Relationship between Compulsory Employment System for persons with Disabilities and Health Status (장애인 의무고용제도와 건강상태 간의 연관성)

  • Aim : The purpose of this study is to investigate the influence of the awareness level of the mandatory employment system for the disabled and the help of the employment increase on the health status of the disabled. Based on this, the purpose of this study is to raise awareness of the compulsory employment system for the disabled and to suggest a way to improve the health of the disabled. Methods : In this study, the first data of the second wave of the 2016-2018 Employment Panel Survey for the Disabled was used to analyze the participation of the disabled in economic activities. a total of 1,648 subjects were analyzed using the chi-square test and generalized estimating equation (GEE). Results : Compared to the group knowing about the compulsory employment system for the disabled, in the group who did not know about the compulsory employment system, the cognitive level was significantly higher for subjective health status (Odds Ratio [OR]: 1.573, 95% Confidence Interval [CI]: 1.252-1.977) and chronic diseases (OR: 1.407, 95% CI: 1.091-1.816), Compared to the group that the compulsory employment system for the disabled is helpful in increasing employment, the group that said it was not helpful at all had depression feel (OR: 2.330, 95% CI: 1.219-4.452) and subjective health status (OR: 2.052, 95% CI: 1.232. -3.416) significantly affected. Conclusion : It was found that the lower the level of awareness and help of the compulsory employment system for the disabled, the negative impact on their health status. Therefore, it is necessary to promote employment growth by raising awareness of the employment system for the disabled. Furthermore, in order to improve the health of the disabled, public relations plans to increase the understanding of the disabled on the system and policies to protect the disabled people who lose their will to work due to difficulties in accessing employment were enacted, and Changes to a disability-friendly working environment should be activated so that participants can participate.

COGNITIVE CHARACTERISTICS OF ADHD CHILDREN ASSESSED BY KEDI-WISC (주의력결핍과잉활동장애 아동의 인지적특성)

  • The purpose of the present study is to investigate cognitive characteristics of ADHD children by comparing their performances on KEDI-WISC according to age and EEG variables. Subjects were 56 ADHD children who visited Seoul National University Children's Hospital during the period from January, 1988 to March, 1989. Group differences on age and EEG variables were tested by ANOVA, and Hierarchical Cluster Analysis was performed to investigate how ADHD children were classified based on their performances on KEDI-WISC. The results Indicated that ADHD children showed low scores on Coding, Digit Span, and Comprehension subtests, suggesting their attention deficits and impulsivity. ADHD children were clustered Into three groups based on only FSIQ. In post-hoc tests three groups showed different cognitive strengths and weaknesses on KEDI-WISC. Group differences on age were not significant, and abnormal EEG group showed lower PIQ than normal EEG group, suggesting the possibility that their attention deficits were related to neurological factors.

A Study on Validity of the Korean Version of the Assessment of Life Habits (LIFE-H 1.0) in Children (아동용 참여평가도구 한글판 Life Habits Assessment(LIFE-H 1.0)의 타당도 연구)

  • Objective: It is important to assess the participation and understand the characteristics of participation for setting goal of the child's treatment and strengthening participation. In this study, we verified the validity of the LIFE-H 1.0, so that domestic occupational therapists can systematically evaluate children's participation. Methods: This study was conducted for about 2 months from February to April 2016 in 47 children with disabilities and non-disabled children. We divided the children into two groups aged 0-4 years and 5-13 years old and conducted a self-report questionnaire to parents. The collected data were verified by SPSS(Statistical Package for Social Science) statistical analysis program. Results: There was no difference between the two groups in general characteristics. In comparison of two groups, a significant difference(p<.05) was found in the LIFE-H 1.0 total performance and satisfaction score and the high discriminant validity was verified. Conclusion: The ultimate goal of occupational therapy is to increase participation. Therefore, the occupational therapist must be able to assess the level of participation and than set the goal of intervention. This study confirms the high level of validity and expects that Korean version of LIFE-H 1.0 will be widely used in clinical field and research.

