Correlation between Lumbar Malposition and Disc Herniation in Lumbar Disc Herniation Patients: Focused on L4-L5, L5-S1 (요추 추간판 탈출 환자에서 나타나는 요추부 변위와 추간판 탈출의 상관관계: L4-L5, L5-S1 요추 간 추간판 탈출을 중심으로)

  • Objectives This study was conducted to find out the correlation between lumbar malposition and lumbar disc herniation. Methods Among the patients who visited Gwangju Jaseng Hospital of Korean Medicine from January 2019 to January 2021, taking 92 patients under age 60 who had the records of X-rays and magnetic resonance imaging (MRI) images as the research subjects, Cobb's angle was measured in anterior-posterior (AP) view and lateral (LAT) view, the number of displacements was scored by listing categories defined by The Korean Society of Chuna Manual Medicine for Spine and Nerves. The degree of lumbar intervertebral disc herniation was expressed as a percentage according to the method of Kato, etc., and the correlation between each factor was analyzed. Results AP curvature and MRI herniation index showed significant positive correlation in L4-L5 level but there was no significant difference in L5-S1 level. LAT curvature and MRI herniation index had no correlation in L4-L5 level, but there was a significant negative correlation in L5-S1 level. Malposition score and MRI herniation index had a significant positive correlation in L5-S1 level, whereas there was no correlation in L4-L5. Conclusions As a result of the study, AP curvature and MRI herniation index showed a significant positive tendency in L4-L5 lumbar spine, and LAT curvature and MRI herniation index showed a significant negative tendency in L5-S1 lumbar spine. Malposition score and MRI herniation index were found to have a significant positive tendency in L5-S1 lumbar spine.

Korean Medical Treatment after Surgical Treatment of Necrotizing Fasciitis on Thigh : A Case Report (대퇴부 괴사성 근막염의 외과적 수술 후 한방치료에 대한 증례 보고)

  • Objectives : The aim of the present study was to report the clinical progress of a patient who underwent surgical treatment of necrotizing fasciitis. Methods : During the admission period, The patient was treated with acupuncture, cupping, Korean Medicine, Chuna therapy every day and observed by Numeric Rating Scale(NRS), Pain Disability Index(PDI), Beck Depression Inventory(BDI), and assessment about range of motion(ROM) and manual muscle test(MMT) for hip & knee joint. Results : After treated by using above treatment, NRS, PDI, BDI and ROM, MMT were improved. Conclusions : Korean Medical treatment are estimated to be good for patients after surgical treatment of necrotizing fasciitis. But more cases are required to prove the effectiveness of the treatment.

A Study on the Architectural Planning of the Rehabilitation Department in General Hospital (종합병원 재활의학부의 건축계획에 관한 연구)

  • As we make every effort to improve our health, the needs and interests of the rehabilitation medicine is increasing. And as society gets more advanced and as the quality of life become higher, rehabilitation department will be important for general hospital and we need the certain guidelines about it. In this study, selected 8 general hospitals in the metropolitan area are analyzed by the analysis of drawings and comparison. The important architectural planning of rehabilitation department are the connection with orthopedics and neurosurgery and the position for convenience approach. Rehabilitation department has 0.63~1.07% of general hospital area. And the rehabilitation department is divided into four branches - medical service space, office space, public space and support space. As the number of programs in the rehabilitation department is increasing, the planning will be considered for the flexible use. And this study proposed the programs, necessary spaces, space composition and basic planning of "rehabilitation department".

  • Rehabilitation medicine is an area which tries to help people who have physical inconvenience in everyday activities to live like healthy people as much as possible by training programs. As society gets more advanced and as the standard of living continuously rises, the need of the "Rehabilitation medicine" is also increasing. Also as we are entering into the aging society, especially geriatric rehabilitation medicine is getting more and more attention as a method to maintain the healthy lives of the elders. However, the standards related to medical care space for rehabilitation medicine is not thoroughly prepared. Although requirement for geriatric hospital is increasing, the standard of geriatric hospital is not following up to regulate them properly. Therefore, in this study, I will propose a 'suitable planning' needed in geriatric rehabilitation by analyzing the rehabilitation sections of 5 general hospitals and 1 geriatric hospital.

