MAXILLARY SINUS BONE GRAFT USING PARTICULATED RAMAL AUTOBONE AND BOVINE BONE (하악지 분쇄자가골과 이종골을 이용한 상악동 골이식술)

  • The maxillary sinus bone graft procedure is one of the predictable and successful treatments for the rehabilitation of atrophic and pneumatized edentulous posterior maxilla. Materials used for maxillary sinus floor augmentation include autogenous bone, allogenic bone, xenogenic bone and alloplastic materials. Among them, autogenous bone grafts still represents 'gold standard'for bone augmentation procedures. We selected the mandibular ramus area as a donor site for the autogenous bone graft because of low donor site morbidity. We performed maxillary sinus bone graft procedures with implant placement using particulated ramal autobone and bovine bone mixture, and got good results. This is a preliminary report of the maxillary sinus bone graft using particulated ramal autobone and bovine bone, requires more long-term follow up and further studies.

The Clinical and Histopathologic Findings of Lymphonodular Hyperplasia of the Colon in Infancy and Childhood (소아에서 대장 림프결절증식의 임상적 및 병리조직학적 소견)

  • Purpose: Lymphonodular hyperplasia of the colon (LNHC) is a rare finding in children and its significance as a pathologic finding is unclear. The aim of this study was to investigate the clinical significance of LNHC by analyzing clinical and histopathologic findings in children with LNHC. Methods: We analyzed data from 38 patients who were confirmed to have LNHC by colonoscopy. We checked age, birth history, past history, family history, and clinical symptoms. A hematologic exam, stool exam, and image studies were performed and biopsy specimens were examined by a pathologist. All patients were asked to have short- and long-term follow-up. Results: The mean age of the patients was 12.5${\pm}$14.4 months. All patients presented with complaints of bloody stool. They appeared healthy and the hematologic findings were within a normal range, with the exception of one case. There was no other identified source of bleeding. On histologic exam, 36 patients (94.7%) had lymphoid follicles and 34 patients (84.5%) fulfilled the criteria of allergic colitis. Regardless of diet modification and presence of residual symptom, there was no recurrence of bloody stool through long-term follow-up in all patients. Conclusion: LNHC is more common in infants who are affected by allergic colitis, but it can appear even after infancy. LNHC should be regarded as the etiology when there are any other causes of rectal bleeding, especially in healthy children. We suggest that LNHC has a benign course regardless of diet modification and it might not require excessive concerns.

Recurrent Hemoptysis after Bronchial Artery Embolization (기판지동맥색전술 후 객혈의 재발에 관한 연구)

  • Background : To observe the immediate and long-term results of bronchial artery embolization(BAE) for hemoptysis and the factors influencing the recurrences. Methods : This study involved 75 patients with massive, or moderate and recurrent hemoptysis, who underwent bronchial artery embolization(BAE) from 1994 to 1999. The underlying diseases included pulmonary tuberculosis in 35, bronchiectasis in 22, aspergilloma in 12, lung cancer in 3, and 3 with other diseases. Results : After BAE, bleeding was controlled immediately in 61 patients(82.7%). One patient died of another medical problem, 3 patients were referred to surgery and 5 patients could not be followed-up. In the remaining 66 patients who were followed for more than one-year after BAE, 37(56.1%) patients had another hemorrhage (26 hemoptysis, 11 minor hemosputa). Among the recurred 37 subjects, 19(51.4%) experienced hemorrhage within 1 month after BAE, 31(83.8%) within 1 year, and 36(94.1%) within 3 years. The underlying lung diseases, the amount of bleeding and the extent of the involved lungs were factors affecting the outcome, especially blood loss >500cc was an important factor affecting the recurrence rate. BAE for two cases with lung malignancy was ineffective. Long-term control of bleeding (3-year cumulative non-recurrence) was achieved in 30 subjects(45.5%). Conclusion : Bronchial artery embolization(BAE) is effective as an initial treatment for moderate to massive hemoptysis. Because most of the recurrences occurred within 3 years, it is important to follow-up such patients for at least 3 years after BAE and the most significant factor affecting the prognosis was amount of blood loss.

