Effect of the Height of Occlusal Appliance on Masticatory Muscles in Various Body Postures (다양한 자세에서 교합장치의 두께가 저작근에 미치는 영향)

  • 다양한 자세에서 교합장치의 두께가 저작근에 미치는 영향을 근전도학적으로 평가하고자, 측두하악장애의 징후와 증상이 없고 정상교합을 가진 성인 8명을 대상으로, 교합장치를 장착하지 않은 상태에서와 4.3, 6.0, 8.1mm 두께를 가진 교합장치를 장착한 상태에서 직립위, 식사위 그리고 앙와위에서 최대 이악물기 상태의 좌, 우측 교근 몇 전측두근의 근활성도를 채득, 분석하여 다음과 같은 결과를 얻었다. 1. 전측두근에서는 교합장치를 장착하지 않은 상태보다 교합장치를 장착한 상태에서 근활성이 유의하게 감소하였고 (p<0.05), 교근에서는 차이가 없었다. 2. 직립위에서 전측두근의 근활성은 교합장치를 장착하지 않은 상태보다 8.1mm 두께의 교합장치를 장착한 상태에서 유의하게 감소하였다 (p<0.05). 3. 식사위에서 전측두근의 근활성은 교합장치를 장착하지 않은 상태보다 8.1mm 두께의 교합장치를 장착한 상태에서 유의하게 감소하였다 (p<0.05). 4. 앙와위에서 전측두근의 근활성은 교합장치를 장착하지 않은 상태보다 4.3, 6.0, 8.1mm 두께의 교합장치를 장착한 상태에서 유의하게 감소하였다 (p<0.05).

CHARACTERISTICS OF CRANIOFACIAL STRUCTURES IN SEVERE ADULT CLASS III MALOCCLUSION (심한 골격성 III급 부정교합을 가진 성인 두개안면골격의 특성)

  • The purpose of this study is to define the characteristics of the skeleton and soft tissues of severe adult class III malocclusion. The materials selected for this study were lateral cephalograms of 112 adult class III malocclusion patients with ANB difference below -2 degrees. and the mean age was 22.9 years old. The normal control sampler consisted of lateral cephalograms of 50 adults in normal occlusion and the mean age was 22.1 years old. The Horizontal reference line was FH line and the vertical reference line was nasion perpendicular to FH line. The skeletal and soft tissue characteristics of Class III malocclusion are as follows : 1. In the skeletal profile evaluated by vertical reference line (Nasion perpendicular to FH), the forehead and maxilla was similar to normal, but the mandible was protruded significantly. 2. The soft tissue profile is concave. The thickness of soft tissue covering forehead area and nose is within normal range. but the upper lip is thicker and the nasolabial angle is smaller than normal. The lower lip and inferior labial sulcus is thinner than normal. The degree of eversion of lower lip is lesser than normal. 3. The cranial base of class III malocclusion is shorter and saddle angle is smaller than normal. 4. The location of midface evaluated in relations to cranial base is within normal range but, the length of midface is shorter than normal when compared from the deep portion of the facial skeleton. 5. The location of maxilla in reference to cranial base is within normal range but the length of maxilla was shorter in class III malocclusion. 6. The mandible was protruded, ramus height and body length, gonial angle were greater than normal, and the chin angle was smaller. 7. Upper incisor was proclined, lower incisor was retroclined.

Comparative Study on Structural Behaviors of Skull in Occlusions for Class I and Full-CUSP Class II (정상 I급 교합과 Full-CUSP II급 교합의 두개골 구조거동 비교 해석연구)

  • Recently, finite element analysis technique has been widely used for structural and mechanical understandings of human body in the dentistry field. This research proposed an effective finite element modeling method based on CT images, and parametric studies were performed for the occlusal simulation. The analyses were performed considering linear material behaviors and nonlinear geometrical effect, and validated with the experimental results. In addition, the skull models with two different molar relations such as Class I and full-CUSP Class II were generated and the analyses were performed using the proposed analytical method. As results, the relationships between the mandibular movement and occlusal force of both two models showed similar tendency in human occlusal force. However, stress was evenly distributed from teeth to facial bone in the skull model with Class I, while stress concentration was appeared in the model with full-CUSP Class II due to the changes of occlusal surfaces of the model.

