Effect of Treadmill Gradient Training on Lower Limb Muscle Activity in Chronic Stroke Patient (만성 뇌졸중 환자의 하지 근육의 활성도에 트레드밀 경사도 훈련이 미치는 영향)

  • The purpose of this study was to identify the effect of treadmill gradient training on lower limb muscle activity in chronic stroke patients. The subject were 32 stroke patients.. Subjects were randomly divided into three group which were control group($0^{\circ}$treadmill training(n=10)) and experimental group($5^{\circ}$treadmill training (n=10) and $10^{\circ}$treadmill training(n=12)). Three groups received treadmill gradient training for 30 minutes while 3 times per week for 6 weeks in addition to conventional physical therapy. Muscle strength was measured by EMG on rectus femoris, biceps femoris, tibialis anterior and gastrocnenius for muscle activities. In comparison of activity of rectus femoris and gastrocnemius between pre and post value, the activity of rectus femoris was significant in the experimental and control group(p<.05) and the activity of biceps femoris was significant in the $5^{\circ}$treadmill gait training group and $10^{\circ}$treadmill gait training group(p<.05). The activity of tibialis anterior was significant in the $5^{\circ}$treadmill gait training group and $10^{\circ}$treadmill gait training group(p<.05). In comparison of the difference of activity of rectus femoris among 3 groups, there was a significant difference between the $5^{\circ}$treadmill gait training group and control group(p<.05). and difference of activity of tibialis anterior was significant difference between the $10^{\circ}$treadmill gait training group and control group(p<.05). These findings suggest that $5^{\circ}$treadmill gait training group and $10^{\circ}$treadmill gait training group can be used to improve lower limb muscle activity in chronic stroke patient. In conclusion, these treadmill gradient training helped improving function of gait ability in chronic stroke patient.

A Comparative Study on the Immediate Effect of Performing Gastrocnemius Stretching with and without Myofascial Release of the Sole on Ankle Dorsiflexion Angles and Gastrocnemius Muscle Tone in Subjects with Limited Ankle Dorsiflexion (발등 굽힘 제한이 있는 사람에게 장딴지근 신장운동과 발바닥 자가근막이완이 발등 굽힘 각도와 장딴지근 긴장도에 미치는 즉각적인 효과 비교)

  • PURPOSE: This study compared the effect of performing gastrocnemius stretching with and without the self-myofascial release of the sole on the active and passive ankle dorsiflexion angles and muscle tone of the gastrocnemius muscle in subjects with short gastrocnemius muscle. METHOD: A total of 23 subjects with short gastrocnemius muscles were included in this study. The study participants were divided into two experimental groups. Group A performed gastrocnemius muscle self-stretching exercises only, while group B performed self-myofascial release of the sole using a massage ball after the gastrocnemius muscle self-stretching exercises. For both groups, the active and passive ankle dorsiflexion angles were measured using a goniometer, and the tone of the gastrocnemius muscle was assessed using the MyotonPRO®. RESULTS: Within-group comparison showed that the participants in both groups A and B had significantly increased active and passive ankle dorsiflexion angles and decreased gastrocnemius muscle tone (p < .05) after performing their respective exercises. However, no significant differences in the said criteria were observed between groups A and B (p>.05). CONCLUSION: The results of this study showed that both methods were effective in increasing active and passive dorsiflexion angles and decreasing muscle tone. Thus, it is recommended to tailor gastrocnemius stretching exercises according to the patient's condition. If the patient does not experience discomfort in the plantar fasciae, it is recommended to perform the gastrocnemius stretching exercise only without myofascial release and use a massage ball afterward.