Clinical Value of Physical Examination and Electromyography in Acute and Chronic Lumbosacral Radiculopathy (급, 만성 요천추부 신경근병증 환자의 신체진찰과 근전도의 임상적 의미)

  • Objective: To investigate the diagnostic accuracy of two physical examinations (straight leg raise [SLR] and Bragard test) and electromyography (EMG) in patients with lumbosacral monoradiculopathy in acute and chronic state on confirmation of different diagnostic criteria (MRI vs MRI and diagnostic selective nerve root block [DSNRB]). Method: We identified 297 participants retrospectively from the departmental database. MRI evidence of L5 or S1 nerve root compression and a positive result in diagnostic SNRB served as reference standards. They were divided into two groups by the symptom duration: lasting more than 12 weeks in the chronic group and less than 12 weeks in the acute group. The diagnostic value of clinical tests and EMG were compared. Results: The clinical tests (SLR and Bragard test) done in acute stage on detection by MRI and DSNRB had the highest sensitivity (68%) compared to the chronic stage (63%), but sensitivity was low (57%) on confirmation of MRI alone. However, there was no significant difference on sensitivity and specificity of EMG regardless of reference standards and symptom duration. Electromyography was a significant predictor of neuropathic abnormalities on both acute (OR, 6.3; 95% CI, 2.4 to 16.7; p<0.01) and chronic (OR, 6.8; 95% CI, 2.9 to 16.3; p<0.01). Conclusion: In general, individual physical tests are easy to do and a combination of those tests could be a sensitive indicator of L5 or S1 radiculopathy. Furthermore, the use of provocation tests could provide useful information, especially in proceeding therapeutic selective nerve root block.

Optimal Combination of Acupoints Based on Network Analysis for Chemotherapy-Induced Peripheral Neuropathy (네트워크 분석에 기반한 항암화학요법으로 유발된 말초신경병증의 최적 경혈 조합)

  • Objectives This study aimed to identify optimal combinations of acupoints used to treat chemotherapy-induced peripheral neuropathy (CIPN). Methods We searched four international databases (MEDLINE, EMBASE, the Allied and Complementary Medicine Databases [AMED], and China National Knowledge Infrastructure [CNKI]) and five Korean databases (DBpia, Research Information Sharing Service [RISS], Korean Studies Information Service System [KISS], Oriental Medicine Advanced Searching Integrated System [OASIS], and KoreaMed) to identify randomized controlled trials (RCTs) that used acupuncture to treat CIPN. Network analysis was performed on the acupoints used in more than three included articles. We constructed a network by calculating the Jaccard similarity coefficient between acupoints and applied minimum spanning tree. Then, modularity analysis, degree centrality (Cd), and betweenness centrality (Cb) were used to analyze properties of the acupoints. Results A total of 25 articles were included. 24 acupoints were extracted from 25 articles. The combinations of acupoints having the highest Jaccard similarity coefficient were {EX-UE9, EX-LE10} and {ST36, SP6}. In the modularity analysis, acupoints were classified to six modules. ST40, EX-UE11, and KI6 had the highest Cd value while ST40, GB34 had the highest Cb value. Conclusions This study found the systematic framework of acupoint combinations used in CIPN studies. This study is expected to provide new perspectives of CIPN treatment to therapists. A RCT is in progress of using the network of this study as a guideline. If significant results are derived from the RCT, it will be possible to lay the groundwork to consider acupuncture for CIPN treatment.

