Neurilemmomas of the Cervical Vagus Nerve (경부 미주 신경에 발생한 신경초종)

  • 인체에서 발견되는 전체 신경초종중 $25{\sim}40%$가 두경부에서 발견되나 경부미주신경의 신경초종은 매우 희귀하여 1988년까지 영문문헌상 88 예가 보고되어 있다. 임상적으로 경부종괴가 주증상이며 때로는 애성, 종괴촉지시 기침 혹은 방사통, parapharyngeal space 의 종괴성장으로 연하곤란등이 초래되기도 한다. 치료는 종괴의 완전적출과 미주신경의 기능을 보존시키는 것이 주요 목표로 되어 있다. 저자들은 최근 5년간 4 예의 정부미주신경외 신경초종을 경험하였는데, 3 예는 측경부의 상부에, 1예는 측경부의 하부에 발생한 것이었다. 연령은 25세에서 50세까지 성인연령이었고, 성별은 남자 l 예, 여자 3 예이었다. 내원시 주소는 전예가 무통성 경부종괴이었으며, 종괴의 크기는 직경 3cm에서 10cm까지 다양하였다. 전예에서 종괴로 인한 신경학적 증상은 없었으나, 1 예에서 parapharyngeal space 의 거대종괴 때문에 다소간의 연하곤란이 있었다. 2 에에서는 종괴촉지시 기침이 유발됨을 호소하였다. 수술은 경부횡절개로 흉쇄유돌근을 제치고 총경 동맥과 내경정맥을 종괴의 상하부위가 완전히 노출되도록 박리한 후 종괴의 피막을 미주 신경의 주행방향에 따라 절개하여 적출 (enucleation) 하였다. 적출술시 종괴가 유착되어 있는 피막 부위는 동시에 절제하되 미주신경의 신경경로 (neural pathway) 는 유지되도록 하였다. 수술직후 2 예에서 일시적 애성을 호소하였다, 추적은 최단 20 개월에서 최장 80 개월까지 하였는데 일시적 애성은 호전되었고 전예가 재발없이 건강하게 지내고 있었다. 따라서 본 종양수술은 종양적출술시 이환된 미주신경의 절단을 결정하기 전에 신경경로를 유지시킬 수 있는 적출술 (enucleation) 을 먼저 고려하는 것이 바람직하다고 사료되었다.

  • Mammography, the process of using X-ray to examine the woman breast, is the one of the effective tools for detecting breast cancer at an early state. In screening mammogram, Computer-Aided Detection(CAD) system helps radiologist to diagnose cases by detecting malignant masses. A mass is an important lesion in the breast that can indicate a cancer. Due to various shapes and unclear boundaries of the masses, detecting breast masses is considered a challenging task. To this end, CAD system detects a lot of regions of interest including normal tissues. Thus it is important to develop the well-organized classifier. In this paper, we propose an enhanced sparse representation (SR) based classifier using Fisher discrimination dictionary learning. Experimental results show that the proposed method outperforms the existing support vector machine (SVM) classifier.

A Case of Mesenteric Myxosarcoma in a Dog (개에서 발생한 장간막 점액육종의 한 증례)

  • A 6-year-old male Shih tzu dog was presented for evaluation of abdominal distention. Abdominal radiography and ultrasonography revealed a soft tissue density mass containing large amount of fluid. Ultrasonography-guided fine needle aspiration of the mass was performed and cytologic impression was granulation tissue with hematoma and fibroplasias. On exploratory laparotomy a mass was identified at the root of mesentery adhered to distal jejunum. Because the mass could not be separated from the attached jejuna loops, the mass and the adhered sites were surgically removed all together and enteroanastamosis was performed. Histologically a low grade myxosarcoma was diagnosed. Tumor cells were positive with alcian blue stain and Ki67 index by immunohistochemistry was 2.5. The dog recovered from surgery uneventfully, and has been in good condition without any signs of recurrence or metastasis for about 30 months after surgery.

  • There are various diseases in soft tissue tumors that occur in subcutaneous tissue, and hematoma generally would be occurred in traumatic or coagulopathic condition. We report a case of hematoma on left gluteal area that was diagnosed by ultrasonography and was resected in 70 year old woman who visited the hospital because of left gluteal mass detected by chance two month ago and slowly growing.

