한국 노인의 잔존치수와 저작능력과의 관계

  • 본 연구는 한국인 노년층이 대표적으로 섭취하는 강도별 음식을 조사하고 각 음식별로 씹어 먹을 수 있는 음식들이 무엇인지를 조사하여 자신이 보유하고 있는 자연치아수와의 상관관계를 알아보아 자연치아의 건강도를 유지, 관리, 예방하는데 하나의 지표로 사용하고 더 나아가 국민구강건강향상에 이바지 하고자 실시되었다. 전국의 60세 남 녀 586명을 대상으로 치과의사와 치과위생사가 구강검사와 면접법으로 설문내용을 기록하는 방식으로 진행하였다. 수집된 자료는 SPSS WIN 14.0 프로그램을 이용하여 분석하였다. 분석기법으로는 잔존치아수와 저작능력간의 관계를 상관계수와 회귀분석으로 분석 검증을 실시하여 다음과 같은 결과를 얻었다. 1. 저작능력과 구강상태와는 치석, 동요도, 충전점수, 우식경험점수, 발거치아수, 임플란트 수, 자연치아수와 나이, 고혈압, 당뇨병, 기타질환, 흡연이 유의한 것으로 나타났다(p<0.05). 2. 회귀분석 결과 나이가 높을수록 저작능력이 감소하며, 자연치아수가 많을수록 저작능력이 높은 것으로 나타났으며 회귀방정식은 저작능력=79.417-(연령$\times$0.281)+3.813$\times$치아수와 같았다(p<0.05). 3. 회귀분석결과 저작능력점수와 자연치아수와의 관계가 가장 높은 것으로 나타났고 회귀방정식은 저작능력=3.817$\times$자연치아+58.694와 같았다(p<0.05). 4. 자연치아로 식사를 하기 위해서는 한국 노인의 주식인 밥과 김치정도를 잘 씹어 먹으려면 잔존 자연치아수가 18개 정도는 있어야 아주 잘 씹어 먹을 수 있고, 잘 먹으려면 12개의 자연치아가 있어야 하며, 육류를 잘 먹기 위해서는 최소한 18개는 있어야 잘 먹을 수 있고, 25개가 있으면 아주 잘 씹어 먹을 수 있다는 연구결과가 나왔다.

Periondontal Disease Detection in Dental Radiography by ROI segment (관심영역을 이용한 치과용 방사선 영상에서의 자연치아 주위 미세변화 검출에 관한 연구)

  • In this paper, we propose a medical image processing method for detection of periodontal disease. The proposed method is the method of an automatic image alignment and detection of minute changes, to overcome defects in the conventional subtraction radiography by digital image processing technique, that is necessary for getting subtraction image and ROI(Region of Interest) focused on a selection method using the structured features in target images. And the method services accuracy, consistency and objective information or data to results. In result, easily and visually we can identify minute differences in the affected parts whether they have problems or not, and using application system.

Spontaneous Recovery of an Intruded Tooth Bounded by Implants: a Clinical Report (임플란트에 둘러싸인 채 압하된 치아의 자연회복: 증례보고)

  • Reports about the intrusion of a natural tooth bounded by implants are very rare, although some concerns have been discussed on the intrusion of teeth connected to implants. A female aged 56 years received an implant (mandibular right first premolar) and post/core onto root rest (mandibular right second premolar) and was restored by single zirconia crown, respectively. Molars were implant restorations. Four month after loading, second premolar was intruded and prominent gap was shown between opposite tooth. Because nonspecific discomfort was expressed, observation was decided after explanation of prognosis of tooth without treatment. Three month later reversal to original position was detected on the periapical radiographs and fully recovered position with intimate contact was completed 11 more months later. Till now 2 years and 3 month observation is being performed. Through the observation of spontaneous recovery of a natural tooth bounded by implants, the cause of intrusion and a mechanism of spontaneous recovery could be estimated.

  • Autotransplantation is the surgical repositioning of an autogenous erupted or unerupted tooth from one site to another in the same individual. This treatment is indicated in traumatic tooth loss, teeth with severe caries, congenitally missing teeth, teeth with bad prognosis and in case of developmental anomalies of teeth. The following 2 cases describe the potential to utilize autotransplantation to replace hopeless teeth with sound wisdom teeth.

