Costs of Korean Clubhouses for Community Mental Health Service (한국 클럽하우스 모델의 지역사회 정신재활 비용)

  • Purpose: The purpose of this study is to identify clubhouses general characteristics, core services, funding sources and costs in Korean Clubhouse Model, and to compare with Korean and international clubhouses. We explored the annual budget, cost per member, and cost per visit for 1 year. Methods: The data were collected from 14 Korean clubhouses and analyzed using descriptive statistics and Spearman's rank correlation with the SPSS 14.0 program. Results: The average of clubhouse operating period was 8.2 years. There were an average of 40.4 active members; among them, 84.1% were schizophrenia. In addition, there were an average of 5.8 staff and 15.3 services in each clubhouse. Cost estimates were as follows: annual budget (excluding housing) $223.633, cost per member $5,704, and cost per visit $21.35. There were significant difference among the annual budget, number of staff, number of service, and active members, but hours of Work-Ordered Day and social activities hours were not statistically significant. Conclusion: Findings provide a more understanding of operations, programs, and costs of Korean clubhouses.

Development and Application of a Web-based Expert System using Artificial Intelligence for Management of Mental Health by Korean Emigrants (해외 이민 한국인의 정신건강관리를 위한 웹기반 지능형 전문가시스템 개발 및 적용)

  • Purpose: The purpose of this project was to develop an international web-based expert system using principals of artificial intelligence and user-centered design for management of mental health by Korean emigrants. Using this system, anyone can access the system via computer access to the web. Methods: Our design process utilized principles of user-centered design with 4 phases: needs assessment, analysis, design/development/testing, and application release. A survey was done with 3,235 Korean emigrants. Focus group interviews were also conducted. Survey and analysis results guided the design of the web-based expert system. Results: With this system, anyone can check their mental health status by themselves using a personal computer. The system analyzes facts based on answers to automated questions, and suggests solutions accordingly. A history tracking mechanism enables monitoring and future analysis. In addition, this system will include intervention programs to promote mental health status. Conclusion: This system is interactive and accessible to anyone in the world. It is expected that this management system will contribute to Korean emigrants' mental health promotion and allow researchers and professionals to share information on mental health.

  • Depression has been causing huge direct and indirect losses to people's health because of its high prevalence, various clinical patterns, drug reaction and diverse courses different among individuals, but its treatment has not been systematic but dependent on individual clinicians' experience and knowledge. To correct this problem, it has been highly necessary to develop clinical guidelines defined as "systematically developed statements to assist practitioners' and patients' decisions about appropriate healthcare for specific clinical circumstances." Currently, countries throughout the world are making efforts to establish evidence-based guidelines among different levels of guidelines and to evaluate and test them. Although such efforts have been unsatisfactory in Korea because of several constraints including lack of high-quality RCT, the Korean Depression Clinical Practice Guideline has started to develop evidence-based guidelines, which are established through strictly designed processes. Thus, the present study purposed to review methods adopted in the development processes and to present the processes of developing the evidence-based guidelines clearly and transparently.

  • 발달 장애는 신체 및 정신이 해당하는 나이에 맞게 발달하지 않은 상태로, 다른 아동에 비해 신경정신과적 질환 발생 확률이 높기 때문에 발달장애 아동의 치료는 매우 중요하다. 그러나 주관적 판단에 의해 이루어지는 기존 작업치료의 경우, 정량적 성과 지표를 확인하기 힘들고 대상자 스스로 지속적으로 진행하기에 한계가 있다. 본 연구에서는 치료 모델을 가상 공간상에 구현하여 공간에 구애받지 않고 치료를 진행할 수 있으며, 수행 결과에 대한 자료를 정확하고 지속적으로 기록하며 확인할 수 있도록 하였다. 또한, AI 스피커를 통해 치료에 대한 피드백을 줌으로써, 대상자 스스로 실시하여 치료자의 개입을 줄여 심리적 부담을 덜어 더욱 정확한 수행이 이루어지도록 하였다.