  • Computerized Tomography is now well established and important noninvasive method of diagnosting mediastinal mass lesions because of its superior imaging of their size, location and internal composition. Authors ana lysed and present CT findings of 30 surgically proven mediastinal tumors and cysts that were studied and treated at the Yeungnam University Hospital during recent 6 years. The most common tumor was thymona(9 cases), and teratoma(6 cases), lymphoma(6 cases), bronchogenic cyst(4 cases), neurogenic tumor (4 cases), pericardial cyst(1 case) were next in order of frequency. There were 5 cases of thymoma showing homogenous solid density mass, 2 cases were malignant thymoma and myasthenia gravis was present in 2 cases. A case of thymolipoma and a case of thymic carcinoma were included. All teratomas were cystic masses but pathognomic fat, and calcified density were seen only in 4 cases. 5 cases were located in anterior mediastinum and 1 case was in posterior mediastinum. Lymphoma(3 Hodgkin's and 3 non-Hodgkin's) appeared as irregular lobulated mass in anterior mediastinum. Neurogenic tumor(2 ganglioneuroma and 2 neurilemmoma) appeared as homogenous density mass located in posterior mediastinum. Among the 4 bronchogenic cysts, 2 were located in retrotracheal area, 1 was located in subcarinal and 1 was in parathoracic area. One case of pericardial cyst was oval shaped cystic mass located in left pericardiac border.

A Case of Uterine Adenocarcinoma in A Spayed Female Dog (중성화된 암캐에서 발생한 자궁 선암종 증례)

  • A 10-year-old spayed female Pekinese dog was referred to veterinary medical teaching hospital of Chonbuk National University with signs of dysuria, abdominal distension, and hematuria. Abdominal radiography revealed a large oval soft tissue mass in cranial abdomen, which was a fluid filled cyst containing a round mass by ultrasonography. Exploratory laparatomy found a 4 cm cyst containing reddish brown fluid and a 1.5 cm small round mass attached to the wall. Cytological impression of the imprint smears of the resected mass were highly malignant adenocarcinoma, which was confirmed by histopathology of the mass and the cyst. After the surgery the patient recovered uneventfully and discharged. The tumor recurred about one year after surgery, and second surgery performed at the owner's request. However the tumor recurred in two months with metastatic disease in the lung and the dog survived three more months after surgery.

  • Ewing sarcomas constitute a group of small, round, blue cell tumors of the bone and soft tissue. Extraskeletal Ewing sarcoma (EES) is a rare malignant neoplasm that arises from soft tissues, and it usually affects children and young adults. EES of the thoracopulmonary region commonly presents with a palpable mass or pain. Although rarely reported, EES affecting the anterior chest wall may present as a breast mass. We report a case of EES arising from the chest wall and manifesting as a palpable breast mass in a 22-year-old woman. The large mass was initially misdiagnosed as a breast origin mass on ultrasonography, but subsequent CT and MRI showed that the mass originated from the chest wall. Radiologists should be aware of the imaging findings of EES, and they should understand that chest wall lesions may be clinically confused as breast lesions.

  • Amyloidosis has been reported to be associated with non-Hodgkin lymphoma. Amyloidosis and lymphoma can be related in two ways: lymphoma-associated systemic amyloidosis and peritumoral amyloidosis with lymphoma. We report a rare case of peritumoral amyloidosis in a patient with head and neck mucosa-associated lymphoid tissue lymphoma. On CT, the oropharyngeal mass showed an irregularly shaped soft-tissue density with multifocal amorphous calcifications and heterogeneous enhancement. On MRI, the mass showed heterogeneous low signal intensity on both T1- and T2-weighted images. On contrast-enhanced MR images, the mass showed good enhancement with several inner non-enhancing foci. Concurrent pathologies, such as peritumoral amyloidosis, should be considered when calcifications are noted in patients with pre-treatment lymphoma.

Calcifying Aponeurotic Fibroma : A Case Report

  • Calcifying aponeurotic fibroma is a rare benign soft tissue tumor that usually involves distal extremities in children and adolescents, especially the hands and feet. We report a case of calcifying aponeurotic fibroma arising in a 14-year-old boy who complained of right thumb mass. Surgical excision was performed. The resected specimen showed a $2.0{\times}1.5cm$ grayish white, fibrotic tissue. Histologic examination showed proliferation of fibroblastic cells with infiltrative growth pattern. Foci of calcification and chondroid differentiation were present.

Cardiac Fibroma Involving Left Ventricle (좌심실에 발생한 심장 섬유종)

  • A 9-month-old male infant was admitted for evaluation of incidentally noticed cardiac mass. The patient had no symptoms and there was no hemodynamic abnormality. Echocardiographic finding showed a huge left ventricular free wall mural mass, which did not obstruct the left ventricular outflow tract. Maximal excision of the left ventricular free wall mass was performed. The pathologic finding revealed cardiac fibroma. During the 7-month of follow-up after surgery, there was no evidence of arrhythmia or tumor recurrence.
