Ultrasonographic and Magnetic Resonance Imaging Findings of Testicular Lymphoma (고환림프종의 초음파검사 및 자기공명영상 소견)

  • Purpose : To evaluate the specific radiologic findings of testicular lymphoma which will be able to differentiated from other testicular tumors. Materials and Methods : Pathologically confirmed eight cases were included in this study. All eight cases were performed ultrasonography and four cases were performed magnetic resonance image. On ultrasonography, the size, location, shape, margin, internal echogenicity, homogeneity and vascularity were evaluated. On magnetic resonance image, the shape, margin, homogeneity, signal intensity on T1- and T2-weighted images, degree and homogeneity of the contrast enhancement and contrast enhancement change on dynamic enhancement study. Results : The margin of the mass was smooth on 6 of 8 patients. Internal echogenicity of the mass lesion was hypoechoic than normal testicular parenchyme on 7 of 8 patients. Four cases were homogeneous, 3 cases were relatively homogeneous and 1 case was heterogeneous. All 8 cases showed increased vascularity. The mass lesion was iso-signal intensity on T1-weighted image and low-signal intensity on T2-weighted image. All four cases were enhanced homogeneously and mildly than enhancing normal testicular parenchyme. On dynamic enhancement study, the mass lesion is progressively enhanced with time. Conclusion : The possibility of testicular lymphoma should be considered when testicular mass was homogeneously hypoechoic and low signal intensity on T2-weighted image in old age patients.

Diagnostic Imaging of Isolated Splenic Torsion in a German Shepherd Dog (독일셰퍼드 견에서 단독으로 발생한 비장 염전의 영상진단)

  • A 5-year-old male German shepherd dog was presented with a 4 day-history of anorexia and depression. Abdominal radiographs showed a large soft tissue mass and loss of serosal detail. Ultrasonographs revealed splenomegaly with the lacy pattern parenchyma, perivenous hyperechoic region at the splenic hilum, and loss of splenic venous flow. Computed tomographic findings included the enlarged non-enhanced spleen except perivenous region at the splenic hilum and a whirled soft tissue mass at the splenic hilum. Exploratory laparotomy confirmed the diagnosis of splenic torsion. Ultrasonographic findings including lacy appearance, hyperechoic perivenous echogenicity and absence of splenic blood flow and CT findings including no or minimal enhancement of the spleen and a whirled appearance of splenic pedicle could be useful for diagnosing isolated splenic torsion in the dog.

Dynamic Contrast-Enhanced MR Imaging in Detecting Local Tumor Progression after HIFU Ablation of Localized Prostate Cancer (국소적 전립선암의 고강도 집속 초음파 치료 후 국소적 암 재발의 발견과 역동적 조영증강 자기공명영상의 역할)

  • Purpose : To retrospectively evaluate the diagnostic performance of dynamic contrast-enhanced MR imaging (DCE-MRI) in detecting recurrent prostate cancer after HIFU of clinically localized cancer, as compared with T2-weighted imaging (T2WI). Materials and Methods: Twenty-six patients with increased prostate-specific antigen levels after HIFU were included in this study. All MR examinations were performed using T2WI and DCE-MRI, followed by transrectal ultrasound-guided biopsy. MRI and biopsy results were correlated in six prostate sectors. Residual or recurrent cancer after HIFU was defined as local tumor progression if biopsy results showed any cancer foci. Two independent readers interpreted the MR images. Results: Of 156 prostate sectors, 51 (33%) were positive for cancer in 17 patients. For detecting local tumor progression, the sensitivity of DCE-MRI and T2WI was 80% and 57% for reader 1 (P < 0.001) versus 84% and 61% for reader 2 (P < 0.001), respectively. The specificity and overall accuracy between DCE-MRI and T2WI showed no statistical difference in both readers (P > 0.05). Interobserver agreement of DCE-MRI and T2WI was moderate and fair, respectively. Conclusion: For detecting local tumor progression of prostate cancer after HIFU, DCE-MRI was more sensitive than T2WI, with less interobserver variability.

Comparative Study on Signal Strength of Mechanical Index Using Ultrasound Machines with SonoVue Contrast (Sonovue 초음파 조영제를 이용한 장비 간 Mechanical Index의 변화에 따른 신호 강도 비교연구)

  • The purpose of this study was to compare the MI using SonoVue along with different machines output and to infer the meaning of the signal difference under the same condition. All of the comparative instruments showed strong signal values at early stage as MI value increased. Over time, the inter-instrumental signal values showed signal attenuation under all conditions except for 10 min of the condition of MI 0.1 of RS85A. E9 and EPIQ7 showed signal degradation due to microbubble collapse over time at all MI values. In the comparison of equipment, the signal strengths of MI 0.1, 0.2, and 0.4 were high in order of EPIQ7, RS85A and E9. In the quantitative analysis, there were statistically significant from the SNR and CNR that were obtained from RS85A and E9 (P-value<0.05). In the quantitative analysis, Epiq7 was statistically significant except for CNR as the MI value was changed In the contrast-enhanced ultrasound, even though MI value was low (MI <0.05), it will be helpful for diagnosis, controlling the MI and scan time because a difference in signal intensity was shown between the three machines.