  • Ehlers-Danlos syndrome (EDS), an inherited connective tissue disorder, is caused by mutations in genes encoding different types of collagen or collagen-processing enzymes. EDS most typically affects the joints, ligaments, skin, and blood vessels. Oral health may be severely compromised in EDS as a result of specific alterations of collagen in orofacial structures. Dental hard tissue defects, root dilaceration, pulp stones, ectopic or delayed eruption, impaction, and periodontal disease could be observed. Therefore, a number of tissue responses related to collagen and precautions should be anticipated when considering dental treatment in EDS. Long-term and comprehensive dental management is required. The purpose of this report is to describe a clinical case of eruption disorders in a patient with EDS.

  • This study has some issues about the elderly, the disabled, and the vulnerable groups whose economic ability is low even if the disaster or disaster situation is not a big problem for the people. we should emphasize the necessity of disaster preparedness action manual considering the characteristics of disaster vulnerable classes and taking serious risks to them even in the same situation. We also want to conduct research only for people with disabilities who are choosing the same definition of the world among the vulnerable groups. I would like to provide a basis for developing a site action manual for people with disabilities by comparing the cases of Korea and Japan. Therefore, through the cases of Japan and Korea, it is possible to maintain the independence of everyday life during disasters, the communication function in disasters, the method of moving information and mobile information, the function of recognizing emergency response, Action Emphasize the urgency of developing action manuals. From this domestic situation, we will conclude the discussion on the disaster safety and action manual for the disabled.

A Development of Telephone for the Hearing Impaired to Improve Listening Ability of Telephone Speech (난청인의 통화 청취도 향상을 위한 전화기 개발)

  • We developed a new hearing aid telephone which helps the hearing impaired person to improve the listening ability of telephone speech. Recently, the hearing impaired person and the elderly who has hearing loss have been continuously increased and their desire for participating society as a producer has been increased also. So they strong1y want the hearing aid devices which make compensation fortheir handicap. The hearing aid telephone is one of the basic aid devices that helps the hearing impaired to communicate well with other poeple and to acquire easily useful information through the phone. We analyze the hearing ability of the hearing impaired, design the new model of the hearing aid telephone and test the telephone in three fields-electrical, word perception, user test. Our new tolephone has lour band pass filter channels and the center frequencies of these filters are 500, 1000, 2000, 3000Hz which are considered psychoacoustic factors and telephone line characteristics. The hearing impaired can adjust the total gain characteristics of receiving sound to his hearing ability by setting four volumes in the telelphone. This procedure is called fitting which is a very important factor for the hearing impaired to take meaning of speech. The total gain of this telephone is over 20dB from 250Hz to 3200Hz range. From the results of the tests we certify that our new model is better for the hearing impaired to understand the meaning or telephone speech than the old general models. The next step of developing the hearing aid telephone is to study about compressing sidetone and noise, dividing frequency bands, selecting hearing aid pattern and compensating psychoacoustic loudness. we expect that the advanced hearing aid telephone can be developed by the research about speech perception characteristics of the hearing impaired in engineering and clinical side.

The Analysis of K-WISC-IV Profiles in Children with High-Functioning Autism Spectrum Disorder (고기능 자폐 스펙트럼 장애 아동의 K-WISC-IV 프로파일 분석 및 융합적 적용)

  • The aim of this study is to distinguish children with high-functioning autism spectrum disorder (ASD) from the norm group by identifying their Intelligence with Korean Wechsler Intelligence Scale for Children-Fourth Edition (K-WISC-IV) profile analysis. The article were administered to 90 children with high-functioning ASD (6-16) years and has surveyed the average of the Full scale IQ, index scores, and subtest scores of K-WISC-IV. Also, this study has conducted a single-subject T-test in order to verify whether Full scale IQ, index scores, subtest scores are different from those of the norm group. The results show that children with high-functioning ASD achieved significantly lower scores on Processing Speed Index, compared to the norm group. Furthermore, their scores in Comprehension, Picture Concept, Picture completion, Coding, and Symbol Search were significantly lower than those of the norm group. It is likely that what have turned out to be the cognitive weaknesses of high-functioning ASD children by K-WISC-IV analysis, including slow process speed, low social judgement, and difficulty in visual stimuli in everyday life are interrelated to their unique characters.

