Due to the high population prevalence of major depression and the strong emphasis on pharmacotherapy for this disorder, antidepressants are among the most frequently prescribed pharmacological agents. But the clinicians are still unable to predict accurately the response of their depressed patients to medication. This article reviews the biological predictors of treatment response including monoamine, neuroendocrine, pharmacogenetic, and psychophysiologic markers. The biological predictors of response, despite some interesting leads that may in the long term be of considerable importance, are not yet sufficiently established to be of routine clinical usefulness. Many of the predictive factors explored in this article are examples of mediators and moderators that affect outcomes. Each one alone may not provide definitive answers for predicting response to treatment, but each must be taken into account at the outset of treatment. It is clear that treatments must be individualized for each patient. It would be necessary to develop the algorithm in order to predict the responsiveness of antidepressant treatment with integration of the results from the previous studies.
우울증 환자들에게 항우울제를 처방하는 임상의들이 흔히 겪게 되는 두 가지 어려움은 약물의 치료 반응 유무를 판단하기 위하여 처음 약물을 투여한 후 4~6주 이상을 기다려야 하는 것과 어떤 종류의 항우울제라도 처음 4~6주 이후에도 반응을 보이지 않는 환자들이 30~40% 이상이 된다는 것이다. 이와 같은 어려움을 극복하기 위해서는 환자 개개인의 항우울제에 대한 반응성을 미리 예측하는 것이 필요하다. 이 논문에서는 연구자들의 과거 실험들과 이미 발표된 연구들을 중심으로 하여 항우울제에 대한 치료 반응성을 예측하는데 약리유전학적 방법을 이용한 현재까지의 연구들과 연구 결과를 해석 할때 고려해야 할 사항을 살펴보고자 한다. 세로토닌 수송체(serotonin transporter, 5-HTT)는 항우울제가 신경세포에 작용하는 주요 작용부위 중 하나이다. 최근의 연구들에 의하면 5-HTT 유전자 promoter 부위의 기능적인 다형성(5-HTT linked polymorphism repetitive element in promoter region, 5-HTTLPR)이 항우울제에 대한 치료 반응성과 관련이 있는 것으로 알려져 있으며, 5-HTTLPR 유전형의 분포빈도는 인종들 간에서 차이가 있는 것으로 알려져 있다. 연구자들은 최근의 실험을 통하여 5-HTTLPR 유전형들의 endophenotype을 혈소판 막에 분포하는 5-HTT의 약동학적 특성으로 측정할 수 있음을 발견하였다. 흥미로운 사실은 5-HTTLPR 유전형의 분포가 인종적으로 다른 양상으로 나타났듯이, 그 endophenotype인 혈소판막의 5-HTT의 약동학적 특성 역시 전혀 반대되는 양상으로 나타났다. 하지만 이 endophenotype의 특성만으로 항우울제의 치료반응을 예측하는 것은 아직까지 한계가 있으며, 향후 이러한 과제를 해결하기 위한 방법중 약리유전학적 방법을 사용할 수 있음을 제안하였다. 예비적으로 시행한 실험을 통하여 연구자들은 세로토닌 수송체의 구조와 특징이 비슷한 생체아민 수송체들의 유전자 다형성들 간에 유의한 상관관계가 있음을 발견하였으며, 이들 유의한 상관관계가 있는 유전자형을 연합하여 조합할 때 세로토닌 수송체의 유전형만의 기여도보다도 항우울제에 대한 반응 예측도의 odds ratio가 유의하게 상승함을 발견하였다. 이러한 연구 결과들은 임상의가 항우울제를 처방 할 때에 환자들의 유전적 그리고 인종적인 배경을 고려하여 개별화된 전략을 사용하여야 한다는 가설을 뒷받침한다. 앞으로 항우울제의 작용기전과 그 대사과정에 관여하는 유전자들들 중심으로 유전자 간의 상호 작용을 밝히고 그 표현형이 약물의 치료 반응도에 미치는 기여도를 평가하는 작업들은 항우울제의 치료 반응과 그 부작용을 미리 예측할 수 있는 평가 도구를 개발할 수 있는 가장 최선의 길이 될 수 있을 것이다.
