The purpose of this study was to evaluate the relationships between the occlusal plane angle and craniofacial skeletal pattern in relation to anterior overbite. Methods: Lateral cephalograms of 90 adults with skeletal class III malocclusions were traced and measured to analyze skeletal factors and occlusal plane angles. In terms of anterior overbite, all patients were classified into 3 subgroups of positive overbite, edgebite, and negative overbite groups. All measurements were evaluated statistically by ANOVA and Duncan's Post Hoc, and correlation coefficients were evaluated among measurements. Results: In this study, some skeletal measurements (saddle angle, articular angle, Y axis, AFH, SN-FH, SN-Mn, FH-Mn) showed a significant difference among the 3 groups in relation to overbite changes. Correlation coefficient showed that PFH/AFH, SN-Mn, Mx-Mn, and FH-Mn showed a significant difference with FH-Occ, Mx-Occ, and Mn-Occ. Regression analysis showed that Mx-Mn had a determination coefficient of 0.714, 0.560, and 0.677 in relation to FH-Occ, Mx-Occ, and Mn-Occ, respectively. Conclusion: This study suggests that consideration of the occlusal plane in relation to the maxillomandibular vertical skeletal state enable the establishment of a more predictable orthognathic surgery result.
Class III malocclusion patients can be approached with many different types of treatment methods, and thus, each patient's problems must be accurately evaluated to allow selection of the best possible treatment method. Cephalometric analysis is an essential part of diagnosis and treatment planning of orthodontic patients, and it would certainly be helpful if reliable cephalometric guidelines could be set. The author divided 482 Class III malocclusion patients(253 males and 229 females) into fourgroups according to different types of treatment methods they have received to correct imbalance between upper and lower jaws: 1) orthopedic appliance (face mask & RPE), 2) camouflage treatment with fixed appliance, 3) surgical-orthodontic treatment, 4) cross-bite correction with removable plates/ functional appliance. Cephalometric values at the time of first clinical examination were compare among the four groups. Cephalometric analysis indicates the following results: 1)the amounts of antero-posterior and vertical skeletal discrepancies and dental compensation were greatest in surgery group 2) SNB, Wits, distance from Nasion Perpendicular Plane to point a facial angle, facial convexity, and APDI were greater in orthopedic appliance group than fixed appliance(camouflage) group, but there was no statistical difference 3) removable plates/ functional appliance group showed least amounts of skeletal discrepancies and dental compensation with statistical significance.
Objective: The purpose of this study was to evaluate the dentoskeletal and soft tissue profile changes after extraction of two lower first or second premolars in "borderline" adult skeletal Class III cases. Methods: Twenty-eight patients with "borderline" skeletal Class III malocclusion were studied. All of them were treated by extraction of two lower first or second premolars. Lateral cephalometric radiographs taken at the start and end of treatment were analysed. Twenty-five cephalometric variables were calculated and paired $t$-tests were performed. Results: After treatment, no significant changes were noted in the skeletal parameters ($p{\geq}0.05$). Regarding the dental parameters, the L1-MP angle decreased by $8.1^{\circ}$, the U1-L1 angle increased by $7.7^{\circ}$ ($p$ < 0.01), the overjet distance increased by 5.7 mm ($p$ < 0.01), the L1-NB angle decreased by $7.3^{\circ}$ and the L1-NB distance decreased by 4.8 mm ($p$ < 0.01). The soft tissue parameters of Li-E, Li-H and Li-RL2 distance decreased by 3.2 mm, 3.4 mm and 4.1 mm respectively ($p$ < 0.01). Conclusions: Orthodontic camouflage treatment by extraction of two lower first or second premolars provides a viable treatment alternative for "borderline" skeletal Class III cases to achieve a good occlusal relationship.
Objective: The aim of this study was to compare the initial lateral cephalometric characteristics in two groups of patients: those that had mandibular setback surgery only and those that had mandibular setback surgery with advancement genioplasty. Methods: The lateral cephalograms of thirty-one patients were studied. Twenty-one Class III patients (group A) had only madibular setback surgery Twelve Class III patients (group B) had mandibular setback surgery with advancement genioplasty. Results: Differences between two groups were found in N-Me, ANS-Me, Occlusal Plane angle, Palatal Plane to U1, Mandibular Plane to L1, Mandibular Plane to L6, SN to U1, Sn-Stms, and Pog' projection. Compared to group A, group B showed more linguoversion and extrusion of upper incisors, more extrusion of lower incisors and lower first molar, and more steepness of the occlusal plane. N-Me, ANS-Me, and Sn-Stms were also longer in group B. But Pog' projection was shorter than group A. Conclusion: We conclude that certain initial lateral cephalometric characteristics may help indicate the inclusion of advancement genioplasty when mandibular setback surgery is planned in skeletal Class III patients.
