Effects of Aromatherapy on Stress, Sleep, Nausea and Vomiting during Patient Controlled Analgesia Treatment of Patients with Hysterectomy (자가통증조절장치 (PCA) 사용자에서 향기흡입법이 자궁적출술 후 스트레스, 수면 및 오심과 구토에 미치는 효과)

  • Purpose: The purpose of study was to examine effects of aromatherapy on stress, sleep, nausea and vomiting of women after laparoscopic hysterectomy. Methods: The participants were 60 women who had laparoscopic hysterectomy: experiment group for aromatherapy (n=30) and control group for routine care (n=30). The experimental group received inhalation aromatherapy for 5 minutes, twice; the first was done right after the operation, the second was at 9 pm before sleep on the same day-while the control group had no inhalation. Data were collected from July to September, 2012 at G hospital. Results: The degree of psychological stress was not significantly different between two groups (t=-1.96, p=.054). Yet, there were significant differences between two groups for degree of physiological stress (t=-3.20, p=.002), the level of cortisol (t=-2.01, p=.049), the score of sleep status (t=2.47, p=.016), the score of sleep satisfaction (t=2.43, p=.018), and the score for nausea and vomiting (t=-2.58, p=.012). Conclusion: Inhalation aromatherapy using the mixed oil of lavender, mandarin, and marjoram was effective in decreasing the level of physiological stress, cortisol, and the score for nausea and vomiting, and also allowed the participants to have a better sleep. Therefore, inhalation aromatherapy could be effective in improving the quality of life of these women during recovery.

Effects of Music Therapy on Pain, Anxiety and Length of Stay of Patients with Laparoscopic Hysterectomy in the Postanesthesia Care Unit (선호 음악요법이 복강경 자궁적출술 환자의 수술 후 통증, 불안 및 회복실 체류시간에 미치는 효과)

  • Purpose: This study examined the effects of music therapy on pain, anxiety and length of stay of patients undergoing laparoscopic hysterectomy in the postanesthesia care unit (PACU). Methods: Sixty patients who received laparoscopic hysterectomy under general anesthesia from a PACU in a university hospital located in Cheonan city participated in this study. The experimental group (n=30) was offered the option to listen to their preferred music by using MP3 players and headphones for 30 minutes. The control group (n=30) received routine postoperative nursing care. Visual analogue scale was used to measure participants' pain and anxiety, length of stay in the PACU was examined by using their medical records. Data were collected from December 2013 to February 2014 and analyzed by descriptive statistics, ${\chi}^2$-test, Fisher's exact test, and independent t-test using SPSS version 21.0. Results: The result showed that the level of post-operative pain (t=2.44, p=.018), anxiety (t=2.37, p=.021), and the length of stay in PACU (t=3.06, p=.004) significantly decreased in the experimental group as compared to the control group. Conclusion: This study indicated that music therapy with the patients' preferred music showed positive effects. Therefore, it can be used as a therapeutic intervention for postoperative pain management of patients with laparoscopic hysterectomy.

VEGF Expression Patterns in Eutopic Endometrium of Patients with Endometriosis (자궁내막증 환자에서 자궁내막의 VEGF 발현 양상)

  • Objective: The purpose of this study was to evaluate the correlation between the expression pattern of vascular endothelial growth factor (VEGF) in endometrium and the pathogenesis of endometriosis by investigating VEGF expression patterns and their difference between eutopic endometrium of patients with endometriosis and that of normal controls without endometriosis. Methods: Endometrial sections were obtained from 64 hysterectomy specimens from women under age of 40, who had undergone hysterectomies and had histological evidence of endometriosis, with stage 3 and 4 according to the revised American Society for Reproductive Medicine classification. As for controls, 37 sections were gained from women diagnosed as having cervical intraepithelial neoplasia (CIN) of the uterine cervix and without evidence of pelvic endometriosis or adenomyosis during their operation. The VEGF content was evaluated immunohistochemically in the eutopic endometrium from 64 patients with endometriosis and 37 normal controls. Histological semiquantitative score (H-score) was calculated and compared between study group and control group throughout the menstrual cycle. Results: There was no significant difference in the H-score of VEGF in the eutopic endometrium between patients with endometriosis and controls without endometriosis when compared according to the same phase of the cycle, although the H-score of the study group was significantly higher in the secretory phase than the proliferative phase. Conclusion: The VEGF expression in the eutopic endometrium of women with endometriosis was not different from that of women without endometriosis. This study suggests VEGF expression in eutopic endometrium is unlikely associated with the pathogenesis of endometriosis.

  • Extrapelvic endometriosis is a rare disease. The majority of extrapelvic endometriosis cases involve scar tissue following obstetric and gynecologic procedures. We have treated two cases of extrapelvic incisional endometriosis. A 39 year old female patient with cyclic vaginal spotting after laparoscopic assisted vaginal hysterectomy due to uterine myoma and a 35 year old female patient with a painful palpable abdominal mass after cesarean section. Both underwent complete excision and were proven to have endometriosis by pathology. Here we report on both cases and review the medical literatures.