Lowe syndrome은 X-염색체 반성열성 유전 질환으로 1952년 Lowe 등에 의해 처음으로 보고되었다. 대부분 남성에게 발생하며, 주요 임상증상으로는 선천성 백내장 및 녹내장 등의 안구증상, 정신지체 및 근긴장저하 등의 근신경계 증상, 신장의 기능이상 등이 있고, 정신지체에 의한 행동조절 문제로 인해 전신마취를 시행할 경우 신장 기능 저하에 따른 대사성 산증과 악성 고열 발생의 위험성이 높아진다. Lowe syndrome으로 진단된 10세 2개월 된 남아가 치석이 많고, 칫솔질이 어렵다는 것을 주소로 전남대학교 치과병원 소아치과에 내원하였다. 임상 검사 시 전반적으로 심한 치석의 침착, 법랑질 형성 부전, 변연성 치은염, 영구치의 맹출 지연, 전반적인 치아 동요 등의 소견을 보였으며, 심한 정신지체로 인해 환자의 협조도가 부족하여 진정요법 하에 외래에서 치과치료를 시행하였다. 행동조절의 어려움과 전신마취시의 위험성, 대사장애 처치에 사용되는 각종 약물로 인한 치아착색과 치석형성의 용이함 때문에 Lowe syndrome 환아의 치과적 관리는 특히 예방에 중점을 두어야 한다.
우울증은 심각하며 재발하는 흔한 정신질환이다. 우울증은 환경 요인과 유전 요인, 그리고 신경생물학적 체계의 구조 및 기능의 변화로 발병한다. 후성유전학적 변화가 우울증과 관련 된다는 여러 연구들이 보고되었다. 후성 유전은 환경 요인이 크로마틴 구조를 변화시켜 DNA 염기 서열 변화 없이 유전자 발현을 조절하는 기전으로 설명된다. DNA 메틸화와 히스톤 아세틸화 및 메틸화를 포함하고 있는 히스톤 변형이 주요 후성유전기전으로 알려져 있다. 우울증 동물모델연구에서는 생애 초기 스트레스 같은 스트레스 환경이 게놈에 지속적으로 후성유전표지를 남기게 되고 이로 인해 유전자 발현이 변화되고 결국 성체가 되었을 때 신경 기능이나 행동 기능에 영향을 미치게 된다고 설명하고 있다. BDNF는 우울증과 관련된 대표적인 유전자로 알려져 있다. 설치류가 출생 전, 후, 그리고 성체 기간에 스트레스에 노출되면 해마에서 BDNF 유전자의 히스톤 변형과 DNA 메틸화 패턴이 변화되고 이로 인해 BDNF 발현이 변화된다. 이러한 과정은 불안과 우울 행동에도 영향을 미치게 된다. 본 종설에서는 BDNF 유전자의 히스톤 변형 및 DNA 메틸화와 같은 우울증 발병에 관여하는 후성유전기전의 최신 지견에 대해 논의하여 우울증 치료의 새로운 타겟 개발에 도움이 되고자 한다.
Risperidone은 D$_2$-수용체와 HT$_2$-수용체 차단효과를 동시에 지녀, 차세대 항정신분열증 치료제로의 가능성을 보이고 있는 약물이다. 그러나 risperidone의 주대사가 CYPIID1에 의할 가능성이 시사되고 있다. 본 연구에서는 향후 항정신병 치료제의 임상 제 1상 연구의 적정 모형 검토를 위해 risperidone율 모델 약물로 체내 약물의 동태, 내약성 및 약물 유전학적 특성등을 검토하였다. CYPIID1의 대사능을 검정한 12명의 정상 성인(11명: extensive metabolizer, 1명: poor metabolizer)을 대상으로 risperidone 투여 시험(placebo, 1, 2 mg)을 무작위 단일맹검 교차시험으로 시행하였다. 투약후 72시간까지 경시적으로 혈액, 뇨 채취 및 ECG lead II, 혈압/맥박의 변동 등을 측정하고, visual analog scale에 의해 중추신경계 주관적 증상들을 관찰 하였다. 혈장 risperidone 및 그 활성형대사물 9-OH risperidone 및 prolactin 농도는 radioimmunoassay법으로 측정하였다.
Autism Spectrum Disorder (ASD) is characterized by persistent deficits in social communication and restricted, repetitive patterns of behavior and interest. Sleep problems are not uncommon in children with autism spectrum disorders. Symptoms of insomnia are the most frequent sleep problems in individuals with ASD. Sleep problems can cause significant difficulties in the daily life of children with ASD and their families. Genetic factor, deregulations of melatonin synthesis, extraneous environmental stimuli and psychiatric and medical conditions may cause sleep problems. The first line treatment of sleep problems in ASD includes managements for potential contributing factors and parent education about sleep hygiene care for child and behavioral therapy. Supplementation with melatonin may be effective before considering other medications, such as risperidone, clonidine, and mirtazapine.