Effect of the marginal position of prosthesis on stress distribution of teeth with abfraction lesion using finite element analysis (보철물 변연의 위치가 abfraction된 치아의 응력 분포에 미치는 영향에 대한 유한요소법적 분석)

  • Purpose: The aim of this study was to evaluate the stress concentration and distribution whether restoring the cavity or not while restoring with metal ceramic crown on tooth with abfraction lesion using finite element analysis. Materials and methods: Maxillary first premolar was selected and made a total of 10 finite element model. Model 1 was natural tooth; Model 2 was tooth with metal ceramic crown restoration which margin was positioned above 2 mm from CEJ; Model 3 was tooth with metal ceramic crown restoration which margin was positioned on CEJ; Model 4 was natural tooth which has abfraction lesion; Model 5 and 6 had abfraction lesion and the other condition was same as model 2 and 3, respectively; Model 7 was natural tooth which had abfraction lesion restored with composite resin; Model 8 and 9 was tooth with metal ceramic crown after restoring on abfraction lesion with composite resin; Model 10 was restored tooth on abfraction lesion with composite resin and metal ceramic crown restoration which margin is positioned on lower border of abfraction lesion. Load A and Load B was also designed. Von Mises value was evaluated on each point. Results: Under load A or load B, on tooth with abfraction lesion, stress was concentrated on the apex of lesion. Under load A or load B, on tooth that abfraction lesion was restored with composite resin, the stress value was reduced on the apex. Conclusion: In case of abfraction lesion was restored with composite resin, the stress was concentrated on the apical border of restored cavity regardless of marginal position. It was favorable to place crown margin on the enamel for restoring with metal ceramic crown.

Association between the Number of Existing Permanent Teeth and Chronic Obstructive Pulmonary Disease (현존자연치아수와 만성폐쇄성폐질환과의 연관성)

  • The aim of this study was to evaluate whether the number of existing permanent teeth is associated with chronic obstructive pulmonary disease (COPD) in a representative sample of Korean adults. Data from 3,107 subjects who participated in the 2009 Korea National Health and Nutrition Examination Survey were examined. The dependent variable was COPD and the independent variable was the number of existing permanent teeth. Spirometry results were classified into three groups (normal pattern, restrictive pattern, and obstructive pattern) by trained technicians. We used dichotomized COPD variables (no vs. yes). The number of existing permanent teeth was evaluated by oral examination and divided into 3 groups (0~19, 20~27, and 28). Demographic factors (age group and sex group), socioeconomic status (education and income), health behaviors (smoking and drinking), oral health and behavior (frequency of toothbrushing; periodontitis; decayed, missing, filled, permanent teeth index; and denture status), and general health status (body mass index, diabetes mellitus, and hypertension) were included as confounders in the analysis. Bivariate analysis and multivariate logistic regression analyses including confounders were applied, and all analyses considered a complex sampling design. Stratified analysis was performed by smoking status. After controlling for various confounders, there was a significant association between the number of existing permanent teeth and COPD (odds ratio [OR], 1.90; 95% confidence interval [CI], 1.20~3.00 for the 20~27 group; OR, 3.93; 95% CI, 1.75~8.84 for the 0~19 group). The association was more significant in current smokers (OR, 8.90; 95% CI, 2.53~31.33). Our data indicate that the number of existing permanent teeth was independently associated with COPD, especially in current smokers. Further longitudinal research is needed to determine whether oral health promotion plays a role in the improvement of lung function and prevention of COPD.

TREATMENT OF ANTERIOR TEETH FRACTURE BY FORCED ERUPTION (치아 정출술을 이용한 전치부 외상치의 치험례)

  • There have been many treatment methods for traumatic subgingival crown fracture and intrusion without spontaneous eruption. The orthodontic forced eruption generally results in favorable clinical findings than crown lengthening with osteotomy and intentional replantation. In first two cases with subgingival crown fracture due to trauma, authors applied orthodontic forced eruption with axed appliance after root canal therapy and then restored them with composite resin. In another case with traumatic intrusive luxation, we observed spontaneous eruption of the corresponding tooth for about 6 months and then returning it to normal position by forced eruption with removable appliance, but root canal filling was conducted after apexification due to devitalization during forced eruption, and so clinically favorable results were obtained.
