AED System using Fuzzy Rules (퍼지규칙을 이용한 AED 시스템)

  • Recently, death number of heart attack in the world is increasing rapidly. Therefore, to solve these problem, it is trend that is making mandatory automatic defibrillator AED establishment to airport, school, at home. However, AED use in an emergency or equipment failure caused malfunctions if equipped with AED may even become obsolete. In this paper, in order to improve this problem, AED Simulator using the fuzzy simulation technique in comparison to existing methods Tilt ambient temperature conditions and in consideration of the conditions, self-diagnostics, error detection at the time to determine whether the development of intelligent simulation. Moreover, in this paper, it proved that fuzzy AED Simulation improved fault detection probability results 30% more than conventional method.

  • In Korea, the "Act on Emergency Medical Care" was enacted in 1994, which was a trigger for first aid training. Currently, first aid training including AED for the general public is done, but the actual use of AED is not enough. The reason for this is that there may be a problem with non-uniform BLS training. The personal and social factors that hinder the intent for AED use were identified through previous studies and applied to BLS training improvement. As a result, 200 subjects (41.6%), who had no intention to use AED, among a total of 481 subjects, were reduced to 43 subjects (8.9%) after receiving the improved BLS training. As a detailed method to increase the intention of using AED, the need for AED location training was highest after the training. Based on this, it is important to convey accurate contents in AED training and the training for location awareness of AED should be done at the same time.

Use of Defibrillator(AED) on Prehospital Cardiac Arrests (심정지 환자 이송 시 구급대원의 자동제세동기 사용현황)

  • Purpose: There has been an increase in the number of prehospital cardiac arrests due to the increasing number of cardiovascular diseases and the increase in the average age of the population. We performed this study to identify the proper resuscitation technique and AED to be used to increase the survival rate in prehospital cardiac arrests. Methods: This studied 159 victims with prehospital cardiac arrests(VF or VT rhythm) by EMT's Reports from January to August, 2005. Results: 108 of 159 victims(67.9%) were shocked by AED. Eighty of 159 victims(50.9%) were recorded with AED shock in prehospital cardiac arrests. A number of shocks is averaged 2.19; 46.2% of one-shock and 86.1% of 1-3 shock. EMS first-tier response interval from time of dispatch to scene arrival was 5.88 minutes, from scene arrival to scene start was 7.36 minutes, from scene start to hospital admission was 9.91 minutes and from scene arrival to AED shock was 6.84 minutes. EMT provided advanced care to prehospital cardiac arrests: 97.5% in CPR, 10.1% in advanced airway management, 67.9% in AED shock. Conclusion: With the increase in cardiovascular disease and old age, the number of prehospital cardiac arrests has risen gradually. However, there were lack of CPR by bystander, defibrillation and advanced cardiac life support(ACLS) in prehospital stage. To improve the adequacy of basic life support and to increase the performance of ACLS, especially AED, we must create challenges to develop new protocols in prehospital care.

Preparation and Self-Confidence to Response to Emergent, Acute or Life-threatening Health Crisis among School Nurses in South Korea (학교내 응급상황에 대한 준비 실태와 보건교사의 응급처치 수행자신감)

  • 배경 및 목적: 본 연구는 학교 내 응급상황(천식 위기, 알레르기 반응, 저혈당 위기, 발작, 심폐정지, 출혈/골절, 머리/목 상해, 열성질환, 중독, 질식)에 대한 행정 차원과 물품구비 차원에서의 준비 실태, 그리고 응급처치 수행자신감을 파악하는데 목적이 있다. 방법: 단면조사연구로 274명의 보건교사를 대상으로 하였으며, 자가보고형 조사지를 이용하여 대상자와 학교의 일반적 특성, 학교 내 응급상황에 대한 행정·물품 준비 실태, 그리고, 응급처치 수행자신감 등 3개 영역의 자료를 수집하였다. 자료수집은 2018년 2월 1일부터 2018년 7월 31일까지 실시하였고, 자료분석은 기술통계, t-검정과 분산분석을 이용하였다. 결과: 대부분의 학교에서 응급상황 관리계획이 있었으나, 응급상황별로 천식 위기는 46.7%, 알레르기 반응은 58.4%에서만 관리계획을 갖추고 있었다. 산소, 혈당측정기, 자동제세동기, 경추고정장치 등은 85% 이상의 학교에서 보유하고 있었으나, 기관확장제 흡입기, 에피네프린 주사제, 흡인기 등은 거의 갖추고 있지 않았다. 응급상황 수행자신감은 5점 만점에 2.67점 (발작관리) 에서 3.55점 (심폐정지관리)이었으며, 일부 응급상황에 대해서 의료기관 근무경력이 증가함에 따라 수행자신감이 증가하였다. 결론: 대부분의 학교에서 응급관리계획을 수립하고 있었으나, 응급상황별 관리계획을 수립한 경우는 반 정도에 불과하였다. 보건교사의 학교 내 응급상황에 대한 응급처치 수행자신감은 전체적으로 낮게 나타남에 따라, 이들의 역량강화를 위한 체계적인 교육과 멘토링 프로그램이 요구된다.

  • Objectives: The purpose of this study is to be conducive to selecting effective educational method for AED(Automated External Defibrillator) education through comparing educational effects between VSI(Video Self-Instruction) and lecturer-centered learning on AED(Automated External Defibrillator) in undergraduates. Method: This study targeted totally 76 undergraduates (39 people for experimental group, 37 people for control group) at J university where is located in J province, from June 8, 2009 to June 12. The analytical method was performed by using SPSS WIN 12.0 Version program. Results: In difference before and after education of the skill performance accuracy in AED between groups, the experimental group(2.89) was higher compared to the control group(2.75). In difference before and after education of knowledge in AED between groups, the experimental group(2.03) was higher compared to the control group(1.82). In difference before and after education of attitude toward AED between groups, the control group(2.14) was higher compared to the experimental group(1.85). In difference before and after education of self-confidence in AED between groups, the experimental group(1.88) was higher compared to the control group(1.76).