The Purpose of this study was to evaluate the effect of the lower third molar on treatment time and distal en masse movement of the lower dentition in Giass III malocclusions. Thirty subjects (9 males and 27 females) were selected, all of whom were diagnosed as Glass III malocclusion and treated by fixed appliances without premolar and/or molar extraction. They were divided into three groups Group 1 consisted of 12 subjects. whose lower third molars were not extracted during the whole orthodontic treatment. Group 2 consisted of 8 subjects. whose lower third molars were extracted after WEAW application and before removal of the orthodontic appliances. Group 3 consisted of 10 subjects whose lower third molars were extracted before MEAW application. For each subject. overall treatment time and duration of MEAW application were determined. In addition. pre-treatment and post-treatment lateral cephalometric radiographs were analyzed. All data were Processed statistically with ANOVA, and the conclusions were as follows: There was no significant difference among the groups in overall treatment time However, duration of MEAW application was longer in Group 2 than in Group 1 or Group 3. The overjet that was established after orthodontic treatment was largest in Group 3, in which the lower third molars were extracted befor MEAW application. After orthodontic treatment, IMPA decreased in Group 3. but increased in Group 1 and Group 2. There was no significant difference among the three groups in the translation of lower second molars However the tipping movement of lower second molars was significantly different, highest in Group 2 and lowest in Group 1 Therefore. it is thought to be better for the orthodontic treatment of Class III malocclusions to extract the lower third molars before MEAW application. In Group 2, the mandibular plane angle was decreased as a result of forward rotation of the mandible This skeletal change was thought to bring about the difficulty of treatment.
The author obtained some useful information for the class III treatment from long term observation on the growing patients with class III malocclusion. 8 patients were selected for this study and presentation. From these observation so far my conclusions might be as follows: First in the early correction of the anterior crossbite, considerable forward growth changes were observed in the maxilla Second, as for the growth modification of jaws by orthopedic treatment only limited effects were recognized from the long-term observation Thrid, at early age of patients with anterior crossbite, any data couldn't make me predict the stability after treatment on the long-term basis. Fortunately, however, genial angle showed a marginal possibility of it prediction. Fourth, at an advanced age/ retraction orthopedic force on the mandible and the rapid change in the mandibular position may cause some trouble in the T.M.joint. Finally, the followings are recommendable. As for the anterior crossbite, correct it early as possible, and use orthopedic force under the age of ten. Do not enter the phase II treatment directly. Just wait and observe until the growth were almost completed, focusiong on some important factors such as airway problem, tongue position, and third molar development. Of course, these factors may have some effects on the mandibular growth. for the female, at the age of around 14 years old and the male, around 17 years old, make a final decision whether the patients will continue to be treated orthodontically or surgically Thereby, (I think) the relapse and retreatment problem after treatemnt we have observed so far might be minimized. Furthermore, the active treatment time may be also reduced.
This study was designed to investigate the attrition pattern in Angle Class III malocclusion with facial asymmetry. The sample consisted of three groups, the 20 subjects of normal occlusion group(Group I), the 12 subjects of class III malocclusion without facial asymmetry group(Group II) and 17 subjects of Class III malocclusion with facial asymmetry group(Group III). In Group I, Maxillary attrition area was larger than mandibular attrition area, but in Group ll and III, there was no significant difference in attrition area between maxilla and mandible. 4. In Group III, the attrition area of deviated side was target than undeviated side 5. There was no significant difference in attrition area between chewing side and non-chewing side in each group. 6. The total attrition area was unaffected by gender.
Distributions trends were examined in 9159 malocclusion patients who had been examined and diagnosed at Department of Orthodontics, College of Dentistry Yonsei University over a 6 year-period from 1992 to 1997. The results were as follows ; 1. The total number of orthodontic patients in 1997 increased in comparing with that of 1992(86%) and after 1995, the number of annual patients showed an increase. 2. Age distribution had shown over-19 year-old group being the largest(39.9%) but percentages of pre-adolescent patients had been decreasing while that of adult patient had gradually increased. 3. Distributions in the types of malocclusion, the Angle's Classification had shown : 33.3% for Class I, 28.6% for Class II and the largest 38.1% for Class III. 4. Among Class I patients, crowding showed the largest(32.6%) and protrusion group had gradually increased to 33.9% in 1996. Otherwise openbite and crossbite groups tend to decrease, annually. 5. Geographic distribution showed a majority of patients from Seoul(above 70%). Among the patients from Seoul, group within the distance 3-6Km from Yonsei dental hospital was the largest(32.3%) and group within 9Km showed 69% of total patients.
