Thoracoscopic Esophagectomy for Esophageal Cancer -One Case Report- (식도암에서의 흉강경 식도적출술 치험 1례)

  • Thoracoscopic esophagectomy can be performed in esophageal diseases to reduce the postoperative complications. Recently, We encountered a case of esophageal cancer and successfully treated it by thoracoscopic esophagectomy with gastric pull-up. A 59-year-old male was presented with swallowing difficulty and an esophagogram, esophagoscopy, and chest CT showed an ulcerating tumor on the lower esophagus. The operation was performed in three stages: mobilization of the esophagus by thoracoscopic surgery, construction of a gastric tube through a laparotomy, and cervical anastomosis between the esophagus and the gastric pull-through. Hoarseness developed postoperatively, and the postoperative esophagogram showed leakage at the esophagogastric anastomotic site. The anastomotic leakage was healed following surgical drainage and the patient was discharged in good health. Hoarseness subsided spontaneously two months after surgery.

Evisceration and Intrascleral Silicone Ball Prosthesis with Penetrating Keratoplasty on Perforated Corneal Ulcer Secondary to KCS in a Shih Tzu Dog (개의 건성각결막염에 의한 각막천공을 안구내용물적출술과 실리콘볼 삽입 후 각막이식을 적용하여 치료한 증례)

  • When severely large corneal perforation occurs, penetrating keratoplasty is a treatment of choice alternative to enucleation. A twelve-year-old male Shih Tzu was referred with perforated corneal ulcer secondary to keratoconjunctivitis sicca (KCS). Perforated cornea was directly sutured using 10-0 non-absorbable suture material, and rotational conjunctival flap was performed. However, re-perforation of cornea by wound dehiscence was observed at 1 month after operation. The yellowish lens escaped outside the orbit during corneal re-perforation, the diagnosis was re-perforated corneal ulcer, moderate corneal edema, moderate KCS (STT; 6 mm) and endophthalmitis caused by escaped lens outside orbit. Accordingly, penetrating keratoplasty (PK) and evisceration through corneal recipient site and intrascleral silicone ball prosthesis were carried out as the planned treatment, and resulted in good cosmetic improvement compared to enucliation. However, exposure of silicone ball occurred at the 9 months after the surgery due to the irritation of implant, thus enucleation was performed. In perforated large corneal ulcer with severe intraocular damage, evisceration with silicone ball insertion with PK would be alternative treatment choice to improve the cosmetic appearance.

  • 평활근육종은 주로 사지와 몸통에 주로 발생하며, 악하선의 원발성 평활근육종은 매우 드물다. 악하선의 원발성 평활근육종은 증상과 방사선 검사 모두에서 비특이적이며, 치료는 광범위 절제술이다. 최근 저자들은 악하선에 발생한 원발성 평활근육종을 광범위 적출술을 시행한 후에 술후 방사선 치료를 시행하여 치험하였기에 문헌 고찰과 함께 보고하는 바이다. 악하선의 평활근육종은 드물지만, 방사선 검사에서 경계가 분명하고 조영 증강이 잘되며, 세침흡입검사에서 방추세포가 발견될 경우에 감별 진단으로 고려해야 한다.

  • 식도의 양성 종양은 흔하지 않았으며 이중 신경초종은 아주 드문 식도 양성종양이다. 봉원에서는 점점 심해지는 연하곤란을 주소로 내워한 52세 여자환자에서 식도에 생긴 신경초종을 우측 개흉술을 통한 종양 적출술로 성공적으로 제거 하였기에 이에 문헌 고찰과 더불어 증례 보고하는 바이다.

