Dimensional Structure of Emotion in Schizophrenic Patients (정신분열병 환자의 정서구조 분석: 2차원 모형을 중심으로)

  • 본 연구에서는 정신분열증 환자들이 정서상태를 개념화하고 얼굴표정을 통하여 타인의 정서 상태를 판단하는 과정을 정상인과 비교하였다. 연구1에서는 만 19세-46세의 정신분열병 환자 19명에게 정서 관련 형용사 15개로 구성된 105개의 형용사 쌍에 대하여 유사성을 평정하도록 하여 MDS로 분석한 결과, 쾌/불쾌와 각성의 두 차원이 일관되게 보고되고 있는 정상집단과는 달리 정신분열병 집단의 자료에서는 쾌/불쾌 차원만이 뚜렷하게 나타나고 각성 차원은 얻어지지 않았다. 연구 2에서는 다양한 정서를 표현한 얼굴 사진 28개를 정신분열병 환자 30명과 대학생 31명에게 제시하고 각 사진의 인물이 경험하고 있는 내적 상태를 쾌/불쾌와 각성의 두 차원에 대하여 각각 7점 척도로 평정하도록 한 결과 정신분열병 집단은 대학생 집단에 비하여 쾌/불쾌 차원에서는 보다 긍정적으로 평정하는 경향이, 각성 차원에서는 상대적으로 낮게 평가하는 경향을 나타냈다. 특히 부정적이고 각성 수준이 높은 내적 상태를 표현하는 사진 자극에 대하여 정신분열병 환자 집단이 각성수준을 과소평가하는 경향이 두드러지게 나타났다. 이와 같은 결과를 종합하여 볼 때, 정신분열병 환자들은 정서상태의 개념화 과정에서 각성 차원에 대하여 상대적으로 낮은 민감도를 보이고 있으며, 특히 부정적인 정서상태를 표현하는 얼굴들에 대란 판단에서는 각성 차원을 과소평가하여 높은 각성수준을 나타내는 표정과 낮은 각성수준을 변별하는데 어려움을 보이는 것으로 해석될 수 있다. 본 연구의 결과는 정신분열병 환자들은 자율신경계의 높은 각성 수준으로 인하여 각성 수준을 더 높일 가능성이 있는 자극을 회피하는 경향이 있다는 Mednick의 가설과 일관되는 결과로 이들의 각성 수준에 대한 낮은 민감도는 방어적인 회피성향의 지속에 따른 둔감화의 결과로 해석될 수 있다. 효과는 GSR를 비롯한 자율신경계지표에서 잘 반영되었다.^2$=.792가 되었다. 설명되는 누적분산값은 67.18%였다.주관적 평가의 결과와 객관적 평가 결과를 이용해 마직물의 태를 평가하는 산출식을 제시하였다. 태 평가치의 경우 16가지 특성치를 모두 넣는 방법과 stepwise 방법, 또 Kawabatark 사용한 순차적 군 회귀법의 세가지 방법의 회귀식 중 16가지 특성치를 모두 넣는 방법의 결정계수가 가장 높았다.tosterone농도는 107.7$\pm$12.0 pmol/l이었고, 남성의 타액내 농도는 274.2$\pm$22.1 pmol/l이었다. 이상의 결과로 보아 본 연구에서 정립된 EIA 방법은 RIA를 대신하여 소규모의 실험실에서도 활용할 수 있을 것으로 사려된다.또한 상실기 이후 배아에서 합성되며, 발생시기에 따라 그 영향이 다르고 팽창과 부화에 관여하는 것으로 사료된다. 더욱이, 조선의 ${\ulcorner}$구성교육${\lrcorner}$이 조선총독부의 관리하에서 실행되었다는 것을, 당시의 사범학교를 중심으로 한 교육조직을 기술한 문헌에 의해 규명시켰다.nd of letter design which represents -natural objects and was popular at the time of Yukjo Dynasty, and there are some documents of that period left both in Japan and Korea.   • A Computerzed Neuropsychological Test(CNT) system using multimedia and object oriented technologies are developed for clinical application in psychiatry. The developed system is composed of 14 neuropsychologial tests which are capable of evaluation of various cognitive functions and functional localization of brain. The system employs Microsoft Window based graphic user interface for easy operation and it has a touch screen and a mouse as input devices from the patient. Speech perception test, color word test, verbal memory test, contingent continuos performance test, and trail making test were translated into korean language, so that usefulness of tests was maximized. Through the results of utilization of this system in the cases of patients with head trauma and psychiatric desorder, this system can be proved to be useful in the evaluation of cognitive function and functional localization of brain.

