Acute Pancreatitis Associated with Varicella Infection in an Immunocompetent Child (정상 면역기능을 가진 소아에서 수두 감염 후에 발생한 급성 췌장염 1예)

  • Blunt trauma, drugs, infection, congenital anomalies of the pancreaticobiliary system, and multisystem diseases are the main causes of acute pancreatitis in children. Various viruses can cause acute pancreatitis, but varicella-induced pancreatitis is unusual and generally observed in adults or immunocompromised patients. We report a rare case of acute pancreatitis associated with varicella-zoster virus infection in a 6-year-old immunocompetent girl. The patient initially presented complaining of severe abdominal pain and repetitive vomiting. The patient had multiple cutaneous crusts that has been caused by preceding varicella infection and had elevated values of serum amylase and lipase. Abdominal ultrasonography demonstrated swelling of the pancreas and pancreatic duct dilatation, findings which were compatible with acute pancreatitis. The patient's clinical and laboratory abnormalities were completely normalized through conservative treatment consisting of fasting, total parenteral nutrition, and analgesic therapy.

Pulmonary Cryptococcosis in Immunocompetent Patients: CT Findings

  • Purpose: To evaluate the computed tomography (CT) findings of pulmonary cryptococcosis in immunocompetent patients. Materials and Methods: CT scans of 25 patients with biopsy-proven cryptococcosis [surgery (n=3), percutaneous needle biopsy (n=21), and bronchoscopic biopsy (n=1)] were analyzed. Thirteen patients were men and 12 patients were women, with a mean age of 53.7 years. Presenting symptoms were cough, sputum, and dyspnea and 12 patients presented with incidentally found chest radiographic abnormalities. Results: Nodule or multiple conglomerate nodules (n=10, 40%) and segmental or lobular consolidation (n=9, 36%) were most common, followed by mixed patterns (n=5, 20%). Predilection sites were lower lobe (n=21/37, 57%) and subpleural areas (n=23, 92%). Air bronchograms within consolidations (n=11/14, 79%) with mild volume loss (n=10/14, 71%) were common. While interlobular septal thickening (n=11, 44%) and cavitation or central low-attenuations (n=11, 44%) were relatively common, lymphadenopathy (n=2, 8%) or free pleural effusions (n=1, 4%) were uncommon. Conclusion: Nodules or airspace consolidation with a predilection of lower lobe and subpleural area are the most common appearances of pulmonary cryptococcosis in immunocompetent patients.

  • Crude 3 M KCI extracts of ovine foetal skin (FLS) were shown to induce serum inhibition of lymphocyte blastogenesis when injected to normal sheep. When fractionated through Sephadex C-150 columns, four protein peaks were obtained, immunosuppressive activity in vitro and in vivo being associated with Peak IV. In discussion, the immunosuppressive activity of FLS is compared with that obtained with extracts of ovine squamous cell carcinoma, and possible identity of the immunosuppressors is suggested.

Screening of the Immuno-stimulatory Activity of the Marine Alga Chlorella capsulate (해수 미세조류인 Chlorella capsulata의 면역활성증진)

  • A purified substance was separated from marine alga, Chlorella capsulate. Immuno-stimulatory activity was measured by observing the growth of human immune T and B cells. The growth of human T and B cells was enhanced by up to about 1.4 times by adding the fraction from C. capsulata, compared with commercially available CGF extracts from the fresh water alga, C. vulgaris. The secretion of TNF-a and IL-6 was also enhanced up to about 1.5 times by adding the separated fraction. Adding 0.4 g/L of the extract increased the production of nitric oxide from macrophages by 1.6 fold. 50% of the AGS cell growth was inhibited by adding 0.4 g/L of the water extracted sample.

  • 산한지통(散寒止痛)의 효능(效能)이 있는 고량강(高良薑)이 체내 면역활성에 미치는 영향을 검토하기 위하여 인삼과 함께 실험을 행하여 아래와 같은 결과를 얻었다. 고량강(高良薑) 및 인삼 수침액을 Sarcoma-180 고형 종양 세포를 이식한 생쥐에 투여하여 생존기간을 연장시켰다. 이를 바탕으로 면역에 대한 효과를 검토한 결과, 정상 ICR 생쥐에서 고량강(高良薑) 수침액은 백혈구수, 체중에 대한 상대적 비장의 무게를 약간 감소시켰으나 통계적으로는 유의성이 없었고, 간에는 영향을 주지 않았다. in vitro 실험에서, splenic IgM PFCs을 유의성있게 증가시켰으며, BSA를 주사한 생쥐에서의 Arthus반응도 증가시켰다. 그러나, 지연형 과민반응에는 영향을 주지 않았으며, 비특이성 면역에 관여하는 Macrophage 등의 탐식 세포의 기능에도 영향을 주지 않았다. 이상의 결과, 고양강은 체액성 면역을 강화할 것으로 보여진다.

