Patellar Inferior Pole: New Landmark for the Anteromedial Instrument Portal for Arthroscopic Surgery of the Medial Meniscus Posterior Horn (슬개골 하극: 내측 반월상 연골판 후각부에 대한 관절경 수술을 위한 전내측 기구 삽입구의 새로운 표식)

  • Purpose: We prospectively evaluated the clinical usefulness of the patellar inferior pole (PIP) as a landmark of the anteromedial (AM) portal for the arthroscopic surgery of the medial mensiscus posterior horn (MMPH). Materials and Methods: Group 1 (50 normal left knees of adults), Group 2 (10 normal knees under anesthesia), and Group 3 (50 consecutive knees undergoing elective arthroscopic surgery for relatively simple intraarticular pathologies, or diagnostic arthroscopy) were included. In Group 1 and 2, the true lateral (A) and valgus stress lateral radiographs (B) on $30^{\circ}$ flexion were obtained, and the lines (AM portal line) passing through the PIP and distal-most medial femoral condyle (MFC) were drawn under the condition without considering the thickness of articular cartilage of MFC (1, 2-A, B group), and considering it as 2.5mm on B (1, 2-C group). Then, we investigated the meeting point of the AM portal line with medial tibial plateau (C-D percentage), and measured the distance between the PIP and the anterior joint line (E-length), and medial tibial-femoral joint space (F-length). In Group 3, the AM portal was made at the PIP level and clinical usefulness of the approach to the MMPH and body of the lateral meniscus (LM) was analyzed. Results: The average C-D percentage came out as 85.8, 101.3, 69.1% for each Group 1-A, B, C, and 102.4, 144.6, 116.8% for each Group 2-A, B, C. Measured E-length was an average of 15.1 (Group 1-A), 15.5 (Group 1-B, C), 13.1 (Group 2-A), and 12.9 mm (Group 2-B, C) and the change by valgus stress had no statistical significance. The F-length increased about 1.2 (Group 1) and 3.6 mm (Group 2) when valgus stress was applied, which had statistical significance (p<0.001, p<0.001). In Group 3, 49, 48 knees were classified as good for the MMPH, and the body of LM in aspect of the clinical usefulness of AM portal made on the PIP level. Conclusion: We identified the clinical usefulness of the PIP as a skin landmark of AM portal for the arthroscopic surgery of the MMPH.

Femoral Tunnel Obliquity between the Transtibial and Anteromedial Portal Technique in Single Bundle Anterior Cruciate Ligament Reconstruction (전방십자인대 단일 다발 재건술에서 경경골 술기 및 전내측 삽입구를 이용한 대퇴골 터널의 기울기)

  • Purpose: To compare the obliquity of femoral tunnels prepared with transtibial (TT) versus anteromedial portal technique (AM) using x-ray in single-bundle anterior cruciate ligament (ACL) reconstruction. Materials and Methods: Among one-hundred thirty two patients who were undergoing ACL reconstruction from January 2007 to December 2009, thirty patients using TT and twenty patients using AM, those who had single-bundle ACL reconstruction, were evaluated with plain radiographs including anteroposterior, intercondylar notch and lateral view to compare the obliquity of bone tunnels. Results: The mean coronal obliquity of femoral tunnel for TT was $71^{\circ}$ (range; $65^{\circ}{\sim}77^{\circ}$), while for AM was $51^{\circ}$ (range; $39^{\circ}{\sim}60^{\circ}$) and the mean sagittal obliquity of femoral tunnel for TT was $22.7^{\circ}{\pm}7.8$, while for AM was $30.2^{\circ}{\pm}6.9$, their differences between them were statistically significant (P<0.05). However, there were no differences between two techniques on the tibial tunnel obliquity in coronal and sagittal plane. Conclusion: Anterior cruciate ligament reconstruction using AM portal technique allows more horizontally oriented and divergent femoral tunnel compared to that of transtibial technique. This seems to enable the graft placement into the femoral footprint and preserve the posterior cortical wall.

주관절 관절경의 해부학 및 Portals

  • 1931년 Burman은 주관절의 제한된 관절 공간, 관절낭 팽창의 어려움, 삽입구 주변에 근접한 신경-혈관 구조물에 의해 주관절 관절경은 불가능하다고 보고하였다. 비록 1935년 자신이 다시 여러 번 시도 끝에 주관절 전방 관절낭을 관절경으로 관찰한 후 주관절도 관절경적 접근이 가능하다는 점을 처음으로 언급하였지만 1980년대 중반까지도 몇몇의 보고를 제외하고 주관절 관절경에 대한 관심은 매우 적었다. 그 후 주관절 관절경은 관절경 기구들의 발달과 소관절을 위한 소구경 관절경의 발전과 함께 하였다. 1985 Andrew와 Carson이 supine position에서 전외측, 전내측 그리고 후외측 삽입구를 이용하여 주관절을 관찰하였으며 1989년 Poehling은 후방 주관절 접근이 용이하도록 prone position으로 관절경을 시행하였고 최근 새로운 접근 방법 및 확대된 적응증에 대한 보고가 점점 늘어나고 있다. 주관절 관절경의 합병증은 적지만 주관절 주변의 해부학적 구조물에 대한 정확한 이해를 가지고 있어야 심각한 합병증을 발생을 예방할 수 있다.

