Patients with Down's syndrome have several dental complications such as small teeth caused by underdevelopment of dentin and enamel, periodontitis, agenesis of teeth, prolonged retention of primary teeth and malocclusion due to narrow palate. Removable denture with maxillary double crowns would be a good treatment option to solve the problems of the patient with Down's syndrome. Double crowns compensate the insufficient support and retention of denture and easily solve the cross bite problem. Double crowns also allow easy repair of denture in case of abutment teeth extraction. In this case, 26-year-old female patient with Down's syndrome and dental phobia had small number of teeth with enamel hypoplasia, prolonged retention of primary teeth and dental cross bite. Prosthetic treatment was done using removable denture with double crowns in the maxilla. In the mandible, teeth preparation was done on enamel margin without anesthesia. Anterior laminate and posterior complete zirconia crown restorations were performed. As a result, the cross bite was effectively corrected by denture with double crowns. Pronunciation and appearance were also improved without extraction of teeth and dental anesthesia.
Tweed-Merrifield directional force technology is a very useful concept, especially for the treatment of Glass II malocclusion. It has contributed to treating a favorable counter-clockwise skeletal change and balanced face, while head gear force using high pull J-hook (HPJH) in an appropriate direction is also essential to influence such results. Clinicians have encountered some problems concerning patients' compliance; however skeletal anchorage has been used widely of late because it does not necessitate patients' compliance, yet produces absolute anchorage. In this case, a good facial balance was obtained by Tweed-Merrifield directional force technology using HPJH together with skeletal anchorage, which provided anchorage control in the maxillary posterior area, torque control in the maxillary anterior area, and mandibular response. This indicates 4hat skeletal anchorage can be used to reinforce sagittal and vortical anchorage in the maxillary posterior area during the retraction of anterior teeth. The author used HPJH for torque control, Intrusion, and the bodily movement of maxillary anterior teeth during on masse movement. However, it is thought that such a result nay also be achieved by substituting mini- or microscrews for HPJH. Consequently, Tweed-Merrifield directional force technology using skeletal anchorage for the treatment of Class II malocclusion not only maximiaes the result of treatment but can also minimize patients' compliance.

This study is performed to investigate the characteristics of the palatal morphology of the children with anterior crossbite in Hellman dental age IIIA by 3-dimensional laser scanner. Totally 40 study casts were taken; 20 were from children with crossbite and another 20 were from normal occlusion as a control. Each cast was scanned by 3 dimension laser scanner and shaped by the 3 dimension image by rapidform 2000 program(INUS, Korea). And finally it was calculated by Rhino 3D program(Rhinoceros, USA). The intercanine, intermolar cross-sectioned transverse plane and sagittal plane were measured. Due to the variations in palatal morphology, each group was standardized into 25mm, 35mm, 35mm. By sectioning standardized curves of the Palatal morphology per 1mm, the palatal depth of each point was calculated. Through these complex methods, the mean curves of the palatal morphology could be obtained and the values were statistically compared and evaluated by T-test with 95% of significance level. The results were as follows: 1. In the intercanine cross-sectioned transverse plane, the mean curve of palatal morphology of crossbite group was flatter V shape than that of control group, however, there was no statistical significance was found between two groups(P>0.05). 2. In the intermolar cross-sectioned transverse plane, the mean curve of palatal morphology of crossbite was deeper all over the area than that of control group, and the statistical significance was found in the middle area from point 8 to 21(P<0.05). 3. In the sagittal plane, the mean curve of palatal morphology of crossbite group was more deepening as approaching posteriorly than that of control group, and the statistical significance was found in all over the area(P<0.01).
Background: This study was done to better understand patterns, trends and local distribution patterns of malocclusion so that we can provide adequate information to patients, to help make the appropriate diagnosis and therapeutic plans, and to assess the future directions of malocclusion treatment. Materials and Methods: Malocclusion patterns, distribution and trends of visiting patients were examined in 993 malocclusion patients who had been evaluated and diagnosed at the Department of Dentistry, Yeungnam University Hospital over a 10-year period from 1995 to 2004. Results: The number of visiting patients per year showed an increasing trend and the visit rate was 1.28 time-higher in females(56.1%) than in males(43.9%). 1) Age distribution showed that the 7-12 year-old group was the largest (36.7%). Geographic distribution showed the majority of patients were from the Dalseo district(28.2%). Angle's malocclusion classification revealed that class III was the largest(38.4%). Crowding was the chief complaint in the highest percentage of patients(33.9%). The therapeutic method used was the fixed appliance in 61% of cases and a combination with extraction in 30.8%. Conclusions: The availability of dental services can accommodate orthodontic needs adequately as well as obtain reliable quantitative information regarding the characteristics of orthodontic patients.
Skeletal Cl III malocclusion is an orthopedic appliance mainly used for growing children with maxillary undergrowth, which largely entails skeletal Cl III malocclusion. It improves anterior crossbite and maxillary position and thus, enables patients to attain favorable Profile but often involves unfavorable profile with protrusive upper and lower lips. Therefore, if orthodontists have knowledge of which condition helps obtain favorable occlusion and profile, they are able to predict the prognosis and limitation of the treatment. This study was done in order to help obtain favorable Profile after treating growing skeletal Cl III children. In the study, we classified childern into two groups, the one with favorable profile(Group 1, n=12) and the other with unfavorable profile(Group 2, n=14) and, with retrospective study using pre- and post-treatment lateral cephalogram, drew the following conclusions. 1. As patients had more serious labioversion of upper incisors, they were more unlikely to have favorable profiles after the treatment. Protrusion of prosthion, which was related with maxillary incisors, also affected profiles. 2. As the NL-ML angle before the treatment was small, it was more likely to get favorable profile. 3. As the degree of lower lip protrusion was high, it was likely to have bialveolar protrusion after the treatment. 4. As the degree of downward and backward rotation of mandible was high, it was likely to get unfavorable profile.