Tracing the Drift Ice Using the Particle Tracking Method in the Arctic Ocean (북극해에서 입자추적 방법을 이용한 유빙 추적 연구)

  • In this study, we analyzed distribution and movement trends using in-situ observations and particle tracking methods to understand the movement of the drift ice in the Arctic Ocean. The in-situ movement data of the drift ice in the Arctic Ocean used ITP (Ice-Tethered Profiler) provided by NOAA (National Oceanic and Atmospheric Administration) from 2009 to 2018, which was analyzed with the location and speed for each year. Particle tracking simulates the movement of the drift ice using daily current and wind data provided by HYCOM (Hybrid Coordinate Ocean Model) and ECMWF (European Centre for Medium-Range Weather Forecasts, 2009-2017). In order to simulate the movement of the drift ice throughout the Arctic Ocean, ITP data, a field observation data, were used as input to calculate the relationship between the current and wind and follow up the Lagrangian particle tracking. Particle tracking simulations were conducted with two experiments taking into account the effects of current and the combined effects of current and wind, most of which were reproduced in the same way as in-situ observations, given the effects of currents and winds. The movement of the drift ice in the Arctic Ocean was reproduced using a wind-imposed equation, which analyzed the movement of the drift ice in a particular year. In 2010, the Arctic Ocean Index (AOI) was a negative year, with particles clearly moving along the Beaufort Gyre, resulting in relatively large movements in Beaufort Sea. On the other hand, in 2017 AOI was a positive year, with most particles not affected by Gyre, resulting in relatively low speed and distance. Around the pole, the speed of the drift ice is lower in 2017 than 2010. From seasonal characteristics in 2010 and 2017, the movement of the drift ice increase in winter 2010 (0.22 m/s) and decrease to spring 2010 (0.16 m/s). In the case of 2017, the movement is increased in summer (0.22 m/s) and decreased to spring time (0.13 m/s). As a result, the particle tracking method will be appropriate to understand long-term drift ice movement trends by linking them with satellite data in place of limited field observations.

Clinical Course of IgA Nephropathy in Children (소아 IgA 신병증의 추적 관찰)

  • Purpose : Present study was undertaken to find out significance of clinical presentation, initial laboratory data and renal biopsy findings on subsequent clinical course of IgA nephropathy in children. Methods : Clinical and laboratory data were analysed retrospectively from 60 children who have been admitted to the Pediatric Department of Kyungpook National University Hospital for the past 11 years and diagnosed as IgA nephropathy. Renal biopsy findings were graded according to the pathologic subclass proposed by Haas. Results : Pathologic grading according to Haas subclassification showed 10 cases in subclass I, 36 in II, 12 in IV and 2 in V and none in subclass II. Sex distribution showed male predominance (male to female ratio = 3 : 1) and mean age at onset of disease was $10.4{\pm}2.8$ years. Episodes of gross hematuria was seen in 71.7% and IgA level increased in 28.3% of children and these were not associated with pathologic grading nor clinical outcomes. With increasing subclass grading, serum protein and albumin decreased and 24 hours urinary protein excretion increased. Normalization of urinalysis (disappearance of hematuria) was seen in 14% at 1-2 years and 37.1% at 3-4 years of follow up period. In 3 cases, renal function deteriorated progressively and they belonged one each to the Haas subclass III, IV and V. Conclusion : In children with IgA nephropathy, progression to chronic renal failure appears to be quite high and pathologic grading according to Haas' subclassification seems to predict patient's outcome faily well. However, firm conclusion cannot be drawn from present study due to the small numbers of patients and short follow-up period. Therefore further multicenter study involving larger numbers of patients and longer periods of follow-up over 10 years was to be undertaken.

