Effects of Training on Sit to Stand on Various Seat Heights on the Balance in Stroke Patients (다양한 좌석 높이에서 일어서기 훈련이 뇌졸중 환자의 균형에 미치는 영향)

  • This study aimed to compare the effects of sit-to-stand training, or training to stand up from sitting positions at various seat heights, on the balance recovery among stroke patients. This study included 20 stroke patients who were randomly divided into two groups. Each group was trained for six weeks, three times a week from March to June 2019. Static balance and dynamic balance were measured, and the variations were analyzed using the paired t-test and the independent t-test. There were significant changes observed in both static and dynamic balance (p<.05) before and after training. However, no significant changes were seen in the static balance in the standing posture with eyes closed (p>.05). This study confirmed that there was a significant effect of training on the balance of stroke patients, especially when progressively lower seat heights were used during the training. This suggests new directions for treatment during rehabilitation for balance recovery of stroke patients. Further studies will need to apply this training to a larger number of subjects, and use various training methods such as randomization of seat height to enable the generalization of the results of this study and application in clinical practice.

Reliability and Concurrent Validity of the Balance Evaluation using Space Balance 3D and Tinetti Mobility Test in Subacute Stroke Patients (아급성 뇌졸중 환자에서 Space Balance 3D와 Tinetti Mobility Test를 이용한 균형 능력 평가의 신뢰도 및 동시타당도 연구)

  • The purpose of this study was to determine the test-retest reliability and the concurrent validity between tinetti mobility test (TMT), berg balance scale (BBS) and space balance 3D which is one of the computerized measurement and visual feedback balance assessment system in subacute stroke patients. Twenty three ambulatory acute stroke subjects were measured the TMT, BBS and space balance 3D. The test-retest reliability(intra-class correlation coefficient: ICC) indicated that the static and dynamic balance in space balance 3D considered moderate reliability and TMT, BBS were good reliability. In case of concurrent validity, there were moderate validity (p<.01) between static balance test with space balance 3D and each TMT, BBS. But there were only poor validity (p<.05) between center to forward-left, center to backward-left phase in dynamic balance test with space balance 3D and each TMT, BBS. These findings suggest that in subacute stroke patients the test-retest reliability and concurrent validity using the space balance 3D and TMT were valuable in balance test but there was limitation to evaluate dynamic balance test.

  • 만성요통 환자 15명을 대상으로 4주간, 주 3회 3차원 척추안정화운동과 허리안정화운동을 실시한 후 3차원 척추안정화검사를 통하여 허리심부근력을 측정하고 Tetrax장비를 통하여 신체균형능력을 평가하였다. 단기간일지라도 3차원척추안정화운동을 통해 정확한 방법으로 운동을 실시하였더니 만성요통환자의 허리근력과 자세균형에 긍정적인 효과를 주었다.

  • 본 논문에서는 기존의 정적인 스케쥴링 모델 즉, 그래프 이론, 정수 프로그래밍 그리고 휴리스틱 모델에 근거한 알고리즘들의 사례를 소개하고 분석하였으며 정적인 스케쥴링 알고리즘의 단점을 보완할 수 있고 고장허용구조를 지원할 수 있는 FAME 알고리즘을 제시하였다. FAME은 정적인 휴리스틱 알고리즘과 동적인 스케쥴링 알고리즘으로 구성되며 부하균형과 기억공간 크기 그리고 최소 통신비용 조건을 만족하고 태스크간 간섭비용을 도입함으로써 태스크간 통신 비용에 의해 나타날 수 있는 태스크들의 동시성 수행 정도의 감소를 방지할 수 있도록 하였다. 또한 기억공간 크기와 간섭비용의 조합은 테스크들이 일부 프로세서에만 할당되는 현상을 방지할 수 있다.

Correlation between balance and thickness of abdominal and quadriceps muscles (복부근육 및 넙다리네갈래근과 균형의 상관관계)

  • This study compares the correlation between muscle thickness (abdominal, quadriceps muscle) and how the muscle thickness affects the static balance when the static balance is measured in a standing position. The subjects of study were to select 29 subjects to find out what relationship muscle thickness had on the static balance when measuring muscle thickness and static balance. rectus femoris muscle thickness showed a significant correlation in statically balanced weight distribution index. In this study, only healthy men in their twenties were selected, and the results could not be generalized to other age groups. As a limitation of this study, muscle strength and peripheral vision were not measured, so it seems difficult to express the accuracy of the study and generalize the results. In future studies, the number of subjects and muscle strength should be measured to proceed with the study.

The effects of combination patterns exercise of proprioceptive neuromuscular facilitation on balance in chronic low back pain elderly patients (PNF 통합 패턴 운동이 만성 요통 노인 환자의 균형에 미치는 영향)

  • The aim of this study was to identify influence that integrated PNF(IPNF) on sprinter and skater pattern exercise had an effect on static and dynamic balance in chronic low back pain elderly adult. The subjects of this study were 34 and they were divided into two groups: IPNF exercise group(n=17), swiss ball exercise group(n=17). They were measured static balance ability using Good Balance System (GBS) and dynamic balance ability using functional reaching test(FRT) and timed up and go (TUG) during pre and post exercise. The results of this study were significant differences in groups about pre and post static and dynamic balance tests the greater part(p<.05). There were significant differences in groups according to exercises(p<.05). The above results from this study indicated that integrated patterns of PNF have improved the static and dynamic balance ability. This study will be enough to provide the basic raw material using a integrated PNF.