DENTAL TREATMENT IN A PATIENT WITH METACHROMATIC LEUKODYSTROPHY UNDER GENERAL ANESTHESIA : A CASE REPORT (이염성 백질 이영양증 환아의 치아우식 치료 증례 보고)

  • Metachromatic leukodystrophy (MLD) is a progressive and degenerative neurological disease caused by a deficiency of the catabolic enzyme arylsulfatase A. Deficiency of arylsulfatase A results in accumulation of sulfatide in the white matter of the peripheral and central nervous system and it occurs demyelination as a result. The patient gradually goes through mental and motor failure. General symptoms of MLD include gait disturbance, mental deterioration, muscle rigidity and impaired swallowing. Inheritance of the disease is autosomal recessive. We report a dental caries treatment of a 3-year old boy with MLD. The patient underwent hematopoietic stem cell transplantation (HSCT) to slow the progression of the disease. He was suffered from difficulties of mastication and swallowing from the degenerative neurological symptom. He was ingesting food by both oral feeding and tubal feeding after he took percutaneous endoscopic gastrostomy (PEG). The cause of multiple caries was mainly presumed as patient's prolonged time of meal. The treatment was performed under general anesthesia considering patient's incompliance. Severely affected lower primary molars were treated with pulp treatment and restored with stainless steel crown. Others were restored with composite resin. There were no postoperative complications. MLD is life threatening progressive disease and also has an impact on unfavorable condition for oral health. Routine home oral care and periodic professional dental care should be emphasized to the caregiver of patient considering the susceptibility of dental caries. Not only the medical care, but periodic dental office visit would benefit the quality of life of the patient.

Hemiplegic Migraine Presenting with Unilateral Facial Palsy: A case report (편측안면마비로 발현한 편마비편두통 1예)

  • Hemiplegic migraine (HM) is a rare subtype of migraine with aura and is accompanied by a fully reversible motor aura. HM can occur in two forms: familial or sporadic. Currently, three genes are related to familial HM. Typically, HM occurs in the first or second decade of life and involves gradually progressing aura symptoms in succession, accompanied by headaches. The aura includes visual, sensory, motor, aphasic and often basilar-type symptoms. Motor aura (weakness) is related to the regions where the sensory aura is involved, and it usually starts at the hand before spreading to the arm and face. Aphasia is a common form of speech aura, but does not typically present as a difficulty in understanding. In this case report, the sensory-motor aura started at the right face and then gradually progressed to the right leg without any symptoms in the ipsilateral upper extremity. To the best of my knowledge, there has been no previous case report for the presentation of a hemiplegic migraine, as in this case report. As there is a possibility of misdiagnosis of Bell's palsy at the early stage of this case, this case report suggests that a physician should consider the rare possibility of stroke or HM when a patient presents with unilateral facial palsy.

Translation and Adaptation of the Children's Home Inventory for Listening Difficulties (CHILD) into Korean (가정환경 아동듣기평가(CHILD) 부모용 설문지의 한국어 번역 및 적용 연구)

  • The Children's Home Inventory for Listening Difficulties (CHILD) questionnaire has been applied for assessing listening and communication difficulties in various home situations for children with hearing loss. The purpose of the study was to translate the CHILD questionnaire for parents into Korean and verify reliability and validity of Korean version of CHILD (CHILD-K). CHILD-K was completed by 55 parents of children (from ages 3~12 years) using cochlear implants (CI). Among the 55 children, 27 were in preschool and 28 in elementary. Internal consistency reliability of CHILD-K was verified by Chronbach's alpha. The mixed factorial ANOVA was conducted to compare the effects of the age group and situation factors (Quiet, Noise, Distance, Social, and Media factors) on the score of CHILD. The results indicated that CHILD-K showed excellent internal consistency reliability (${\alpha}=.96$). The CHILD scores among age groups were significantly different as the older age group resulted in higher scores in all situations except Distance. For both groups the mean scores for the Quiet situation were significantly higher than other situations, and the mean scores for the Social situation were significantly lower than other situations. Moreover, analysis showed that children with CI had difficulties in the Social situation combined with other situation factors. The results indicate that the Korean version of CHILD questionnaire is a reliable tool for the assessment of communication abilities in home situation in Korean-speaking children using CI.