Objectives : Genome-wide association studies (GWASs) and meta-analyses indicate that single-nucleotide polymorphisms (SNPs) in the a-1C subunit of the L-type voltage-dependent calcium channel (CACNA1C) gene increase the risk for schizophrenia and bipolar disorders (BDs). We investigated the association between the genetic variants on CACNA1C and schizophrenia and/or BDs in the Korean population. Methods : A total of 582 patients with schizophrenia, 336 patients with BDs consisting of 179 bipolar I disorder (BD-I) and 157 bipolar II disorder (BD-II), and 502 healthy controls were recruited. Based on previous results from other populations, three SNPs (rs10848635, rs1006737, and rs4765905) were selected and genotype-wise association was evaluated using logistic regression analysis under additive, dominant and recessive genetic models. Results : rs10848635 showed a significant association with schizophrenia (p=0.010), the combined schizophrenia and BD group (p=0.018), and the combined schizophrenia and BD-I group (p=0.011). The best fit model was dominant model for all of these phenotypes. The association remained significant after correction for multiple testing in schizophrenia and the combined schizophrenia and BD-I group. Conclusion : We identified a possible role of CACNA1C in the common susceptibility of schizophrenia and BD-I. However no association trend was observed for BD-II. Further efforts are needed to identify a specific phenotype associated with this gene crossing the current diagnostic categories.
노쇠는 스트레스 요인에 대한 취약성이 증가하여 생리적인 예비능력이 감소되고 항상성을 유지하는 능력이 저하되는 증후군으로 입원, 장애 및 사망의 위험을 증가시킨다. 노쇠는 다양한 원인에 의해 발생하며 다차원적인 접근이 필요하고 조기에 발견해서 관리하는 것이 중요하다. 노쇠는 통증이나 우울증과 같은 신경정신과적 문제와도 깊은 관련이 있다. 노쇠를 평가하는데 있어서 질병, 영양, 움직임, 감각기능 등의 신체적 영역뿐만 아니라 심리사회적인 영역까지 포괄적으로 고려하는 것이 바람직하며, 대표적인 척도로 Fried의 신체적 노쇠 기준과 Rockwood의 노쇠 척도를 들 수 있다. 노쇠 관리를 위해서는 신체적 활동과 적절한 단백질 섭취가 중요하며, 부적절한 약물 사용을 줄이고 구강관리, 인지기능, 낙상에도 유의해야한다. 노쇠와 통증은 서로 영향을 미칠 수 있으며 통증이 노쇠를 촉진시킬 수 있다. 노쇠와 통증 연구 분야에서 호르몬과 단백질의 이상, 면역체계 활성과 염증반응, 그리고 후성 유전학의 기전이 공통적으로 작용하고 있다는 증거들이 발표되고 있다. 향후 좀 더 광범위하고 수준 높은 연구가 이루어져야 할 것이며, 그 결과를 노쇠와 통증의 억제와 치료에 적용한다면 삶의 질이 높아질 것이다.
Alzheimer's disease(AD) is associated with a characteristic neuropathology. The major hallmarks of AD are senile plaques (SPs) and neurofibrillary tangles(NFTs). ${\beta}$-amyloid protein($A{\beta}$) is derived from the proteolysis of amyloid precursor protein(APP) and then converted to SPs. Mature SPs produce cytotoxicity through direct toxic effects and activation of microglia and complement. NFTs are composed of paired helical filaments(PHFs) including abnormally phosphorylated form of the microtubule-associated protein(MAP) tau and increased tau level in cerebrospinal fluid may be observed in most AD. The aggregation of $A{\beta}$ and tau formation are thought to be a final common pathway of AD. Acetylcholine, dopamine, serotonin, GABA and their receptors are associated with AD. Especially, decreased nicotinic acetylcholine receptors(nAChRs) in AD are reported. Genetic lesions associated with AD are mutations in the structural genes for the APP located on chromosome 21, presenilin(PSN)1 located on chromosome 14 and PSN2 located on chromosome 1. Also, trisomy 21, Apo-E gene located on chromosome 19, PMF locus, low density lipoprotein receptor-related protein and ${\alpha}$-macroglobulin increase risk of AD. In this article, we will review about the neurobiology of AD and some newly developed research areas.
Objective : Since some studies have shown that the brain-derived neurotrophic factor(BDNF) has an important role in the pathophysiology of depression, this study investigated the relationship between BDNF genetic polymorphism and the long-term outcome of the antidepressant treatment. Method : One hundred and eight patients with major depressive disorder were evaluated for the long-term outcome(up to 3 years) of antidepressant treatment. The severity and improvement of depression were assessed with the Clinical Global Impression(CGI) Scale. The genotypes of BDNF 196A/G polymorphism in the patients were determined using Restriction Fragment Length Polymorphism(RFLP). Result : The genotypes of 128 patients were investigated and 95 patients of those have been evaluated for 3 years. No significant differences were noted comparing three-genotype groups for CGI scales at baseline, 4 weeks, 8 weeks, 1 year, 2 years and 3 years. Conclusion : This result shows that BDNF polymorphism investigated in this study was not associated with the long-term outcome of the antidepressant treatment. However, further studies with another BDNF polymorphism should be needed.