Arthroscopic Reduction of Subluxed Medial Meniscus using Suture Anchor for Restoration of Hoop Stress - Technical Note - (버팀테응력 회복을 위한 아탈구된 내측 반월상 연골의 Suture anchor를 이용한 정복술 - 술기 보고 -)

  • Purpose: The authors introduce a new technique of arthroscopic reduction of subluxed medial meniscus using suture anchor for the restoration of hoop stress. Operative Technique: Anterolateral, anteromedial, and medial midpatellar arthroscopic portal are used. Arthroscope was inserted through anterolateral portal. Through the scope, we confirmed subluxation of medial meniscus. Transection of menisci including radial and root tear were excluded. We released the anterior horn of medial meniscus through anteromedial and burred the future insertion site of suture anchor. After inserting suture anchor through medial midpatellar portal, we used 90 degree suture hook and no.2 Nylon to retrieve the suture of inserted anchor. We tied the suture by sliding knot-tying method. Weight bearing was limited for 6 weeks postoperatively. Conclusion: Arthroscopic retightening of medial meniscus is less invasive, conserving and progressed method for subluxed meniscus.

All-inside Arthroscopic Capsular Imbrication and Lateral Release in Patellofemoral Instability (Operative technique) (슬개대퇴관절 불안정성에서의 관절경적 All-inside 관절막 중첩술 및 외측 지대 유리술 (수술 술기))

  • Purpose: We introduce a technique of all inside arthroscopic capsular imbrication and lateral release used to treat patellofemoral instability. Methods: With the arthroscope in the anteromedial portal for best viewing, the arthroscopic scissor is placed through superolateral portal for proximal to distal release. The release performed 5mm to 1cm from the edge of the patella. After completion of the procedure, with the arthroscope in anterolateral portal, we inserted 5mm cannula in superolateral portal and made working portal from superomedial portal. Medial reefing was performed with all inside technique by using curved needle of the spectrum suturing system and No. 1 monofilament PDS suture is passed through the superomedial portal percutaneously and retrieved through a superolateral portal. Conclusion: Several methods for arthroscopic patella realignment have been proposed, but they have consisted primarily of arthroscopically assisted techniques using a medial incision. We believe that our procedure is preferable to arthroscopically assisted methods commonly used, in that an incision is avoided and the vastus medialis obliqqus is not violated. Our technique is minimally invasive and is easy to control the tightness of the medial patellofemoral ligament (MPFL) under direct vision.

Observation of Lateral Compartment through Posteromedial Trans-posterior Septal Portal in Knee Joints (슬관절 후내측 후격막 통과 도달법을 이용한 외측 구획의 관찰)

  • Trans-posterior septal posterior arthroscopic portals are useful in performing arthroscopic diagnosis and treatment of the lesions located in posterior aspect of knee joints. Except inferior surface of posterior horn of lateral meniscus, we easily observed entire lateral compartment, especially inferior surface of anterior horn and lateral tibial condyle by figure of 4 position through posteromedial trans-posterior septal arthroscopic portal. And it was possible to obtain visual field from posterior aspect and we could use anteromedial and anterolateral portals for instrumentation without clouding. So, this procedure may be useful in performing surgery of lateral compartment especially, anterior aspect in the knee joints.

Arthroscopic Reduction and Fixation of an Anterior Cruciate Ligament Avulsion Fracture From the Tibial Eminence Using Bioabsorbable Pins - Technical Note - (생흡수성 핀을 이용한 소아 경골 과간부 견열 골절의 관절경적 정복 및 고정 방법 - 수술 술기 -)

  • Purpose: We describe a new and simple technique for arthroscopic fixation of tibial intercondylar eminence avulsion fracture using bioabsorbable pins in skeletally immature patients. Operative Technique: Diagnostic knee arthroscopy is performed using anterolateral and anteromedial portals. Fracture debris and blood clot are debrided to expose the injured site well. The fragment is reduced with the probe and fixed temporarily with a 1.1-mm diameter K-wire that is inserted percutaneously from the anterosuperior aspect of the knee joint. The drill guide is introduced into the joint and the fragment is secured by bioabsorbable, poly-p-dioxanone 1.3-mm pins inserted from different angles. The pins are 40 mm in length. The knee is placed in a long leg cast in extension for 4 weeks to assure that full extension is obtained. Conclusion: Arthroscopic fixation of an tibial intercondylar eminence avulsion fracture using bioabsorbable pins is not a technically demanding, suitable method that ensures fracture healing and restores the stability of the joint.