THE PALATAL MORPHOLOGY OF THE CHILDREN WITH CLASS II DIV.1 MALOCCLUSION IN MIXED DENTITION : A STUDY USING THREE-DIMENSIONAL LASER SCANNER (혼합치열기 II급 1류 부정교합 어린이의 구개형태 : 3차원 레이저 스캐너를 이용한 연구)

  • The purpose of this study was to clarify the palatal volume and anterior palatal slope of the children with class II div.1 malocclusion and normal occlusion in mixed dentition(Hellman dental age III A) using three-dimensional laser scanner. Samples were consisted of 31 children with skeletal class II div.1 malocclusion in mixed dentition and 29 children with normal occlusion and profile among the contestants in 2000-2004 Healthy Dentition Contest in Seoul. Totally 60 maxillary study model were taken. Each cast was scanned by three-dimensional laser scanner (Breuckmann opto-TOP HE, INUS, Korea) and shaped into the three-dimension image by Rapidform 2004 program(INUS, Korea). And the palatal volume and anterior palatal slope of each cast were calculated by Rapidform 2004 program(INUS, Korea). The values were statistically compared and evaluated by independent samples t-test with 95% of significance level. The results were as follows: 1. Palatal volume was significantly lesser in children with class II div.1 malocclusion than that of normal occlusion in mixed dentition(p<0.05). 2. No significant difference in the anterior palatal slope and palatal height was found between the children with class II div.1 malocclusion and normal occlusion in mixed dentition(p>0.05). 3. Palatal length was significantly greater in children with class II div.1 malocclusion than that of normal occlusion in mixed dentition(p<0.01). 4. Intercanine and intermolar width were significantly lesser in children with class II div.1 malocclusion than those of normal occlusion in mixed dentition(respectively p<0.05 and p<0.01).

Posteroanterior cephalometric characteristics in skeletal Class III malocclusion (골격성 III급 부정교합자의 정모 두부규격방사선 계측학적 특징)

  • In proper diagnosis of skeletal Class III malocclusion, it was important to know the pattern of three dimensional skeletal & facial disharmony. The purpose of this study was to obtain P-A cephalometric characteristics in skeletal Class III malocclusion comparing with normal occlusion. The samples were consisted of 120 subjects, divided into four groups : Male normal occlusion, Female normal occlusion, Male skeletal Class III malocclusion, Female skeletal Class III malocclusion. Posteroanterior and lateral cephalogram were taken from the subjects with a x-ray apparatus (ASHAI CX90SP, Japan) and traced on acetate paper with routine manner. The transverse and vertical values from posteroanterior cephalometry, the sagittal values from lateral cephalometry and their ratio were obtained. The results were as follows: 1. The anteroposterior discrepancy in skeletal Class III group was not due to short maxillary length(Cd-A), but to longer mandibular length(Cd-Gn) than normal occlusion group. 2. The faces of skeletal Class III group were longer than normal occlusion group. It was not due to increase of upper face height(Cg-ANS) but to increase of the lower face height(ANS-Me) especially mandibular height(Cd-Me). 3. There was no difference in the facial width values between normal occlusion group and skeletal Class III group, except upper molar width(U6-U6), lower molar width(L6-L6) and mandibular width(Ag-Ag) of female skeletal Class III group which were larger than normal occlusion group. 4. The increase of mandibular length of skeletal Class III group was reflected in the increase of lower facial height but did not have an effect on the mandibular width.

THE POSITION OF THE INCISORS IN CORRELATION TO ANB ANGLE, PROCUMBENCY AND INCLINATION IN KOREAN NORMAL OCCLUSION (한국인 정상교합에 있어서 전치의 위치와 ANB각, 돌출도, 경사도와의 상관관계에 관한 두부 방사선 계측학적 연구)

  • 저자는 한국인 정상교합에 있어서 상하악 전치의 위치가 ANB각과 어떠한 상관관계가 있는지 그리고 전치의 위치가 돌출도와 경사도와 상관관계가 있는 지를 조사 연구하기 위하여 $20\~24$세 연령의 남자 54명, 여자 56명 합계 110명의 한국인 정상교합의 측모두부 X-선사진을 사용 분석했다. 이의 통계분석은 Computer 과정을 거쳤으며 다음과 같은 결과를 얻었다. 1. 상하악 전치 경사는 ANB각과 밀접한 상관관계가 있고, 상악전치의 위치는 ANB각에 (-)상관관계 하악전치의 위치는 ANB각에 (+) 상관관계가 있으며 이는Steiner 분석법의 acceptable compromise에서 보여준 것과 유사한 경향을 나타냈다. 2. 상악전치 경사는 상악돌출도와 (-) 상관관계가 있다. 3. 1-NA각은 상악경사도와 상호독립적이다. 4. 1-NA각은 하악돌출도와 하악경사도와 의의있는 상관관계를 나타냈다. 5. 1-NA거리와 상악경사도와의 상관관계는 여성군에서만 의의있는 것으로 나타났다. 6. 하악전치치축경사는 하악돌출도와 하악경사도와는 남성군에서만 상관관계가 있고 여성군에서는 의의가 없는 것으로 나타났다.

