The Meaning of Dignified with Death (존엄한 죽음의 의미)

  • Purpose: We explored Koreans' perception of the meaning of death with dignity that Korean people. Methods: A phenomenological research methodology was applied. A total of 13 participants were sampled based on their age and gender. Participants were interviewed in depth from September 2015 through February 2016. Colaizzi's phenomenological analysis method was used for data analysis. To establish the validity of the study, we evaluated its realistic value, applicability, consistency and neutrality of the qualitative evaluation criteria of Lincoln and Guba. Results: Koreans' perception of death with dignity was structured as 19 themes, nine theme clusters and four categories. The four categories were "comfortable death", "good death", "resolving problems before death", and "death with good reputation". The theme clusters were "death without pain", "death submitting to one's fate", "death that is not ugly", "leaving good memories to others", "dying in a way we want", "death after proper settling of things", "dealing with chronic resentment before death", "death after living a good life", and "death with recognition". Conclusion: For Koreans, death with dignity meant not burdening others, settling things right and leaving good memories to their families and friends. Such perceptions can be applied to hospice care for terminally ill patients.

Dignity Therapy for End-stage Patients: Concept Analysis (말기 환자의 존엄요법 개념분석)

  • Purpose: Dignity therapy is a very effective intervention to improve the dignity of end-stage patients. A concept analysis by Walker and Avant (2005) was adopted to define, describe, and delimitate the concept of dignity therapy. Methods: Nursing literature in the National Digital Science Links (NDSL) and Medline database were searched for the definitions of "dignity" and "dignity therapy". Definitions, uses, and defining attributes of dignity therapy were identified; model and contrary cases were developed; and antecedents, consequences, and empirical references were determined. Results: Through dignity therapy patients and their families share their stories, and that in turn improves the quality of life and death. Five attributes were identified: higher quality of life and death, therapeutic conversation, respect of human dignity and worth, expressing thoughts about life and death and systematic process. Conclusion: Patients at the end of their lives feel more comfortable about death. Hospice care providers should try to protect dignity of patients in their care. The attributes of the dignity therapy clarified in this study should be applied for terminally ill patients to improve their quality of life and death.

Attitudes Toward Legalization of Death with Dignity (존엄사의 법제화에 대한 태도)

  • The purpose of this study is to research on attitudes toward legalization of death with dignity. The respondents are 561 adults aged 20 years old and over, living in Seoul and Kyunggi Province. Research questionnaire consist of 28 questions concerning general background, personal experiences of death, attitudes toward death, pros and cons on death with dignity and legalization. Statistical analyses employ frequency, mean, cross tab, and t-test. 87.3% of respondents agree the legalization of death with dignity. The persons who are older, self-employed, and production employees, get married, live with spouse, believe buddhism, experience care for the death, believe life after death, recognize the necessity of testament agree more on death with dignity. The first requisite for legalization of death with dignity is the standard of judgement concerning self decision on death with dignity. The decision makers are prioritized by self, family member in order. The necessary services for decisions on death with dignity are the mediation role between medical team and family, medical information about illness prognosis and prolongation, psychological counselling on depression.

  • In this article, this study examines the issue of "Physician-assisted Suicide" and how Ramon Sampedro asserts his right to a dignified death through litigation. Ramon, the protagonist of the movie <The Sea Inside> is a patient, severely paralyzed man who has spent more than 26 years in bed. The only thing he can do is verbally ask his family for help. Ramon can no longer support this worthless existence, so he pursues death with dignity. Ramon files a lawsuit to authorize death with dignity within a legal framework, but is denied on the grounds that life is a duty. Ramon eventually fulfills his desire for death with dignity with the help of his friends. Ramon sets up a camera to document the process of his death and introduces the cyanide, which is used in assisted dying, by inhaling cyanide in front of the camera and dying quietly. Although Ramon is not a terminally ill patient, who can blame him for practicing death with dignity as he chooses to do so. We will need to work to build social consensus and legislate for death with dignity for seriously ill patients like Ramon.

Concept Analysis of Nurses' Acceptance of Patient Deaths (간호사의 환자죽음 수용에 대한 개념분석)

  • Purpose: The purpose of this paper is to identify a theoretical basis of end-of-life care by examining attributes of the concept of the nurses' acceptance of patient deaths. Methods: Walker and Avant's approach to concept analysis was used. A literature study was performed to check the usage of the concept. To identify the attributes of the concept and come up with an operational definition, we analyzed 16 qualitative studies on nurses' experiences of death of patients, published in a national science magazine from 1999 to 2015. Results: The nurses' acceptance of death of patients was identified as having four attributes: acceptance through mourning, attaining insight on life and death while ruminating life, facing with fortitude and practicing human dignity. Antecedents of the concept were experiences of patient's death, confusion and conflict, negative emotions, passive responses, denial of patients' death. The consequences of the concept were found as the holistic end-of-life care and active pursuit of life. Conclusion: This study on the attributes of the concept of the nurses' acceptance of death of patients and it's operational definition will likely lay the foundation for applicable end-of-life care mediations and theoretical development.

