Purpose: Current results of autologous stem cell transplantation (SCT) suggest that this procedure may prolong disease free survival In patients with acute myeloid leukemia (AML). Autologous SCT is increasingly used as treatment for AML in first remission. The aim of this study was to evaluate the outcome of autologous SCT for patients with AML in first remission treated by autologous SCT using cytarabine, melphalan and total body irradiation (TBI) as the conditioning regimen. Materials and Methods: Between January 1995 and December 1999, 29 patients with AML in first remission underwent autologous SCT. The median age of patients was 33 years (range, 16 to 47). The conditioning regimen consisted of cytarabine ($3.0\;gm/m^2$ for 3 days), melphalan ($100\;gm/m^2$ for 1 day) and TBI (total 1000 cGy in five fractions over 3 days). Results: The median follow up was 40 months with a range of 3 to 58 months. The 4-year cumulative probability of disease free survival was 69.0%, and median survival was 41.5 months. The 4-year relapse rate was 27.6%. The factor Influencing disease free survival and relapse rate was the French-American-British (FAB) classification ($M_3$ group vs. other groups; p=0.048, p=0.043). One patient died from treatment-related toxicity. Conclusion:: Although the small number of patients does not allow us to draw any firm conclusion, our results were encouraging and suggest that the association of cytarabine, melphalan and TBI as a conditioning regimen for autologous SCT for AML on first remission appears to be safe and effective.
Twenty patients with biopsy-proven Waldeyer's ring lymphoma were treated with radiotherpy between 1984 and 1990 at the Department of Radiation Therapy, Inje University Paik Hospital and seventeen evaluable patients were analysed retrospectively. Dose of radiation ranged from 35 to 50 Gy to Waldeyer's ring structure with an additional 5 and 10 Gy boost dose to the primary site. The lower cervical nodes received 35 to 60 Gy. The median follow-up period was 24 months (range;9 to 80 months). The 5-year overall survival rate was $50.2{\%}$ and 5-year disease free survival rate was $47.1{\%}$. The final local control rate was $82.4{\%}$. The relapse developed average 10 months after treatment. Most of relapses were systemic ($87.5{\%}$). The patients with stage I disease fared better than advance stage. The favorable histology of lymphoma showed better prognosis than unfavorable histology. There was no significant difference in survival rate between radiotherapy alone and combination of chemotherapy and radiotherapy in early stage lymphomas. But of the patients with advanced stage, those who received chemotherapy and radiotherapy had better prognosis than those treated with radiotherapy alone.
To evaluate if the apoptotic fragment assay could be used to estimate the dose prediction after radiation exposure, we examined apoptotic mouse crypt cells per 1,000 cells after whole body $^{60}Co$$\gamma$-rays and 50MeV ($p{\rightarrow}Be^+$) cyclotron fast neutron irradiation in the range of 0.25 to 1 Gy, respectively. The incidence of apoptotic cell death rose steeply at very low doses up to 1 Gy, and radiation at all doses tigger rapid changes in crypt cells in stem cell region. These data suggest that apoptosis may play an important role in homeostasis of damaged radiosensitive target organ by removing damaged cells. The curve of dose-effect relationship for the data of apoptotic fragments was obtained by the linear-quadratic model $y=0.18+(9.728{\pm}0.887)D+(-4.727{\pm}1.033)D^2$ ($r^2=0.984$) after $\gamma$-rays irradiation, while $y=0.18+(5.125{\pm}0.601)D+(-2.652{\pm}0.7000)D^2$ ($r^2=0.970$) after neutrons in mice. The dose-response curves were linear-quadratic, and a significant dose-response relationship was found between the frequency of apoptotic cell and dose. These data show a trend towards increase of the numbers of apoptotic crypt cells with increasing dose. Both the time course and the radiation dose-response curve for high and low linear energy transfer (LET) radiation modalities were similar. The relative biological effectiveness (RBE) value for crypt cells was 2.072. In addition, there were significant peaks on apoptosis induction at 4 and 6h after irradiation, and the morpholoigcal findings of the irradiated groups were typical apoptotic fragments in crypt cells that were hardly observed in the control group. Thus, apoptosis in crypt cells could be a useful in vivo model for studying radio-protective drug sensitivity or screening test, microdosimetric indicator and radiation-induced target organ injury. Since the apoptotic fragment assay is simple, rapid and reproducible in the range of 0.25 to 1 Gy, it will also be a good tool for evaluating the dose response of radiation-induced organ damage in vivo and provide a potentially valuable biodosimetry for the early dose prediction after accidental exposure.