Judgment on the Criminal Responsibility of Perpetrators with Mental Disorders and Their Mental Examination (정신장애 범죄인의 책임능력 판단과 정신감정)

  • This article focuses on §10 of Criminal Act of the Republic of Korea to discuss how to determine criminal responsibility of a perpetrator suffering from mental disorders, while reviewing existing process and standards of mental examination at each stage of the criminal procedure as well as exploring suggestions regarding how they should be complied. The determination of the sanity or criminal responsibility of the mentally ill as defined in the §10 of Criminal Act, by its nature, cannot be approached with a traditional, clear-cut dichotomy of biology by medical practitioner and psychology·normative science by lawyer. Looking into the actual procedure of determining mental and physical disorder with special consideration of mental illness reveals the inevitability of collaboration between lawyers and psychiatrists. In the meantime, the process and standards of mental examination at each stage of the criminal procedure must be definitive and clear. First of all, during the investigation stage, examination prior to prosecution should be more actively encouraged, considering that judging sanity of the perpetrator at the time of committing a criminal act is important. During the trial stage, the mandatory examination must be conducted depending on the sensitivity and gravity of the case. Next, medical examination to determine criminal responsibility and the one to order treatment and custody must be separately conducted in order to properly execute medical treatment and custody. Obligatory mental examination could be considered both during the stage of request for and execution of the treatment and custody. Lastly, the procedure of examination and format of examination documents need to be standardized for better objectivity and reliability.

The Relationship between Insomnia and Somatization According to Types of Work of Firefighters (소방공무원의 직무유형에 따른 불면과 신체화증상과의 관계)

  • Objectives : Firefighters are always under stress due to their job environment, and they are likely to become psychologically vulnerable due to continuous exposure to traumatic events, which is a stressful situation that requires emergency standby at all times. The aims of this study were to examine mental health factors for each division of firefighters and to see the relationship between sleep and somatization symptoms among them. Methods : General characteristics and related inspections were conducted through self-reporting questionnaires for 1,264 firefighters working at Chungbuk fire stations. Several mental health factors were investigated by the Insomnia Severity Index, Severity of somatic symptoms, Impact of Event Scale, Perceived Stress Scale, Resilience Scale, Alcohol Dependent Screening Scale, and the Center for Epidemiologic Studies Depression Scale Depression Scale and Mini-International Neuropsychiatric Interview. Results : Among the mental health variables, the relationship between insomnia and somatization varies significantly depending on types of work. It was reported that the first aid team was significantly higher than the fire suppression and rescue team. Post-traumatic stress, depression and drinking also differ significantly depending on types of work. The first aid team reported more posttraumatic stress than the fire suppression team and more depressed mood than the rescue team. The administration part had more alcohol consumption than the fire suppression or first aid team. Resilience was significantly higher in the rescue team than the first aid team. No significant differences between the types of work about stress and suicide risks. Insomnia, stress, and post-traumatic stress were significant predictors of somatization among firefighters. Above all, insomnia was significant mental health variable affecting somatization. Conclusions : Various mental health factors were different according to the types of work in firefighters. The First aid team was more vulnerable to mental health variables such as insomnia and somatization than other divisions. Insomnia is the most important cause of somatization in firefighters, so it is recommended that they be treated for insomnia and to prepare institutional policies.