A PRELIMINARY STUDY OF RELATIONSHIP AMONG TEMPERAMENTAL CHARACTERISTICS, FAMILY ENVIRONMENT AND DEVELOPMENTAL HISTORY (기질과 가정환경 및 발달사이의 관계에 관한 예비연구)

  • The objective of this study was to compare the family environment and developmental history of three groups of children classified by their temperament. The parents of 484 Korean children aged between 3 and 7 years completed the Korean version of Parental Temperamental Questionnaire developed by Thomas and Chess and Developmental Questionnaire created by Division of Child and Adolescent Psychiatry, Samsung Medical Center. After clustering these children into 5 temperamental groups according to the method proposed by Fullard et al, 98 Easy, 36 Difficult, and 21 Slow-To-Warm-Up children were included in the analysis. Statistically meaningful differences observed among three groups were as follow:1) Marital conflict of parents was more frequent in Difficult and Slow-To-Warm-Up children than in Easy children. 2) Parentchild conflict was more frequent in Difficult children than in Easy children. 3) Conflict among siblings was more frequent in Difficult children than in Easy children. 4) Average monthly income of family was less in Difficult children than in Easy children. 5) Toilet training was achieved later in Difficult children than in Easy children. 6) Motor development was slower, between 2 and 5 years old, in Slow-To-Warm-Up children than in Easy children. 7) Fear of stranger started earlier in Slow-To-Warm-Up children than in easy children. 8) Physical health was poorer in Difficult and Slow-To-Warm-Up children than in easy children. The findings indicate that Difficult child or Slow-To-Warm-Up child group have unfavorable family environment, different developmental milestone and poorer physical health in comparison with Easy child group.

Current Practices of the Customized Bibliographic Instructions in Academic Libraries (대학도서관 맞춤형 이용자교육의 운영 현황 분석 연구)

  • The purpose of this study was to investigate the current practices of the customized bibliographic instructions (CBI) at four-year academic libraries in Korea. Among the entire 206 academic libraries, a total of 149 libraries participated in the survey. Study result shows that the libraries providing CBI was 63.1%. The findings of the study revealed the overall picture of the CBI operated in Korean academic libraries. The findings of the study suggested useful strategies to apply in adopting and operating the CBI in academic libraries in Korea.

Phenolic Acid Composition and Antioxidative Activity of Chestnut Endoderm (밤 삽피의 페놀산 조성과 항산화 활성)

  • Free, soluble esterified and insoluble bounded phenolic acids were separated from Eungi chestnut endoderm. The composition and contents of phenolic acid were analyzed by gas chromatography, and their antioxidant activity was examined by DPPH assay, 2-deoxyribose oxidation, and ferric thiocyanate method. Gallic, ellagic, salicylic, and gentisic acids in free phenolic acid fraction, gallic, ellagic, and protocatechuic acids in soluble esterified fraction, sianpic and gentisic acids were the major phenolic acids in insoluble bounded fraction. Marked differences were observed in the phenolic acid composition and contents among the fractions. Free phenolic acid fraction showed the strongest antioxidant activity. Results revealed chestnut endoderm could be a potential antioxidant source containing gallic and ellagic acids.

The Association of Life Event Stress, Family Function and Cancer (암과 생활사건스트레스 및 가족기능의 연관성)

  • Purpose : To clarify the relation between psychosocial variables and cancer in Korea. Methods : Case-control study. Participants: 239 subjects in 2 university hospitals in Seoul completed a series of psychometric instruments(the Olson's FACES III and the Lee's 98-items life event scale). Results : In bivariable analysis, there were statistically significant difference in age and economic status(income): marginal significance in education status and marital status between the cases and controls. The family function type and stress score were not significantly different. The result of multivariable logistic regression, analysis showed that the risk of cancer was associated with economic status and marital status, but neither the family function nor the life event stress. Conclusion : In this study, we cannot prove the statistical association between the family function, life event stress and cancer. It is necessary to persevere in our efforts to clarify the relation between stress and disease and to develop the useful tools to measure the Korean family function and life event stress.

