Macroscopic Serosal Invasion in Advanced Gastric Cancer (진행성 위암에서 육안적 장막침윤의 의의)

  • Purpose: The macroscopic findings of tumors are not always identical with the microscopic findings. This study investigated the oncologic implications of macroscopic serosal invasion in advanced gastric cancer to find out how to improve the accuracy for the depth of invasion assessed by the surgeon during an operation. Materials and Methods: The medical records of 789 patients with advanced gastric cancer who underwent a gastrectomy at Kyungpook National University Hospital between 1995 and 1999 were reviewed. The prognoses and the recurrence patterns were analyzed according to macroscopic serosal invasion and microscopic serosal invasion, and the clinico-pathological factors of cT3/ss cancers were compared with those of cT3/se cancers. Results: Difference of survival rates according to macroscopic serosal invasion and microscopic serosal invasion revealed statistically significant. Recurrence rates were similar in patients with macroscopic and microscopic serosal invasion (42.2% and 41.4%, respectively). Peritoneal recurrence rates were also similar (19.8% and 21.9%, respectively). The sensitivity and the specificity of macroscopic assessment of serosal invasion were 70.3% and 77.8%, respectively, On univariate and multivariate analyses, Borrmann type I/II cancers and the absence of distant metastases revealed the risk factors for overestimating of serosal invasion. Conclusion: Macroscopic serosal invasion assessed by a surgeon intraoperatively can be used to give a prognosis and to predict the recurrence pattern precisely, although there is a risk for overestimation when the tumor is a Borrmann type I/II cancer or the tumor has no distant metastases. Effect of Omentum, Pleura, Diaphragm on Tracheal Autograft Survival (자가이식기관의 생존에 필요한 장막, 흉막, 횡격막의 역할 분석)

  • Pleura, diaphragm, pericardial fat pad, intercostal muscles and omentum can be used to protect and revascularize the bronchial suture line of tracheal transplantation, lung transplantation and pulmonary resection. The purpose of the present study is to compare the influence of the pleura, diaphragm and omentum in survival of isolated tracheal segments in the experimental animals. Material and Method: Sprague-Dawley rats weighing 250- 350g were used. The animals were divided in three groups; the pleura, omentum and diaphragm. Following intraperitoneal anesthesia, endotracheal intubation was performed. Then the trachea was exposed. A three-ring sec- tion of cervical trachea was excised. The resected trachea was implanted at each sites. After 2 weeks, rats were sacrificed. Histopathological examination of the tracheal segments was performed. For comparison of each groups, histopathological viability of resected tracheal segment was scored by three tissue layers; epithelium, submucosa, and cartilage. The results were presented as average score. Result: In histopathological examination, submucosa and cartilage using tracheal segment necrosis scoring system. The pleural group showed well preserved tissue. There was minimal necrosis and inflammation compared with other groups. In the pleural group, tracheal necrosis scores were $2.17\pm0.983$at epithelium, $1.67\pm0.516$ at submucosa and $2.17\pm0.753$ at cartilage. At the omental group, scores were $1.00\pm0.00,\;1.60\pm0.548\;and\;1.80\m0.447$. In the diaphragmatic group, scores were $1.40:\pm0.894,\;2.40\pm0.547\;and\;2.20\pm0.447$. Total necrosis score were $6.00\pm1.789$ in the pleural group, $4.40\pm0.894$ in the omental group and $6.00\pm1.414$ in the diaphragmatic group. Conclusion: There were no significant viability differences in terms of total necrosis score for the viability of resected tracheal segment. But the best result was achieved in the omental group. Therefore, omental wrapping on tracheal graft site will be beneficial for the prevention of graft necrosis.

Gartner´s Duct Cyst in a Maltese Bitch (말티즈 암캐에 있어 Gartner´s Duct 낭종례)

  • 2001년 4월 19일에 11살 된 말티즈 암캐의 비정상적 자궁에 대한 검사의뢰가 들어왔다. 육안적 검사에서 얇은 막으로 된 다양한 자궁 낭종이 관찰되었다. 그 수는 5개이며 그중 2개는 자궁체의 장막면에 위치하고 있었으며 7∼8 mm 정도의 직경을 가지고 있었다. 나머지 3개는 직경 1∼2 mm 정도의 크기이며 자궁각의 장막면에 위치하고 있었고, 절개시 모든 낭종들은 장액성 액체들로 가득 차 있었다. 병리조직학적 검사에서는 모든 낭종들이 자궁의 장막하에 존재하고 있었다. 낭종의 상피세포들과 이들의 위치는 모두 Gartner's duct와 동일한 것들이었고, 따라서 Gartner's duct cyst로 진단하게 되었다.

Efficacies of Sodium Carboxymethylcellulose, Oxidized Regenerated Cellulose and Vitamin E in Prevention Against Postoperative Adhesion Formation in Dogs (개에서 복강수술 후 유착방지에 대한 Sodium carboxymethylcellulose, Oxidized regenerated cellulose, Vitamin E의 효과)

  • This study was performed to compare the efficacy of the sodium carboxymethylcellulose (SCMC), oxidized regenerated cellulose (ORC), and their combination with vitamin E for the prevention of postoperative adhesions in the dog. Twenty five dogs were randomly divided into five groups ; non-treated group (Control Group), SCMC-treated group (SCMC Group), ORC-treated group (ORC Group), vitamin E and SCMC-treated group (SCMC + E Group), and vitamin E and ORC-treated group (ORC + E group). After laparotomy, 5 abrasions were made to induce intraperitoneal adhesions on the surface of the ileal serosa. The adhesions were occurred in serosa to mesentary (37.6%), serosa to serosa (24%), serosa to omentum (8.8%) and serosa to parietal peritoneum (3.2%). The incidences of adhesions were 92%, 84%, 64%, 56% and 68% in Control, SCMC, ORC, SCMC+E and ORC + E Group, respectively. The adhesion scores in SCMC+E Group were significantly lower than those in the other groups (p < 0.01). In conclusion, this study showed that oral supplements of vitamin E and intraperitoneal administration of 2% SCMC solution were effective on reducing intraperitoneal adhesions in the dog.