Here, we compared the effectiveness of 50 MeV($p{\to}RBe^+$) cyclotron fast neutrons versus $^{60}Co$${\gamma}$-rays by the apoptotic fragment frequency in both rat peripheral lymphocytes and crypt cells to check a radiobiological endpoint. The incidence of apoptotic cell death was increased in all irradiated groups, and radiation at all doses trigger rapid changes in both crypt cells and peripheral lymphocytes. These data suggest that apoptosis may play an important role in homeostasis of damaged radiosensitive target organ by removing damaged cells. In addition, $y=3.5+(118.410{\pm}10.325)D+(-33.548{\pm}12.023)D^2(r^2=0.992)$ after ${\gamma}$-rays in rat lymphocytes. A significant dose-response relationship was found between the frequency of apoptotic cell and dose. These data show a trend towards increase of the numbers of apoptotic cells with increasing dose. Dose-response curves for high and low linear energy transfer (LET) radiation modalities in these studies were different. The relative biological effectiveness (RBE) value for crypt cells was 1.919. In addition, there were significant peaks on apoptosis induction at 4 and 6h after irradiation, and the morphological findings of the irradiated groups were typical apoptotic fragments in crypt cells that were hardly observed in the control group. Thus, apoptosis induction in both crypt cells and peripheral lymphocytes could be a useful endpoint of rat model for studying screening test and microdosimetic indicator to evaluate the biological effects of radiation-induced cell damage.
In this study, a linear accelerator (LINAC) through 3 Gy of radiation per body irradiated mice of the small intestine and the liver to produce in order to protect the cells after radiation exposure that caspase (caspase 3 &caspase 9) and NO (nitric oxide), and looked like to know cytokine of IL-6 and TNF-${\alpha}$, the result is as follows. First, caspase 3 & caspase 9 showed a noticeable increase in the radiation group than in the control group both small intestine and liver tissues (P <0.001). Second, NO are both intestine and liver tissue showed a marked increase in the radiation group than in the control group (P <0.001). Third, one of Cytokine IL-6 and TNF-${\alpha}$ showed a significant increase in the irradiated group than the control group both small intestine and liver tissues (P <0.001).
Total body irradiation in the treatment of childhood leukemia, which is one of the pre-treatment with stem cell transplantation is being used, the current organization using compensators are treated. However, under the terms of the compensator organization long-term impact on the human body, it is difficult to assess directly. In this study, we use the mathematical simulation of radiation exposures body energy and the distance to the crew and the patient (source surface distance, SSD), and patients with tissue compensators change of the distance along the body of the organ doses were evaluated. As a result, the surface dose of energy 4 MV, SSD 280 cm, tissue compensators and the patient when the distance 30 cm 5.84 G / min showed the highest levels. In addition, patients with tissue compensators and the distance apart when 30 cm TBI represents the ideal dose distribution was found.
Purpose : The main Purpose of this study is to develop new payment rates for services of Radiation Oncology, considering costs of treating patients. Material and Methods : A survey of forty hospitals has been conducted in order to analyze the costs of treating patients. Before conducting the survey, we evaluated and reclassified the individual service items currently using as Payments units on the fee-for-service reimbursement system. This study embodies the analysis of replies received from the twenty four hospitals. The survey contains informations about the hospitals' costs of 1995 for the reclassified service items on Radiation Oncology. After we adjust the hospital costs by the operating rate of medical equipment, we compare the adjusted costs with the current Payment rates of individual services. Results : The current payment rates were 5.05-6.58 times lower than the adjusted costs in treatment planning services, 2.22 times lower in block making service, 1.57-2.86 times lower in external beam irradiation services, 3.82-5.01 times lower in intracavitary and interstitial irradiation and 1.12-2.55 times lower in total body irradiation. Conclusion : We could conclude that the current payment system on Radiation Oncology does not only reflect the costs of treating patients appropriately but also classify the service items correctly. For an example, when the appropriate costs and classification are applied to TBI, the payment rates of TBI should be increased five times more than current level.