Emotional State and Personality Characteristics in Patient with Panic Disorder (공황장애 환자의 정서상태와 성격특성)

  • Objectives: The purpose of the present study was to investigate the emotional state and personality characteristics of patient with panic disorder. Methods: Twenty patients with panic disorder, 21 patients with somatoform disorder, and 20 normal healthy controls were studied. Korean version of Beck Depression Inventory(BDI), State and Trait Anxiety Inventory(STAI) and Korean standardized edition of Cattell's 16 Personality Factors Questionnaire(16-PF) were used for assessment. Statistically, One-way ANOVA with Scheffe test were used by SPSS/PC for windows. Results: 1) Total score of BDI was higher in the panic group than the somatoform group and normal control group(p<0.001). 2) Total score of state anxiety and trait anxiety in the panic group was higher than the somatoform group and normal control group(p<0.001). 3) In 16 PF, there were no definitely abnormal range of scores. But in first-stratum source traits of 16PF, the panic group was higher than the somatoform group and normal healthy group in O-factor (p<0.01) and Q4-factor(p<0.001). In second-stratum source traits of 16PF, the panic group was higher than the somatoform group in ANX-factor(p<0.05), but lower than normal control group in TOUfactor(p<0.05). 4) There were no differences in the panic subgroup according to sex and cutoff points of BDI score 16 and STAI-T score 54. 5) According to cutoff point of STAI-S score 52, the panic subgroup above 52 was higher than the panic subgroup under 52 in G-factor(p<0.001), Q3-factor(p<0.05) and SUP-factor(p<0.001), but lower in L-factor(p<0.05). Conclusion: These results suggest that emotional state of patients with panic disorder are depressive and anxious as compared with patients with somatoform disorder and normal controls. Patients with panic disorder did not show any abnormal personality characteristics but were more guilt-prone, anxious, emotionally sensitive. We propose that the understanding of panic patients' emotional state and personality characteristics will helpful to treat and manage in patient with panic disorder.

Association between Cognitive function, Behavioral and Psychological Symptoms of Dementia and Temporal Lobe Atrophy in Patients with Alzheimer's Disease and Mild Cognitive Impairment (알츠하이머형 치매 및 경도인지장애 환자에서 인지기능 및 행동심리증상과 내측두엽 위축의 연관성)

  • Objectives : The aim of this study was to compare severity, neurocognitive functions, and behavioral and psychological symptoms of dementia (BPSD) according to the degree of temporal lobe atrophy (MTA) in Korean patients with dementia due to Alzheimer's disease and mild cognitive impairment due to Alzheimer's disease. Methods : Participants were 114 elderly subjects diagnosed with Alzheimer's disease or mild cognitive impairment in this cross-sectional study. MTA in brain MRI was rated with standardized visual rating scales (Scheltens scale) and the subjects were divided into two groups according to Scheltens scale. Severity was evaluated with Clinical Dementia Rating (CDR) and Global Deterioration Scale (GDS). Neurocognitive functions was evaluated with the Korean version of Short Blessed Test (SBT-K) and the Korean version of the Consortium to Establish a Registry for Alzheimer's Disease assessment packet (CERAD-K). BPSD was evaluated with the Korean version of the Neuropsychiatric Inventory (K-NPI). Independent t-test was performed to compare severity, neurocognitive functions, and BPSD between two groups. Results : The group with high severity of MTA showed significantly lower scores in CDR, SBT-K, MMSE-KC, modified Boston naming test, word list recognition, and word list memory (p<0.05). There were no differences in K-NPI scores between two groups. Conclusions : Severity and neurocognitive functions of dementia had significant positive association with MTA, but BPSD had no association with MTA. Evaluating MTA seems to have potential benefit in diagnosing and treating neurocognitive impairments in the elderly. Further evaluation is needed to confirm the association between certain brain structures and BPSD.