The Effect of a Preparation Method using Hospital Play on Anxiety and Role Behaviors of Children Undergoing Tonsillectomy (병원놀이 교육이 편도선 적출술 아동의 수술 전 불안과 수술 후 환아 역할행위에 미치는 영향)

  • Purpose: The purpose of this study was to examine the effect of a preparation method using hospital play on anxiety and role behaviors of children undergoing tonsillectomy. Method: Twenty-eight children aged 5 to 7 years who were admitted for tonsillectomy were assigned to either an experimental group or a control group purposefully. The preparation method included provision of information using hospital play. The data were analyzed using Mann-Whitney test and Chi-square test. Result: There was no Significant difference in anxiety before surgery between the experimental and control groups. Also, there was no Significant difference in role behaviors on the 1st and the 2nd day of surgery. Conclusion: Even though there was no statistical significance, the result showed that the preparation method had some positive effect on children and parents. It is needed to develop systematic preparation methods for children undergoing surgery to enhance their coping.

Complications Following and Supplementary Procedures for a Pancreas-preserving Total Gastrectomy (위 전절제술에서 췌장보존 비장적출술의 합병증 및 보완술식)

  • Purpose: A pancreas-preserving total gastrectomy (PPTG) was introduced to decrease the postoperative complications due to pancreatic resection. However, some complications, such as leakage of pancreatic juice, are still reported. Thus, the purpose of this study was to propose a supplementary procedure based on the results of treatment for gastric cancer at our hospital. Materials and Methods: From Jan. 1997 to Dec. 2004, the cases of 141 patients who underwent a PPTG for gastric cancer were reviewed retrospectively. The patients were divided into Group A (38 cases), patients who were treated using a conventional PPTG, and Group B (103 cases), patients who were treated using a new and improved PPTG. Their postoperative complications were compared. Results: No statistically significant differences in clinicopathologic data were noted between the two groups. The comparison of complications showed for groups A and B, respectively, 4 and 0 cases of pancreatic fistula, 1 and 0 cases of intraabdominal abscess, 2 and 0 cases of intraoperative pancreatic necrosis, and 2 and 2 cases of minor leakage. The difference in the prevalence of complications between the two groups was statistically significant (P=0.0001). Conclusion: In order to reduce the risk of PPTG-related complications, we used vascular clamps to observe the necrosis of the pancreatic tail before dividing the splenic artery, and this method resulted in a significant decrease in postoperative complications. Thus, we conclude that our use of vascular clamps in a PPTG is a simple and useful method for preventing postoperative complications.

  • The majority of esophageal foreign bodies can be removed by esophagoscopy, but some cases may require removal by external route. Recently, we experienced an impacted denture in the third esophageal constriction area, and the patient has pre-existing progressive muscular dystropy. We tried esophagoscopic removal for several times, but failed because the denture clasps were penetrated into the esophageal wall. Open thoracotomy was performed and impacted denture was successfully removed.

Surgical Management of Dentigerous Cyst for Preserving Permanent Tooth Buds (영구치 보존을 고려한 함치성 낭종의 외과적 처치)

  • Dentigerous cyst is the most common developmental odontogenic cyst of the jaw which is associated with unerupted teeth. Treatment modalities range from enucleation to marsupialization. Enucleation is the process in which the cyst is completely removed, and this is usually indicated for smaller lesions. Larger cysts can be treated by marsupialization, which is a process that consists of making a surgical cavity on the wall of the cyst, emptying its content and maintaining the continuity between the cyst and the oral cavity. This procedure allows decompression of the cyst, regeneration of the bone-defected area, and also lets the wall of the cyst change into normal mucosa. This technique will protect adjacent structures and will promote spontaneous eruption of succedaneous teeth previously surrounded by the cyst. These are two case reports of a 5-year-old and an 11-year-old boy with dentigerous cysts found regarding supernumerary tooth and pulpotomized mandibular primary molars, respectively. Due to the large size of the cysts, both cases were treated with marsupialization. This case report shows that in cases regarding large-sized dentigerous cysts, marsupialization can be an effective surgical technique that promotes spontaneous eruption of cyst-involved succedaneous teeth and the preservation of adjacent teeth.
