology nurses from various oncology and hemato-oncology units. Professional quality of life (ProQOL), which is conceptualized with three sub-dimensions (compassion satisfaction, burnout, and secondary traumatic stress), job satisfaction, job stress, demographic and work-related variables were measured. Results: The participants were all women, with the mean age of 28.4. The participants with high ProQOL was only 14.8%, and 75.3% and 77.6% of the participants reported a moderate to high level of burnout and secondary traumatic stress respectively. Job satisfaction, age, and the total years of nursing practice have positive correlations with compassion satisfaction. Also, compassion satisfaction was significantly different according to age group, work units, the level of education, nursing positions, and the years of practice. Conclusion: Since the level of professional quality of life among oncology nurses was relatively low, the program for oncology nurses to improve compassion satisfaction and to decrease compassion fatigue needs to be developed.

종양전문간호사의 역할규명을 위한 연구 (The Role Behaviors of Oncology Nurse Specialist)

  • The purposes of this study was to identify and propose the expected role of the oncology nurse specialist by embodying role theory to oncology nurse specialist. The subjects of this study were 149 persons in 14 hospitals, who were classified to 4 groups, oncology nurse specialists(ONS) group, head nurses and charge nurses(HN & CN) group in hemato-oncology ward, registered nurses(RN) group in hemato-oncology ward, and hematologists & oncologists(H&O) group. The questionnaire which was consisted of 89 items for role of oncology nurse specialist, was made by researcher with a field study and literature review about role of oncology nurse specialist and verified by matrix delphi technique about content validity and construct validity. The data were collected from October 22, 2002 to November 5, 2002. All 4 groups proposed that ONS should perform an expert practitioner role first of all. But ONS group, RN group and H&O group proposed orderly expert practitioner, educator, researcher, consultant, and administrator & change agent, but HN & CN group did expert practitioner, educator, consultant, researcher, administrator & change agent. Expert practitioner had the most highest necessary degree in all groups and most highest performance degree in ONS group. That was consistent with results that all groups proposed role of expert practitioner at first. 4 items out of 20 items showed the meaningful differences between groups. For role of educator, oncology nurse specialist group proposed necessary degrees over 4.0 point out of 5.0 in all items. 4 items out of 18 items showed the meaningful differences between groups. For role of researcher, 3 nurses groups proposed a high necessary degree, but performance of ONS group was most lowest among 5 roles. 6 items out of 14 items showed the meaningful differences between groups. The role of consultant had high necessary degree in some items related to hematopoietic stem cell transplantation. 2 items out of 17 items showed the meaningful differences between groups. In nursing behaviors of administrator & change agent, those items about enacting principle, cost development and participation of professional academy had a high necessary degree. 4 items out of 18 items showed the meaningful differences between groups. Oncology nurse specialists group performed 5 roles orderly, expert practitioner, consultant, educator, administrator & change agent, researcher. This result was different from expected role of themselves as well as the other groups. There was a different necessary degree between role and embodied nursing behaviors of role. ONS group and RN group proposed orderly educator, researcher, administrator & change agent, expert practitioner, consultant, but the other groups did educator, expert practitioner, researcher, consultant, administrator & change agent. The expected standards of oncology nurse specialist in this study were usually master's degree, total career of 5-7 years, oncology career of 3-5 years and certification. But for the post, qualification and qualification institution, various opinions were suggested. In the conclusion, there was a different necessary degree between role and embodied nursing behaviors of role. All groups proposed expert practitioner at first in abstract role, but educator at first in embodied nursing behaviors of role. So we have to consider this difference carefully in the future research. ONS acted the role of expert practitioner first of all, but we should develope and expand the roles of researcher, and administrator & change agent. We should enact roles by role behaviors induced from mutual agreements in necessary degree and performance degree, and bargain the role behaviors that showed the meaningful differences between groups But, we should consider carefully which group's opinion we have to select. I suggested 36 items out of 89 items, in which ONS proposed necessary degree over 4.0 out of 5.0 and half of them performed as the nursing behaviors of oncology nurse specialist that did not induce role stress. For the future, We should role bargain the role with other groups based on these items.

복강경 보조 원위 위 절제 환자의 장기적인 '삶의 질' 평가 (Long Term Impact of Laparoscopic Assisted Distal Gastrectomy on Quality of Life)

  • 목적: 조기위암 수술에 복강경 적용의 용이성 및 종양학적 안정성에 대해서는 많은 연구가 아루어지고 있으나, 복 강경 적용의 또 다른 목적인 '삶와 질' 향상에 대한 연구 는 아직 미흡한 현실이다. 이에 저자들은 위암 수술에 복강경 적용이 '삶의 질' 향상에 미치는 장기적 영향을 평가 하고자 본 연구를 고안하였다. 대상 및 방법: 2003년 1월부터 2004년 12월까지만 2년간 순천향대학병원 외과에서 위 절제 수술 후 위암 1기로 진단된 154명의 환자들을 대상으로 수술 후 1년 이상 경과한 시점에서 설문조사를 시행하였다. EORTC OLQ-C30(3판)과 EORTC QLQ-ST022 한글판을 이용하여 한 명의 전문 간호사에 의해 외래 방문 시 직접 면담 혹은 전화 상담을 통해 설문조사를 시행했으며, 통계처리는 SPSS 12.0 for Windows를 이용하였다. 결과: 전체 대상 환자 154명 중 86명에서 설문 조사가 이루어져 58%의 응답률을 보였으며, 86명(개복군: 57명, 복강경군: 29명)의 평균 연령은 55세였고, 남자가 60명 여자가 26명으로 남녀 비는 2.3 : 1 이었다. 개복 군의 종양 크기는 평균 2.42 cm로 복강경군의 평균 종양 크기 1.8 cm에 비해 유의하게 크기가 컸으며(P=0.003), 평균 획득 림프절 수도 개복 군에서 36개로 유의하게 더 많았다(P=0.000). 그 외 종양관련 인자인 원발암와 침윤도, 림프절 전이 및 TNM 병기에 있어서 양 군 간에 차이는 없었다. 전반적인 건강 상태와 관련된 2 항목의 평균 점수는 개복 군이 57 (${\pm}20.6$), 복강경 군이 60.3 (${\pm}20.4$)으로 양 군 간에 유의한 차이는 없었으며, 기능과 관련된 15 항목과 증상과 관련된 13항목에 있어서도 양군 간에 유의한 차이는 없었다. EORTC QLQ-STO22를 이용한 '삶의 질' 비교 결과 개복군의 평균 점수는 94.5 (${\pm}97.3$), 복강경군은 68.9 (${\pm}64.9$)로 개복군에서 전반적으로 불편한 증상을 많이 호소하였으나 통계학적인 차이는 없었다. 결론: EORTC QLQ-C30 (3판)과 EORTC QLQ-STO22 한글판을 이용한 설문조사 결과 장기적인 측면에서 위 절제술에 복 강경 적용이 환자의 '삶의 질' 향상에 영향을 미치지 못했다.