  • In August 2000, the American Heart Association and the European Resuscitation Council published the conclusions of tile International Guidelines 2000 Conference on Cardiopulmonary Resuscitation and Emergency Cardiovascular Care which contains both the new recommendations and an in-depth review. The most important changes in the recommendations according to the authors are discontinuation of the pulse-check for lay people, 500 ml instead of 800∼1,200 ml tidal volume during bag-valve-mask ventilation (FiO2 > 0.4) of a patient with an unprotected airway, unifying correct endotracheal intubation size as 8.0 mm, vasopressin (40 units) and epinephrine (1 mg) as comparable drugs to treat patients with ventricular fibrillation, early prehospital survey and intravenous lysis for patients who have suffered coronary artery syndrome and stroke.

Awareness level of basic emergency treatment by airline cabin crew (항공기 승무원의 기본응급처치 의식 조사)

  • In the event of the emergency patient care, cabin crew must take the charge of the first responder quickly. The basic emergency treatment knowledge of the cabin crew consisted of 80.5%~97.8% when the emergency scene showed up in the passenger and the types of emergency care with that the cabin crew could cope were bleeding control, fever, seizures, myocardial infarction, airway management, and partial airway obstruction management. Considering these cares, the improper emergency types revealed approximately 3.2%~20.0%. In airway obstruction there was followed by loss of consciousness and this led to cardiac arrest. In case of cardiac arrest, the cabin crew must know how to check breathing and to use the automated external defibrillator(AED). The life-threatening cardiac arrest can happen to any passenger in any time, so the cabin crew should meet with the emergency accident and apply the AED to the cardiac arrest victim.

  • Cardiopulmonary resuscitation (CPR) is an emergency treatment that stimulates blood circulation and breathing when the function of the heart stops or stops breathing. CPR can be divided by basic life support (BLS) and advanced life support (ALS). BLS involves the use of chest compression to force the blood flow to the main organs, rescue breathing to improve the breathing to the respiratory failure patient and the use automated external defibrillator (AED). The categories of advanced life support include advanced cardiovascular life support (ACLS) for adult and pediatric advanced life support (PALS) for children. In the treatment of dental care for children, which is extremely difficult to deal with, and for a variety of reasons, the use of sedation is considered to treat the children who are unlikely to cooperate with dentistry. This is why there is an increasing possibility of an emergency situation involving cardiac arrest. PALS includes the BLS, and it presents a systematic algorithm to treat respiratory failure, shock and cardiogenic cardiac arrest. In order to manage emergency situations in the pediatric dental clinic, respiratory support is most important. Therefore, mastering professional PALS, which includes respiratory care and core cases, particularly upper airway obstruction and respiratory depression caused by a respiratory control problem, would be highly desirable for a physician who treats pediatric dental patients. Regular training and renewal training every two years is necessary to be able to immediately implement professional skills in emergency situations.

Technological Governance Regarding Life-Sustaining Technologies: The Limitations of RRI and Bioethics ("한국의 연명의료정책과 기술 거버넌스: 사회에 책임지는 기술혁신(RRI)의 적용 한계와 생명윤리")

  • Recently, as DNR prevails more and more in Korea, discussions regarding meaningless medical life-sustaining-treatment (LST) intensified. Some of the Supreme Court decisions are even discussed in mass media, causing public debates. These cases tell us that, as life-sustaining medical technologies are highly developed, more sociological and policy-related analyses are needed on them. Firstly, this study will review 40 previous studies that analyze recent discussions in Korea about LST. Secondly, this study also shows that in bioethical and policy-related perspectives, governance about LST calls for a new implications regarding thanatoethics and thanatopolitics. In this new theoretical framework, death with dignity (DwD) can be understood as a process of giving back the thanatopower to the subject who chooses his way of ending based on his sound and free will. Thirdly, some of the new LST or resuscitation technologies such as automated external defibrillators (AED) are developed in RRI framework. However, if subjects themselves choose not to apply those technologies on them, as in the case of DNR (do not resuscitate) vows, meaning of developing such technologies are to be questioned. But currently such questions regarding the limitations of RRI are seldom asked. I argue that in order to properly apply RRI framework on existing technology, we also need to consider these points.

South Korean and Japanese intention to use automated external defibrillators in out-of-hospital cardiac arrest situations (일반인의 AED 사용에 관한 한·일 비교 연구)

  • Purpose : The purpose of the study is to investigate the factors influencing using AED use in South Korea and Japan. Methods : I conducted a questionnaire survey from February 25 to March 4, 2013, receiving responses from 517 people in Korea and 520 people in Japan. The questionnaire included sociodemographic factors, history of heart disease, AED knowledge, and other variables. A logistic regression analysis was conducted. Results : Among the 517 Korean respondents, 220 (42.6%) intended to use AEDs. Among the 520 Japanese respondents, 384 (74.4%) had similar intentions. Multiple logistic regression analysis found the following significant predictors of intent to use AEDs: gender (odds ratio [OR] = 0.419), household income (OR = 0.991), and AED knowledge (OR = 2.833) in Korea; and gender (OR = 0.582), age (OR = 0.968), cardiac disease history (OR = 2.099), and AED knowledge (OR = 2.984) in Japan. Conclusion : It would be helpful to teach AED use to encourage performing public access defibrillation. In addition, it is necessary to design AED education programs based on country-specific factors.