The Effects of Yoga Exercise on Balance and Gait Velocity in Stroke Patient (요가운동이 뇌졸중 환자의 균형과 보행속도에 미치는 영향)

  • The purpose of this study was to analyse the effects of yoga exercise on balance ability and gait velocity in stroke patients. Subjects were categorized in to a control group and yoga program group with 9 for each group. Yoga program was conducted for 60minute for 8weeks, three times a week. For the purposes, the study measured Stability Index(SI, postural sway) and Weight Distribution Index(WDI) using Tetrax, Functional Reach Test(FRT), Dynamic Gait index(DGI) and 10 meter walking test. At pre- and post-exercise after appling the yoga exercise, the data was analyzed. Yoga exercise group's SI and WDI were decreased, FRT and DGI were increased in comparison with control group. But 10 meter walking test was no significance. It suggests that the yoga exercise could promote recovery from balance disorder after stroke.

Effect of Olfactory Stimulation on Balance, Spasticity and Quality of Life in Chronic Stroke Patients (후각자극이 만성 뇌졸중 환자의 균형, 경직 및 삶의 질에 미치는 효과)

  • The purpose of this study was to determine whether olfactory stimulation would improve spasticity, balance ability and quality of life in stroke patients. Twenty-one stroke patients were recruited and were randomly divided into two groups: olfactory stimulation group (n=10) and sham stimulation group (n=11). Participants in both groups received conventional physical therapy for 30 minutes before the intervention. Additionally, subject in the olfactory stimulation group performed olfactory stimulation using lavender oil for 6 minutes (2 minx3 set), five times a week for two weeks, while the sham stimulation group conducted olfactory stimulation using water for the same amount of time. Composite-Spasticity-Score was used to assess spasticity level of ankle plantar-flexors. Dynamic balance was measured using a TUG. Postural-sway distance was measured using a force platform. Quality of life were measured by SF-36. There was no significant difference within group and between the groups in the spasticity. Significant improvement in postural-sway and TUG were observed in the olfactory stimulation group compared to the sham stimulation group (p<0.05). The Mental Componnt Summary of the SF 36 in the olfactory stimulation group improved significantly greater than the sham stimulation group (p<0.05). Our findings indicate that olfactory stimulation is beneficial and effective to improve balance ability and quality of life in stroke patients.

Relationship between Plantar Tactile Sensory Thresholds and Balance for Life-Care Increase in Patients with Stroke (뇌졸중 환자의 라이프케어 증진을 위한 발바닥 감각역치와 균형의 상관관계)

  • Sensory impairment is common following a stroke. Tactile afferent inputs from the plantar foot provide important information to the central nervous system to generate balance. The present study, which recruited 50 stroke patients, aims to determine the relationship between plantar tactile sensory threshold (PTST) and balance in patients with stroke. The PTST was evaluated at two sites (hallux and heel) using Semmes-Weinstein monofilaments. The balance abilities were measured by the degree of weight distribution in quiet standing (QS) and sitting-to-standing position (STS) using a MTD system and Berg balance scale (BBS). The hallux and heel sensory thresholds (ST) in the affected side were higher than the non-affected side (P < 0.05). The degree of weight distribution in the affected side was lower than the non-affected side. QS and STS showed P < 0.05, with BBS score of 36.00 ± 1.53. The hallux ST in the affected side correlated with BBS (r = -0.444, P < 0.05), QS (r = -0.332, P < 0.05), and STS (r = -0.390, P < 0.05), whereas the heel ST in the affected side correlated with BBS (r = -0.467, P < 0.05), QS (r = -0.532, P < 0.05), and STS (r = - 0.516, P < 0.05), suggesting that higher detected PTST might have an influence on the decreasing balance abilities. The study of sensory threshold in patients with stroke will help their rehabilitation and sensory evaluation.

Effectiveness of Sensory-Feedback Ankle Training on Motor Functions in Hemiparetic Patients (시지각을 이용한 발의 감각되먹임 적용이 편마비 환자의 운동기능에 미치는 영향)

  • 본 연구의 목적은 뇌졸중으로 인한 만성 편마비 환자를 대상으로 발에 시지각 자극을 적용하여 균형과 보행에 관한 운동기능에 미치는 효과를 알아보고자 하였다. 6개월 이상 된 편마비 환자 16명이 연구에 참여하였고, 무작위로 실험군(EG) 10명과 대조군(CG) 10명으로 구분하였다. 두 군 모두 기본적인 물리치료를 적용하였고 실험군은 정적균형도구인 Tetrax Multiple System을 이용하여 발목에 감각되먹임 시지각 자극을 매회 30분, 4주간 주 3회 실시하였다. 운동 중재 전과 후의 운동기능에 해당하는 균형(TUG), 보행(DGI)을 측적하여 본 연구의 효과를 비교하였다. 실험군에서 정적 균형, TUG, 그리고 동적 균형지수(DGI) 지수는 중재 전후 통계적으로 유의한 차이를 나타냈고(p<.05), 실험, 대조군간 전후의 변화율에서도 유의한 차이가 나타났다(p<.05). 대조군에서는 DGI 만 중재 전후 통계적으로 유의한 차이가 나타났다. 순수하게 유산소 운동만을 실시하였다. 통계처리 방법으로 전 후 차이를 검증하기 위하여 대응표본 t 검정을 실시하였고 대조군과의 차이 검증을 위하여 독립표본 t 검정을 실시하였다. 모든 통계적 유의수준은 0.05로 하였다. 본 연구에서는 편마비 환자에게 흥미를 촉진하고 운동 기능 회복의 효과를 강화할 수 있는 감각 되먹임이 동반된 시지각 자극을 적용하여 균형 및 보행기능의 향상을 얻을 수 있었고, 차후 연구는 장기간의 추적 관찰 연구가 지속되어야 하고 발목 뿐만 아니라 다른 관절에서의 비교연구가 필요할 것이다.