  • A theroretical study was made on the psychodynamism of somatoform disorder. Somatoform disorder is caused by a defense mechanism of somatization. Somatization is the tendency to react to stimuli(drives, defenses, and conflict between them) physically rather than psychically(Moore, 1990). Ford(1983) said it is a way of life, and Dunbar(1954) said it is the shift of psychic energy toward expression in somatic symptoms. As used by Max Shur(1955), somatization links symptom formation to the regression that may occur in response to acute and chronic conflict. In the neurotic individual psychic conflict often provokes regressive phenomena that may include somatic manifestations characteristic of an earlier developmental phase. Schur calls this resomatization. Pain is the most common example of a somatization reaction to conflict. The pain has an unconscious significance derived from childhood experiences. It is used to win love, to punish misdeeds, as well as a means to amend. Among all pains, chest pain has a special meaning. Generally speaking, 'I have pain in my chest' is about the same as 'I have pain in my mind'. The chest represent the mind, and the mind reminds us about the heart. So we have a high tendency to recognize mental pain as cardiac pain. Kellner(1990) said rage and hostility, especially repressed hostility, are important factors in somatization. In 'Psychoanalytic Observation on Cardiac Pain', psychoanalyst Bacon(1953) presented clinical cases of patients who complained of cardiac pain in a psychoanalytic session that spread from the left side of their chests down their left arms. The pain was from rage and fear which came after their desire to be loved was frustrated by the analyet. She said desires related to cardiac pain were dependency needs and aggressions. Empatic relationship and therapeutic alliances are indispensable to psychotherapy in somatoform disorder. The beginning of therapy is to discover a precipitating event from the time their symptoms have started and to help the patient understand a relation between the symptom and precipitating event. Its remedial process is to find and interpret a intrapsychic conflict shown through the symptoms of the patient. Three cases of somatoform disorder patients treated based on this therapeutic method were introduced. The firt patient, Mr. H, had been suffering from hysterical aphasia with repressed rage as ie psychodynamic cause. An interpretation related to the precipitating event was given by written communication, and he recovered from his aphasia after 3 days of the session. The second patient was a dentist in a cardiac neurosis with agitation and hypochondriasis, whose psychodynamism was caused by a fear that he might lose his father's love. His symptom was also interpreted in relation to the precipitating event. It showed the patient a child-within afraid of losing his father's love. His condition improved after getting a didactic interpretation which told him, to be master of himself, The third patient was a lady transferred from the deparment of internal medicine. She had a frequent and violent fit of chest pains, whose psychodynamic cause was separation anxiety and a rage due to the frustration of dependency needs. Her symptom vanished dramatically when she wore a holler EKG monitor and did not occur during monitoring. By this experience she found her symptom was a psychogenic one, and a therapeutic alliance was formed. later in reguar psychotherapy sessions, she was told the relaton between symptoms and precipitating events. Through this she understood that her separation anxiety was connected to the symptom and she became less terrifide when it occurred. Now she can travel abroad and take well part in social activities.

Psychological Characteristics of Psychiatric outpatients with High Suicide Risk : Using MMPI-2-RF (정신건강의학과 외래 환자 중 자살 고위험 집단의 심리적 특성 : MMPI-2-RF를 이용하여)

  • Objectives : The purpose of this study was to examine whether the MMPI-2-RF serves as a useful tool to differentiate between the subtypes of high risk of suicide among psychiatric outpatients. Methods : Patients were recruited from the department of psychiatry of university hospital. Participants were diagnosed using DSM-5 criteria by board certified psychiatrists. Their medical records were reviewed retrospectively. And participants were put into 4 groups (Suicide ideation, Suicide attempt, Non-suicidal self-injury, and general psychiatric diagnosis as a control group). For statistical comparison, the MANCOVA with gender as a covariate was used. Results : The results indicated that as previous research with non-clinical sample suggested, psychiatric outpatients with high suicide risk also have significantly higher Emotional/Internalizing Dysfunction, Helplessness/Hopelessness, Suicidal/Death Ideation, Demoralization, Cognitive complaints, Cynicism, Dysfunctional negative thoughts than general psychiatric patients group. But group differences within the high suicide risk patients have not been observed. However, suicide attempt group and NSSI group has higher Behavioral/Externalizing Dysfunction, RC4, AGG than general psychiatric patients group. But there was no difference between suicidal idea group and general psychiatric patients group. Conclusions : There was no group difference observed between all three subtypes, which mean the MMPI2-RF may not be the useful diagnostic tool to navigate high suicide risk subtypes. Even though there was no difference observed in the suicide ideation group, suicide attempt group and NSSI group have higher aggression and externalization. So those indexes could serve as a useful marker to investigate riskiness of suicide related symptoms.

