A Study on Effects of the Fall Prevention in the Rehabilitation Pilates Exercise (노인의 재활 필라테스 운동이 낙상 예방 효과에 대한 고찰)

  • This study was understood the risk mechanism of falls in the elderly and reviewed previous research data to see if the principle of Pilates rehabilitation exercise could have a positive effect on balance ability and postural stabilization in the elderly, and the purpose of this study is to present literature-based data on the fall prevention effect of rehabilitation Pilates exercise. First, the rehabilitation Pilates exercise makes it available for strengthening the center of the body to stabilize the spine and pelvis and stimulates the nerve root, thereby having an effect on the balance and the joint stabilization. Second, a proprioceptive sensory impairment and a musculoskeletal degenerative disease due to aging increase the loss of balance ability and the instability in posture maintenance, thereby coming to raise the risk of a fall damage caused by the difficulty in performing motor functions and by the gait disturbance. Third, the rehabilitation Pilates exercise leads to improving the core muscle strength in older adults, resulting in being capable of expecting the betterment in the balance and the reaction time motor function. And there is a positive impact on the improvement in body imbalance and on the stability in movement involved in the fall prevention, resulting in being able to suggest the possibility of likely contributing to a reduction in a fall risk rate. In conclusion, the rehabilitation Pilates as the elderly exercise program showed effects on the improvement in the body's muscular strength, on the upgrade in a sense of balance, and on the stabilization of core. Thus, it was considered to diminish the risk factors for a fall damage caused by musculoskeletal degeneration and to be capable of preventing a serious disorder of activity due to a chronic senile disease.

A Qualitative Study about Volunteer Work Experiences of Senior Citizens: On the Subject of Senior Citizen Volunteers in the Welfare Center of K City (노인의 자원봉사 경험에 대한 질적 연구: K시 노인종합사회복지관 노인자원봉사자를 대상으로)

  • The purpose of this study is to understand volunteer work experiences of senior citizen volunteers serving in the welfare center in K district in the aspect of an insider and to apprehend the meaning. The objects of this study are fifteen senior citizen volunteers and two social workers who do volunteer works in the welfare center, and this study utilized the qualitative study method through observations and in-depth interviews from 2009.09.02 to 2009.10.21. The data analysis is processed by the qualitative analysis program, Nvivo 2.0, and the processed data are analyzed in accordance with the grounded theories of Strauss and Corbin. As a result of reviewing data, the meanings about volunteer work experiences of senior citizens are induced as eight factors; senior citizens' identity in an old aging society, desire fulfillment through volunteer works by senior citizens, various leisure activities to help their health in mind and body, acceptance attitude about aging, mental supports from their families, finding activities suitable for each person's personality and capability, changes of perception about the handicapped, and endeavors to spend meaningful old age. Through results and discussion obtained by this study, the conclusion is as below: First, participation induction factors are identities of elderly people in aging society, acceptance attitudes toward aging and emotional support from families in the matter of senior citizens' volunteer work. Second, programs suitable for individual aptitude and capability should be implemented for activating senior citizens' volunteer work. Third, participation in senior citizens' volunteer work indicates desire satisfaction through senior citizens' volunteer work, leisure activities helping health for body and mind, and life changes such as changes of perception about handicapped people etc.

Genetic Counseling in Korean Health Care System (한국 의료제도와 유전상담 서비스의 구축)

  • Over the years Korean health care system has improved in delivery of quality care to the general population for many areas of the health problems. The system is now being recognized in the world as the most cost effective one. It is covered by the uniform national health insurance policy for which most people in Korea are mandatory policy holders. Genetic counseling service, however, which is well recognized as an integral part of clinical genetics service deals with diagnosis and management of genetic condition as well as genetic information presentation and family support, is yet to be delivered in comprehensive way for the patients and families in need. Two major obstacles in providing genetic counseling service in korean health care system are identified; One is the lack of recognition for the need for genetic counseling service as necessary service by the national health insurance. Genetic counseling consumes a significant time in delivery and the current very low-fee schedule for physician service makes it very difficult to provide meaningful service. Second is the critical shortage of qualified professionals in the field of medical genetics and genetic counseling who can provide the service of genetic counseling in clinical setting. However, recognition and understanding of the fact that the scope and role of genetic counseling is expanding in post genomic era of personalized medicine for delivery of quality health care, will lead to the efforts to overcome obstacles in providing genetic counseling service in korean health care system. Only concerted efforts from health care policy makers of government on clinical genetics service and genetic counseling for establishing adequate reimbursement coverage and professional communities for developing educational program and certification process for professional genetic counselors, are necessary for the delivery of much needed clinical genetic counseling service in Korea.