Effect of fludrocortisone in cervical spinal cord injury patients with orthostatic hypotension - randomized controlled trial (경수부 척수손상 환자의 기립성 저혈압에서 플루드로코티손의 효과 - 무작위 대조군 연구)

  • The purpose of this study was to investigate the therapeutic effect of fludrocortisone in patients suffering from cervical spinal cord injury (SCI) with orthostatic hypotension (OH). Twenty-six patients with cervical SCI diagnosed with OH through a head-up tilt test were randomly assigned, and they were given either conservative treatment or additional fludrocortisone treatment. Fludrocortisone was administered for 2 weeks, increasing from 0.1 mg to 0.2 mg week . Blood pressure (BP), heart rate (HR), and blood parameters were measured at the beginning and after 2 weeks. After 2 weeks of treatment, there was a significant increase in the baseline BP of the treatment groups (p<.05). When analyzing the drop ratio, there was a tendency for a lower orthostatic BP drop in the treatment groups. Mild adverse events were reported in 7.69% of the treatment groups. Fludrocortisone exhibited therapeutic effects such as preventing cardiovascular complications and continuing rehabilitation through increased baseline BP and reduced OH, and can therefore be considered as a treatment option for OH in patients with SCI.

Chuna Manual Therapy for Carpal Tunnel Syndrome: A Systematic Review (손목 터널 증후군에 대한 추나요법의 효과: 체계적 문헌 고찰)

  • Objectives This systematic review aimed to provide evidence for the effectiveness and safety of Chuna manual therapy (CMT) for carpal tunnel syndrome (CTS). Methods We searched 12 electronic databases (PubMed, Ovid-MEDLINE, EMBASE, The Cochrane Library, CNKI, CiNii, ScienceON, KMbase, KISS, OASIS, KMTANK, and RISS) up to the end of October 2021. We only included randomized controlled trials (RCTs) investigating CMT for the treatment of CTS. The methodological quality of the included RCTs was evaluated using the Cochrane Risk of Bias Tool 1.0. Results Six RCTs were eligible for inclusion in our study. A meta-analysis of four studies demonstrated positive results for the use of CMT when used in conjunction with Korean medicine treatment compared to those for Western conservative treatment for CTS. Conjunction treatment significantly improved the total efficacy rate compared to conservative treatment alone (P=0.0007, n=4). Conclusions There is reliable evidence for the use of CMT in treating CTS based on a published meta-analysis. To measure only the effect of Chuna, an experimental group should be designed with Chuna alone to evaluate its effectiveness. However, it should be noted that the studies included in this systematic review were heterogeneous and of low quality, thus warranting further investigation using well-designed RCTs.

A Case Report of Upper Extremities Rehablitation of 2 Cervical Spinal Cord Injured Patients by Functional Electrical Stimulation with Korean Medical Interventions (경추부 척수손상 사지마비 환자의 한방치료와 기능적 전기 자극요법을 통한 상지 재활 치험 2례)

  • Two patients, diagnosed as cervical spinal cord injury, were hospitalized and treated by Korean medical interventions with Functional Electrical Stimulation (FES). The range of motion of the shoulder joint, manual muscle test were adopted to measure the resulting motor recovery after treatment. And the Korean version of Barthel index (K-MBI) was adopted as a method of measuring the improvement of quality of life after treatment. The range of motions and muscle strengths of the shoulder joints were improved. The activities of daily living were slightly improved in case 2, but showed no improvement in case 1. FES was effective for upper extremities rehabilitation of spinal cord injured patients. Further studies are needed to set up an Korean medical protocol for spinal cord injury rehabilitation.

Study on the effect of Post Open laser Lumbar Micro-discectomy on the Cross Section Area of Deep Muscles in Patients (요추부 미세 현미경 레이져 디스크 수술(OLM)이 환자의 심부근육 단면적 크기에 미치는 영향)

  • Purpose : The purpose of this study is to figure out the effects of Open Laser Microdiscectomy(OLM) on deep muscles by comparing multifidus and longissimus muscle size (cross section area; CSA) of pre and post operation. Methods : The subjects consisted of forty patients who had OLM. The data were analyzed with paired t-test comparing left and right deep muscle CSA of pre and post-operation, and both the deep muscle CSA of pre and post-operation, using SPSS ver. 15.0 program. Results : The results of this study showed a significant difference in deep muscle size (CSA) between pre and post operation (p<.05). Although there was not a meaningful difference between right and left deep muscle size (CSA) in pre operation (p>.05), there was a significant difference between both of them in post operation (p<.05). Conclusion : Therefore we made the conclusion that the operation causes decrease of muscle tone in deep muscles and muscle imbalance by causing muscle atrophy in the lumbar deep muscle after the operation.