Correlations between Neurologic and Psychiatric Symptoms in Acute Stroke Patients (급성기 뇌졸중 환자에서 신경 및 정신 증상간 상호관계)

  • Objectives : This study aimed to investigate the correlations between neurological and psychiatric symptoms at two weeks after stroke. Methods : For 412 stroke patients, stroke severity was evaluated by the National Institutes of Health Stroke Scale(NIHSS), disability by the Barthel Index(BI) and modified Rankin Scale(mRS), cognitive function by the Korean Mini-Mental State Examination(K-MMSE), and muscle power by grip strength. Psychiatric symptoms were assessed by Symptom check list-90-Revision(SCL-90-R), consisted of nine symptom domains : Somatization, Obsessive-compulsive, Interpersonal sensitivity, Depression, Anxiety, Hostility, Phobic anxiety, Paranoid ideation, Psychoticism, and Additional items. The correlations between the neurological and psychiatric symptoms were investigated at the time of admission and before discharge(i.e. before and after treatment). Results : At the time of admission, NIHSS score was associated with scores on Phobic anxiety and Additional items ; and scores on BI and mRS were associated with Depression, Phobic anxiety and Additional items. At the time of discharge, NIHSS score was associated with scores on Somatization, Depression, Phobic anxiety, and Additional items ; scores on BI and mRS were associated with scores on Depression, Phobic anxiety and Additional items ; MMSE score was associated with Obsessive-compulsive, Depression, Phobic anxiety, and Additional items ; and grip strength was associated with Somatization, Depression, Anxiety and Additional items. Conclusions : More severe neurological symptoms were associated with higher psychiatric morbidity particularly in depression, phobic anxiety, sleep and appetite disturbance at acute stage of stroke. More intensive psychiatric care and intervention are needed for the high risk group.

  • This study validates the effectiveness of Hackathon Education which is popular entrepreneurship education method. And to reveal how entrepreneurship affects willingness of entrepreneurship and its team commitment. In addition, this study is to examine the moderating effect of transformational leadership in the relationship between entrepreneurship and its performance. The results show that entrepreneurship has no statistically significant result, but has positive impact on the willingness of entrepreneurship. However, moderating effect was proved between entrepreneurship and willingness of entrepreneurship. Therefore, entrepreneurship has positive effect on willingness by complementary relationship of transformational leadership Furthermore, entrepreneurship has positive impact on its team commitment and even though statistically not proven but also transformational leadership can affect its team commitment by interaction of entrepreneurship.

Mediating Effects of Self-reassuring and Self-attacking on Relationship between Mental Health and Psychological Well-being among College Students (대학생의 정신건강과 심리적 안녕감 간의 관계에서 자기위안과 자기공격의 매개효과)

  • This study examined the mediating effects of self-reassuring and self-attacking in the relationship between mental health and psychological well-being among college students. The study subjects were 230 (98 males and 132 females), and the collected data were analyzed using the SPSS PROCESS Macro based on regression analysis. The results of the study were as follows. First, there were significant relationship among mental health, psychological well-being, self-reassuring and self-attacking. Second, depression, obsessive compulsive disorder, interpersonal sensitivity and psychosis had a significant negative effect on psychological well-being. Third, self-reassuring and self-attacking had a partial mediating effect on the relationship between mental health and psychological well-being. The indirect effect of mental health on psychological well-being through self-reassuring was significantly higher than the indirect effect of psychological well-being through self-attacking. Based on the results of this study, we discussed the role of self-reassuring in the relationship between mental health and psychological well-being.

A Study on the Coping Experience of Mental Disorder Symptoms (정신장애인의 정신질환 증상 대처 경험에 관한 연구)

  • This work aims to explore personal coping and insight experience in mental disorder symptoms from the perspective of the parties in order to lay an empirical basis for the transition of the mental health service paradigm from a medical model to a human rights model. For this purpose, in-depth interviews with 8 persons with mental disorders were conducted and a model of practice was suggested through analysis using the grounded theory. As a result, 11 categories, 23 sub-categories and 132 concepts were identified. According to the analysis of this study, people with mental disorders have changed their perspective on symptoms through in-depth insight into their identities and symptoms as mental disorders, and discovered their own autonomous ways to cope with symptoms, managing their daily lives. Therefore, in developing a Korean alternative model for people with mental disorders, it is necessary to prepare conditions to find their own countermeasures through opportunities for insight.
