Purpose: This study propose basic materials to prepare the mentally disabled for rehabilitation nursing mediating plans, by comparing the degree of self-care of the chronically mentally disabled residing in community mental health centers between those who participate in daytime rehabilitation programs and those who do not as well as the degree of perceived burden of the two groups' families. Method: The subjects of the study were 51 chronically mentally disabled men, who participated in daytime rehabilitation programs in three mental health centers in Gyeonggi-do and their families, and 53 in-house chronically mentally disabled men, who did not participate and their families. The measuring instrument for self-care consisted of ten spheres (65 questions and 5 points a standard). To quantify the feeling of burden of families, the measuring equipment developed by Montgomery et al. was used. Results: The degree of self-care of chronically mentally disabled men participating in rehabilitation programs in community mental health centers were higher than that of men not participating, and there was a significant difference between them. Families in the group not participating in rehabilitation programs had higher score than those in the group participating, but there was no significant difference. Conclusion: The participation of the chronically mentally disabled in rehabilitation programs influenced improvement of self-care and also lessened a feeling of burden of families somewhat.
The attention of case management currently has been highlighted and its importance has been highly emphasized in mental heath field. At this point, We conducted a qualitative study on the experience of case managers from the Suwon Community Mental Health Center where it has implemented the ACT model. Through the result of this study, we verified feasibility of ACT model into Korea. and it has shown that assertive service and team approach has been a critical ingredients of ACT and it extended case manager's ability and brought improvement in relationship between clients and them. the clients has brought a positive changes. However case managers have experienced the dilemma between experts initiatives and clients dependency, and they empathised the importance of the familism and mental health system in Korea. In order to implement ACT extensively, we suggested several implications.
This study was conducted to explore the relationship between the efficiency and its organizational determinants of tile Community Mental Health Centers(CMHCs). Data are obtained from 81 personnel of 27 CMHCs from Sept. to Oct. in 1999(19 in Kyunggi province, 7 in Seoul City and 1 in Chunchon, Kangwon province). Major findings of this study are as follows. 1. DEA is a mathematical programming technique that optimizes the relative efficiency ratio of inputs over outputs for each decision-making unit(DMU). It produces a summary scalar efficiency ratio for each DMU of CMHCs. It assessed multiple inputs and multiple outputs simultaneously, and compared to specific peer group of CMHCs. 2. Organizational determinants of DEA efficiency of CMHCs we proved as advertisement(+), location of CMHCs(in public facility)(+), area of facility(+), period of operation(+), job satisfaction(+), clarity of work-role(vague), cohesion(-), rate of certified personnel(+), number of referral(+), and voluntary service time(-).
Suicide is a complex behavior associated with various neurobiological and psychosocial factors. It is considered that genetic polymorphism combined with environmental stress such as child-adolescent trauma make differences in neurobiological systems, which cause psychiatric disorders or pessimistic personality, impulse-aggressive behaviors, lack of judgment, and finally result in suicidal behavior. Much progress in the neurobiology of suicide has been made over the several decades. There seems to be a hereditary disposition to suicide independent of psychiatric disorder. The changes in neurotransmitters, neurohormones, neurotrophic factors, cytokines, lipid metabolisms related with their genetic polymorphism can contribute to disturbance of signal transductions and neuronal circuits vulnerable to suicide. It is likely that the main factors are dysfunctions of serotonin (5-HT) and hypothalamus-pituitary-adrenal (HPA) axis. Our understanding about the neurobiology of suicide is still limited. However, clinical practice could be assisted by neurobiological findings capable of making the detection of risk populations with higher sensitivity and the development of new treatment interventions. The settlement of biological markers in suicidal behaviors and their relationships is required.

Purpose : Mental health service desire has been diversified according to the increase of economic level and rapid social change. Mental Health and Welfare Center(MHWC) is a provider of mental health services in Japan. This is a basic stage study which will suggest the architectural planning guidelines for MHWC. Methods : Data were collected through literature research, field surveys, and expert interviews to 69 MHWC in Japan. 1) Research for the policy and legal aspects of mental health support system. 2) Research for structural aspects of MHWC: Characteristics of establishment, management, and regional factors. 3) Research for the physical environment aspects of MHWC: Classification and evaluation of MHWC's Type by locational characteristics in Japan. Results : The result of this study can be summarized into three points. The first one, Mental Health and Welfare Center's service has being expanded to suicide, depression and stress from chronic mental illness, to reflect social needs. The second one, The average population of area installed at Mental Health and Welfare Center was 2,307,570 person, and average area of the regions were $5,745m^2$. The third one, Mental Health and Welfare Center is divided into single-structure type and combine-structure type. And combine-structure type is divided into medical-combine type, welfare-combine type, and public-combine type.
This study is a research that investigated organization of case management practice at a mental health center. For organization research of case management practice, this study applied the research method of Institutional Ethnography, and analyzed 'work knowledge' and 'texts' data which were obtained through field work that lasted 3 months with 11 study participants. The analysis is not interpretation of the researcher, but it is work that assembles 'work knowledge' of field workers until they reach discovering institutions that organize case management practice. As a result of this study, case management practice of the mental health center was being standardized as 'a service that is biased towards symptom management without comprehensive and continual resources links' by new public management theory, bio-medicine discussion, 'standardization policy', 'treatment rate improvement policy', 'performance-based budget system', 'performance evaluation system' and 'continual linkage policy', mediating texts. This is a different outcome from usefulness of case management that is claimed to support politically.

Objectives: The purpose of this study is to examine the modern history of public health(PH) and suggest a way forward for PH nursing(PHN). Methods: This paper is a review article that derives results from literature review. Results: In the period of beginning (up to 1944), PHN began as the PH Department was created in the Hygiene Bureau in 1908 and tasks about nurses were legislated. PHN was limited to infectious disease tasks and performed mostly by missionaries. In the period of foundation formation (1945 to 1961), the Republic of Korea was founded, and PH policies and tasks were defined with the establishment of the central government organization and the applicable laws. In the period of foundation establishment (1962 to 1979), the Regional PH Act was amended, and as a result, PH Centers(PHCs) spread across the country. In the period of foundation expansion (1980 to 1994), the PH referral system of PHCs, PH Units, and Primary Health Care Post was established. In the period of organization in each area (1995 to 2005), PH programs reflecting changes in disease structure and public needs for the quality of life. A regional health care plan was launched. In the period of funtion expansion (2006 to present day), Centers for support health living were established. Conclusions: In the future, PH nurses need to have a macroscopic perspective that views PH through the overall PH system, and to expand from the existing healthcare concept to the national and global healthcare one.

Purpose: The purpose of this study is to provide data on the basis for architectural planning of floor plan type and spatial composition by analysing standard mental health centers in Seoul. Methods: Data were collected through literature research, field surveys, and expert interviews to 22 standard mental health centers in Seoul. Results: The results of this study could be summarized as follows; Firstly, Mental Health Center is divided into five types according to the location. Location types of mental health center were 'Director type', 'Health center connection type', 'Public facilities connection type', 'Complex center connection type' and 'Commercial Facilities connection type'. Secondly, Depending on the type of management a mental health center is divided into two types. Types are 'Complex type' and 'Independent type'. Average area of 'Complex type' is $192.99m^2$ and 'Independent type' is $266.87m^2$. This difference affects the various spaces. Implications: It is necessary to give and architectural suggestion of mental health center in response to the proposal of the system.