THE ANTERIOR-POSTERIOR AND VERTICAL RELATIONSHIP OF THE GROWING CHILDREN WITH CLASS III MALOCCLUSION BY LATERAL CEPHALOMETRIC MEASUREMENT (측모두부방사선 사진을 이용한 성장기 III급 부정교합아동의 전후방적, 수직적 악골관계에 대한 연구)

  • While making diagnosis and the treatment plan for the growing children who visited at Chonnam National University Hospital for orthodontic treatment, authors obtained 8 lateral cephalometric measurements in antero-posterior and vertical relationship such as APDI, WITS, ANB, SN-MP, ODI, PFH/AFH, Y-axis, SUM for children aged 7 to 9 with class III malocclusion and compared them with these of 73 children of elementary school aged 7 to 9 with proper profile and normal occlusion in Gwangju. The results were as follows: 1. Between normal occlusion and class III malocclusion, ANB, SN-MP, ODI, SUM, except PFH/AFH and Y-axis showed statistically significant differences(p<0.05). 2. Between mesurements to describe skeletal disorder of antero-posterior relationship such as APDI, WITS, ANB and skeletal disorder of vertical relationship such as SN-MP, ODI, PFH/AFH, Y-axis, SUM, all of them in both normal occlusion and Class III malocclusion showed significant correlation, except Y-axis, SUM correlation(p<0.01). 3. Wald' statistics of WITS, ANB and APDI expressing skeletal disorder of antero-posterior relationship showed 7.118, 5.148, 0.741, respectively and Wald' statistics of ODI, Y-axis, PFH/AFH, SN-MP, SUM were presented 28.348, 2.238, 1.376, 0.090, 0.089, respectively. Therefore, WITS and ODI could be considered as useful diagnotic measurements for class III malocclusion.

The structural change in the hyoid bone and upper airway after orthognathic surgery for skeletal class III anterior open bite patients using 3-dimensional computed tomography (3D-CT를 이용한 골격성 III급 개방교합자의 악교정 수술 전, 후 설골 및 상기도의 변화)

  • Objective: The purpose of this study was to investigate the structural changes of the hyoid bone and upper airway after orthognathic surgery for skeletal class III anterior open bite patients, and make comparisons with normal occlusion. Methods: Pre- and post-operative computed tomography (CT) examinations were performed on 12 skeletal class III anterior open bite patients who were treated with mandibular setback osteotomy. Using the V-works $4.0^{TM}$ program, 3-dimensional images of the total skull, mandible, hyoid bone, and upper airway were evaluated. Results: In the Class III open bite group, the hyoid bone were all positioned anteriorly, compared to the Normal group (p < 0.05). The angle between the hyoid plane and mandibular plane in the Class III openbite group before surgery was greater than in the Normal group (p < 0.05), and the difference increased after surgery (p < 0.01). In the Class III openbite group, the volume of the upper airway decreased after surgery (p < 0.001) and the volume of the upper airway was smaller than the Normal group before and after surgery (p < 0.001). Conclusions: The narrow upper airway space in skeletal Class III openbite patients decreased after mandibular setback osteotomy. This may affect the post-surgical stability.

Pattern Analysis of Occlusal Contacts During Lateral Excursion Using T-scan III System (T-scan III 시스템을 이용한 하악 측방운동 시 교합접촉 양상의 분석)

  • The objective of this study was to analyze occlusal contact pattern of 33 young adult males with normal occlusion during lateral excursion using T-scan III system which can measure the dynamic occlusion. Occlusal contact patterns were examined and categorized in non-working side disclusion point(more functionally acceptable position) not canine-to-canine position. Disclusion time, occlusal force, and occlusal force distribution ratio of upper canine at disclusion point during lateral excursion movement was also measured and compared among the groups divided according to contact patterns. The results showed that group function was clearly more dominant than canine guidance and other occlusal contact patterns different with existing patterns were found. There were significantly difference between groups in variables of T-scan measurement. T-scan III system is effective tools to analyze and evaluate occlusal contact patterns, disclusion time, occlusal force, and occlusal force distribution ratio of upper canine at disclusion point.