A convergence study of the effects of asymmetric standing posture on knee joint position and lower extremity muscle activity in subjects with hyper-extended knee (무릎 과다 폄을 가진 대상자에게 비대칭 선 자세가 무릎 관절 위치와 근활성도에 미치는 영향에 대한 융합적 연구)

  • The purpose of this study was to investigate the effect of hyper-extended knee and asymmetric standing posture on knee joint position and lower extremity muscle activity. Thirty-three voluntary participants participated and included sixty legs in the study. The sixty legs were divided into two groups: hyper-extended knee and normal group. The muscle activity and knee extension angle were measured. In the asymmetric standing posture, the knee joint extension angle and the muscle activity of the gastrocnemius were statistically significant between the normal group and the hyper-extended knee group. Based on the results, we confirmed that the asymmetric standing posture increases the hyper-extended knee. Therefore, it will contribute to the establishment of therapeutic guide for the subjects with hyper-extended knee to maintain the symmetrical standing posture, and future studies need to be conducted including the effects of the hip joint and ankle joints.

The Effects of Speed Variations in Treadmill Training on Thickness of Lower Extremity Muscles (속도가 다른 트레드밀 훈련이 다리 근두께에 미치는 영향)

  • The purpose of this study was to determine the effects of speed variations in treadmill training on the thickness of lower extremity muscles. A total of 36 university students were divided into three groups: MVTG (n=12), HVTG (n=12), and ATG (n=12). Subjects in MVTG underwent treadmill training with their own average speed; those in HVTG underwent treadmill training with 130% speed of their own average speed; and those in ATG underwent treadmill training with alteration of speed, between 100% and 130% of their own average speed. Treadmill training was performed for 60 minutes a day, three times per week, for a total of 6 weeks. Ultrasonography was used to compare the muscle thickness between rectus femoris, vastus lateralis, gastrocnemius, and tibialis anterior. The result was as follows: The rectus femoris, vastus lateralis, and gastrocnemius were significantly increased after the training period, and they also were statistically significant in interaction. Moreover, tibialis anterior was also significantly increased. Therefore, compared to the average-speed treadmill training, speed variations had a greater effect on thickening the lower extremity muscles. In the near future, we will conduct a study applying the findings from this study in a rehabilitation program for patients with gait disturbance due to nervous or musculoskeletal system disease.

Effect for Wellness of Blood Flow Restriction Aerobic Exercise Program - Focusing on Mscle Ativity and Mtor Nurons - (혈류제한 유산소운동 프로그램의 웰니스를 위한 효과검정 - 근활성도와 운동신경원을 중심으로 -)

  • This study quantitatively compares and analyzes lower extremity muscle activity and motor neurons by performing blood flow-restricting aerobic training in the lower extremities, which is closely related to aerobic capacity for health, in normal people, and provides basic data to suggest the effectiveness of an effective blood-restricting exercise program. would like to provide A group of 10 people who applied aerobic exercise on a treadmill by restricting blood flow to 140 mmHg of pressure was set as Experimental Group I. And 11 people who applied only aerobic exercise on a treadmill were randomly assigned as a control group. The intervention program was implemented on a treadmill for 4 weeks, 3 times a week, once a day, for 30 minutes once. In addition, muscle activity and motor neurons were measured and analyzed using surface electromyography before intervention. As a result of the study, the muscle activity of the rectus femoris, biceps femoris, tibialis anterior and gastrocnemius was significantly increased (p<.001) in the pre-and-poster comparison within the group of experimental group I (p<.001). In the pre-and-poster comparison of the control group, the muscle activity of the rectus femoris, biceps femoris, tibialis anterior and gastrocnemius was significantly increased (p<.001). In comparison of changes between groups, there was a significant difference in the activity of the rectus femoris muscle (p<.05). Combining aerobic exercise in parallel with lower extremity blood flow restriction can be developed into an injury prevention exercise program that can restore functional activity in rehabilitation training for elite athletes and elderly people with weak joints. In addition, based on these results in future research, it is considered that it is necessary to expand the scope of non-normal subjects and conduct various studies according to the pressure intensity.