  • Purpose : $Henoch-Sch\"{o}nlein$ purpura(HSP) nephritis has a variable range of prevalence from 25 to 50% among HSP patients and is a common cause of chronic glomerulonephritis in children. In our study, we evaluated the distribution and the association of the angioten-sinogen(AGT) M235T polymorphism with the clinical manifestations, particularly proteinuria in children with HSP with or without nephritis. Methods : The AGT M235T polymorphism was determined in children with HSP nephritis (n=33) or HSP without nephritis(n=28) who had been diagnosed at Busan Paik hospital from January 1996 to June 2001. The M235T polymorphism of the AGT gene was determined by PCR amplification of the genomic DNA. Results : The M235T polymorphism of AGT gene frequency was MM 75%, MT : 25%, TT : 0% in HSP and MM : 64%, MT : 36%, TT : 0% in HSP nephritis, there was no significant differences in the genotype and allele frequencies between the two groups. No significant differences in clinical manifestations at onset and last follow-up were seen between the two genotypes. When statistical analysis was done according to the presence of the M allele, the amount of 24-hour urinary protein excretion and the incidence of moderate to heavy proteinuria(>500 $mg/m^2/day$) at onset and at last follow-up were higher in the MT genotype than in those of in the MM genotype but these difference were not statistically significant. Conclusion : We suggest a lack of association between M235T polymorphism of the AGT gene and clinical manifestations in children with HSP nephritis. However, further follow-up studies based on sufficient number of patients and long term follow up periods are necessary to confirm the role of M235T polymorphism of AGT gene in children with HSP nephritis.

임진강대 석류석의 성장과 다변형작용의 시간적-공간적 관계

  • 임진강대의 변성이질암은 전형적인 바로비안형 변성분대를 보이며, 남쪽으로 갈수록 변성도가 증가하여 석류석$\longrightarrow$십자석$\longrightarrow$남정석 대를 정의한다. 우리는 반상변정의 성장과 여러번에 걸친 광역변형작용의 연관성을 밝히기 위해 광물의 반응관계와 성장순서 그리고 미구조(microstructure)를 -특히 석류석에 대해서- 연구하였다. 임진강대는 크게 세 번에 걸쳐 변형작용을 받은 것으로 해석된다: (1) 지각 두께의 증가에 수반된 압축변형작용 (D$_{n-1}$), (2) 주 엽리(Sn)를 만든 변형작용(Dn), 그리고 (3) 연성전단작용에 수반된 신장변형작용(D$_{n+1}$ ). 석류석대의 석류석 반상변정에서는 약간 휘어진 포유물 궤적(inclusion trail)이 주 엽리면에 대해 연속적이며, 이는 Dn과 동시기에 반상변정이 생성되었음을 지시한다. 이러한 석류석은 녹니석과 백운모로 구성된 주 엽리를 치환하면서 자라기 때문에, 녹니석+백운모+석영=석류석+흑운모+$H_2O$의 반응에 의해 만들어진 것으로 해석된다. 석류석 자형변정(idioblast)이 주 엽리를 자르면서 성장하기도 하는데, 이는 Dn 이후에도 석류석이 후구조(post-tectonic) 광물로 성장했음을 지시한다. 또한, 이러한 석류석은 흑운모를 치환하기 때문에, 동구조(syn-tectonic) 석류석의 생성반응에서와는 달리 흑운모가 반응물임을 알 수 있다. 한편, 십자석대의 석류석은 포유물 궤적에 의해 정의되는 S$_{n-1}$면이 주 엽리면과 사각을 이루며 단속적이기 때문에, D$_{n-1}$과 Dn 사이에 자란 것으로 해석된다. 이와는 대조적으로 십자석은 주 엽리를 치환하면서 자라고 있어서 Dn과 동시기 혹은 Dn 이후에 자랐을 것으로 해석된다..의 환경문제를 발생하지 않으며, 공정액에 첨가제를 투입하지 않으므로 순환형 친환경공정으로 각광받을 수 있다. 본 연구에서는 고온, 고농도의 NaOH 수용액의 처리에 적합한 막소재와 발생될 수 있는 제반 문제점 등을 파악하였고, 장기간의 실험을 거쳐 최적 투과 압력(Trans membrane pressue), 세정 조건 및 주기, 막재질에 있어서 보강하여야 할 Point, 최적 운전 조건들을 토출해 내었고, 향후 실제 Plant에 적용할 계획이다.는 양적으로 다른 두 가지의 유사한 마그마가 수반된 것으로 추정된다. 것으로 추정된다.를 사용하지 않음으로써 효과적이고 만족할 만한 심근보호 효과를 보였다.를 보였다.4주까지에서는 비교적 폐포는 정상적 구조를 유지하면서 부분적으로 소폐동맥 중막의 비후와 간질에 호산구 침윤의 소견이 특징적으로 관찰되었다. 결론: 분리 폐 관류는 정맥주입 방법에 비해 고농도의 cisplatin 투여로 인한 다른 장기에서의 농도 증가 없이 폐 조직에 약 50배 정도의 고농도 cisplatin을 투여할 수 있었으며, 또한 분리 폐 관류 시 cisplatin에 의한 직접적 폐 독성은 발견되지 않았다이 낮았으나 통계학적 의의는 없었다[10.0%(4/40) : 8.2%(20/244), p>0.05]. 결론: 비디오흉강경술에서 재발을 낮추기 위해 수술시 폐야 전체를 관찰하여 존재하는 폐기포를 놓치지 않는 것이 중요하며, 폐기포를 확인하지 못한 경우와 이차성 자연기흉에 대해서는 흉막유착술에 더 세심한 주의가 필요하다는 것을 확인하였다. 비디오흉강경수술은 통증이 적고, 입원기간이 짧고, 사회로의 복귀가 빠르며, 고위험군에 적용할 수 있고, 무엇보다도 미용상의 이점이 크다는 면에서 자연기흉에 대해 유용한 치료방법임에는 틀림이 없으나 개흉술에 비해 재발율이 높고 비용이 비싸다는 문제가 제기되고 있는 만큼 더 세심한 주의와 장기 추적관찰이 필요하리라 사료된다.전 도부타민 심초음파는 관상동맥우회로술 후