Delayed Contrast-enhanced Computed Tomography for Adrenal Masses in 3 Dogs (개 부신종양의 지연형 조영증강 전산화단층촬영 적용 3증례)

  • Three dogs having adrenal masses detected on ultrasonographic examination were underwent computed tomography (CT) for surgery. After adrenalectomy, each mass was diagnosed pheochromocytoma with myelolipoma, adrenocortical carcinoma and adrenal adenoma through histopathology. Five minutes were used to get delayed enhanced CT images. Attenuation value was measured in each mass and the absolute and relative percentage of enhancement washout were calculated.

  • Testicular lymphoma is an uncommon testicular tumor that usually presents as a painless mass. It usually shows hypervascularity on color Doppler ultrasound (US) and a mild enhancement on enhanced CT or MRI. We present an unusual case of a testicular lymphoma mimicking a missed testicular torsion in a 67-year-old male patient with right scrotal swelling and intermittent pain for 2 months. Color Doppler US demonstrated the absence of vascularity in the enlarged right testis, and the initial diagnosis was a missed testicular torsion. CT demonstrated a poorly enhancing mass rather than a missed testicular torsion with enhanced small nodular foci at the periphery. The final pathological diagnosis was testicular lymphoma.

  • International guidelines recommended screening with ultrasonography (US) every 6 months for patients at risk for hepatocellular carcinoma (HCC). However, US demonstrates low sensitivity for the early detection of HCC. Magnetic resonance imaging (MRI) plays an important role in the noninvasive diagnosis of HCC, but it is not suitable for surveillance due to its lengthy examination and high cost. Therefore, several studies have been using various abbreviated MRI strategies, including noncontrast abbreviated MRI, dynamic contrast-enhanced abbreviated MRI, and abbreviated MRI using hepatobiliary phase image for HCC surveillance. In this article, we aim to review these various strategies and explore the future direction of HCC surveillance considering the cost-effectiveness aspect.

Hepatic Pseudolymphoma Mimicking a Hypervascular Tumor: A Case Report (과혈관성 종양으로 오인된 간의 가성림프종: 증례보고)

  • Hepatic pseudolymphoma is a rare benign liver mass that is characterized by proliferation of non-neoplastic lymphocytes extranodally. To the best of our knowledge, only 46 cases have been reported in the English literature. We described the case of a 75-year-old woman with hepatic pseudolymphoma mimicking a hypervascular tumor. After the histological confirmation of the rectal neuroendocrine tumor, CT scan revealed a 1.0 cm-sized, poorly-defined and low-density nodule in the liver. On MRI, the hepatic nodule showed an arterial enhancement and a low-signal intensity on the hepatobiliary phase. On diffusion-weighted imaging, the hepatic nodule showed a high signal intensity on a high b-value. On fluorodeoxyglucose (FDG) positron emission tomography (PET)/CT, it revealed a high standardized uptake value nodule. The US showed the hypoechoic nodule and the US-guided biopsy confirmed the hepatic pseudolymphoma.

  • Staphylococcus epidermidis is a part of the normal skin flora of humans. However, it can cause serious infections in people exposed to foreign bodies or in immunocompromised patients. A 13-year-old boy was hospitalized with fever and myalgia. Painful nodular lesions were detected on the scalp, arms, and legs. Pancytopenia and blasts were present in the peripheral blood. He was diagnosed with acute myeloid leukemia. Magnetic resonance imaging of the whole body showed multiple peripheral rim-enhancing, cyst-like lesions. Ultrasonography showed echogenic nodules inside the cystic lesions in the intramuscular space of the arms and legs. Therefore, cysticercosis was strongly suggested initially. However, an abscess was confirmed on sono-guided biopsy and S. epidermidis was isolated from a microbial culture of the tissue. We report a case of multiple disseminated lesions caused by S. epidermidis in a leukemia patient, initially mistaken for cysticercosis.

Incidental Breast Lesions on Chest CT: Clinical Significance and Differential Features Requiring Referral (흉부 전산화단층촬영에서 우연히 발견된 유방 병변: 임상적 중요성 및 진료 의뢰가 필요한 특징적 영상 소견)

  • Purpose: To evaluate the CT features of incidental breast lesions on chest CT and to suggest useful criteria for referral to a specialized breast unit. Materials and Methods: Between May 2009 and April 2014, enhanced chest CT examination reports containing the key word 'breast' were reviewed retrospectively. Patients who had incidental breast lesion and were referred to a specialized breast unit and then underwent pathological confirmation or follow-up over a 1-year period were included. Finally, 86 patients (all female, mean age, $48.9{\pm}12.6years$) were enrolled. Two radiologists evaluated lesion characteristics, including size, shape, margins, and enhancement. The correlations between the CT features and pathologies were evaluated, and the diagnostic accuracy of CT features in various combinations was assessed. Results: Among the CT features, irregular shape, non-circumscribed margin, and high contrast enhancement were different between malignant and benign lesions (p < 0.05). The combination of non-circumscribed margin and high contrast enhancement had the highest accuracy (97.7%). Conclusion: Reliable CT features for incidental malignant breast masses are irregular shape, non-circumscribed margin, and high contrast enhancement. The combination of non-circumscribed margin and high contrast enhancement could help distinguish incidental malignant breast lesions and indicate referral to a specialized breast unit.