Effects of Exercise Program by Type on Balance Ability and Muscle Activity In A Standing Posture (유형별 운동프로그램이 선 자세에서의 균형능력과 근활성도에 미치는 영향)

  • This study, in order to establish the effect of three exercise groups on the static and dynamic balancing abilities and the muscular activity, targeting adult males aged 20-29, assigned 15 men to each aquatic exercise group, trunk stabilization exercise group, and balance exercise group. The study was conducted from June, 2013 to August, 2013, and measured and compared the balancing ability and the muscle activity(tibialis anterior and gastrocnemius muscle) of the participants after performing intervention for 30 minutes a day, 3 days a week, for 6 weeks. As a result, on the comparison between before and after the intervention, there were significant differences in changes of the surface area and the whole path length in all the three groups(p<.05)(p<.01), and also on the dynamic balance, there was a significant difference in change of limited of stability(p<.05)(p<.01). On change of the muscle activity of tibialis anterior, both left and right sides showed statistically significant differences in all the three groups(p<.05)(p<.01), and gastrocnemius muscle showed a statistically significant difference in all the three groups except for the left side of the trunk stabilization exercise group(p<.05)(p<.01). It could be established that aquatic exercise is effective for improvement of the balancing ability and increase of the muscular activity, and we intend to propose specific aquatic exercise program development by conducting a study to determine the objective effect of aquatic exercise on the elderly or patients who have a poor balancing ability.

The Effect of the Insole Height on Lower Limb Joint Angle and Muscle Activity at Landing when the Maximal Ground Reaction Force of Male in Their 20s (착지 동작 시 깔창 높이가 20대 남성의 하지 관절 각도와 근활성도에 미치는 영향)

  • The purpose of this study is to analyze the effect of the height and insole height upon landing on the lower limb joint angle and muscle activity during maximum ground repulsion in young men. For a male in their twenties, a landing motion was performed with a force plate on a 40cm-high platform by wearing one of 0, 3, 5cm polyurethane insoles per week for a total of 3 weeks. During the landing motion, the joint angle of the lower extremities and the muscle activity of the rectus femoris, biceps femoris, anterior tibialis and calf muscles were measured during the maximum ground repulsion. In order to compare the changes in the joint angle and muscle activity of the lower limbs according to the height of the insole, a one-way ANOVA with repetitive factors was performed. As a result of the analysis of the lower limb joint angle, the higher the height of the insole affected the angle of the left ankle joint. As a result of the muscle activity analysis, the higher the height of the insole affected the right anterior tibialis muscle and biceps femoris. It is thought that it is possible to protect the body when landing through sufficient muscle strength increase of the lower limb muscles. As the angle of the affected muscle and lower limb joint may be different depending on the type of insole, it is considered necessary to study it.

Analysis of Muscular Activity of Tibialis Anterior Muscle and Gastrocnemius Muscle in Functional Reach Test of Elderly According to Different Surfaces (기능적 팔 뻗기 시 지면에 따른 앞정강근과 장딴지근의 근활성도 비교)

  • Purpose : This study aims to examine the changes of muscle activity of the tibialis anterior muscle and the gastrocnemius muscle on the ground and unstable ground for functional reach test. Methods : This study chose 24 elderly men and 24 elderly women who could walk independently, reach their arm over 25 cm, show shoulder joint $90^{\circ}$ flexion, had no operations on lower limb joints, and don't take medication affecting the ability to keep balance. Muscular activity of the tibialis anterior muscle and the gastrocnemius muscle was measured using functional reach test and electromyogram. Functional reach test was conducted at a total of four sections, 0cm, 15cm, 20cm, and 25cm. Results : It was known that the gastrocnemius muscle was used more than the tibialis anterior muscle to keep balance and the elderly with good balance ability showed no great change of muscular activity on both the stable and unstable ground. Conclusion : It was found that the subjects used their gastrocnemius muscle more and lower limbs of frequently used parts to keep their balance. As the elderly have good balance ability, they showed no great change of muscular activity on both the stable and unstable ground.