Endoscopic Resection of Early Gastric Cancer in Korea: Recent Results and Future Directions (조기위암 내시경절제술-최근 국내 성적과 발전방향)

  • Endoscopic resection has been established as one of the treatment options for selected cases of early gastric cancer in Korea. Yet the indications, best treatment methods, management of complications and follow-up methods after the procedure are still uncertain. Some large-scale multicenter studies have recently been reported in Korea. In this review, I will provide an overview of the recent published articles and future directions for endoscopic resection of early gastric cancer.

천안호 침몰해역의 해상조건 분석

  • 2010년 3월 26일 21시 45분경, 백령도 서남쪽 1.6km(1마일)해상에서 대한민국 해군의 초계함 천안함이 원인 미상의 사고로 침몰한 사건이 발생하였다. 이에 연안공학자의 입장에서는 수색 및 구조에 필요한 기초자료인 해상조건들을 제공하고 시뮬레이션을 통한 보다 자세한 예측 및 유추가능한 자료를 제공한다는 것은 뜻깊은 일임에 틀림없다. 이에 본 연구에서는 백령도-대청도 부근해역의 기상, 파랑, 조석 및 조류, 저질, 부유사 상태등을 조사 분석하고 이를 기초로 해역특성을 분석하였다. 사건당시의 유속상황은 소조기(neap tide)-중조기 (mean tide) 사이에 해당하며 3월 26일이 지나고 4월 3~4일까지는 유속이 가장 강한 대조기 (spring tide)가 진행되는 시점으로 수색 및 구조작업에 에로사항이 있는것으로 파악되었다. 또한, 21:00-22:00 경은 낙조가 진행 중에 있기 때문에 물질이동은 남동쪽이 우세할 것으로 보이며 특히, 불규칙한 해저지형으로 인하여 급격한 와류 등이 존재할 것으로 판단되어 입자추적실험을 수행하였다. 수행 결과, 입자는 유속상황에 따라서, 초기에는 남동쪽으로 이동하지만 장기 예측결과, 외해쪽으로 흘러가는 것으로 나타났다. 이를 통하여 추후, 수색작업의 범위를 외해쪽으로 확대시켜야 할 것으로 사료된다.

Simulation of Monthly Streamflow for the Soyang Basin Using Water And Snow balance MODeling System (융설을 고려한 물수지 모형을 이용한 소양강 댐 상류 유역의 월 유출량 산정)

  • This study describes the WASMOD, water balance model which can consider the snowmelting. The pilot study basin is the Soyang River basin with outlet at Soyang Dam Site and compute long-term monthly streamflow, The advantage of the WASMOD is that the input data is simple and the user can operate easily. To optimize for the parameters of the model, the WASMOD used VA05A of automatic fitting technique. The observed and simulated monthly streamflow hydrographs were compared. The model performance on corrleation coefficient between the observed and the simulated streamflow for the verification periods was above 0.89. It was shown that the WASMOD reproduces the observed monthly streamflow hydrographs very well. This evidence suggests that the WASMOD might be appropriate for the simulation of monthly streamflow