Qualitative Study on Social Workers' Experiences and Roles during End-of-Life Care in Elderly Long-Term Care Facilities (노인 장기요양기관에 종사하는 사회복지사의 임종 케어 경험과 역할에 관한 질적 연구)

  • This study was to investigate the social workers' psycho·social experiences and roles during the end-of-life care process in elderly long-term care facilities. As a result of data analysis through in-depth interviews, social workers experienced great exhaustion and burnout due to frequent death experiences during the end-of-life care process, and expressed regret for not being able to provide better service for the deceased and longing for loved ones. And in the event of a sudden death, social workers would undergo criticism and complaints. The main roles of social workers during end-of-life care were to contact and communicate with family members when signs of death appeared and support them after the death. The necessity of standardized manuals and education for end-of-life care, a recharging program and support group to prevent burnout of social workers, and a legal safety net for emergency preparedness and emergency measures were suggested. The necessity of death preparation education, hospice care, and advance medical directive was also emphasized for the dignified death of the elderly.

The Subjective Perception and Characteristics of the Middle-aged on Death with Dignity (존엄사에 대한 한국 중년층의 인식유형과 특성)

  • This study is to grasp fundamental data to prepare the policy about death with dignity(DwD) by grasping perception type of the middle-aged about DwD. 32 middle-ages test Q-sort with 30 statements which is related to DwD. The result of analysis shows that subjective perception types are permission type, opposition type, and limited permission type. First, 'permission type' indicates that DwD by self-determination of patients should be accepted; it is necessary to patients with extreme pain. Second, 'opposition type' insists that there will be more people dying unfairly; a trend to make light of human life could be in everywhere if DwD is permitted. Third, 'limited permission type' agrees with permitting DwD but insists on preparing specific legal system before that. The subject, DwD, itself seems not to be able to draw a complete agreement from people, but national opinions should be reflected during the process of DwD system.

A Study on Aid in Dying (조력사망(Aid in Dying)에 대한 고찰)

  • "Aid in Dying" means that when a decision-making patient suffers from an incurable disease, a drug that can speed up death is prescribed by a doctor and used to lead to death. Since the suspension of life-sustaining treatment was institutionalized based on human dignity and patient autonomy, the question of whether assisted death can be legally justified in relation to the right to receive medical help to shorten one's life to die with dignity has recently been actively discussed. In Korea, since the suspension of life-sustaining treatment was institutionalized by the enactment of the Life-sustaining Treatment Decision Act in 2016, an amendment to the Life-sustaining Treatment Act was recently proposed to legalize Aid in Dying. The global trend is that human "Right to Die" is discussed in the division of life and death, from the suspension of life-sustaining treatment to assisted death, and again in the order of euthanasia. In this paper, we started discussing dignified death and institutionalized patients' right to self-determination, looked at the controversy in the United States, which legislated assisted death in many states since the 2000s, and analyzed the main contents of California's End of Life Option Act and the data after enforcement. The strict requirements for Aid in Dying, such as voluntary confirmation of patients' intentions and doctors' obligation to provide information, and the results of California's Aid in dying system, composed of relatively diverse races, were reviewed.

The study on the view of death in the Buddhism for well dying's culture formation (웰다잉(well dying) 문화 형성을 위한 불교의 죽음관 연구)

  • Recently, the social concerns on well dying come to the fore as important discourse on dignity of human death, which detonation by the problem of euthanasia. Well dying means 'good death' the meaning of a word, and it means 'prepared death', 'decented death' 'beautiful death' by extention of sematic context like this as a general rule. In this paper the writter have considered that how the view of Buddhism's death contribute to the discourse on well dying, which regarding death as the starting point, death conquest as the finishing point of the theory. The Buddhism's discourse on the attitude interpretation conquest process of death contributed to formation of abundant discourse on well dying, especially the view of Buddhism is able to contribute to conquest of death anxiety and death education, that death conquest interpret to spiritual psychological phenomenon not physical physiological eternal life (or immortality) and conquer death through enlightenment on reality of things by spiritual psychological change.