The Effect of Sports Elastic Compression Stockings on Lower Extremity Edema by High Intensity Exercise (스포츠용 탄력압박스타킹이 최대 운동 시 하지의 부종에 미치는 영향)

  • 본 연구는 스포츠용 탄력 압박 스타킹(Essential compression calf sleeve, SKINS, Australia)을 착용하고 최대 운동을 하였을 때, 하지 부종에 어떠한 효과가 있는지를 알아보고자 한다. 실험 대상자는 젊은 성인 남녀 30명으로(남자 15명, 여자15명) 근 골격계 및 심혈관계에 특이 병력이나 사고에 의한 외상 등이 없었으며, 규칙적으로 운동을 하는 자는 제외하였다. 대상자들의 신체적 특성을 파악하기 위해서 자동 신장계(BSM330, Biospace, Korea)를 사용하였다. 부종정도를 알기위해서 줄자로 운동 전후의 장딴지 근(종아리)의 둘레와 체성분 분석기(InBody 720, Biospace, Korea)를 사용하여 하지의 부종 수치를 측정하였다. 최대의 운동은 호흡가스 분석기와 연결된 트레이드밀(CPEX-1, IRS, Japan)을 사용하여 변형된 BRUCE Protocol TR-3에 따라 호흡 교환율(R) 값이 1.15이상 되거나 all-out 될 때까지 하였다. 실험 대상자들은 동일 집단으로 전이효과를 방지하기 위하여 일주일의 간격을 두고 스포츠 탄력 압박 스타킹 착용 유무에 따른 동일한 실험을 두 번 하였다. 측정은 최대 운동 전후의 장딴지 근(종아리)의 둘레와 부종 수치를 하였다. 스포츠용 탄력 압박 스타킹 착용 유무와 운동 전후를 비교하기 위하여 MANOVA(다변량 분산분석)를 사용하였고 유의수준은 .05로 설정하였다. 탄력 압박 스타킹을 착용한 상태에서 최대 운동 전후의 부종 정도를 측정한 결과 스타킹을 착용하지 않았을 때 운동 전후의 둘레가 유의하게 증가하였고(p<0.05), 스타킹을 착용 하였을 때 운동 전후의 둘레가 유의하게 증가하지 않았다(p>0.05). 부종 수치(세포외액/전체체액)는 탄력 압박 스타킹을 착용하였을 때 오른쪽 다리는 유의하지 않게 감소하였고(p>0.05), 왼쪽 다리는 유의하게 감소하였다(p>0.05). 탄력 압박 스타킹을 착용하고 운동할 경우 하지의 부종 경감에 어느 정도 효과가 있다고 사료된다.

Effects of Dynamic Tubing Gait Training on Postural Alignment, Gait, and Quality of Life in Chronic Patients with Parkinson's Disease : Case Study (동적탄력튜빙 보행훈련 프로그램이 만성 파킨슨병 환자의 자세정렬과 보행능력과 삶의 질에 미치는 영향 : 사례연구)

  • The present study investigated the effects of dynamic tubing gait training(I and II) on the postural alignment, gait, and quality of life in chronic patients with Parkinson's disease. This study is based on the case study that recruited a total of 3 patients with chronic Parkinson's disease (Hoehn and Yahr Stage of 1 to 3 each one person). Dynamic tubing gait training (I and II) applied to chronic patients with Parkinson's disease for 25 sessions, 30 minutes a day, 5 days a week, over 5 weeks period. To investigate the effects of this study, evaluating using the postural alignment test, muscle activity tests, gait analysis, and quality of life scale for patient with Parkinson's disease. After the intervention of Dynamic tubing gait training (I and II), Trunk flexion was decreased. Also, during walking from initial contact (IC) to mid stance (Mst), muscle activity of Quadriceps, Hamstring, and Tibialis Anterior (TA) was increased and muscle activity of Gastrocnemius was decreased. The muscle activation of Erector Spinae (ES T12, L3) was increased in the H&Y I and III stages and decreased in the H&Y II stage. Length of gait line, single support line, ant/post position and lateral symmetry of center of pressure (COP) parameters improved. The spatio-temporal gait parameters including of step length, stride length, and velocity was increased, and cadence decreased. Further the quality of life of patients with Parkinson's disease was improved. Based on these findings, Dynamic tubing gait training (I and II) could be applied as a new approach to improve posture, gait, quality of life in chronic patients with Parkinson's disease for more than 5 years, whose drug resistance is halved.