Mercury, one of the most diffused and hazardous organ-specific environmental contaminants, exists in a wide variety of physical and chemical states. The murcury with the nature which evaporates easily can cause an acute or chronic mercury poisoning to workers at mercury-handling workplaces. Although many studies indicate that mercury induces a deleterious damage, little has been reported from the investigations of mercury effects at surrounding levels in living things. The purpose of this study was to evaluate the biological effects of mercury chloride and ionizing radiation. Prepubertal male F344 rats were administered mercury chloride in drinking water throughout the experimental period or were given wholebody irradiation with a dose of 6.5 Gy. The amount changed of body weight during the experimental period showed a 4.9% rise in the mercury-treated group and 14.4% decline in the irradiated group compared with the level of the control group. The results of hematological analysis (red blood cells, white blood cells, hemoglobin, and hematocrit) indicated the differential effects of mercury chloride and ionizing radiation. However the concentration of cortisol as assessed by radioimmunoassay increased in both of the groups. Relative expressions of mRNA related to mitochondrion-mediated apoptosis were investigated using semiquantitative reverse transcription polymerase chain reaction on gonad and urinary organs of the experimental groups. While the expression of Bcl-2 mRNA exhibited different patterns depending on the organs or the experimental groups, both of the experimental groups showed a conspicuous expressions of Bax mRNA. In conclusion, the target organ of mercury chloride seems to be a urinary organ and the pattern of damage induced by mercury chloride differs from that by ionizing radiation.

Whole spine scanography (WSS) is a radiological examination that exposes the whole body of the individual being examined to x-ray radiation. WSS is often repeated during the treatment period, which results in a much greater radiation exposure than that in routine x-ray examinations. The aims of the current study were to evaluate the patient dose of WSS using computer simulation, image magnification and angulation of phantom image using different patient position. We evaluated the effective dose(ED) of 23 consecutive patients (M : F = 13:10) who underwent WSS, based on the automatic image pasting method for multiple exposure digital radiography. The Anterior-Posterior position(AP) and Posterior-Anterior position( PA) projection EDs were evaluated based on the PC based Monte Carlo simulation. We measured spine transverse process distance and angulation using DICOM measurement. For all patient, the average ED was 0.069 mSv for AP position and 0.0361 mSv for PA position. AP position calculated double exposure then PA position. For male patient, the average ED was 0.089 mSv(AP) and 0.050 mSv(PA). For female patient, the average ED was 0.0431 mSv(AP) and 0.026 mSv(PA). The transverse process of PA spine image measured 5% higher than AP but angulation of transverse process was no significant differences. In clinical practice, just by change the patient position was conformed to reduce the ED of patient. Therefor we need to redefine of protocol for digital radiography such as WSS. whole spine scanography, effective dose, patient exposure dose, exposure direction. protocol optimization.
Radiation accidents which occured in the A.E.R.I. during last ten years are described (table 1). It seemed to the authors that some of these accidents were considered to be hazardous to man body and associated installations. This report deals with the following four major accidents involving body contamination incidents that our health physicists have been experienced. 1. Over-exposures (up to 130 rem) to the total body due to the mismanipulation in the Cobalt-60 gamma irradiation facility. 2. Floor surface contamination (up to 13 mrad/hr) and its spread out due to the mishandling of radioiodine contained in the bottle. 3. Body surface contamination and 0.36 uCi radioactivity accumulated in the thyroid gland of a worker due to the inhalation of gaseous iodine-131. 4. A void capsule due to the leakage out of the radium therapeutic source (3mg\ulcorner) These accidents were treated by definitely prompt action to protect the workers and associated installations from any radiation hazards and every possible efforts were made to confine the spread of radioactive contamination as small area as possible by means of elaborate decontamination work and monitoring.