An Exploratory Study on the Legislation of the Right for Cultural Welfare (문화복지의 법적 권리화에 대한 탐색적 연구)

  • Cultural welfare is the cultural activity provided and supported by state, a local autonomous entity, and private organizations to improve the quality of people's life. And as a basic concept as well as a premise of discussion on cultural welfare, cultural right is a social right, a right which can make cultural express, gain access to cultural activities. A statue concerning cultural activities contributed to the promotion of our people's cultural welfare interwoven with cultural right. But laws were made for the purpose of enforcing national policy and supporting regime rather than promoting fundamental human rights, and they became effective and were understood as a mere part of national government, not as perspective of their execution, security of right or realization of them. On the other hand, based on laws concerning social welfare, cultural life means human life consisting of minimum welfare system, and minimum level of life which guarantees least cultural life is becoming an objective standard. This means the standard level of cultural right, and for the handicapped cultural rights are concretely guaranteed as a right in that they will not be discriminated for their approaching to cultural activities. However, laws concerning cultural activities have limitation in that there are ambiguous concept and limitation of cultural life, and there are no concrete and active laws about cultural rights to guarantee cultural life. In a constitution and laws concerning social welfare, cultural welfare must have concrete contents about the range and the level of cultural rights. In order cultural rights to be turned into complete ones which can assert its rights, as seen in the Supreme Court, a court has to try to understand the violation of cultural rights as the violation of legal rights.

  • Objective : This study aimed to understand the experience of taking a bus, investigate successful strategies to overcome the psychological difficulties of people with disabilities. Method : In this study, a phenomenological and hermeneutic approach was used to gain a deeper understanding of the behavior, the language, the process. For participants through interviews and participant observation data were collected. Phenomenological interpretation of the way through technology and to evaluate the results. Result : Ambulatory persons with disabilities in the community have confidence in the bus-independent movement in the community possible. And get on and off the bus at the moment of urgent strategies to use in the bus seats were taken. To cope with unexpected situations like that do not put stress was confirmed. In addition, by bus and go sit on the seat to look at the patient's burden was to. Conclusion : Ambulant disabled people's confidence in the bus had a significant impact on the independent movement. Always on the lookout for falling, and many psychological difficulties and to overcome unforeseen circumstances to predict and know that public transportation could be.

A Systematic Review of Sensory Integration Intervention for Children in Korea (아동을 대상으로 한 감각통합치료의 중재효과에 대한 체계적 고찰: 국내 연구를 중심으로)

  • Objective : The purpose of this study is to summarize the best-available intervention evidence for children's sensory integration therapy, drawn from studies published domestically in Korea over the last 10 years. Methods : The articles evaluated in this study were collected from the RISS and DBpia databases using the search terms "sensory integration," "sensory processing," and "Ayres Sensory Integration (ASI)". A total of 19 papers were analyzed. The selected studies were then assessed using the Population, Intervention, Outcomes, and Comparison method, the International Classification of Functioning, Disability and Health (ICF) method, and the modified Evidence Alert Traffic Light Grading System. Results : Development delay was the most commonly applied diagnosis for children's sensory integration therapy and individual sensory integration therapy was the most frequently used intervention method. The intervention effect was 91 percent in the body structure and function of ICF. The areas concentrated on were sensory modulation, sensory processing, fine and gross motor, body scheme, body-self concept, balance, basic movement, postural control and hand function, attention, and self-esteem. Conclusion : This simple overview of the efficacy of children's sensory integration therapy provides a basis for easy understanding and use by therapists, researchers and families with children.