De Grouchy syndrome or Distal 18q- is a genetic condition caused by a deletion of genetic material within chromosome 18, and the deletion involves the distal section of 18q. It causes a wide range of medical and developmental concerns. Congenital orthopedic anomalies, cleft lip and palate are relatively common. People with distal 18q- are often small for their age. Most individuals with distal 18q- fall in the mild to moderate range of intellectual disability. Strabismus and nystagmus, changes in the optic nerve as well as colobomas are also fairly common. People with distal 18q- frequently have conductive and/or sensorineural hearing loss. At present, treatment for distal 18q- is only symptomatic. This article presents a case report: Caries treatment of a 4-year-old female patient with de Grouchy syndrome under general anesthesia. The special considerations of dental care, especially caries treatment for the patient with de Grouchy syndrome are discussed.
The purposes of this study were to evaluate the nasolabial angle changes between closed lip position at centric occlusion and relaxed lip position at which the bite is open so that the lips do not touch and to elucidate the significance of the relaxed lip position for dentofacial diagnosis. Methods: The subjects consisted of 60 (35 Males, 25 Females) skeletal Class III malocclusion adult patients (mean age 23.3 years) with anterior crossbite. Results: In Class III malocclusion adult patients, there were significant differences in the nasolabial angle changes between closed lip position and relaxed lip position. Using the cluster analysis, the subjects were divided into three groups according to the pattern of nasolabial angle change: Group 1 (N = 27, 45%, $-8\;{\sim}\;1$), Group 2 (N = 30, 50%, $2^{\circ}\;{\sim}\;17$), and Group 3 (N = 3, 5%, over 18). Conclusion: The results showed that the pattern of the nasolabial angle change between closed lip position and relaxed lip position varies in skeletal Class III malocclusion patients. Thus, relaxed lip position should be taken into account when diagnostic records are obtained and analyzed to accurately to evaluate the facial soft tissues and predict facial esthetics after surgical-orthodontic treatment.

Class III malocclusion with mandibular protrusion can be divided into skeletal and pseudo malocclusion due to tooth displacement. For skeletal malocclusion, favorable treatment results can be obtained by establishing an appropriate vertical and horizontal intermaxillary relationship in order to secure a restoration space and obtain aesthetic and functional results. In this case, complete mouth rehabilitation was performed using an implant and a fixed prosthesis in a patient with mandibular protrusion and anterior teeth wear and reduced occlusal vertical dimension. After cast analysis and digital diagnosis, a provisional restoration with increased vertical dimension was fabricated to secure posterior support and evaluate stable centric occlusion. With the definitive prosthesis reflecting the provisional restoration, favorable function and aesthetics were obtained.

Early treatment is recommended for class III malocclusion in the primary dentition, though it is difficult to diagnose correctly. It was recommended dental maturation can possibly be used to make a differential diagnosis of skeletal class III malocclusion. This study aimed to compare dental maturation of first molars in children with skeletal or non-skeletal class III malocclusion in the primary dentition and to determine if dental maturation could be used to make a differential diagnosis of class III malocclusion. Among the children visiting the department of pediatric dentistry in Pusan National University Dental Hospital for anterior crossbite in the primary dentition, 18 were categorized into the non-skeletal class III malocclusion and 34 into the skeletal class III malocclusion. Panoramic radiographs were used to make comparative analysis of dental age and the eruption rate of the first molars. The following results were obtained. No difference was found between chronologic and dental age by the skeletal features or gender, with the latter being older than the former (p < 0.05). The discrepancies in eruption rate of first molars were significantly different between skeletal (18.91%) and non-skeletal groups (16.53%) (p < 0.05). This result implies that maturation of the first molars might be used to make a differential diagnosis of class III malocclusion.

The authors treated three patients who chiefly complained functional anterior crossbite due to premature loss of primary molars by using removable space maintainer and functional appliances. In orthodontic practice, the mandibular rest position and the possibility of taking construction bite have been as the criteria for evaluation of functional factors involved in anterior crossbite. Functional anterior crossbites, if left untreated, may have deleterious effects on the development and function of craniofacial complex and TMJ. Objectives of the treatment were as follows: 1) to recover vertical dimension 2) to eliminate functional disharmony 3) to correct anterior reversed occlusion 4) to attain good facial esthetics 5) to prevent unfavorable growth of jaw & dentition Characterized craniofacial morphology resulting from the premature loss of deciduous molars could be recovered following the correction of crossbite. Therefore, it is recommended that in orthodontic diagnosis of functional anterior crossbite due to premature loss of deciduous molars, the craniofacial abnormality affected by that should be considered.