MR Imaging of Uterine Malignant Mixed M$\ddot{u}$llerian Tumor: Comparison with Endometrial Carcinoma (자궁의 악성혼합뮬러리안 종양의 자기공명영상: 자궁내막암과의 비교)

  • Background: Generally, it is difficult to differentiate uterine malignant mixed M$\ddot{u}$ llerian tumor(MMMT) from endometrial carcinom in radiological and clinical aspects. Our purpose is to investigate MR findings that distinguishes MMMT from endometrial carcinoma. Materials and Methods: We retrogradely evaluated the magnetic resonance imaging findings of pathologically proven 5 cases of malignant mixed M$\ddot{u}$llerian tumor(MMMT) and 14 endometrial carcinomas to know the differential points of these two tumors originating in the endometrial cavity. The size of the mass, presence or absence of myometrial or uterine cervical invasion, growth pattern of the mass, signal intensity and degree and pattern of contrast enhancement were analyzed and compared. Results: The length of the long axis of the MMMT was 1.5-9.0cm(average, 5.7cm) but that of the endometrial carcinoma was 0.5-6.0cm(average, 2.5cm). Invasion of uterine cervix which was found in 3 MMMT cases, dilated the endometrial cavity and the lumen of the uterine cervix and showed the pattern of growing into the external os. Invasion of uterine cervix was found in only one case of endometrial carcinoma. The presence or absence of myometrial invasion, the signal intensity and homogeneity on T1- and T2-weighted images, and the degree and patterns of contrast enhancement showed no significant difference. Conclusion: Any specific finding to differentiate MMMT from endometrial carcinoma was not ascertained. However, MMMT can be suspected if the size of the endometrial mass is greater than 5cm and if the mass dilates the enocervical canal and invades the uterine cervix.

Dose Distribution of 3-Channel Ovoid Applicator (3-Channel Vaginal Ovoids의 선량분포 특성)

  • This study was aimed to develop a new ovoid applicator for vaginal high-dose rate intracavitary radiation therapy, evaluate uniformity of dose distribution, and assess clinical applicability. The authors evaluated dose uniformity of vaginal mucosa according to 5-different ovoid-separation using 2-channel and modified 3-channel ovoid applicator. There were no significant differences in the dose distribution along the vaginal mucosa with 2 and 2.5 cm separations, but there were between the 2-channel and 3-channel ovoid applicator with a separation of 3 cm or more. Although a low dose area was shown between two ovoid applicators with the 2-channel ovoid applicator, the dose distribution along the vaginal mucosa with the 3-channel ovoid applicator was very uniform.

  • Objectives: This paper was aim to report the clinical study for five cases received Korean medicine hospital treatment because of hysterectomy sequelae within 5 days after surgery.Methods: The patients having treated with hospital management, from March 1st in 2014 until March 24th in 2015, was enrolled in this study. We collected their age, chief complaint, vice complaint, diagnosis, visit days after surgery, treatment period, and etc. We treated the patients with acupuncture, moxibustion, and herbal medicine.Results: All of the cases treated with Korean medicine in this study revealed the recovery. The vice complaint also had improved.Conclusions: Korean medicine is effective on patients having variable postoperative symptoms following hysterectomy. And it is necessary to conduct an additional treatment about menopause symptoms.

A clinical report of thermographic change after Korean Medicine Treatment on post-hysterectomy syndrome patients (자궁적출술 후 증후군의 한방치료 전후 체온분포 변화에 관한 임상보고)

  • Objective : The aim of this study is to analyze the effectiveness of Korean Medicine treatment on post-hysterectomy syndrome patients by thermographic change during treatment. Methods : 5 female patients who had a hysterectomy within a year were treated with Korean Medicine concluding acupuncture, herbal medicine, cupping therapy, herbal bath. We evaluated the thermographic differences by checking Digital Infrared Thermal Imaging(D.I.T.I.) at CV17(膻中), CV4(關元), ST3(巨髎), BL15(心兪), and BL23(腎兪). Results : In consequence of Korean Medicine Treatment for 3 weeks, thermographic differences between CV17(膻中) and CV4(關元), ST3(巨髎) and CV4(關元), BL15(心兪), and BL23(腎兪) tended to decrease. Conclusion : After Korean Medicine treatment, thermographic differences between upper body and lower body tended to decrease cause shangrexiahanzheng(上熱下寒證) made by hysterectomy became moderated.

  • Objectives: The purpose of this study is to report the effect of Korean medicine treatments on Postoperative symptoms. Methods: The patient was treated with herbal medicine, acupuncture and electronic moxibustion. The efficacy of treatment was evaluated with visual analogue scale (VAS), the degree of symptom relief and presence or absence of symptoms. Results: After hospital care with korean medicine treatments, symptoms such as fatigue, lower abdominal pain, thirst, hot flush, cold hypersensitivity of hands and feet and tinnitus were disappeared. Conclusions: To promote physical strength and reduction of fatigue, intensive hospital care with korean medicine treatment seems to be effective. After discharge, continued outpatient treatment for complications of surgery and menopausal disorder seems to contribute to quality of life and satisfaction for patient who undergoing hysterectomy.

Adjustment after a Hysterectomy (자궁적출술후 여성의 성생활 변화와 적응)

  • Purpose : This study examined the relationship between sexual changes and adjustment and identified the factors which affect adjustment after a hysterectomy. Method : The subjects were 89 women under 50 years of age registered at gynecology departments of general hospitals in Seoul. Result : $60.7\%$ of the women restarted coitus during six weeks to three months post operation. They felt a decrease in vaginal secretions$(68\%)$, and abdominal and pelvic pain$(59.8\%)$, but2/3 of them didn't change the frequency of coitus and level of orgasm. With respect to the adaptability of the sexual life, there was a significant difference in the time to restart coitus, coitus, lack of vaginal secretions, abdominal and pelvic pain, change of frequency of coitus, experience of orgasm, importance of sex and avoidance of coitus, according to job, income, and health condition. Conclusion : It is appropriate to restart coitus six weeks to three months after surgery and preliminary information should be given to patients after surgery as abdominal and pelvic pain could be relieved after twelve months. Also, sexual adjustment can be improved if they can recognize the changes after surgery from sexual life before surgery.