  • 생체재료란 손상된 생체조직의 기능을 가능한 한 정상에 가까운 상태로 회복시킬 때 이용되는 재료이다. 생체재료는 보건 의료 분야 중 치료, 재활 및 예방의 수단으로 생체에 적용되기 때문에 인간 수명연장 및 사고, 질병의 증가 등에 따라 그 중요성이 점점 증가되고 있는 추세이다. 그럼에도 불구하고, 생체재료 이식에 따른 면역기전에 대해서는 명확하게 밝혀지지 않고 있다. 따라서 본 연구에서는 미생물의 특이 구조를 인식하여 염증반응을 유도하는 TLR (Toll like receptor)을 통한 면역학적 기전을 생체재료 이식시 나타나는 면역반응에 적용하고자 macrophage cell line인 RAW264.7을 이용하여 확인하였다. 생체재료(칼슘-인산계 뼈이식제, PCL microphere)의 특성은 electron microscope(SEM, TEM)을 통하여 관찰하였고 RAW264.7에 의한 세포 독성실험 (MTT와 LDH assay) 결과 세포독성은 없는 것으로 확인되었다. 생체재료가 TLR pathway에 미치는 영향을 Western blotting과 RT-PCR을 이용하여 확인하였으며, 면역반응 시 유도되는 cytokine을 RT-PCR과 ELISA를 이용하여 확인하였다.

Changes of CGRP immunoreactivity in rat trigeminal ganglion neurons during tooth movement (백서 삼차신경절내 신경세포체의 치아이동에 따른 CGRP 면역염색성의 변화)

  • GRP was known as the modulator of Pain transmission in central nervous system and local effector to peripheral tissue causing vasodilation, increased blood flow, modulation of immune sysem, stimulation of endothelial cell proliferation, and stimulation of bone formation. Numerous study, therefore, were done to elucidate involvement of CGRP to tooth movement. To investgate the response of CGRP immunoreactive nerve cells according to cell size in trigeminal ganglion during tooth movement, immunohistochemical study was performed using rat. Experimental rats(9 weeks old, 210 gm) were divided as six groups(normal(n=6), 3 hour group(n=5), 12 hour group(n=4), 1 day group(n=5), 3 day group(n=5), 7 day group(n=5)), and were applied orthodontic force (approximately 30 gm) to upper right maxillary molar. After frozen sections of trigeminal ganglions were immunostained using rabbit antisera, the changes of CGRP immunoreactive cells in regard to cell size distribution(small cell(upto $20{\mu}m$), medium cell($20-35{\mu}m$), large cell(above $35{\mu}m$)) were observed. The results were as follows 1. The percentage of CGRP immunoreactive cells to all nerve cells in trigeminal ganglion was 33.0% in normal control group, was decreased to 24.5% in 1 day group, and was increased to 41.8% in 7 day group. 2. The percentage of small, medium, and large cells expressing CGRP immunoreactivity in normal trigeminal ganglion to all CGRP immunoreactive cells were 51.3%, 44.0%, 4.7%, respectively. 3. The percentage of small cells with CGRP immunoreactivity to all CGRP immunopositive cells was increased in 3 hour and 12 hour groups. 4. The percentage of medium cells with CGRP immunoreactivity was increaed in 3 day and 7 day groups. 5. The percentage of large cells with CGRP immunoreactivity was increaed in 7 day group. Conclusively, the small cells with CGRP immunoreactivity in trigeminal ganglion respond to orthodontic force during initial phase of tooth movement, and later the medium and large cells with CGRP immunoreactivity respond

  • To investigate the role of myelin-associated glycoprotein (MAG) in the normal aging process, aging-related morphometric and ultrastructural analyses of the MAG-positive (MAG-(+)) oligodendrocytes were carried out in the cerebral cortex of the Sprague-Dawley rats. In the aged rats, the density of MAG-(+) oligodendrocytes was significantly decreased in the cortical layer (IV-VI) compared with that of the adult rats. However, the percentage of medium and dark types of oligodendrocytes was significantly increased by aging. In the aged rats, the mean nuclear area of the MAG-(-) oligodendrocytes was interestingly reduced compared with that of MAG-(+) oligodendrocytes. In addition, MAG immunoreactive products were markedly decreased in the medium-dark type of oligodendroglial cytoplasm and processes, and were scarcely localized in the dark type of oligodendrocytes of the aged rats. These results suggest that degeneration of oligodendrocytes-myelin system by aging is associated with down regulation of MAG, and that may contribute to further understanding of the biology of MAG in the oligodendrocytes-myelin system.

An Adaptive Anomaly Detection Model Design based on Artificial Immune System in Central Network (중앙 집중형 망에서 인공면역체계 기반의 적응적 망 이상 상태 탐지 모델 설계)

  • The traditional network anomaly detection systems execute the threshold-based detection without considering dynamic network environments, which causes false positive and limits an effective resource utilization. To overcome the drawbacks, we present the adaptive network anomaly detection model based on artificial immune system (AIS) in centralized network. AIS is inspired from human immune system that has learning, adaptation and memory. In our proposed model, the interaction between dendritic cell and T-cell of human immune system is adopted. We design the main components, such as central node and router node, and define functions of them. The central node analyzes the anomaly information received from the related router nodes, decides response policy and sends the policy to corresponding nodes. The router node consists of detector module and responder module. The detector module perceives the anomaly depending on learning data and the responder module settles the anomaly according to the policy received from central node. Finally we evaluate the possibility of the proposed detection model through simulation.