Hemiplegic Experience after Stroke among Middle Aged Man (중년남성의 뇌졸중 후 편마비 경험)

  • This study is a qualitative one that used a phenomenological study method to provide basic data for nursing practice by understanding the essence and meaning of middle aged men's hemiplegic experience after stroke. At D hospital which is located in S city, 8 hemiplegic participants who were hospitalized between 24th November, 2014, and 6th December, 2014, participated in in-depth interviews. The experiences of the participants were categorized into 6 criteria, viz. 'Thorny path on a long journey', 'Broken brakes', 'Crossroads of the mind', 'Burden of head of household', 'Signpost that lights the darkness', and 'Route to a second life', and 16 theme clusters were included in these categories. As the sequelae and complications, such as physical pain, psychological conflict and role disorder, due to stoke in middle aged men, seriously affect their quality of life, this study attempted to understand the effect of these problems and provide new insight into nursing practice. Also, it is important to prevent the recurrence of stroke in the patients, facilitate their recovery, and help them to maintain healthy lives through the reestablishment of their role in the home and society, including through the resolution of their physical and psychological problems.

Understanding Policies related to Home Modification for the Elderly: An Occupational Therapist's Perspective (고령자 가정환경수정 관련 정책에 대한 이해: 작업치료사의 관점에서)

  • The purpose of this study was to explore the domestic and overseas home modification policies, and review the direction of domestic home modification policies to enable 'Aging In Place; AIP' and the competencies required of occupational therapist as and expert in home modification. In order to expand occupational therapy for domestic home modification and strengthen occupational therapist competency, it is required to establish a clear and practical home modification process that reflects the design and intervention of occupational therapy. Appropriate selection and standardization of the evaluation tools that identifies the needs of the elderly and reflects the characteristics of domestic housing types are also required. In addition, it is necessary to understand policies and systems, and endeavour to establish a relevant knowledge base for communication and cooperation with experts in other fields. In terms of the policy, there is a need for the home modification support system that allows the elderly to adapt to their aging regardless of their income level, beyond the existing support system, which was implemented mainly for low-income people and disabled. In addition, as in overseas cases, the efforts need to be made to provide various and efficient measures, such as support for barrier-free housing, expansion of the scope of support through cooperation between the national government and local governments, low-interest loans and tax reductions, and establishment of governance with organizations and institutions related to home modification services.

Recent Studies on Anorexia and Tissue Wasting Induced by Cancer Cachexia (암 악액질의 주요 병인 기전 및 최근 연구 동향)

  • Cancer cachexia-anorexia is a multi-organ metabolic syndrome characterized by anorexia and weight loss. Generally, such symptoms are a serious problem in cancer patients, adversely affecting chemotherapy success and survival rate. Cachexia has been reported to accompany up to 80% of gastrointestinal cancers, such as pancreatic, lung, and colon cancer, though it is relatively rare in lymphoma or breast cancer patients. It is also known that cancer-induced anorexia occurs independently of chemotherapy, although decreased appetite due to chemotherapy is well reported. In terms of pathoflammatory cytokines that are excessively increased by tumor tissues. Since the mechanism of cancer cachexia is not yet fully understood, there are currently no therapeutic agents or diagnostic markers to treat it. A recently published study identified a substance secreted from cancer cells that induces cancer anorexia, and the molecular mechanism causing the eating disorder was discovered. An increase in the expression of this substance has been shown to be statistically correlated with the symptoms of cachexia in cancer patients, and it is therefore expected to be applicable in the diagnosis and development of therapeutic agents for cancer cachexia. This review article aims to provide an overview of the key molecular mechanisms of the anorexia and tissue wasting caused by cancer cachexia.

A Grounded Theory Approach to Person Centered Communication between People Living with Dementia and Their Caregivers (사람중심 치매커뮤니케이션에 대한 근거 이론적 연구)

  • Communication breakdown has been mentioned causing a heavy burden for dementia caregivers. This study aims to inspect and structure the process and results of communication between people with dementia and their caregivers. The impeding/facilitating elements of communication are also extracted. Interviews with 21 of dementia care experts about the direct and indirect experiences of communication with people with dementia were analyzed based on the grounded theory. Results show that combination of the cognitive and communication decline of the people with dementia, confusing environment and caregivers' inappropriate attitude and lack of communication skills leads to communication breakdown and relations severance. Minimal contacts and task-oriented conversation results in conflicts and people with dementia's increasing agitation, anxiety and violent behaviors while understanding of individuality and listening with heart lead to recovered lucidity in the state of serious dementia, recovered pleasure and voluntary participation in the daily activities for people with dementia. Core paradigm was defined as 'Person Centered Care through relation formation'. There are 4 types of communication with people with dementia : partnering, patronizing, conflicting, avoiding types. Researchers suggest that Person Centered based communication skills be educated and trained for dementia caregivers.
